Step 2: Accomplished Application Form with complete set of Signatures and Requirements

OVCA-BCO specific attachments: a. Memorandum of Agreement (MOA) b. Sanitary Permit c. Health Certificate d. Layout Plan e. Approval of Academic/Unit canteen operator

Procedure for Requests

3.1 Location on only approved UPD areas;3.2 Requests must be filed one (1) week before the scheduled activity;3.3 No vending of glass bottled beverages;3.4 Non-usage of styrofoam and plastic cups; only biodegradable materials maybe used;3.5 Only white tents are permitted with no labels to align with the

Academic Core’s character and significance;3.6 A maximum of five (5) concessionaires are allowed per activity

request;3.7 Must wear proper attire: (For Concessionaire) - Hairnets and Apron for food handlers - Closed shoes - Uniform as required - Complete Identification Card (ID)3.8 Waste Management: (For Concessionaire) - Two (2)Black Garbage Bags: Biodegradable and Non-Biodegradable

3.9 Stall size: (3meters x 3 meters)3.10 For electrical requirements – C/o OVCCA3.11 Duration: 8AM to 5PM (excluding ingress/egress)3.12 Violations
