11-77 Part 6: Baseline Physical Examination Findings Henry R. Black, Deanne Unger, Alfredo Burlando, John C. Wright, Sara L. Pressel, Ralph Allen, Robert H. McDonald, and Hemantha Surath P resented are results of the physical examina- tions performed on the participants random- ized into the Systolic Hypertension in the Elderly Program (SHEP). Included are data on 4,226 of the total cohort of 4,736 (89.1%). Randomized participants not included were examined, but at least one element of the physical examination was not recorded. All centers performed the physical exami- nations in a standardized fashion (see Methods). Data are presented by age (in 5 -year increments), by race (black or white), by systolic blood pressure (SBP) at randomization (by 10-mm Hg intervals), and by whether participants were on medication when initially screened for SHEP. The actual num- bers and percentages or the means and standard deviations are given in each table. In addition to the data presented in detail, other findings not directly pertinent to the cardiovascular system are noted and summarized at the end. Methods Complete physical examinations were performed at baseline visit 1 by the clinicians at each of the 16 centers. All of those examined had provisionally qualified for enrollment in SHEP at the initial screening or after withdrawal of their antihyperten- sive medication. The physical examination was done as part of good medical care as well as to determine the presence or absence of abnormalities related to eligibility for further participation in SHEP. Exclusions based on physical examination included definite evidence of uncontrolled congestive heart failure, tissue necrosis due to peripheral arterial disease, or a carotid bruit in the appropriate anatom- ical site of a definite transient ischemic attack (TLA) by history. The physical examination was done in a well-lighted, draft-free room at a comfortable tem- perature. A firm-mattressed bed or examining table with a pad cover was used. Participants disrobed completely (with the exception of undershorts or panties) and wore gowns for the examination. Partic- ipants were asked not to smoke immediately before or during the examination. For a description of the methods of measurement of blood pressure and pulse rate, see part 5. 1 Weight was measured in pounds on a calibrated beam balance scale. At each baseline visit, partici- pants were weighed without shoes or outer clothing. Weight was recorded to the nearest pound. Once at baseline, height without shoes was measured and rounded to the nearest inch. A standard direct opthalmoscope was used to visu- alize the optic fundi. The heart was examined with the bell of the stethoscope while the participant was in the supine position. Presence of a cardiac murmur or third or fourth heart sound was recorded. The abdomen was examined with the participant in the supine position. Abdominal bruits were identified by auscultation with the diaphragm of the stethoscope. Abnormal abdominal pulses, hepatomegaly (liver span more than 10 cm), and abdominal masses were also noted; these were found in fewer than 1% of random- ized participants and are not further reported here. The extremities were evaluated for evidence of absent or diminished pulses. The femoral, popliteal, and posterior tibial arteries were examined. Presence of femoral bruits and pitting ankle edema was also recorded. The neurological examination consisted of assess- ing the presence of abnormalities of gait, heel and toe walking, station, cranial nerves, visual fields, strength, coordination, or reflexes. These abnormal- ities were very unusual and are not detailed here. Results Overall Findings Table 6.1 summarizes the physical examination findings for all randomized participants. Part 6: Baseline PhysicalExamination Findings

Presented are results of the physical examina-tions performed on the participants random-ized into the Systolic Hypertension in the

Elderly Program (SHEP). Included are data on 4,226of the total cohort of 4,736 (89.1%). MethodsComplete physical examinations were performed

at baseline visit 1 by the clinicians at each of the 16centers. All of those examined had provisionallyqualified for enrollment in SHEP at the initialscreening or after withdrawal of their antihyperten-sive medication. The physical examination was doneas part of good medical care as well as to determinethe presence or absence of abnormalities related toeligibility for further participation in SHEP.

The neck was examined for carotid pulsations andbruits. Absent or diminished pulsation was recorded.The thyroid was also examined.

The chest and lungs were examined with theparticipant in the sitting position. Recorded abnor-mal findings on auscultation included wheezing andbilateral rales that did not clear with coughing.

The point of maximal cardiac impulse (PMI) wasdetermined. Presence was noted of a PMI that waslaterally displaced more than 2 cm from the midcla-vicular line. The heart was examined with the bell ofthe stethoscope while the participant was in thesupine position. Presence of a cardiac murmur orthird or fourth heart sound was recorded.

The abdomen was examined with the participant inthe supine position. ResultsOverall Findings

Table 6.1 summarizes the physical examinationfindings for all randomized participants. Included are

the means and standard deviations for weight, height,and body mass index as well as the numbers andpercentages of the cohort with positive findings forthe other aspects of the physical examination.

Sample Size and Anthropometric MeasurementsSample size. Table 6.2 gives the numbers and

percentages by age, sex, and race of SHEP random-ized participants who had a complete baseline phys-ical examination; stratification is by level of SBP anduse of antihypertensive medication at initial screen-ing. For both men and women, a higher percentage ofwhites were on medication when screened. For bothblacks and whites, women were more likely than mento have been on medication when screened.

Weight. Mean weight was 162.3 lb (Table 6.3).Weight was lower with greater age, and there were noconsistent differences between those who were on andthose who were not on medication at screening. Meanweight also tended to be lower with higher SBP, andblack women tended to be heavier than white women.

Height. Mean height was 64.3 in. (Table 6.4).Slightly smaller stature was recorded with greaterage. There were no clear differences in relation toSBP or between blacks and whites.

Body mass index. Mean body mass index was 27.5kg/m2 (Table 6.5). As expected, similar to lowerweight and age, mean body mass index was alsoinversely related to age. Black women had a highermean body mass index than white women.

Ocular FundiFundi visualized. Overall, it was possible to visual-

ize the ocular fundi of 79% of randomized partici-pants (Table 6.6). Proportion of fundi visualized waslower with age.

Abnormal fundi. Proportion of visualized fundiwith abnormalities was 30% overall (Table 6.7). Itwas greater with age, greater for blacks than forwhites, and slightly greater for those who were onmedication at screening than for those who were not.For those who were not taking medication at screen-ing, those with SBP of 180 mm Hg or more had thehighest proportion of abnormal fundi. For the groupon medication at screening, frequency of abnormalfundi was lowest in the 160-169 mm Hg SBP stratum.

NeckCarotid bruits. Carotid bruits were noted in 7% of

the cohort (Table 6.8). They tended to be moreprevalent in those who were on medication at screen-ing than in those who were not; the opposite was truefor those with SBP of 180 mm Hg or more. Whiteshad carotid bruits more frequently than blacks. Fre-quency of carotid bruits was highest in those 80 yearsold or older, regardless of medication status atscreening, but there was no consistent pattern amongthe other age strata.

Carotid bruits— Unilateral. Unilateral carotid bruitswere present in 4% of the cohort (Table 6.9). Theywere found equally in those who were on and in those

who were not on medication when screened forSHEP. These bruits were more frequent in whitesthan in blacks and in those 80 years old or older.

Bilateral carotid bruits were present in 3.1% of therandomized participants (data not shown). Thosewith the highest level of SBP at randomization weremost likely to have bilateral carotid bruits. There wasno consistent relation with respect to age, race, or sexfor this physical finding.

Any carotid pulses —Absent or diminished. Carotidpulses were absent or diminished in 2% of the cohort(Table 6.10). This finding was noted more frequentlyin those on than in those not on medication atscreening for SHEP. There was no consistent patternbased on race, age, or level of SBP.

Chest and LungsBilateral rales. Rales were present in 2.2% of the

SHEP cohort at randomization (Table 6.11). Theprevalence of rales was lower in blacks than in whitesfor either sex and lower in younger (60-74 years old)than in older (75 years old or older) age groups.There was no consistent correlation with level of SBPor medication status at screening.

Wheezing. Wheezing was noted in 2.2% of thecohort (Table 6.12). There was no consistent patternfor wheezing in relation to age, level of SBP, ormedication use at screening. Prevalence of wheezingwas higher among black men than among the otherthree race-sex groups.

HeartAny murmur. Murmurs were detected in 26% of

SHEP participants (Table 6.13). Frequency wasgreater with older age and level of SBP but was notrelated to medication status at screening. Blackwomen had a considerably higher prevalence ofmurmurs (41.4%) than did the other three race-sexgroups.

PMl more than 2 cm lateral to midclavicular line.PMI was displaced more than 2 cm lateral to themidclavicular line in only 1% of the SHEP cohort(Table 6.14). This finding was more prevalent witholder age and in blacks than in whites. There were noconsistent patterns in relation to level of SBP ormedication status at screening.

Third heart sounds. Third heart sounds were rarelyheard in the SHEP cohort (0.9%) (Table 6.15). Theywere more frequently found in blacks than in whitesand in those on medication at screening. Level ofSBP at randomization did not relate to frequency ofthird heart sounds.

Fourth heart sounds. Fourth heart sounds werenoted in nearly 9% of randomized SHEP participants(Table 6.16). They were slightly more frequent inthose who were on than in those who were not onmedication at screening, except among white men.Black women had a higher prevalence (14.4%) of thisphysical finding than did the three other race-sexgroups. There was no apparent pattern in relation toage or SBP level.

Irregular pulse. Any patient with atrial fibrillation atscreening or during qualifying visits was excludedfrom participation in SHEP (Table 6.17). Nonethe-less, an irregular pulse during the physical examina-tion was noted in 5.4% of the SHEP cohort. Theprevalence of this abnormality was progressivelygreater with age but did not correlate with level ofSBP. Blacks, especially women, were more likely tohave irregular pulses. There was no consistent corre-lation between irregular pulse and medication statusat screening. The precise nature of the irregularitiesnoted is not recorded here. More detailed data onarrhythmias detected by electrocardiogram at base-line in SHEP are reported elsewhere.2

Abdominal BruitsAbnormal abdominal bruits were detected in

nearly 3% of randomized SHEP participants (Table6.18). They were less frequent in black men than inthe other three race-sex groups. There was no con-sistent difference in prevalence in relation to medi-cation status at screening, age, or level of SBP.

ExtremitiesPitting ankle edema. Pitting ankle edema was pres-

ent in almost 9% of the cohort (Table 6.19). It wasmore frequent in SHEP participants on antihyper-tensive medication than in those not on medication atscreening. Pitting ankle edema was found more fre-quently with older age. It was less frequent amongblack men than among the other three race-sexgroups, and it was not related to SBP level.

Femoral bruits. Femoral bruits were heard in nearly7% of SHEP participants (Table 6.20). They weremore frequent in white men than in the other threerace-sex groups. Those on medication at screeninghad a greater prevalence of femoral bruits, as did the

group with SBP of 180 mm Hg or more at random-ization. There was no consistent trend with age.

Peripheral pulses—Diminished or absent. Peripheralpulses were diminished or absent in 21% of the ran-domized participants (Table 6.21). This abnormalitywas more prevalent with older age and higher level ofSBP. Those on medication at screening were also morelikely to have diminished or absent peripheral pulses.

Overall Freedom From Abnormalities, IncludingNeurological Abnormalities

Overall, 15.8% of SHEP participants had no ab-normalities on physical examination (Table 6.22).This finding was inversely related to age. The fre-quency varied inversely in relation to level of SBP butwas slightly lower in those on antihypertensive med-ication at screening than in those not on antihyper-tensive medication at screening. The percentage ofparticipants with no abnormalities was similar inblack men (17.1%), white .men (16.7%), and whitewomen (15.9%), but it was lower in black women(10.5%).

In addition to these findings, 5.6% of the SHEPcohort (2.0% of the men and 8.3% of the women)had breast abnormalities, 3.4% had an abnormalthyroid, and 16.8% had some abnormality found inthe examination of head, ears, nose, or throat (de-tailed data not shown).
