Welcome Message from the President

Welcome to our 13th Annual Conference. For most of us, this is a return to a well-known forum, while forthose new at this conference; you are in for a treat. The OAG annual meeting is like no other. We have anopportunity to catch up with friends and acquaintances, establish new links with trusted members of ourcommunity and, as always, to learn.

The topic this year, THE GI WATER COOLER, is a highlight of the subjects that are on our mind, the stuffyou want to know, so you can answer the classic question: What’s up doc? This year’s swine flu has merelyreminded us of some of the infection issues we have to deal with on a daily basis, issues that affect us, our

patients, and our equipment. Our local environment in the endo unit is also changing, with more people, forms and requirements to dealwith. We used to look to the US with some envy, but perhaps we should re-evaluate. We will hear about changes to GI practice in theUS and what may be around the corner for Ontario. There will be lots to hear and ask questions about on-call endoscopy and the evolvingrole of Cancer Care Ontario in our practices. And to top it all off, we have our annual debate … this year asks the question “what is thebest GI anesthesia for now and the future?”

Our companion program is always popular, and this year’s private tour of the new AGO should be in that tradition of success.

Our Saturday Gala looks to be another hit: Formal and Motown. On Sunday, we will hear important information about OMA and CPSOissues, billing information and the issues we, as gastroenterologists, will face in the coming year.

For me, this is a sweet end to my three years as your president. It is with both sadness and eager anticipation of continuing success thatI pass the torch to Dr. David Baron, your next president. I am sure that he, working with our Board, staff and a growing and enthusiasticmembership, will take OAG to new heights. See you at the Water Cooler!

Message from the Conference Co-Chairs

Gastroenterologists take great pride in the care that we provide to our patients, continually improving ourknowledge and updating our skills through the multitude of CME activities available to us. Despite the everincreasing availability of web-based education, many of us still get the most out of the live meetings that weattend. At these meetings it is not always the symposia themselves but rather the "shop talk" betweensessions that we find most helpful in our day-to-day activities. These discussions naturally lend themselvesto similar discussions between clinical encounters whether it be in the coffee room, in the endo suite, thenursing station on the ward or our office "water cooler."

From AASLD to UEGW, there is an alphabet soup of available CME choices. For more than a decade ourannual meeting has provided plenty of opportunity to interact on a personal level with colleagues while providingeducation from a local perspective. Thanks again for making this weekend a highlight on your calendar. I amsure the program will give you more to chew on in the coffee room Monday than last weeks leftovers.

Ivan Thomas Beck, MD, PhD, FRCPC, FACP, MACG

Dr. Ivan Thomas Beck was born in Budapest, Hungary in 1924. He married

his beloved Marjorie Favre in 1948 and has a daughter, Cynthia Beck, a

practicing psychiatrist in Calgary, Alberta.

Dr. Beck completed his medical training in Geneva, Switzerland in 1949

and did his postgraduate training at McGill University in 1952. He went on

to complete his Doctor of Philosophy (PhD) in Experimental Medicine at

McGill in 1963. Dr. Beck is a Fellow of the American College of Physicians, American College of Gas-

troenterology and Master of the American College of Gastroenterology.

He has worked as a clinical and basic investigator where he studied gastrointestinal motility, pancre-

atitis, intestinal absorption and response of the small bowel to injury. Dr. Beck established the first clin-

ical motility laboratory in Canada while at McGill in 1956 and his second at Queen’s University in

1966. He has published over 230 papers, books and chapters and has held MRC grants uninterrupt-

edly from the early 1960’s until 1996. He established the Gastroenterology division at the Hôtel Dieu

Hospital and the unit was named in his honour at a ceremony presided over by the Honourable David

Peterson, former Premier of Ontario.

Dr. Beck is a founding member and past chair of the Canadian Digestive Health Foundation. He is a

founding member of the Canadian Association of Gastroenterology and has served as its first sec-

retary, is a past president and continued to serve as its archivist.

Dr. Matthew Muller completed a degree in biochemistry at McGill University,

followed by a medical degree and internal medicine and infectious diseases

residencies at the University of Toronto. Following the completion of my clinical

training, he completed a PhD in clinical epidemiology, also at the University of

Toronto. Currently, he is the associate medical director of infection prevention and

control at St. Michael’s Hospital and assistant professor of medicine at the Univer-

sity of Toronto. His research interests are in the epidemiology and control of hospital

acquired infections, MRSA, ESBL and C. difficile. Dr. Muller also has a research

interest in hand hygiene, particularly in novel technology to support the improve-

ments in, and measurement of, hand hygiene compliance in healthcare settings.

Session Overview

In this presentation Dr. Muller will describe the evolving epidemiology and clinical

relevance of several important infectious pathogens in the community, outpatient

and inpatient settings, including C. difficile, MRSA, and multi-drug resistant gram

negative bacteria. He will then present a practical approach to infection control

strategies that can and should be used to prevent the transmission of these (and

other) pathogens in the office, clinic and inpatient setting.

David K. H. Wong, MD, FRCPC

Hepatologist, Toronto Western Hospital

Dr. David Wong is a Hepatologist at the Toronto Western Hospital. Dr. Wong

graduated from the University of Toronto Faculty of Medicine in 1988. Following

his clinical training in Internal Medicine and Gastroenterology, Dr. Wong received

further training as a research fellow with Dr. Bruce Walker at the Massachusetts

General Hospital, Harvard Medical School, where he conducted studies on the

cellular immune responses to hepatitis C viral infections in humans.

Dr. Wong was initially appointed full time staff in the Division of Gastroenterology

at McMaster University but moved to the University Health Network (UHN)/Mount

Sinai Hospital (MSH) in 2002, where he is the Clinical Director of the Liver Clinic

at UHN Toronto Western Division. He is also on staff of the Immunodeficiency

Clinic at UHN - Toronto General Division and is currently an Assistant Professor

of Medicine at the University of Toronto.

Dr. Wong's clinical interests lie in the area of viral hepatitis and liver disease in

HIV. His educational responsibilities include co-ordinating the Clinical Hepatology

training program for the University of Toronto and co-coordinating the Gastroen-

terology-Hepatology clinical training program for General Internal Medicine at the

UHN/MSH. Dr. Wong also co-ordinates the annual Hepatology Update meeting

in Toronto.

Bill Thompson graduated from The University of Western Ontario with his Med-

ical Degree in 1985. He completed his training in Internal Medicine at Western in

1989 and in Infectious Diseases at the University of Manitoba in 1990. He has

worked in Moncton, New Brunswick and London, Ontario. While in Moncton he

was the Director of the Infection Control unit and was a member of the Canadian

Nosocomial Infection Surveillance Program. He has been involved in the devel-

opment of policies of infection control as they relate to patients and physicians

alike and in outbreak investigations. Outside of the realm of Infectious Diseases

he has an interest in Distributed Medical Education at both the undergraduate

and postgraduate levels.

Session Overview

By the very nature of their use, endoscopes may bring microbial pathogens

directly into contact with mucosal surfaces or tissues. Inadequate cleaning and

high level disinfection post procedure and subsequent reuse may result in the

transmission of dangerous infectious agents from patient to patient or infection

with nosocomial pathogens. While the risk of patient to patient transmission is

estimated to be one per 1.8 million examinations, outbreaks of infection with a

variety of bacterial and viral pathogens have occurred. This talk will review the

evidence for transmission and the reasons identified for its occurrence. Dedicated

protocols and trained staff who strictly adhere to them are required to adequately

process endoscopes to prevent endoscopy-associated clinical infections. A

systematic process to review these procedures and the adherence to them is

required to minimize infection risk.

Dr. John I. Allen graduated with honors from Rice University in Houston, Texas

in 1973. He attended medical school at the University of New Mexico, graduating

in 1977. His internship, residency and specialty training in gastroenterology were

completed at the University of Minnesota. Dr. Allen spent ten years on the faculty

of the Department of Medicine. In 1991 Dr. Allen was recruited to be Associate

Medical Director of the Virginia Piper Cancer Institute at Abbott Northwestern

Hospital and joined Minnesota Gastroenterology. Currently Dr. Allen is the Medical

Director for Quality at Minnesota Gastroenterology- the largest fully integrated

single specialty gastroenterology practice in the country.

Dr. Allen has over 45 published articles and book chapters on topics ranging

from basic molecular biology to practice management. In 2001 he returned to

the University of St. Thomas (Minneapolis) to complete an MBA program in Medical

Group Management. Since that time, he has developed expertise in practice man-

agement, clinical performance measures and process improvement.

He was, until recently, the Chair of the American Gastroenterology Association

(AGA) Institute Committee on Clinical Practice and Quality. His work in defining

quality measures for colonoscopy and gastroenterology is extensive. He

co-chaired the Physician Performance Improvement Task Force that developed

the current Colonoscopy Measures for the Physician Quality Reporting Initiative

(PQRI) for the Centers for Medicare and Medicaid (CMS). As Chair of the Board

of the Institute for Clinical Systems Improvement (ICSI – www.icsi.org), he helped

develop statewide measures for colonoscopy and colon cancer prevention with

Minnesota Community Measurement. He developed a large practice registry for

endoscopic outcomes and has published extensively on quality measurement in

community practice gastroenterology.

He has spoken nationally about physician performance measures, public account-

ability and quality improvement as it relates to specifically to colonoscopy and colon

cancer prevention. In 2006, he published the first “Outcomes Booklet” for a

community-based GI practice (found at www.mngastro.com). Within the AGA he has

led initiatives concerning transitioning gastroenterology to Electronic Medical Records

and has worked with national health plans to develop a GI Quality Measure Set.

Dr. James C. Gregor is an Associate Professor of Medicine in the Departments

of Medicine and Epidemiology & Biostatistics in the Schulich School of Medicine

and Dentistry at the University of Western Ontario.

Dr. Gregor graduated from the University Western Ontario in 1988, and specialized

in Internal Medicine and Gastroenterology. He did further training in Clinical

Epidemiology and Health Economics at the University of Toronto.

Dr. Gregor joined the Medical Staff at The London Health Sciences Centre in

1995. He served as Program Director for the Division of Gastroenterology from

1998 to 2002. Currently he is the Site Chief for the Department of Medicine at

the Victoria Hospital, Associate Chair of Medicine at the University of Western

Ontario, Site Chief and Program Director for the Division of Gastroenterology.

Dr. Gregor is on the Board of Directors of the Ontario Association of Gastroenterology

and is Chair and organizer of the annual Scholars’ Program for the Canadian

Association of Gastroenterology. He is the Chair of the Specialty Committee in

Gastroenterology for the Royal College of Physicians & Surgeons of Canada.

His research interests include therapeutic endoscopy, inflammatory bowel disease,

economic analysis, and quality of life assessments in Gastroenterology.

Session Overview

The practice of medicine is in continual evolution and our specialty is no excep-

tion. It wasn't that many years ago that Gastroenterologists were one of the pil-

lars of a hospital's internal medicine call schedule, managing everything from the

bleeding peptic ulcer to the acute myocardial infarction. Recent years have seen

us begin to focus our practice, and in many cases our call, on our specialty.

Though our care and knowledge may be available 24/7 in many centres, our

trusty friend the endoscope may not be.

How vital is it to have 24/7 endoscopy? For conditions such as non-variceal upper

GI bleeding there is no clear consensus, though few would argue that availability

times of more than six hours are unreasonable. For other other indications such as

food impaction and colonic decompression, the literature is even more limited.

In this session, we will review some of the indications for on-call endoscopy, some

of the local and international data we use to guide us and some local experience

about what we actually do on nights and weekends when resources are committed

to such an endeavour.

Dr. Terrence Sullivan is President and Chief Executive Officer of Cancer Care

Ontario. He joined the provincial cancer agency in 2001 and occupied succes-

sively responsible positions in preventive oncology/research. From 1993-2001,

Dr. Sullivan held the position of President of the Institute for Work & Health (IWH),

a private not-for-profit Institute affiliated with University of Toronto which he de-

veloped into North America’s leading research center on work related injury. Dr.

Sullivan has held senior roles in the Ontario Ministries of Health, Intergovernmental

Affairs and Cabinet Office. He served two successive First Ministers of Ontario as

Executive Director of the Premier's Council on Health Strategy, including a period

of time as Deputy Minister.

Dr. Sullivan is an active behavioral scientist with research and practice interests

in cancer prevention and health system performance. He holds faculty appoint-

ments in the Departments of Health Policy, Management and Evaluation and

Public Health Sciences at the University of Toronto. Among his voluntary commit-

ments, he is Vice Chair of the Ontario Agency for Health Protection and Promo-

tion and he chairs the Performance Committee of the Canadian Partnership

Against Cancer.

Session Overview

The implementation of colorectal cancer (CRC) screening in Ontario has brought

the cancer community and the OAG together in a common effort to reduce the

burden of this common disease. CRC will result in 8100 new cases and 3100

deaths in 2009 in Ontario. Colon Cancer Check, Ontario's colorectal screening

program initiated in 2007 has had a strong engagement with endoscopists in

Ontario as we work to implement a province wide colorectal screening program.

This presentation reviews the initial ramp-up of hospital only endoscopy to

support the expansion of endoscopic investigation of gFOBT+ individuals and

individuals with a first degree family history of colon cancer. This has occurred in

parallel to the secular growth in colonoscopy which has been under way for some

time. Clinical guidance, the evolution of program standards and data systems

have been key enablers in this program. Public reporting on enrollment and

access indicators have also been features of the program as has regional and

hospital feedback on volumes by indication, wait times and perforation rates. The

feasibility of non-hospital colonoscopy as part of the program is a currently a

discussion point for the program, the professional community and the Ministry of

Health and Long Term Care.

Dr. Stephen Brown is a practicing Anaesthesiologist and Chief of the Anaesthesia

Department at North York General Hospital in Toronto. He is also Past-Chair of the

OMA Section on Anaesthesiology and an Assistant Professor at the University

of Toronto.

In his roles as Chief and Section Chair, Dr. Brown has created and promoted

innovative models of Anaesthesia care delivery. He currently co-chairs the Provincial

Anaesthesia Care Team Implementation Advisory Committee.

His 25-member Department provides anaesthesia coverage for thousands of

in- and outpatient endoscopic procedures every year.

Dr. Nilesh Chande is an Assistant Professor of Medicine and the Program

Director of the Gastroenterology Subspecialty Training Program at The University

of Western Ontario. His interests include medical education, inflammatory bowel

disease and the practice of endoscopy.

Session Overview

Dr. Frederick T. Bray practices Gastroenterology in Ottawa,

Ontario Canada.

He received his medical education from McGill University,

Montreal, Quebec, where he specialized in Internal Medicine.

He further specialized in Gastroenterology, having done a

fellowship at the University of Ottawa, Ottawa, Ontario. At the University of

Ottawa he received an award for Teaching Excellence.

He began his practice in 1994 at the Riverside Hospital, a community based

hospital in Ottawa. At the Riverside Hospital he initiated and ran the ERCP

program. In 1999 the Riverside Hospital was amalgamated with the Ottawa Civic

and Ottawa General hospitals to form the Ottawa Hospital. While at the Ottawa

Hospital, he was able to fulfill one of his natural interests, medical education at

the University of Ottawa.

In June 2001 he resigned from the Ottawa Hospital and opened up an outpatient

Endoscopy Clinic where he performs colonoscopies and gastroscopies.

He is married with two boys, nine and ten years old. In his spare time he enjoys

running marathons and doing triathlons.

Dr. Earle Morgan is the Chief of Gastroenterology at Hôtel

Dieu-Grace Hospital and Windsor Regional Hospital. He is an

Adjunct Professor and co-ordinator for Digestive Diseases and

Nutrition at the University of Western Ontario, Windsor Campus.

He received his medical degree from the University of

Saskatchewan in 1978. Dr. Morgan did his residency in Internal

Medicine at the University of Saskatchewan and a Fellowship in Gastroenterology

at Dalhousie University. Dr. Morgan is a recipient of several awards, has written

a number of publications and is an active member of the Canadian Association

of Gastroenterology, Ontario Association of Gastroenterology and Southwestern

Gastroenterology Association. He is married to Mavis and has four adult daughters.

Dr. Paul Kortan graduated from McMaster Medical School in Hamilton and com-

pleted his Gastroenterology training at the University of Toronto. He completed

two years of post-graduate training at the University of Leuven, Belgium and one

year of Therapeutic Endoscopy in Hamburg, Germany. He joined the Division of

Gastroenterology at The Wellesley Hospital, University of Toronto in 1982. He

and his colleagues established a very successful training programme in Thera-

peutic Endoscopy, which moved to St. Michael's Hospital in 2000. Each year the

Therapeutic Endoscopy group at St. Michael's Hospital trains three clinical

fellows and one research fellow.

Dr. Kortan's expertise is in endoscopic management of pancreaticobiliary

diseases, as well as endoscopic treatment of early gastrointestinal cancer and

palliative modalities for malignancies. Dr. Kortan is also on staff at The Hospital

for Sick Children where he performs paediatric ERCPs and advanced therapeutic

endoscopy procedures.

Shelley Hornstein, PhD

Associate Professor of Architectural History & Urban Culture,

York University

Dr. Shelley Hornstein is a graduate of the Université des Sciences Humaines,

Strasbourg, France. Professor Hornstein has taught at York University since 1985.

Her courses include Memory and Place, Paris as Modernist Dream, The Celluloid

City, No Place like Home, and The Metropolis Revisited. Prior to York University,

she taught at Concordia and Laval Universities. She is a member of York’s grad-

uate programs in Art History, Culture and Communications, and Social and Polit-

ical Thought. She has served as Associate Dean, Co-Director of the Centre for

Feminist Research, and twice Chair of Department of Fine Arts, Atkinson College.

Session Overview

Dr. Shelley Hornstein, will be addressing the Art Gallery of Ontario (AGO)

building itself as a container shaping the ideas we think about as we visit the

exhibits and how we read works in different institutions differently. Why does the

AGO transformation by Frank Gehry matter? How do paintings dialogue with

each other and with us in different museum spaces? Why is visiting an exhibition

in one museum an altogether different experience from visiting the same exhibition

in another?
