Terms & Conditions Who is Eligible to Participate: Any person who is directly responsible for the decision-making, order placement and/or approval of Smucker’s® Snack’n Waffl es™ brand waffl es for his/her school district. Only one individual per school district can be designated to submit qualifying purchases and accrue associated points. Both new and existing customers are eligible to accrue points. New customers are defi ned as school districts that have never previously purchased Smucker’s® Snack’n Waffl es™. Exclusions: if you work for the U.S. Government you are not eligible to participate; if your employer has a policy prohibiting employees from participating in reward promotions, you should not participate. All participants will have the option to redeem points and receive the associated award personally, or assign any award to his/her school district. Qualifying Product Purchases and Points Earned: Any 72-count case purchase of 2.2 oz. Maple, Cinnamon, Blueberry or Chocolate Chip Snack’n Waffl es™ brand waffl es between the dates of September 1, 2011 and March 31, 2012 and reported as directed below will earn 10 points per case.Bonus Entries Available: New customers will receive a one-time bonus of 500 points when submitting their fi rst qualifying purchase and 10 points per case thereafter. Existing customers who order a new fl avor that they have never ordered before will receive a one-time 100-point bonus when they order a fl avor they have never ordered before. Existing customers can order up to three new fl avors and receive the one-time 100-point bonus for each new fl avor ordered. All subsequent orders of the new fl avors will receive 10 points per case. All Bonus point awards are subject to review/approval by Smucker Foodsevice. Reporting Your Qualifying Product Purchases: (1) In order to accrue points to your Point Account you must: have an active email account, circle the qualifying case count and the date of order on your purchase order, submit the purchase order via one of the following methods: email scanned pdf or jpg fi le to: [email protected], fax (without a cover sheet) to: 480-443-6815, or send a copy by regular mail to: Extra Mile Marketing ATTN: Smucker Foodservice, 16611 N. 91st Street, STE 106 Scottsdale, AZ 85260-1523. (2) Purchase orders must be received by the 15th of the month following the month of purchase, with the exception of the fi nal month(March 2012). All purchase orders must be received by April 6, 2012. (Example: purchase order dated October 28, 2011 must be submitted for receipt no later than November 15, 2011 to receive points) (3) Registered account holders will receive a report of their accumulated points sent via email on a monthly basis. Establishing Your Smucker’s® Point Account: In order to start reporting qualifying purchases and accumulating points, you must fi rst register to create your Point Account. Here’s how: Complete the registration form with all of the requested information and submit via one of the methods as directed above for reporting. To get the maximum benefi t from the program, submit your registration bySeptember 1, 2011. However, you can enroll at any time with the fi nal deadline of March 31, 2012. Redeeming Your Points: You can redeem your accumulated points as soon as you have reached any one of the following redemption levels - $50 Gasoline Card: 1,000 Points | 250 Gift Card to Offi ce Supply Store* or 500 Smucker’s® branded pencils and scented erasers: 4,000 Points | FREE ANC 2012 Registration OR iPad® with Wi-Fi + 3G: 6,000 Points | FREE ANC 2012 Registration + Hotel* for 4 nights: 12,000 Points. Complete the Offi cial Reward Claim form with the requested information and submit via one of the methods used for reporting qualifying purchase orders. Points have no cash value and are for use only in claiming the above rewards. The number of points you choose to redeem must be based on the Point Accumulation Report that you receive for each month. Points can be used to redeem any combination of rewards within the total number of points you have accumulated. Points redeemed/used will be deducted from your account and will be refl ected on your Point Accumulation Report.
