1. Yourpartnership’sprojectorinitiative.TheCommunityNavigatorSchemeoperatesacrosstheareasofDacorum,ThreeRivers,Watford,StAlbansandHertsmere.Theschemereceivesreferralsfromhealthandsocialcareprofessionals(GPs,SocialWorkers,etc.)thathavepeoplepresenttothemwithissuesthatneednon-clinicalornon-statutorysolutionsandthehealthandsocialcareprofessionalsareunsureofwhichvoluntarysectororuniversalserviceisavailabletohelp.HealthandSocialcareprofessionalsthenreferthepersontoCommunityNavigatorScheme(referralstriagedbyHertsHelp).TheCommunityNavigatorthenworkswiththeindividualandestablishestherelationshipswithallofthevoluntarysectororganisationsandactivitieswhichwillsupportthepersonwiththeissuestheyface;e.g.theymaybeafamilycarer,couldbeindebt,orareisolated;andneednetworkofcommunitysupportwrappedaroundthemtoensuretheystaywell,independentandfeelsupportedbytheircommunity.
2. Howdidyourpartnershipcomeaboutanddevelop?
TheMarmotReport‘FairSociety,HealthyLives’,foundthat20%ofsomeone’shealthandwell-beingdependsonclinicalinterventions.Therestisconditionedbysocialorenvironmentalfactors.ResearchcarriedoutbytheCitizensAdviceBureaufoundthatonefifthofGPtimeisspentonnon-clinicalissues.AlmostahalfofGPssawvalueinunderstandinglocalcommunitiesbutmanydon’thavethetimetodothis.HertsHelpalreadyexistedastheone-stopentrytothevoluntarysector;however,itwasachallengeforvulnerablepeopletocontactHertsHelpandactontheinformationprovided.Thesepeopleneededsupporttoaccessalloftheservicesavailabletothemandneededsomeonetobuildtherelationshipsbetweenthemandtheorganisations.InresponsetothisHVCCGandHCCcommissionedtheCommunityNavigators(deliveredbyvoluntarysectorproviders)toprovidefacetofacesupporttovulnerablepeople,andbuildrelationshipswithhealthandsocialcareprofessionalsandthevoluntarysector.Throughthecommissioningoftheschemethepartnershipwasborn.TheSchemeManager,employedbyHCCandhostedbyHVCCG,ensuredthatthepartnershiphadoversightandcouldbedevelopedcollectivelyandwouldsupporteffectivedeliveryoftheproject.
3. Impact.Pleasedescribewhatoutcomesyouareachieving,yourimpactorhowyouaremakingadifference.
TheCommunityNavigatorSchemereceives1,200referralsayearandhas:-Improvedandsupportedpeople’saccesstoexistinglocalcommunityservices,helpingthemtostaywellandindependent,whilereducingthedemandonprimaryandacutecareservices.-Reducedtheamountoftimethatstatutoryprofessionals(GPs,SocialWorkersetc.)spendonsupportingpeopletoaccessinformalservicesfromthevoluntarysector.HertsHelpandCommunityNavigatorsareseenasthe"onestopshop"forsocialprescribing;andanecdotalevidencehasshownthatcliniciansarenowhavingmoreconstructive,two-wayclinicalengagementwithpeopleabouttheirclinicalhealthneeds;allbecausetheCommunityNavigatorshaveconnectedpeopletotheinformalsupportavailableinthecommunityinordertoaddresstheir‘widerdeterminantsofhealth’.-Preventedacuteadmissionsduetonon-clinicalreasonsanddelayedtheneedforpeopletoaccessmoreformalHealth&SocialCareactivities(seeGPvideoforexample:https://www.youtube.com/watch?v=LUoLM-bVaq8&list=PLxGLedelF6pnf2bwd4bjmA1E5iAzbjwG3&index=4)-Improvedpeople’semotionalwellbeingandreducedtheirriskofdevelopingdepression(asmeasuredbytheSF12QualityMetrictool).Thosewhohavebeensupportedarealsodemonstratingincreasedresilience.-Developedapreventativemodelofcarethataddressesthewiderdeterminantsofhealthatacommunitylevel,bysuccessfullyintegratingexistingvoluntaryandcommunitysectorprovisionwithHealthandSocialCareproviders.“Idon’tknowwhatisavailablefromthevoluntarysectorandIdon’talwayshavethetimetofindout,buttheCommunityNavigatordoes,andmakessurethesupportmypatientsgetlasts.”
4. Howpartnershipworkingishelpingyoutoachievemore.Thewaysinwhichworkinginpartnershiphasachievedmoreinclude:1.Providebetterconnectionstothelocalvoluntarysector.Eachpartnerhasauniqueinsightintothewideandvariedrangeofsupportthatisavailablefromthevoluntarysectorinaspecificlocality.EachpartnerhasbeenabletochampiontheCommunityNavigatorSchemewithintheirlocalarea,growconnections,increasereferralsandsolutions.Statutorypartners,suchasHertsValleysCCG,provideaccesshealthcarepathwaysincreasingreferralsfromGPs.2.Abletobeagiletoseizenewopportunities.Withalargenumberofpartners,ithasenabledtheschemetobeagileandadaptable.WhenfundingwasavailabletorecruitaFrequentAttendersCommunityNavigator,andtesttheconceptwithpeoplewhoregularlyattendA&E;havingarangeofpartnersmeantthatthemostsuitedone(WatfordHousingTrust)couldensurethatthishappenedquickly.3.SpecificexpertiseandKnowledge.TheexpertisethateachorganisationbringstotheschemeisavailabletoalloftheCommunityNavigators,regardlessoflocalityorhost.WatfordHousingTrustcanprovidehousingexpertise,whileCarersinHertfordshirecanprovidetheirexpertiseandknowledgearoundsupportingcarers;astwoexamples.4.Abletomarketthevoluntarysectorasoneprofessionalentitytofrontlinehealthandsocialcareprofessionals.Byworkingasanetworkofpartnersthevoluntarysectorisabletobemarketedtostatutorysectorprofessionalsasone,professionallyrunsector,whichprovidesreal,credible,solutionsforchallengesfacedbypeople.
5. Howyouaremakingsurethatthepartnershipisworkingeffectively.
ThegovernanceofthepartnershipandtheCommunityNavigatorSchemeisthroughasteeringgroupwhichincludesallpartnerorganisations,andbothleadcommissioningpartners.TheSteeringGroupalsohasrepresentativesfromSocialcaretoensuretheprojectissupportingandcomplimentingsocialcaredeliveryandstrategy.TheSteeringgroupalsohasaGPrepresentativetoensuretheschemeisworkingeffectivelywithPrimaryCare.TherearealsorepresentativesfromHertfordshireCommunityNHSTrustandWestHertsHospitalTrustandalsoHertsHelponthesteeringgroup.Together,thesteeringgroupmemberssupportthestrategicdirectionoftheCommunityNavigatorSchemeandprovideacriticalvoicetooperationaldeliveryofthescheme.Havingthesteeringgrouphasbuiltexcellenttrustandrelationshipsbetweenallofthepartnerswithinthisproject.TheCommunityNavigatorschememanagerisemployedbyHertfordshireCountyCouncilandishostedbyHertsValleysCCG.TheschemeManagermanagesthedaytodayoperationsoftheCommunityNavigators,providingoversightofdeliveryacrossallofthehosts,aswellasensuringthattheschemelinkseffectivelyintoappropriatenetworksacrosshealthandsocialcare.Open,regular,communicationanddialoguebetweentheschememanagerandallofthehostorganisationsandtheCommunityNavigatorsthemselves,hasbeenacriticalelementinbuildingeffectivepartnershipworkingfortheproject.ThishasalsoenabledtheCommunityNavigatorstodevelopasateamworkingtoacommongoal,despitethefactthattheyarehostedbydifferentorganisationswithinthepartnership.
6. Whatyouarelearningfromeachother.
1.Effectiveengagementwithfrontlinehealthcareproviders.WebetterunderstandthatonesimpleroutefromtheHealthandSocialcaresectortothevoluntarysectoristheeasiestwaytoensurepeoplegetthehelptheyneed.PrimaryCareisnowthebiggestsinglereferrertoCarersinHertfordshire.Moreover,CommunityNavigatorshavecomplimentedthecarersChampionsandtheCarerSupportAdvisors.2.Widerdeterminantsofhealtharebeingidentifiedbyfrontlinestaffofallpartners,regardlessoftheirprojectarea.SolutionstoaddressingthesearebeingfoundthroughinformalnetworkingwiththeCommunityNavigatorsandothervoluntarysectorpartners.AllhostsarenowpartoftherecentlyestablishedWestHertsCommunityResilienceForum,whichhelpstodevelopthestrategicdirectionofcommunitysupportandsocialprescribingacrossWestHertfordshire.3.EngagementwithMultispecialtyteams(MST).Throughthescheme,eachpartnerunderstandshowMSTs(integratingsocialcare/mentalandcommunityhealth)operate.CarersinHerts,AgeUKDacorum(Dacortiummembers),POhWERadvocateshaveallattendedMSTstocontributesolutionstosharedcareplans.4.Asexternalpartners.Wealsonowunderstand“thelanguageofhealth”andinturnpeoplearealsoawareofthelanguageofhousingandvoluntarysector.
