❖ Application without the Demand Draft will not be accepted. The name of thecandidate and the name of the course (Paramedical Course) should be writtenon the reverse of the Demand Draft. SC/ SCA and ST candidates who are nativesof Tamil Nadu are exempted from paying the cost of the Application Form.

❖ The template with requisite details available for download should be printed and pastedon the cover.

❖ AR Number will be assigned by the Selection Committee.

❖ Candidates can track their applications on the above websites by entering either theirApplication number or the Higher Secondary Course / Equivalent Examination RegisterNumber of first appearance with the year.

❖ Random number generated by computer will be assigned to each candidate.

❖ Eligible candidates who have not received their call letter for counselling can downloadtheir call letter from the websites www.tnhealth.org & www.tn.gov.in by entering theirAR No allotted by the Selection Committee and their Date of Birth. They are instructedto attend the counselling as per the date and time mentioned in the counsellingschedule which will also be available on the websites. Candidates are requested topaste their photograph on the call letter which is to be attested by a Gazetted Officer.(The photograph must be a copy of the same one pasted on the application).

GENERAL INSTRUCTIONS:-1. (a) Candidates seeking admission to Paramedical courses should submit the

common application form with necessary enclosures which should reach:-The Secretary,Selection Committee,No.162, E.V.R Periyar High Road,Kilpauk, Chennai – 10.

on or before 05-08-2016 upto 5.00 p.m.(b) The candidates shall ensure that the completed application form with all particulars

and enclosures reaches the Selection Committee within the date and timespecified. Applications received by Speed post / courier or any other meansafter the last date & time will not be accepted irrespective of the date of booking.

(c) The Selection Committee shall not be responsible for any delay in the receiptor for the loss in transit of application form etc.,

(d) Request for extension of time for submission of documents called for, after thespecified date and time will not be entertained.

(e) Any supporting documents received after the due date of application withoutrequired documents / particulars will not be entertained.

(f) Candidates seeking admission under Special Category for the paramedicalcourses (Orthopaedically Physically disabled & Children of Ex-Servicemen) shall submit the separate Special Category Form along with themain application in the prescribed format available along with an additional feeof 100 /- by way of a Demand Draft for each special category enclosing therelevant certificates. If the certificates are not enclosed, the candidates will notbe considered under that category without any intimation to the candidate.

(g) All the Demand draft should be drawn in favour of “The Secretary, SelectionCommittee, Kilpauk, Chennai - 600 010” payable at Chennai.

(h) Candidates selected under General Category will not be allowed to claim thevery same status as the candidates selected under Special Category.

(i) Mutual Transfer is not allowed.(j) Photocopy/ Fax copy of application will not be accepted.(k) Both upper limbs, vision and hearing should be normal for all candidates including

candidates applying under Orthopaedically physically disabled category.II. ELIGIBILITY:-2. Candidate should be a citizen of India.3. NATIVITY:-

i) Candidate should be a Native of Tamil Naduii) Candidates who have studied VIII to XII standard in schools of Tamil Nadu need

not enclose the “Nativity Certificate”.iii) Candidates who are native of Tamil Nadu but studied from VIII to XII Std either

partly or completely in schools situated outside Tamil Nadu should producethe “Nativity Certificate”.

iv) The Certificate of Nativity in Tamil Nadu should be issued by the competentauthority only in the specified format available in the application form andsupported by a copy of Passport/ Ration Card / Birth Certificate.If thecopy of the supporting document is not produced, then the application form willbe rejected. Nativity Certificate submitted later will be summarilyrejected.

v) Applications without proper Nativity Certificate will not be considered.

vi) Permanent Residence Certificate will not be accepted.vii) Candidates who are native of Tamil Nadu alone will be considered for communal

reservationsNote: Nativity Certificate should be produced as per “Instructions for Nativity

Certificate” in Annexure XIV(a). Producing false nativity certificate if detectedat any time later during the course of study will result in expulsion of thecandidate and also initiation of criminal proceedings against the candidateas well as the Parents as per law.

4. AGE LIMIT - FOR B.Sc. (NURSING) COURSE:-Candidates should have completed 17 years of age and maximum age is 30 years ason 31st December 2016. For SC / SCA / ST candidates, maximum age is raised to 35years. Evidence should be produced.

FOR ALL OTHER COURSES:-Should have completed the age of Seventeen years on or before 31st December2016. Evidence should be produced

5. EDUCATIONAL QUALIFICATIONS: -i) Candidates should have passed in all the subjects of the qualifying Examination of

the Higher Secondary Course conducted by the Tamil Nadu State Board or any otherequivalent Board in a single sitting in the following group of subjects with minimumeligible marks.(a) Physics, Chemistry, Botany and Zoology. (or)(b) Physics, Chemistry, Biology or Mathematics with any other subject.

A failure in examination/ reappearance/ first appearance in compartmentin one or more subjects will be treated as an attempt.

6. MINIMUM ELIGIBILITY MARKS: -i. B.Pharm. /BASLP Courses:

For OC / BC / BCM / MBC / DNC Candidates : -Pass in Higher Secondary Course with Physics, Chemistry, Biology or Botanyand Zoology or Mathematics not less than 40 % aggregate marks.For SC / SCA / ST Candidates :-Pass in Higher Secondary Course with Physics, Chemistry, Biology or Botanyand Zoology (or) Mathematics.

ii). B.Sc. Nursing Course:For OC / BC / BCM / MBC / DNC Candidates :-Pass in Higher Secondary Course with Physics, Chemistry, Biology or Botanyand Zoology with not less than 45% aggregate marks.For SC / SCA / ST Candidates :-Pass in Higher Secondary Course with Physics, Chemistry, Biology or Botanyand Zoology with not less than 40 % aggregate marks.

(iii) B.Sc. Optometry Course:Pass in Higher Secondary Course with Physics, Chemistry, Biology or Botanyand Zoology or Mathematics.

(iv) B.P.T /B.Sc.Radiology & Imaging Technology / B.Sc. Radio –TherapyTechnology / B.Sc. Cardio Pulmonary Perfusion Technology / B.O.TAll Categories:Pass in Higher Secondary Course with Physics, Chemistry, Biology or Botanyand Zoology.

NOTE: - (i) Only the marks obtained in the science subject in the Higher SecondaryCourse qualifying examination as a whole in the first appearance willalone be taken into consideration for the allotment of seats to thecandidates as per G.O. Ms No. 184/HE (J2)/Dept, dated:09/06/2005 andG.O.(Rt)No.143/HE(J2)/Dept dated 06.05.2008.

(ii) Candidates who have passed any other equivalent examinations in the firstattempt with same group of subjects mentioned above should produce theeligibility certificate from the Tamil Nadu Dr. MGR Medical University beforeadmission to the course.

(iii) In the case of candidates who have furnished the mark sheets of the qualifyingexamination, only in GRADES, the minimum marks of the grades alone will betaken into consideration while determining the marks in the select Sciencesubjects of Physics, Chemistry and Biology or Botany and Zoology.

(iv) Candidates who have submitted mark sheets with GRADES should obtain thestatement of marks indicating the actual marks secured from the examinationauthority and attach a certified copy as an enclosure to the application.

(v) Normalization method will be adopted for calculating the minimum eligibility markfor the candidates who have passed any other equivalent examination.

7. i. Candidates who have passed Higher Secondary Course in vocational stream /open university and private candidates are not eligible to apply to Para MedicalCourses.

ii. Married candidates are also eligible for admission to B.Sc.(Nursing) Course.All the selected candidates should stay compulsorily in the Hostel during theentire period of study.

iii. The ratio between men and women shall be 1 : 9 respectively in the selection ofcandidates.

8. Normalization of Marks:-i) As per the Tamil Nadu Act No 3 of 2007, dt 05.03.2007 and G.O(Ms) No.77

HE(J2) Dept.dt:05.04.2007, the marks obtained by the students in the relevantsubjects in the qualifying examination conducted by various Boards or Authorityshall be equated with the marks obtained by the students in the same subjectsin the qualifying examination conducted by the State Board (Government ofTamil Nadu) by adopting the method of normalization.

Explanation:-Under the method of normalization, the highest mark obtained by the studentsof various Boards in each subject will be equated to the highest mark obtainedby the students of State Board in that subject and the relative marks obtained byother students in that subject will be determined accordingly.

Illustration:-i. If the highest marks secured by the student of State Board in Physics is 100 and

the highest mark secured by a student of any other Board in the same subject is90, both the highest marks will be considered to be equal to 100. If a student ofthe other Board secures 60 marks in Physics when the first mark in Physics inthe same Board is 90, then 60 marks will be considered to be equal to 66.66marks as arrived at below:-

100 x 60————— = 66.66%

90(ii) After normalization of marks in the relevant subjects in the qualifying examination

conducted by different Boards, the qualified students of different Boards will bemerged into a common merit list.In case of candidates who have qualified from National Boards such as CBSEand ISCE, the highest marks obtained by the candidates at the National Levelalone will be taken into consideration for normalization.

III. 9. PROCEDURE FOR FILLING AND SUBMISSION OF APPLICATION : -Before filling up the application form and OMR sheet/Scrutiny form, read all theinstructions carefully.Each application form bears a unique number which is printed on the Applicationform and the OMR. Candidates are requested to check the number andensure that the same number is printed on the Application form, OMR,Outer and Inner Covers. In case any discrepancy is noted, it should beimmediately brought to the notice of the selection committee before the lastdate for the receipt of application. However the Serial Number of the OMR willbe considered as the final number for correspondence purposes.

10. a). Use one common application form for all Paramedical Courses. No candidateshould submit more than one application.

b). If a candidate applies for Paramedical Courses in forms other than the oneprescribed for the academic year i.e., 2016-2017 session, the application will besummarily rejected.

11. Fill up the application form and OMR sheets legibly, neatly and correctly. Theinformation will be processed by using a computer. Any mistake committed by thecandidate will prejudice the selection.

12. Do not fold OMR sheets under any circumstance. Making a hole, using a pin or astapler on the OMR sheets should be avoided. Cutting, overwriting and erasing in theApplication form and in the OMR Sheet or Scrutiny form (for Downloaded applications)should be strictly avoided.

13. Application with incomplete or insufficient particulars or without enclosures or thosereceived after the last date mentioned will be summarily rejected without any intimationto the candidate.

14. Filled up application without the signature of the candidate will be rejected.15. The Government Orders issued and to be issued from time to time pertaining to any of

the matters contained in the prospectus should be read as part and parcel of thisprospectus and such terms and conditions in the Government Order are deemed tohave been incorporated in this prospectus.

16. i) Candidates should affix recent passport size photographs taken with nameand date (as shown in the Instructions-Annexure XII) in the places providedin the application form and the OMR.The photograph pasted in the commonapplication form alone should be duly attested by Group A or Group B officer ofthe Government of Tamil Nadu or Central Government or the Head of theInstitution last studied.

ii) The photograph pasted in the OMR need not be attested.iii) The Registration Roll number allotted by the Directorate of Government

Examination/concerned Boards should be indicated along with the year of HigherSecondary Course examination / equivalent in the application.

17. All the entries in the application form and OMR sheets should be written legibly by thecandidate in his/her own handwriting. The candidate should ensure that all informationincluding the marks in the qualifying examination furnished by him/her in the applicationand the enclosures are correct. The candidate is informed that if after proper scrutinyof his/her application, the marks furnished by him/her in the application are not corrector any other information is found to be incorrect, then;a. He/She will forfeit the admission no matter at what stage of the course, he / she

will be at that time.b. He/She will be debarred from pursuing any other course for a period of three

years;c. Legal action will be initiated against the candidate and the parents/guardian for

furnishing incorrect information;d. Criminal action will be initiated against him/her for furnishing false Mark Sheets/

false Community Certificate/ false nativity certificate

18. No document enclosed with the application will be returned. Candidates are instructedto enclose only the photocopies (not originals) of the following certificates in theorder indicated below along with the application.(a) Mark sheet of Higher Secondary Course (Academic) examination (both sides of

mark sheet should be photocopied) or any other equivalent examination of thefirst appearance.

(b) Transfer certificate obtained after the completion of Higher Secondary Course(Academic) or equivalent courses.

(c) Nativity certificate (if applicable )(d) Permanent Community Certificate Card (if applicable)(e) X standard Mark Sheet .(f) Certificate for proof of study from VIII standard to XII standard.(g) First graduate Certificate (if applicable). Certificate submitted along with

application alone will be considered. If submitted later to the collegeauthorities, it will be summarily rejected (Annexure XV-a&b).

(h) Special category form with Demand Draft (s) and relevant certificates(ifapplicable).

Note: i) The candidates belonging to Scheduled Tribe should produce CommunityCertificate obtained from a Competent Authority not below the rank of a RevenueDivisional Officer. If he/she claims ST status in common application withoutproducing any evidence or claiming that ST status is under process of scrutinywill be treated as open category candidate.

ii) Producing a false community, Nativity or any other certificate if detected later atany point of time during the course of study will result in expulsion of the candidateand also criminal proceedings will be initiated against the candidate as well asthe parents as per provision of law.

iii) As number of bogus community certificates are being encountered off late andin order to curb such practices, the authentication of the above certificate has tobe verified and confirmed by the concerned authority, before the individualcompletes 1st year Para Medical course. The concerned candidate and parentsshould take necessary steps to that effect. Unless proof of authenticity ofcommunity certificate is received, the candidate will not be allowed to continuethe 2nd year.

(iv) Community Certificates obtained from other States will not be considered andsuch candidates will be treated as open category.

IV. METHOD OF SELECTION AND ADMISSION:-19 (i) Ranking will be made on merit on the basis of marks obtained by the eligible

candidates in the prescribed science subjects in the qualifying examinationsreduced to the base of 200 marks. The marks obtained through differentBoards will be taken into account after normalization

ii) Ranking of the candidates will be done based on the total marks obtained in theScience Subjects after normalization of marks computed to a maximum of 200.

iii) Only the marks obtained in the Science subjects in the Higher SecondaryCourse qualifying examination as a whole in the first appearance willalone be taken into consideration for the allotment of seats to thecandidates and the improvement marks in Higher Secondary Courseexamination of any year will not be considered. Also the marks obtainedthrough part by part appearance in Higher Secondary Courseexamination will not be considered including for natives of Tamil Naduwho have passed any other equivalent examination from other Boards/States.

iv) a) If claiming of improvement marks for selection to Para Medical Courses isdetected at any point of time,it will lead to cancellation of selection andremoval of his/her name from rank list.

b) Any application submitted without the latest attested photo copy of thefirst appearance mark sheet of the qualifying examination will summarilybe rejected.

Note:- In respect of the candidates who have taken the subjects of Physics, Chemistry,Biology and Mathematics, the marks obtained in Physics, Chemistry and Biologywill be taken into account for arriving the General merit list and for thosecandidates who have taken Physics,Chemistry, Mathematics along with othersubjects (other than Biology, Botany and Zoology) the marks obtained in Physics,Chemistry and Mathematics will be taken into account for arriving the merit listfor B. Pharmacy and BASLP courses only.

20. Determination of Rank of Candidates obtaining equal marks in all paramedicalcourses (Inter-se Merit)In cases where two or more candidates obtain equal aggregate for 200, the rank ofsuch candidates shall be determined on the basis of the following order namely:-a) Percentage in Biology or Botany and Zoology taken together in Higher Secondary

Course Examination.b) Percentage in Chemistry in Higher Secondary Course examination.c) Percentage in Fourth Optional subject in Higher Secondary Course Examination.d) Date of Birth – Weightage given to seniority in age.

Computerised Random number assigned. Higher value of random number willbe taken into consideration.

V. MERIT LIST21. The Secretary, Selection Committee, will publish the tentative merit list in the website:

www.tnhealth.org / www.tn.gov.in22. The candidates will be called for counselling as per their merit and they may exercise

their option as per their eligibility norms mentioned in the Prospectus.VI. COUNSELLING PROCEDURE- SINGLE WINDOW SYSTEM:23. The selection will be made on merit basis and by Counselling applying the rule of

reservation by the Government of Tamil Nadu as follows:-Open Competition – 31%Backward Class – 30%Most Back ward Class – 20%Scheduled Caste – 18%Scheduled Tribe – 1%Within 30 % reservation for BC, 3.5% reservation will be provided for BC Muslims and16 % of seats out of 18 % quota earmarked to Scheduled Caste shall be allocated tothe Arunthathiyar Community.BC, BCM, MBC / DNC, SC, SCA & ST candidates are eligible for Selection underOpen Competition as per merit in addition to the reservation made for them.

a) Any remaining unfilled seats for want of adequate number of eligible candidatesbelonging to SCA community shall be filled by candidates belonging to SCcommunity other than Arunthathiyar as per merit and vice versa. (G.O.(Ms).No.55P & AR Department dated 08.04.2010)

b) Any remaining reserved seats in ST Quota for want of adequate number ofeligible candidates belonging to ST community shall be allotted to candidatesbelonging to SC Community as per merit. (G.O.(Ms).No.77 Health & FamilyWelfare Department (MCA1) dated 24.02.2011)

c) Any remaining unfilled seats in BCM Community for want of adequate number ofeligible candidates after exhausting the merit list shall be filled up on merit basisby candidates belonging to BC community without exceeding the quota for BCcommunity as a whole(G.O.Ms.No.30/H&FW Dept(MCA1)dated 18.01.2012)

d) Even after filling up of the required seats reserved for Arunthathiyars onpreferential basis, if more number of qualified Arunthathiyars are available, suchexcess number of candidates of Arunthathiyars shall be entitled to compete withthe Scheduled Castes other than Arunthathiyars in the inter-se merit amongthem (As per G.O.Ms.No.65 dated 27.07.2009 of P &AR(K) Department).

24. Counselling Schedule will be available only on the website. Candidates who have notreceived the call letter in time can also attend the counselling session as per scheduleand as per their merit with call letters downloaded from the website.

25 (a) Counselling for the Special Categories will be done before the General category(b) There will be no reallotment / change of college allotted for the Special

Category.26 (i) Option once exercised during counselling will be final and cannot be

changed.(ii) (a) Candidates who fail to attend the counselling at the specified date and

time will forfeit their right to admission and their name will not beconsidered later.

(b) Due to unforeseen reasons, if a candidate is not able to attend thecounselling on the specified date and time, he/she can attend on asubsequent time/date during that counselling phase and allowed to takethe course and college available at the time of counselling when he/sheactually attends.

(or)He / She can authorize his / her parent on his / her behalf and the parent’sdecision will be binding on the candidate. The authorization letter must bein the prescribed format in the Annexure XIII.

(c) If the candidate is absent during the 1st phase of counselling, forwhich he/ she is called, cannot attend the subsequent phase ofCounselling.

27. One of the Parents/ Guardian will be allowed to attend the counselling. They shouldmake their own arrangements for attending the counselling session. Individualintimation will be sent for regular counselling through call letter.

28. a. All communication will be sent only to the mailing address given in the applicationand self addressed envelopes (Size 23 x 10 cms) Candidates are requested tointimate any change of address immediately. The intimation / call letters will besent either by Registered Post, Speed Post, Courier Service or by e-postwhichever is feasible. Selection Committee is not responsible for any postaldelay or non receipt of call letters.

b. Any change or modification and relevant information pertaining to this admissionprocess will be made available immediately on the websites. (www.tnhealth.org /www.tn.gov.in.) The candidates are instructed frequently to visit the website fromthe date of application till the end of counselling without fail. Please do not wait

for counseling information to be received by post, as all information includingthe date of counselling will be displayed only on the websites. Call letters canbe downloaded from the websites. Candidates will be permitted forcounselling accordingly

c. All candidates attending the counselling for Para Medical DegreeCourses will have to remit a non-refundable amount of Rs.250/- by meansof Demand Draft drawn in favour of “The Secretary, Selection Committee,Kilpauk, Chennai - 600 010” as processing fee.

29. Those who get Retotalled / Revalued marks can also attend the counselling on theday and session according to their new marks. If the candidate produces the Retotalled/ Revaluation marks after the counselling is over and within the cut off date, then his/her name, will be considered in the existing vacancies available and he/she cannotclaim any preferential rights for allotment according to merit under Retotalling /Revaluedmarks. Candidates who submit the Revaluation / Retotalling marks after theadministrative cut-off dates fixed by the Government will not be considered for selection.

30. The vacancies arising after the first phase of counselling and also to the new collegesif any added will be filled up from the waiting list through counselling, if necessary.

31. The movement of candidate from one category to another category (from self financingto Government colleges ) and from one college to another college (only for Governmentcolleges) will be based on the merit following the rule of reservation. The date ofsuch counselling will be published in the website and also will be informed to the Deansof respective Government Colleges. No individual intimation will be sent to thecandidates.

32. If a candidate chooses a Government Quota seat in Self-Financing College categorywhen seats are available in Government colleges, he/she has to give a declaration tothe effect that he/she will not be permitted to claim the Govt. seats later during re-allotment / subsequent phase of counseling

33. Candidates can exercise their option for the course and the college of their ownchoice at the time of counselling and allotment will be subject to availability ofseats. Individual request for transfer of college or change of course willnot be entertained after counselling is over.

34. Mutual transfer will not be permitted.VII. CATEGORIES OF SEATS:35. Choice of Candidates : -

a. Candidates can exercise their option for allotment of seats subject to availabilityin any of the categories. If no seats are available in the particular category, he/she can be waitlisted for one course only.

The candidates are instructed to produce a certificate from the District MedicalBoard of the area concerned constituted for this purpose assessing the natureand the extent of physical disability in the format prescribed in the Annexure-III A& B.The Medical Certificate issued by the District Medical Board of the area concernedfor this purpose is subject to the confirmation of the Expert Committee constitutedby the Director of Medical Education at the time of counselling.The Candidates seeking admission under this category should produce a fullsize photograph exhibiting the deformity.The Certificate must have been obtained WITHIN THREE MONTHS priorto submitting the application for seeking admission under this category.If the Certificate has been obtained earlier, the application will berejected.

The selection and admission of candidates of other Universities / Boards will beprovisional subject to approval of the qualifying examination by the Tamil Nadu Dr.M.G.R.Medical University under which the candidate gets admitted. In case, the recognition isnot accorded for the admission of the candidate on any ground, the provisionaladmission will not entitle the candidate for the continuance of the course and theadmission will be cancelled.

41. The Selected Candidates should produce all the original documents at the time ofadmission to the Deans/Principals of respective Colleges concerned. Selection will becancelled if the candidates do not produce the original documents at the time ofadmission.

42. The candidates will be required to undergo Medical Examination at the College to whichthey are allotted before the Medical Board on the date notified by the authorities of theCollege. Only those candidates found medically fit will be admitted and be permitted topay the fee.

43. Candidates selected for admission and his / her parent / guardian will have to signdiscipline declaration form in Annexure – IX, X and XI. Failure to sign the declarationwill result in the cancellation of selection.

44 (a) As per the Tamil Nadu Dr.MGR Medical University regulations, all the ParamedicalCourses except B.Sc. Nursing Course, no admission shall be made after 30thSeptember 2016 for the academic year.

(b) As per Indian Nursing Council, the last date for admission to B.Sc.Nursing Courseis 31st October 2016

45. Candidates can type the necessary certificates in the prescribed format separatelyand are requested to attach the same along with the application.

46. All selection and admission for Para Medical Courses are only provisional. The Selectionand admission is liable to be cancelled at any time as and when the eligibility conditionsare found to be defective or incorrect or there has been any suppression ormisrepresentation of facts. The Selection Committee reserves the right to cancel theallotment of Para Medical courses and Colleges.

47. Candidate selected for admission to Para Medical Courses are not guaranteed anyappointment in Government Service.

48. COMMUNICATION :-a) All intimation for communication to the candidates will be sent only to the mailing

address given by the candidate as in the application. The intimation / Call Letterswill either be sent by Registered Post or Speed Post or Courier Service or e.Postor they can be downloaded from the Websites www.tnhealth.org & www.tn.gov.inas per instructions on Page 2 of this Prospectus whichever is feasible dependingon the availability of time before counselling. Selection Committee will not beheld responsible for Postal Delay and non viewing of websites.

b) Candidates should inform the change of address immediately to the SelectionCommittee. If the change of address is not intimated to the Selection Committeein time, the Selection Committee will not be responsible for delay incommunication or its consequences.

IX 49. Hepatitis – B Vaccination :-All candidates selected for admission to first year para medical course should take theHepatitis – B vaccine before admission and produce a certificate as in Annexure –VI.

X. Ragging :-50. The candidates who join Paramedical courses should not indulge or participate in any

kind of ragging throughout their study period. If they are found to have indulged inragging at any time, the candidate will be removed from the roll of the Institution atwhatever stage of study and criminal action will be taken against the candidate.

51. Candidates applied for Paramedical courses are deemed to have read the contents inthe prospectus and agreed with all the conditions / clauses and will not have the rightto challenge any of the clauses contained in the prospectus.

ANNEXURE XIV (a)INSTRUCTIONS FOR NATIVITY CERTIFICATE

1. This Certificate should be issued by an officer of the Revenue Department notbelow the rank of Tahsildar in the Taluk concerned.

2. This Certificate should not be issued by the Special Tahsildars, Deputy Tahsildars such asLoans, Land Acquisition, Election, Excise and HQs, Deputy Tahsildars, Special Deputy Collectors,Assistant Commissioner of Agricultural Income, Excise, Elections etc.

3. The Certificate should bear the stamp of the office of the Officer signing the Certificate.4. Any foreign national irrespective of the period of study in the State will not become

eligible to apply for this course. No certificate of Nativity in the State of Tamil Nadushould therefore be issued to Nationals of other countries for this purpose.

5. The certifying officers should insist upon clear proof of the Nativity of the Parent orGuardian of the candidate and satisfy themselves on the genuineness of the residentialqualification. They should be particularly careful in the case of candidates whose mothertongue is not Tamil and whose place of birth is other than Tamil Nadu and who havestudied VIII to XII standards outside Tamil Nadu.

6. Residence Certificate will not be considered as Nativity Certificate.7. The guidelines prescribed for the issue of Nativity Certificate are as per Letter

1. APPLICATION CONSIST OF TWO PARTS: -Part 1 : Application FormPart 2 : OMR SheetThe Application Form contains 24 items. Sub group of the items of the application is reproducedin the OMR Sheet.

2. METHODS OF ENTERING PARTICULARS IN THE APPLICATION: -Particulars required by an item are to be entered in the Application in long hand or in codedform depending on the item. “Address for Communication “ for example, is written in longhand. For certain items, like “ Medium of Instruction “, several alternative responses are listedin tabular form. Each alternative is assigned a code. You must encircle the code which willsignify your response to that item. In the case of certain items, like Native District, yourespond by writing a code in the box or boxes provided.
