OVERVIEW

This document contains sample demographic questions, along with response options. Demographic data may be collected through responses to survey questions, or uploaded as part of a participant file that is linked to survey responses. When demographic information is provided through a participant file and linked to survey responses, participants must be informed of this approach.

For more general information on the effective use of demographic questions on surveys, first consult Tips and Guidelines to Create an Effective Survey.

While sample demographic questions are included in this document, every organization is unique. Use these as a starting point but customize to fit your organization and the survey's purpose. Some common areas for customization are [between brackets].

What’s Typical

Most organizations include questions related to employees’ employment status and workgroup (e.g., location, job level, organizational tenure, division). In addition, some organizations also choose to include demographics of a more personal nature (gender, race, age, disability status, sexual orientation). However, these types of questions are typically avoided outside of the United States and are even illegal in some countries. It is strongly encouraged that you consult with your organization's legal department before including these questions in surveys that will involve any employees outside of the United States.
