Current research shows that individuals who have difficulties in any of these areas are at significant risk for infections and falls. This is most often noted during the

adjustment period to their new living situation.

Our goal as part of the Transitions program is to quickly identify potential problems and provide education to individuals and their family members.

We then work with individuals, family members and facility staff to develop a therapy program that is not only enjoyable but also helpful in reducing or eliminating the risk factors associated with the four areas of the Transitions program.
