The Sally Ride Science Junior Academy offers fascinating and fun learning experiences in science, technology, engineering, arts and math (STEAM) designed for students entering grades 4 through 12 during the 2020-2021 school year. During these classes and workshops, students assume the roles of space explorer, marine biologist or computer engineer as they immerse themselves in hands-on projects.

Top-notch STEAM instructors lead these classes and workshops, serving as both teachers and role models. The classes and workshops also incorporate real-life stories of vibrant women conducting research in each field. Their examples inspire students and help make STEAM careers more accessible.

PARKING

Parents are required to sign their students in the first day of each week. Students can go straight to class all other days. Parents are also required to sign their students out at the end of each day, unless students are granted self check-out as indicated by the parent on the authorization form. Please allow ample time for parking so your student can get to the academy on time. Free parking is in the structure and on the street. Please follow all designated parking rules, as violators may be towed.

ABSENCES

If your student is not going to attend their scheduled workshop for the day, please report this absence by calling the SRS Junior Academy office at (858) 534-0804 or emailing [email protected]. If a student is absent on the first day and this absence is not reported to SRS, they will be dropped from their workshop(s) and no refund will be provided. You may call the office at any time during academy hours (8am-5pm) in case of emergency or other concerns.

DROP-OFF

If you and your student did not attend one of the pre-registration days, on the first day of your student’s workshop, you and your student must sign in at the front desk located at the University City Center (6256 Greenwich Drive, San Diego, CA 92122). If you and your student did attend one of the pre-registration days, your student can go straight to class. If a student is driving themselves, they are not permitted to leave campus during the duration of class or lunch. Please note that your student will not be supervised prior to the start of their workshop unless they are signed up for early drop-off.

PICKUP

All sign-outs will be in the front office (located in the lobby of the University City Center building). Please be on time for pickup unless you have registered your student for late pickup. Please note that your student will not be supervised past the time of their workshop if they are not registered for late pickup. Students who are participating in late pickup must be picked up by 5pm. You must sign your student out each day, unless you consent to your student checking themselves out as noted on the authorization form. If your student has consent to sign themselves out each day, they must report to the front office to sign out before leaving. One instance of excessively late pickup (more than 10 minutes) will be permitted. Additional late pickups may result in your student being dropped from the program. Please bring a photo ID with you to show the office staff when checking your student out.

LUNCH

Students participating in both morning and afternoon sessions should bring a non-perishable lunch and beverage each day. There will be a 60-minute supervised lunch break from 12-1pm for students participating in both AM and PM workshops. You will need to enroll your student in the 60-minute lunch break. There is no fee for this.

Please do not send food or drinks in glass containers. Lunches will not be refrigerated; therefore, we ask that you send lunches that do not contain mayonnaise or other food items that will spoil if not kept cold. Freezing lunches and juice the day before and/or inserting a cold pack in the lunch container will help preserve the food. Please mark all lunches (and all containers inside lunches) with the student’s first and last name and the date. Please also add your student’s name to their water bottles. There will be drinking fountains and a location where students can refill their water bottles as needed throughout the day’s activities.

JUNIOR ACADEMY SWAG

Each student will receive a Sally Ride Science T-shirt, nametag and lanyard. Students are required to wear their lanyards with nametags each day. Students are encouraged to wear their T-shirts on the first day of the workshop (Monday) and the last day of the workshop (Friday).

DRESS CODE

Students are expected to exercise sound judgment, safety, good taste and modesty regarding their dress and grooming.

• Stomach must be covered and shirts must touch the tops of pants

• Undergarments may not be exposed• No strapless shirts, completely bare

shoulders or sheer clothing• No sagging pants• Bottom of a student’s shorts/skirt must

reach or extend beyond the fingertips when the student’s arms are at their side

• Wallet chains are not permitted• Footwear (sandals or shoes) must be

worn at all times• Sleepwear, blankets and pajamas are not

permitted• Hats and hoods are not permitted to be

worn indoors

ACCIDENTS AND EMERGENCIES

All precautions will be taken to prevent serious health risks to students. In the event that a minor injury occurs, the trained staff will administer first aid on-site and record any concerns. In the event of a medical emergency, the trained staff will call 9-1-1 and the student will be taken to the nearest hospital for any necessary treatment.

EARLY REGISTRATION DAY

Junior Academy now offers one day of early registration for participants. This will provide you and your student the opportunity to complete all required forms and pick up materials prior to the first day of classes and workshops. We encourage you to attend the early registration day, but if you are unable, registration will be available the first day of Junior Academy.

CODE OF CONDUCT

Good behavior is encouraged in a positive manner. The staff will work cooperatively with parents and guardians, keeping them informed of behavior concerns and methods used to teach and guide students toward socially acceptable behavior, which include:

• Strive to do the best work possible.• Tolerate and respect individuals of different races, cultures, religions, genders,

sexual orientations, disabilities and national origins.• Behave in a friendly, cooperative and responsible manner toward all persons in the program

and extended programs and in the larger local communities.

If a behavior incident does occur, the following protocol will be followed:

1. A Junior Academy staff member and the student will discuss the student’s behavior and collaboratively work on a positive plan moving forward. The staff member will document the incident, conversation and forward plan.

2. A Junior Academy staff member and the student will discuss the student’s behavior and contact the parent or guardian immediately. The staff member will inform the parent of the prior occurrence and if there is a third incident, the student will be removed from Junior Academy and a refund will not be provided.

3. A Junior Academy staff member and the student will discuss the student’s behavior and contact the parent or guardian immediately. The parent will be notified they need to pick up their student immediately and that the student will be dropped from the workshop schedule without a refund.

HARASSMENT POLICY

Sally Ride Science is committed to providing a positive learning environment. Furthermore, all students and staff of the Sally Ride Science Junior Academy program have the right to an environment free of harassment or violence of any type. Because of this, the following will not be tolerated:

• Sexual harassment includes, but is not limited to, unwelcome sexual advances, requests for sexual favors, or other verbal, visual, written or physical conduct of a sexual nature, which create an intimidating, hostile or offensive educational/social environment for students, faculty or staff. Sexual harassment is demeaning and degrading. It affects an individual’s self-esteem and can have a negative impact on performance. It can make an individual feel angry, powerless and fearful. If an incident occurs, a victim should inform a Sally Ride Science instructor or staff member immediately.

• Bullying harassment includes, but is not limited to, any kind of comment, threat or action that creates an intimidating or hostile atmosphere. Every student has the right to pursue an education without being concerned about physical, verbal or emotional abuse.

• Physical harassment includes, but is not limited to, any kind of physical violence, intimidation, threat or harassment directed toward any Sally Ride Science participant, instructor, staff member or any other individual. Any allegation of harassment at UC San Diego or at any Sally Ride Science related activity during the program will be investigated, and disciplinary action will be taken if allegations prove to be factual.

In order for your student to be eligible for participation in a chosen workshop(s), you are required to complete the digital authorization form in advance of the first day of their workshop. Failure to do so will result in your student being dropped from their workshop(s). The authorization form will be sent to you via email once your student is registered for a workshop. Please follow the link and complete the form within 48 hours. Failure to do so may result in your student being dropped from the program with no refund.

WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT

Waiver: In return for being permitted to participate in the 2020 Sally Ride Science Junior Academy, scheduled from June 29 to July 24, 2020, including any associated use of the premises, facilities, staff, equipment, transportation, and services of the university, I, for myself, heirs, personal representatives, and assigns, do hereby release, waive, discharge and promise not to sue The Regents of the University of California, its directors, officers, employees, and agents (“The University”), from liability from any and all claims, including the negligence of the university, resulting in personal injury (including death), accidents or illnesses, and property loss, in connection with my participation in the Activity and any use of University premises and facilities.

Assumption of Risks: Participation in The Activity carries with it certain inherent risks that cannot be eliminated regardless of the care taken to avoid injury. The specific risks vary from one activity to another, but the risks range from 1) minor injuries such as scratches, bruises, and sprains, to 2) major injuries such as eye injury, joint or bone injuries, heart attacks, and concussions, to 3) catastrophic injuries such as paralysis and death.

Indemnification and Hold Harmless: I also agree to indemnify and hold The University harmless from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney’s fees, arising out of my involvement in The Activity, and to reimburse it for any such expenses incurred.

Severability: I further agree that this Waiver of Liability, Assumption of Risk, and Indemnity Agreement is intended to be as broad and inclusive as permitted by law and that if any portion is held invalid the remaining portions will continue to have full legal force and effect.

Governing Law and Jurisdiction: This Agreement shall be governed by the laws of the State of California, and any disputes arising out of or in connection with this Agreement shall be under the exclusive jurisdiction of the courts of the State of California.

Acknowledgment of Understanding: I have read this Waiver of Liability, Assumption of Risk, and indemnity agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I confirm that I am signing the agreement freely and voluntarily, and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.

PART 4/4 -PHOTOGRAPHY RELEASE & 2020 UC SAN DIEGO ONLINE SERVICES: USER OBLIGATIONS AND RESPONSIBILITIES

PHOTOGRAPHY RELEASE

During the program, pictures may be taken of students in the midst of their learning and social activities. These pictures may be used in future brochures and public information pieces on Sally Ride Science. There may also be a local television news crew filming for a special report.

UC San Diego Standard Model Release Form: The undersigned enters into this Agreement with UC San Diego Extension Programs (herein called “Photographer/Film Crew”). I have been informed and understand that Photographer/Film Crew is producing marketing materials for the Sally Ride Science program and that my name, likeness, image, voice, appearance and/or performance is being recorded and made a part of those materials (“Product”).

1. I grant Photographer/Film Crew and its designees the right to use my name, likeness, image voice, appearance, as embodied in the Product whether recorded on or transferred to videotape, film, slides, photographs, audio tapes, the internet or other media, now known or later developed. This grant includes without limitation the right to edit, mix or duplicate and to use or reuse the Product in whole or part as Photographer/Film Crew may elect. Photographer/Film Crew or its designee shall have complete ownership of the Product in which I appear, including copyright interests, and I acknowledge that I have no interest or ownership in the Product or copyright.

2. I also grant Photographer/Film Crew and its designees the right to exhibit, market, and otherwise distribute the product, either in whole or in parts, and either alone or with other products, for commercial or non-commercial printing and advertising or any other purpose that Photographer/Film Crew or its designees in their sole discretion may determine. This grant includes the right to use the Product for promoting or publicizing any of the uses.

3. I confirm that I have the right to enter into this Agreement, that I am not restricted by any commitments to other parties, and that Photographer/Film Crew has no financial commitment or obligations to me as a result of this agreement. I hereby give all clearances, copyright and otherwise, for use of my name, likeness, image, appearance, and performance embodied in the Product. I expressly release and indemnify Photographer/FilmCrew and its officers, employees, agents, and designees from any and all claims known and unknown arising out of or in any way connected with the above-granted uses and representations. The rights granted Photographer/FilmCrew herein are perpetual and worldwide.

4. In consideration of all the above, I hereby acknowledge I am receiving no monetary compensation for this release, and am signing below with full understanding and free will. I have read the foregoing and understand its terms and stipulations and agree to all of them. I hereby certify that I am the parent or legal guardian of the student named above, and have read, acknowledge and fully understand this waiver and its terms.

2020 UC SAN DIEGO ONLINE SERVICES: USER OBLIGATIONS AND RESPONSIBILITIES

Sally Ride Science students are authorized to use the University’s Internet/Online services in accordance with the following obligations and responsibilities:

1. Users are responsible for proper use of online accounts at all times. Users shall keep private personal account numbers, home addresses, and telephone numbers. They shall use the system responsibly.

2. The Internet shall be used for educational purposes only. Commercial, political and/or personal use is strictly prohibited. The University reserves the right to monitor online communication for improper use.

3. Users shall not use the system to encourage the use of drugs, alcohol, or tobacco, nor shall they promote unethical practices or any activity forbidden by law or University policy.

4. Users shall not transmit or access material that is threatening, obscene, disruptive, or sexually explicit, nor material that could be construed as harassment or disparagement of others based on race, national origin, sex, sexual orientation, age disability, religion or political belief.

5. Materials obtained or copied on the Internet may be subject to copyright laws, which govern the making of reproductions of copyrighted works. A work protected by copyright may not be copied without permission of the copyright owner unless the proposed use falls within the definition of “fair use.” Violation of copyright laws may subject the user to an action for damages and/or injunction.

6. Users shall not read others’ mail or files, they shall not attempt to interfere with other users’ ability to send or review electronic mail, nor shall they attempt to delete, copy, modify, or forge other users’ mail.

7. Users are expected to keep messages brief and use appropriate language.

8. Users shall report any security problems or misuse of the network to any Sally Ride Science Staff.

9. The Internet contains material that may be considered harmful. The University will not knowingly allow the use of the Internet for access of harmful matter. Because the University is a public place shared by students and staff of all ages, staff reserves the right to end the Internet

sessions when such material is displayed. The University maintains all Internet stations.

I have read this contract and understand that Internet privileges are intended for education purposes. I understand that it is impossible for the Regents of the University of California, any of their respective employees, agents and/or representatives to restrict access to all controversial materials, and I will not hold the University responsible for material acquired on the network. I agree that my son/daughter’s inappropriate use of the University’s technology may result indisciplinary action, the loss of technology privileges, dismissal from the program, and/or appropriate legal action. I hereby give my permission for my student to access information utilizing the University network and Internet gateway. I hereby certify that I am the parent or legal guardian of the student named above, and have read, acknowledge and fully understand this waiver and its terms. I further understand that I am signing this agreement freely and voluntarily, and intend by my signature to completely and unconditionally release from all liability to the greatest extent allowed by law the UC Regents, UC San Diego, Sally Ride Science and its agents. I give my consent without reservation to the foregoing on behalf of him or her.
