In August 2014, the United States Chemical Safety Board (CSB) reported the results of an investigation of a December 2010 explosion in a titanium and zirconium scrap metal processing facility in West Virginia. The explosion resulted in three fatalities and

one injury. Here is a brief summary of what is believed to have happened:1. A blender was being used to process zirconium powder. Employees had observed mechanical problems before the explosion. Blender paddles were striking the sidewall of the blender causing damage. Adjustments and repairs were made but problems continued.2. It is believed that sparks or heat from metal-to-metal contact between the blender blades and the blender side ignited the zirconium powder.

3. The burning zirconium dust resulted in a deflagration - hot gases expanded, producing a “wind” observed by two witnesses. The burning zirconium dust ignited open drums of titanium and zirconium stored nearby, propagating the fire.

4. The initial explosion lofted other dust in the plant into the air causing a secondary dust explosion and fire.Did you know? The CSB has investigated 9 serious combustible dust incidents in the USA since 2003. These explosions and fires caused a total of 36 fatalities and 128 injuries. Five of these explosions involved metal dusts, and three occurred in the same plant. On August 2, 2014 an explosion in a plant in Kunshan, China that manufactures aluminum automobile wheels caused at least

75 fatalities and 180 injuries. Initial reports indicate that the incident was an aluminum dust explosion. Most solid organic materials, as well as plastic powders and many metals, can create an explosive cloud if the particles are

small enough and dispersed in the air at a sufficiently highconcentration. What can you do?The CSB report identified many contributing causes for this incident. Most were related to facility design, failure to comply with dust explosion protection standards, and management systems. But there are things that you can do as a plant worker to prevent dust explosions.• Know whether or not you have any dust explosion hazards

in your plant, and what you must do to ensure properoperation of your plant’s protective systems. • Recognize that good housekeeping is critically important for dust explosion

safety. Accumulated dust on equipment, floors, and elevated surfaces such as lighting fixtures and ceiling support beams can contribute to secondary dust explosions.• Report any maintenance issues which could result in sparking or overheating in any dust handling equipment, and do not operate the equipment until it is repaired.Do you have dust explosion hazards in your plant?Aiche.org

YES, if it is a fine powder or dust! Any material which can burn is capable of causing a catastrophic dust explosion if it is suspended as a fine dust or powder in air or other oxidizing atmosphere. On February 7, 2008 there was a severe explosion in a sugar refinery near Savannah, Georgia, USA. The explosion injured more than 30 people, and the death toll had reached 13 as of the end of March 2008. The explosion is still being investigated, and the detailed causes are not yet known. However, the initial investigation indicates that this was a dust explosion. Many people are not aware of the explosion hazard of many dusts and fine powders. Some examples of materials which may be a dust explosion hazard if present as a fine powder include almost any organic material - grain flour, sugar, plastic, corn starch, pharmaceuticals. Powdered metals such as aluminum and magnesium also present a dust explosion hazard.Do you know what conditions are necessary for a dust explosion? The conditions needed for a dust explosion can be represented as a pentagon • FUEL – The presence of a combustible

dust. Particle size is important – smaller particles are more likely to be both ignitable and dispersible.• OXIDANT – usually oxygen in the air, which is generally enough to support an explosion.• SUSPENSION – The dust needs to be dispersed into the air. The dust may

normally be dispersed in air in the process equipment. In a building, this could be done by a large leak or spill, a small initial dust explosion, or any other disturbance which could shake dust layers off of equipment or lift dust off of the floor.• IGNITION SOURCE – Energy is required to ignite the mixture. This may be something with as little energy as static

electricity or a stronger energy source such as an open flame or an electrical fault.• CONFINEMENT – For example, the walls, ceilings, floors and roof of a building create confinement. Plant equipment including process equipment, storage silos, dust collectors, and ducting also creates confinement. Sometimes an initial

explosion occurs and lifts larger amounts of dust that may have accumulated in the plant and disperses this dust into the air. This creates the conditions for a second, much larger explosion, which can be catastrophic.A small amount of dust – a layer as small as 1/32 inch (less than 1 mm) thick on exposed surfaces – can create

an explosive dust cloud once suspended. A dust layer can be considered to create a hazardous condition if it covers an area, on all surfaces, greater than 5 % of the floor area of a room. How can you tell if there is too much dust? Two guidelines which people have used are that there is too much dust accumulation (1) when you cannot tell the color of the equipment or floor beneath the layer of dust, or (2) when you write your name in the dust and very slight ridges form at the edge of the letters. Good housekeeping is a necessary safe work practice when dust hazards are concern. Other safe work practices for dust include reducing static discharge potential through the use of grounding and bonding, proper electrical area classification and equipment selection. If your plant handles potentially explosive dusts, make sure that you understand the hazards, and all of the safe work practices and safety equipment needed for safe operations.

Safety Alert

First Aid Burn Injury from Hot Condensate SprayBapco’s EHS Committee conducted their monthly meeting for

October 2014, in which the following points were discussed:• There were no Lost Time or Non-Lost Injuries; however, 2 First Aid injuries were recorded during September 2014. We recently celebrated 3 million hours without an LTI. Our next milestone is to achieve 5 million hours without an LTI by the third week of November 2014. In order to achieve this milestone we must remain focused by using all the tools available to us such as “4 Keys to Safety” and “Osool Bapco” to prevent all injuries.• There were 14 vehicle accidents during September 2014, out of which 10 involved Bapco drivers and 4 involved contractor drivers. Most accidents were minor and non-contractor / non-employee fault, but the high number is still alarming. YTD there have been 81 vehicle accidents compared to 69 in 2013. The increase in vehicle accidents is a concern; a computer based training package for defensive driving will be introduced soon.• 14 minor Process Safety Incidents out of which 4 have been PSE tiered were recorded during September 2014. YTD there have been 265 process safety related incidents out of which 3 have been classified as Tier 1 incidents. The low number of significant incidents is good and reflective in the world class RRC performance. However, we must focus on PSM to reduce total number of incidents.• The Environmental Performance Index (EPI) remains

at 6.25 versus the target of 7. The EPI is continuing to show a steady improvement. Well done to all and keep up with the efforts to achieve continual improvement in our environmental performance.• The EHSC reviewed the 3rd quarter progress of 2014 EHS Objectives and Targets. Although good progress is being made, we need to ensure that we make all efforts to achieve the targets as agreed at the beginning of the year.• A presentation on the upcoming OE audit by Chevron was made highlighting the framework for the audit. The audit is scheduled to take place towards the end of January 2015 and will last for approximately 12 days. There will be 12 auditors on site which will mean this is the most comprehensive and longest audit the Company will have undertaken. More details will be sent to all concerned nearer the time. All process owners and custodians are requested to ensure that they have all the relevant documentation readily available with full knowledge to the health of their processes prior to the audit.• The third LBAT survey is planned for November 2014 and all are encouraged to participate in this survey.• A presentation was made on the draft standard produced to ensure the safe operation and control of vacuum tankers. It was agreed that the way forward to fully implement all the requirements stipulated in this standard will be further discussed at the operations meeting.

How Well Do You Really Sleep?

Deep, refreshing sleep on a nightly basis is integral to getting you in top shape when you rise and shine in the morning. By learning how to avoid sleep deterrents, you can ensure that you wake up feeling rested and recharged. Like it is commonly said, it’s not just how long you sleep that matters, but how deep you sleep as well.1. Have a regular bedtime, and wake up at the same time every day: Go to bed at the same time every night and more importantly, try not to break this cycle on weekends or holidays when it may be tempting to stay up very late. If you want to change your bedtime, help your body adjust by making subtle changes daily. And if you’re getting enough sleep, you should wake up naturally without the need for an alarm. If you need an alarm clock to wake up on time, it most likely means that you aren’t getting the sleep your body needs.2. Turn off electronic appliances like a TV or a computer: Most of us just flop down in front of the TV or prefer to surf the net at the end of a hard day, but t h e light suppresses melatonin production and keeps your mind engaged and busy even when it’s time to sleep. Listening to

music or trying relaxation exercises before you go to sleep, as you will entertained if you need to and get the sleep that you rightly deserve.3. Try not to use a mobile phone either! If you use an electronic device such as a mobile phone or an iPad, use something which doesn’t produce strong light of its own, as this can again hinder melatonin production in your body which is a basic necessity for deep, quality sleep.4. Sleep in a dark room: It’s very simple - the darker your room is, the better you sleep. Cover electrical displays, use curtains or shades to block light from windows, or wear a sleep mask to cover your eyes completely. Also make sure your room is at a comfortable temperature and that it smells clean and fresh.5. Keep noise down: You should try your best to avoid noise from the outside world like honking cars, the late-night garbage truck or the neighbour’s barking dog. Try drowning out these sounds with a fan, or even white noise which is a favourite for many, however surprising. You may buy a special sound machine or generate your own white noise by setting your radio between stations. 6. Make sure your bed is comfortable: You need to ideally have enough room to stretch and turn comfortably. If you often wake up with a sore back or an aching neck, it may be time to re-evaluate the lumpiness of your mattress. Choose an orthopaedic mattress and sleep on your back so that you are most comfortable and can sleep well.7. Avoid big meals at night: Finish up dinner earlier in the evening, ideally around two hours before you decide to go to sleep. Even better if you keep dinner light and healthy. Fat, greasy foods make you feel uneasy and take a lot of time to digest, which can hinder your chances at a good night’s sleep.

Breakfast is the most important meal of

the day, and how! Let’s have a look at a

few reasons why you should never skip

this important meal (although many

people unfortunately do):

1. By eating

breakfast, you are

essentially “breaking your

fast”. With your first meal

after a long rest, you get

your metabolism going and

actually burn those calories

you need to burn to lose

weight while supplying

your body with all the fuel

you need to be energized

all morning.

2. If you want to

perform better at what you

do, have breakfast because

it wakes you up really well.

It has been proven over and over again

that people who eat breakfast are more

alert and do better than people who skip

breakfast. Similarly, a hungry person

tends to be disinterested, lethargic and

irritable when faced with complex tasks.

3. Breakfast foods might is your

chance to eat the foods you may not eat

the rest of the day. You can have cereal

and fruit with milk which supplies your

fibre, folic acid and calcium in one sitting.

Items like eggs, croissants and lean bacon

are delicious and offer a great start to your

day.

4. If you follow a diet,

you won’t cheat on it.

Distributing your caloric

intake throughout the day

at regular intervals will

help you regulate what

you consume and you

won’t feel the need to

over-indulge when you

finally get your hands on

some food. What’s more,

you tend to actually pick

unhealthy, only tasty foods

when you are in absolute

hunger because the last

thing you would care

about when you

are starving is to follow

a rigorous diet!

Why you should never skip breakfast...

With the trend of capturing self-portrait photographs, or “selfies” catching up rapidly over the past year, inventors have brought

something new to the market which appeals greatly to social media enthusiasts, especially the younger ones – a “selfie pod”! The “pod” is actually an extendable pole, which features a stand in which you can fit your mobile phone and snap the camera shutter with a Bluetooth controller. So the next time you see a bunch of youngsters holding something that looks like it came out of a science-fiction movie, no need to run for cover - they’re just taking a picture!
