PROGRAM PORTFOLIO
The Agenda
Health is seen in isolation whereas by definition, it is an outcome determined by factors that are
outside health sector. We all influence these determinant factors and therefore we all have
stakes in the health sector development in the province and in the country. ‘No one can do it
alone’
Health should be the first and foremost priority of any growing nation. The ongoing paradigm
shifts of global competitiveness, environmental changes, health regulations, patient-provider
interaction, health human resource training and development needs and more over rapid
medical and technological innovations are transforming health management organizations.
Health care industry requires change managers and leaders who can be ready to adapt and
envision the change.
Being aligned with international and national health governing bodies, the Department of Health
and Hospital Management is keen to support the Sustainable Development Goals as proposed by
the United Nations. The Sustainable Development Goals (SDGs) are a proposed set of targets
relating to future international development. They have replaced the Millennium Development
Goals of 2015. The SDGs were first formally discussed at the United Nations Conference on
Sustainable Development held in Rio de Janeiro in June 2012 (Rio+20). On 19 July 2014, the UN
General Assembly's Open Working Group on Sustainable Development Goals (OWG) forwarded a
proposal for the SDGs to the Assembly. The proposal contained 17 goals with 169 targets
covering a broad range of sustainable development issues. These included ending poverty and
hunger, improving health and education, making cities more sustainable, combating climate
change, and protecting oceans and forests. Pakistan, being a signatory of the charter, affirms to
achieve these goals and targets.
Healthcare
systems in
Sindh, Pakistan
Health systems in Sindh faces many challenges that are generally cross-cutting in nature
irrespective of socioeconomic and health development. The health-poverty-illiteracy nexus
coupled with gender inequity and regional disparities results in uneven health coverage. The
distant and disadvantaged become more affected in terms of affordability and accessibility as the
advanced care services are largely concentrated to southern part of province
Over the years, public funded health services come under great stress due to low budgetary
allocations and increasing demand placed on it by an ever growing population at the pace 3.2%,
in case of Sindh, including in-migration from across the country and almost 4.00 million Afghan
Refugees.
Page | 8
KEY CHALLENGES:
Sindh has lowest donor investment in health <1%
Low investment in consumer awareness (Health Education/ Advocacy)
Lack of cohesive mechanisms involving public and private sector
Integration of Vertical Programs
Equipment Availability 31-60%
Drugs availability <61%
Chronic shortage of female staff, nurses, paramedics, pharmacists and specialists.
Only 4-5% of total government budget spent on health versus 8% in other provinces and 10-12% in regional countries.
Total Public Sector Budgets for Health in Sindh; Need VS Actual (PKR 81 Billion : 42 Billion per
annum)
(Sources: PDHS 2012-13; PLSM & Health Sector Strategy 2012-20)
Post devolution/ Constitutional (18th Amendment) Act 2010 situation has offered a policy and
administrative space to the province of Sindh to adjust its roles and responsibilities to the
context of the local needs.
This situation strongly warrants developing robust, innovative, cost-effective and integrated
quality healthcare delivery system for improved health outcomes. The Health Department,
Government of Sindh based on Health Sector Strategy (HSS) 2012-20 has developed the
Operational Plan HSS Phase I (2014-2017), which incorporates the vision, goal, objectives and
lays down the way forward for overcoming priority issues. The roadmap in the shape of the
evidence based Operational Plan with the performance matrix captures following areas of
importance in Phase I of implementation.
