OPEN SER ISSUES - ICSBLIMERICK GENERATING STATION

Instrumentation Setpoints (7.2.2.1)

Open Item: We will require that the applicant revise his methodology fordetermining the trip setpoint and the allowable value so as to includeonly drift associated with that portion of the channel tested when thesetpoint is determined and the tolerance allowed to minimize the needfor frequent adjustments.

At Power Testability of Actuation Instrumentation(7.2.2.3)

Open Item: We will require that the applicant demonstrate that theLimerick design includes an on-line testing capability for the actuationinstrumentation channels, logic and actuation devices associated withthe PCRVICS, RESI, RAIS, HCRI, RCIC and ESW systems.

Lifting of Leads to Perform Surveillance Testing(7.2.2.4)

Open Item: To provide conformance with the recommendations of R.G.1.118we will require that design modifications be implemented to provide thecapability to perform surveillance testing without lifting leads.Further, we will require that design modifications be implemented forthose cases where performance of surveillance testing requires the openingof circuit breakers where the opening of the breakers does not causea trip of the associated instrument channel or the actuation of theassociated component or device.

Engineered Safety Features Reset Controls(7.3.2.3)

Open Item: To provide conformance with the recommendations of IEBulletin 80-06 and the LRG-II position the staff will require that thecontrol systems for the HPCI and RCIC isolation valves be modifiedsuch that they do not automatically reposition open on a reset of theisolation logic. In addition, we will require that design verificationtesting be performed to verify that safety system valves do notreposition on a actuation logic reset signal.

Manual Initiation of Safety Systems (7.3.2.5) |,

Open Item: We will require that the application propose plant modificationswhich will provide separate interlocks for remote manual operation atthe component level for the CS, CSRHR and LPCI injection valves.

Capability for Safe Shutdown Following Loss ofElectrical Power (7.4.2.1)

Open Item: We will require that the applicant either demonstrate thatsufficient alarm and/or indication is provided to alert the operatorto the loss of power of the instrumentation and/or control systemsaddressed in IE Bulletin 79-27, or that design modifications be proposedto provide appropriate alarms or indicators.

Remote Shutdown System (7.4.2.3)

Open Item: To satisfy the requirements of GDC 19 and the guidance of SRPSection 7.4 we will require that the applicant revise the design of theRemote Shutdown System as follows:

(1) The design should provide redundant safety grade capability toachieve and maintain hot shutdown from a location or locationsremote from the control room, assuming no fire damage to anyrequired systems and equipment and assuming no accident has occurred.The remote shutdown station equipment should be capable of maintainingfunctional operability under all service conditions postulatedto occur (including abnormal environments such as loss ofventilation), but need not be environmentally qualified foraccident conditions unless environmental qualification isrequired for reasons other than remote shutdown. The remoteshutdown station equipment, including indicators, should beseismically qualified.

(2) Redundant instrumentation (indicators) should be provided todisplay to the operator (s) at the remote shutdown location (s)those parameters which are relied upon to achieve and verifythat a safe shutdown condition has been attained.

(3) Credit may be taken for manual actions (exclusive of continuouscontrol) of systems from locations that are reasonably accessiblefrom the remote shutdown stations. Credit may be taken formanual actions involving jumpering, rewiring, or disconnectingcircuits.

(4) The design should pecvide redundant safety grade capabilityfor attaining subsequent cold shutdown through the use ofsuitable procedures.

(5) Loss of offsite power should not negate shutdown capabilityfrom the remote shutdown stations.

The design and procedures should be such that following activationof control from the remote shutdown location, a loss of offsitepower will not result in subsequent overloading of essentialbusses or the diesel generator. Manual restoration of power toshutdown loads is acceptable provided that sufficient informationis available such that it can be performed in a safe manner.

(6) The design should be such that if the manual transfer of controlto the remote location (s) disables any automatic actuation ofESF equipment, this equipment can be manually placed in servicefrom the remote shutdown station (s). Transfer to the remotelocation (s) should not change the operating status of equipment.

(7) Where either access to the remote shutdown station (s) or theoperation of equipment at the station (s) is dependent upon theuse of keys (e.g., key lock switches), access to these keys shallnot be precluded by the event necessitating evacuation of thecontrol room.

(8) The design should comply with the requirements of Appendix Rto 10 CFR 50.

Post Accident Monitoring Instrumentation (7.5.2.3)

Open Item: The adequacy of the post-accident monitoring instrumentationis currently under review by the staff and will be addressed in a supplementto this report.

Bypassed and Inoperable Status Indication (7.5.2.4)

Open Item: We will require that the applicant either verify that a systemout-of-service alarm is provided for items 14-17 which is automaticallyactuated by system component and support system failures, or propose designmodifications to provide appropriate alarms or indication.

High Energy Line Breaks and Consequential Control System Failures (7.7.2.1)

Open Item: We will require that the applicant evaluate the effects andconsequences of a HELB to verify that when the safety systems respondto mitigate the effects of the HELB, a single active failure in the safetysystems in conjunction with all possible combindtions of control systemsand non-qualified safety systems failures induced by the HELB are boundedby the FSAR Chapter 15 analyses.

Multiple Control System Failures (7.7.2.2)

Open Item: We will require that the applicant's analysis demonstrate thatfailures of power sources, sensors, headers and impulse lines are boundedby the Limerick FSAR Chapter 15 analysis, and that failures of a headeror impulse line wil' not defeat protection system redundancy, or defeatseparation between control and protective functions.
