Instructions for Submitting STEM OPT Data Validation Report

Due to OIS: Reports can be submitted to OIS up to 30 days before your reporting due date. Late submissions will be accepted, but OIS will be unable to record your submission in your SEVIS record. Missing these deadlines may affect your F-1 status.

Processing time: 7 business days after submission—incomplete and/or incorrect requests will require resubmission.

Step 1: Log into your SEVP Portal, and verify the following: Data validation report due dates in the “STEM Student Due Date Announcement” pop-up message Address, telephone, and employer information and make changes if needed

Step 2: Complete STEM OPT Extension Data Validation Report Form Select which validation report you are submitting; read and initial bottom section of form

Step 3: Email the following to OPTSTEM@USC.EDU: In email subject line write: Data Validation Report: Last Name, First Name — SEVIS ID # Attach the following documents in PDF format:

1) Completed STEM OPT Extension Data Validation Report Form

2) Only if submitting 12- or 24-month validation report — copy of Form I-983 (Pages 1-5) with signed

self-evaluation on Page 5; for more information on self-evaluations, refer to the STEM OPT
