Introduction

The Visiting Professional Programmes in nursing were established to recognise the wealth of experience and expertise that, as a large teaching hospital in the centre of London, Guy’s and St Thomas’ has to offer in relation to clinical expertise. The Trust has acclaimed international success through patient care campaigns, including a recent campaign on care for patients with dementia, and is home to several award winning nurses. The visiting professional programme is a great way to share our learning and expertise with others.

Who should attend?

The nursing programme is aimed at registered nurses and midwives that are currently working outside of the UK who wish to broaden their knowledge of health care and gain insight into working in the NHS. Experience of working in a hospital environment is not essential but would be an advantage for those attending.

Duration

The nursing placements that we offer normally run for 6 weeks, but they can run between 2 weeks and 8 weeks. This is to enable a comprehensive experience and an opportunity to learn about what it is like to work as a registered nurse or midwife at Guy’s and St Thomas’ NHS Foundation Trust.

Format

The nursing programme is delivered entirely within the hospital work place. It is an observational programme, so

visiting nurses will be aligned with a buddy/mentor from within the clinical environments that they have pre-

chosen. The buddy/mentor will then ensure that all of the learning and development needs and expectations are

met. There is a wide selection of departments to be placed within, so we ask that any visitors inform us of their

specific areas of interest

The nursing programme has been devised so that visiting professionals can experience the full authentic experience of Guy’s and St Thomas’ NHS Foundation Trust. There are many unique offerings and benefits that attending the nursing programme can provide.

Available Departments

The Visiting Professional Programme has been designed by the Chief Nurses Office to ensure that those visiting Guy’s and St Thomas’ NHS Foundation Trust gain the most from their experience. This is why we ask those wishing to visit to specify which departments they are interested in observing.

Our Departments

All of the departments that are available for visiting professionals to attend focus on the nursing philosophy of the 6 C’s. This means that our nursing teams centre their work on care, compassion, competence, communication, courage and commitment.

Abdominal medicine and surgery Guy's and St Thomas' is the centre of excellence for the treatment of kidney disease and urological disease and we have the largest and most active living kidney donation programme in the UK. We are a regional centre for kidney transplantation and a supra-regional centre for kidney and pancreas transplantation and for blood group and HLA incompatible transplantation. Our urology services is one of the largest in the South East of England and we are a tertiary referral centre for many urology cancer pathways. We are also a specialist centre for prostate, bladder, renal and testicular cancer, as well as andrology surgery, endourolgoy and continence. The Abdominal Medical Directorate also contains gastrointestinal services. Common surgeries and procedures that are undertaken by these services include elective cholecystectomy, elective laparoscopic nissen fundoplications and thyroid surgeries. We have a state of the art endoscopy department at St Thomas’ and a satellite endoscopy Centre running at Guy’s Hospital which caters for both the diagnostic and therapeutic intervention of gastrointestinal disorders as both a primary and tertiary referral centre.

Acute medicine Acute Medicine and older persons has the second highest adult admissions rate in Guy’s and St Thomas’ (10,408 admissions 2012) and the Emergency Department had 140,000 attendances in 2012-13. We currently have the top performing A&E in South London. Our Acute Medicine Directorate includes the emergency department and urgent care centres, 10 wards and outpatient clinics such as older person assessment unit, chest and sleep in both Guy’s and St Thomas’ locations.

Cardiovascular services The Trust is a major specialist centre for cardiac surgery and cardiac services. The surgical procedures undertaken include coronary artery bypass, valve replacement and repair, major aortic surgery and congenital heart surgery. The centre provides a wide range of diagnostic tests and treatments including ECG, stress testing, diagnostic angiography, angioplasty, electro physiology studies, pacemaker and ICD insertion. We are proud of our nurse led services providing cardiac rehabilitation, heart failure, rapid access and cardiac arrhythmias treatment.

Evelina Children’s Hospital Evelina London Children’s Hospital, which is a state of the art hospital located next to St Thomas’, cares for children and young people from across south London and south east England. We offer a unique range of services from antenatal diagnosis through to childhood, into adolescence and on into adult life.

GRIDA (Genetics, Rheumatology, Infection, Dermatology and Allergy) We have many specialty clinics and the nurses who support these are highly trained in specialist fields. Our centres are identified by the Department of Health as Centres of Excellence for Specialty Care. The allergy service at Guy's is one of only five full-time allergy centres in the UK and it is one of the busiest and largest allergy centres nationally. The majority of the clinical service is outpatient-based. The Louise Coote Lupus Unit, the largest lupus unit in Europe, offers a wide range of services to patients with lupus and other autoimmune and connective tissue disorders. We also have a Directorate of Infection (DoI) which is one of the largest centres for infectious diseases clinical training in the UK. We also specialise in rheumatology and we offer a wide range of services to patients with diseases of the joints and connective tissues through outpatient clinics at both Guy's and St Thomas'.

Medical specialities The directorate of Medical Specialties comprises both clinical and non-clinical support services for ophthalmology, neurology, neurophysiology diabetes endocrine and lipids, and neurofibromatosis. These services predominantly provide diagnosis, treatment and continuing care (where appropriate) to patients with long term conditions. Each service has sub-specialty areas of expertise, which are distinctive and provide clinical excellence locally and nationally. The ophthalmology department offers a wide range of outpatient and surgical services for diseases of the eye and surrounding structures. The neurology department offers diagnosis and treatment to patients with diseases of the nervous system. The department of clinical neurophysiology offers a range of investigations that assess the electrical activity of the brain, nerves and muscles. Diabetes and endocrinology has busy centres on both sites, catering not only for patients with diabetes, but also patients with endocrine and lipid disorders. Finally, the neurofibromatosis department provides care for children and adults with complex and non-complex neurofibromatosis.

Haematology and oncology Our dynamic haematology and oncology directorate includes top haematology and oncology specialists alongside the very latest state-of-the-art facilities. We are building a new, state of the art Cancer Centre at Guy’s Hospital which will allow us to give patients seamless care. The centre will be a hub for south east London, providing specialist cancer services, training, development and research. This will also help us to improve both cancer treatments and outcomes.

Perioperative, Critical Care and Pain The Critical Care Department is one of the largest in the UK, consisting 70 beds. As a tertiary referral centre, it is nationally and internationally renowned for its services. Our therapies are varied and advanced, including renal replacement therapy, plasma exchange, advanced respiratory and ventilatory therapies such as HFOV (High Frequency Oscillatory Ventilation) & ECMO (Extracorporeal Membrane Oxygenation).

Cardiothoracic and surgical intensive care (OIR) provides level 3 support for adult patients who require short term ventilation & invasive monitoring after cardiac surgery and complex general surgery. It is a busy 11-bed unit caring for Level 2 and 3 patients. The medical and surgical high dependency department specialises in the care of patients who need intense monitoring and treatment that cannot be provided on a ward. Offering a wide range of supportive therapies our nurses are highly skilled in the provision of inotropic support, non-invasive ventilation therapy, complex wound management, rehabilitation and tracheostomy management. Our cardiothoracic high dependency team specialise in the care of patients who need intense monitoring and treatment following cardiac and vascular surgery. The Critical Care Response Team is a team of critical care doctors and nurses providing specialist support and advice to ward teams in the management of critically ill ward patients. We are also world renowned for our Lane Fox Respiratory Unit, which is the largest Weaning and Home Mechanical Ventilation Unit in the UK supporting a population of over 1,200 in the use of non-invasive ventilation and ventilated patients at home. We provide inpatient care for those failing to 'wean' from invasive ventilation following critical illness, including tracheostomy management and ventilation.

Surgery Combining one of London’s busiest trauma wards, a specialist elective orthopaedic unit and one of London’s leading plastic surgery units, the Surgical Directorate cares for patients from the local area, as well as those who travel nationally and internationally to access our expert services. There are four main wards within our Surgical Directorate. We have the George Perkins Ward which is a 22 bedded orthopaedic ward specialising in emergency trauma cases. The Queen and Sarah Ward make up the Orthopaedic Unit with a total of 41 beds. We care for patients undergoing elective orthopaedic, plastic, breast and neuro-modulation surgery and for patients who have sustained spinal trauma. The Alan Apley Ward is a 22 bedded unit specialising in a wide range of elective and trauma plastic surgery procedures. As one of London’s leading plastic surgery units, we specialise in breast reconstruction following diagnosis of cancer, complex wound management and highly specialist dressings.

We have an acute bay with monitored beds for the management of patients undergoing major breast reconstruction, offering nurses the opportunity to learn new skills in nursing acute patients. Outpatients within the directorate include Fracture Clinic, Hand Trauma and the Plastic Dressing Clinic at St Thomas’ and the Orthopaedic Centre at Guy’s.

Women’s services Guy’s and St Thomas’ is a local maternity service and a tertiary referral unit providing care to women and their families in the local area. We have approximately 6700 births per year which range from low risk women being cared for at home or on the midwife-led Home from Home Birth Centre, to women with complex social, medical and psychological needs receiving care from a range of healthcare professionals on the obstetric-led Hospital Birth Centre.

Community The District Nurse teams in Lambeth and Southwark work alongside Guy’s and St Thomas’ Foundation Trust and King’s College Hospital to provide skilled nursing care at home, promote and maintain independence, provide patient education, advice and support to individuals, carers and the family. The nursing teams, in a variety of roles, undertake comprehensive nursing assessment and case management. They prescribe medication, dressings and undertake complete episodes of care in conjunction with primary care.

Clinical Imaging and Radiological Services We have one of the largest Imaging departments in the UK, providing a wide range of clinical support services to all directorates. There is an emphasis on cutting edge equipment and the introduction of new clinical techniques giving imaging here a reputation of being at the forefront of innovative radiological interventions. Highly motivated and enthusiastic nurses work in the department of interventional radiology making an important contribution to the care and management of both inpatients and outpatients requiring diagnostic or interventional procedures in an acute Trust setting.

The nursing team have a wide range of clinical expertise in areas including assisting during the recovery of patients undergoing interventional radiology procedures i.e. angiogram/angioplasty and arterial stenting, oesophageal dilatation and stenting, and embolization.

Chief Nurses Office Faculty

Dame Eileen Sills DBE | Chief Nurse and Director of Patient Experience and Infection Control

Dame Eileen Sills was appointed Chief Nurse in 2005. Having qualified as a registered nurse in 1983, Eileen has held a number of general management and senior nursing leadership posts in London. She was awarded a CBE in 2003 for services to nursing, and a DBE in January 2015.

Eileen holds two visiting professorships, at King’s College London and London South Bank Universities. She is a member of the NHS Employers policy board and the Chair of the grant committee for the Burdett Trust for Nursing. Eileen has a national reputation for strong, visible, clinical leadership, and her drive to take senior nurses back to the bedside has earned her a national reputation for her Clinical Fridays initiative.

In August 2013 Eileen was appointed as the Clinical Director for London’s Strategic Network for Dementia.

Guy’s and St Thomas’ NHS Foundation Trust Guy's and St Thomas' NHS Foundation Trust is one of the largest Foundation Trusts' in the UK. In 2013, the Dr Foster Hospital Guide awarded us Trust of the Year for safe care.
