    How to Apply for a Passport:

    1. Get the Passport Application Form: One can apply for Passport through OnlineRegistration, (after doing online registration, get print outs of the application form) click here fordetails for Online Registration . Apart from this applicant can submit directly in the respectivepassport office/ DPCs/Speed post centers, get the relevant application form from here .

    2. How to fill the Form: The Passport Application Form is Machine Readable. Pleasefollow the instructions as follows:

    Use capital letters only. Particulars given in the form will be printed in the passport.Therefore, please be careful in filling up the form and avoid any mistakes.

    Use black/blue ball pen only Do not fill the form with pencils or ink-pen. While filling up the boxes, kindly leave one box blank after each completed word. Write clearly within the box without touching the boundaries. Adjust the information to fit within the number of given boxes. Do not write anything outside the box. Avoid over-writing Incomplete application will not be accepted.

    Note: All original documents are to be shown at the time of submission of thepassport application form. With the Original Passport Application Form , self-attested copies of all required documents need to be attached. Illiterate applicantsshould put only thumb impression in the box meant for thumb impression/signature.

    In case an applicant has stayed at more than one address during the last one year,he/she should furnish two additional photocopies of the PP form for each additionalplace of stay.

    3. Where to Apply : For Online Registration, the applicant has to submit the printouts of the application form along with required documents to the respectivepassport office in the appointment date and time printed. Apart from this PassportApplications can be submitted at

    1. The counter of the Passport office2. The Speed Post centers3. District Passport Cells

    4. Passport Collection Centers

    An application for a passport may be submitted personally OR through arepresentative (any close member of the family or recognized travel agent(s) only)carrying an authority letter.

    4. Urgent Passport: Refer Tatkaal Scheme in case if applicant needs the passport urgently.

    5. Fee Structure: Refer Fee Structure f or the fees to be submitted.

    Application Forms (Applicable for all the Passport Offices)

    Downloaded forms are valid for making Passport Applications in all Passportoffices, District Passport Centers and Speed Post Centers.

    Application Form No. 1

    Form No. 1 is to be used for Fresh/ Reissue/Replacement of Lost/Damaged passports/ Change of Name/ Appearance/ Exhaustion of pages . The sameapplication form is used for passports for minors as well.

    New passport means applying for a passport for the first time or applying for apassport if you have never held any passport earlier. In case an application was evermade at any Passport Office prior to the present application and whether theapplication was processed or closed, or even if passport was issued and not receivedby the applicant, details thereof should be given in the relevant column in thepresent application. Failure to disclose these details, if any, would invite applicablepenalty.

    Reissue means an application for another passport in lieu of an existing one which

    has either expired or is about to expire. An applicant may apply for a new passport inlieu of an expired/ about to expire passport upto 3 years after and 1 year before theexpiry of the Passport. Re-issue of passport should not be mistaken for renewal of passport. Renewal of passport means that a passport which was originally issued fora short validity of one to five years - under certain emergency conditions on the partof the applicant - is now required to be extended to its full validity of ten years fromthe date of issue of the passport. ( Renewal is a zero fee service and the applicationshould be made in Form No. 2 ).

    A New passport is issued in lieu of a lost/damaged passport.

    Application Form No. 2 is for making application for Police Clearance Certificate,Deletion of ECR Stamp, Inclusion of Spouse's name and change of address. Thesame Form is used for renewing a short term validity passport to its full termvalidity.

    PP Form is used for Police Verification Report. This is a part of Application Form No.1. This can be filled up separately if required by the Passport Office for reverification.In case an applicant has resided at more than one place in last one year, anadditional set of PP form for each address/ place has to be filled up.

    INSTRUCTIONS-SECTION III-COLUMN-WISE GUIDELINES FOR FILLING UP THE

    APPLICATION FORM

    At the beginning of the Application Form there are boxes for affixing photograph,

    appending signature and thumb impression and giving details of payment of the fee.

    PHOTOGRAPHS: Recent passport size photographs (Three) in colour showingfrontal view of full face are required. One photograph is to be pasted on form andtwo on the PP forms, which is to be filled in duplicate. Black and whitephotograph, photograph with coloured or dark glassess, Dark Background orin uniform, Polaroid prints or computer prints will not be accepted. The colourphotographs should have a light background (preferably white) and shouldshow the full face of the applicant frontally. In the box meant for affixing thephotograph, please paste your recent and identical colour photograph of size 3.5 cmx 3.5 cm.. Photograph should fit exactly in the box and in any case not smaller thanthe box provided in the form. For Jumbo booklets, two additional photographs are

    required. The photographs on the PP forms have to be cross signed by the applicantwhereas photograph on First page of the form is NOT to be signed.

    Signature/thumb impression should be strictly within the box without touchingthe boundaries. Thumb impression should be of left hand in case of males and righthand in case of females. (Blue/ Black ball pen to be used for signatures).

    The name up to 75 character long name can be given and filled in the form. The fullname as it should appear in the Passport should be furnished here. For example,

    Name: KARUR VAIKUNTA SUBRAMANIYAN RAMANATHAN PILLAI - Write the Surnameas PILLAI in the boxes provided for Surname and put a comma and write

    VAIKUNTA SUBRAMANIYAN RAMANATHAN as the Given name in the rest of the

    columns. In case you do not have a surname, just write the given name. No initialsshould be written and they should be expanded.

    COLUMN 2 If you have ever changed your name, please indicate the previousname in full. This will be applicable to a person who has even marginally changed thename or a lady who has changed her name / surname subsequent to the marriage. If there is no change in name at all, kindly write : Not applicable.

    COLUMN 3 In case of Male/Female option, please write M or F as applicable in the

    box space provided. For others, please write X in this box.

    COLUMN 4 DATE OF BIRTH: The date of birth is filled as dd/mm/yyyy and inwords as shown in the birth certificate issued by Municipal/Government Authorities.Proof of date of birth is to be attached. Please see Section IV concerning documentsto be attached.

    COLUMN 5 PLACE & COUNTRY OF BIRTH: In case born in India, please mentionname of place like Village / Town, District, State and if born outside India, mentionname of place and country. If born before partition of India, at a place, which nowfalls within Pakistan or Bangladesh, please fill up the name of place followed byCountry as Undivided India.

    COLUMNS 6, 7 AND 8: The name of Father, Mother and Spouse is to be entered in

    the respective columns. Surnames, if any, in these columns should be mentionedafter the given names. In case the applicant is unmarried, column 8 asking forinformation on name of spouse may be filled as NOT APPLICABLE.

    COLUMNS 9 AND 10: Please give relevant details along with date since residing atthe given address, telephone No. with area code is required for the purpose of contacting in case additional information or document is required by Passport Office.Mobile phone No. would be useful for sending SMS message to the applicant for thesame purpose. If the period of residence given in Column 9 is less than one year onthe date of application, please furnish the other addresses with duration of residence. Students staying away from their parents have the option of applyingfrom place of study. In such cases, for proof of address, a bonafide certificate fromthe Principal/Director/Registrar/Dean of the educational institution is must. Anadditional set of PP Form is to be submitted for more than one address in last oneyear.

    COLUMN 11: Please give details of previous passport(s) held. Either the PreviousPassport Number or the file number may be mentioned here along with date of issueand place of issue in the relevant boxes. In case previously applied for passport butthe same was not received/issued, then the details such as file number, date appliedand place where applied should be furnished here. Suppression of facts may attractpenal provisions as per the section 12 of the Passports Act, 1967.

    COLUMN 11(a): If ever travelled on Emergency Certificate (EC) or ever deported orrepatriated to India at Government cost, then the details of EC number, date andplace of issue along with original seizure memorandum, place and country fromwhere deported or repatriated should be furnished in this column. Even if the ECdetails are not available readily, at least the place and country from where deportedor repatriated should invariably be given here. All such applicants should furnish

    details of circumstances of their repatriation/loss of passport in a form of notarizedaffidavit.

    COLUMN 12: Details like educational qualifications, visible distinguishing mark andthe height in centimeters are to be provided against the respective item.

    COLUMN 13: The relevant entry as Yes or No should be marked in the box spaceprovided. If working in Central / State Government, Public Sector Undertakings,Statutory Bodies, a Identity Certificate from the concerned office should beattached as per Annexure B .

    COLUMN 14: Regarding citizenship, whether it is by birth, descent, registration or

    naturalization is to be recorded in the box space provided by either B/D/R/N as thecase may be. If held any other citizenship before Indian citizenship, then pleasefurnish the previous citizenship in the blank space provided.

    COLUMN 15: Please write Y or N as applicable. It may be mentioned here thatIndian citizens categorized as ECR before leaving the country are required to get aclearance from the Protector General of Emigrants. Applicants in ECR category willhave the ECR Stamp put on their passports. In case the passport booklet does nothave ECR stamp, the applicant would be deemed to have been granted ECNR status.No ECNR stamp will be affixed on the passport. An applicant would be eligible forECNR status if he/she falls in any one of the following categories and providesdocumentary proof thereof:

    PERSONS ON WHOSE PASSPORTS ECR STAMPS WOULD NOT BE AFFIXED:

    (I) (a) All holders of Diplomatic/official Passports.

    (b) All GAZETTED Government servants.

    (c) All Income-Tax payers (including Agricultural Income-Tax Payees) in theirindividual capacity. Proof of assessment of income tax and actual payment of incometax for last one year or copy of PAN card to be insisted upon, and not merelypayment of advance tax. However, in most cases as an assessment order is not

    issued separately by the income tax department, income tax returns which arestamped by income tax authorities can be accepted.

    (d) All professional Degree holders, such as Doctors holding MBBS degree orequivalent degree in AYURVED or HOMEOPATHY, accredited Journalists, Engineers,Chartered Accountants, Cost Accountants, Lecturers, Teachers, Scientists,Advocates, etc.

    (e) Spouses and dependent children of category of persons listed from (b) to (d).

    (f) All persons having educational qualifications of matriculation and above.

    (g) Seamen who are in possession of CDC or Sea Cadets, Deck Cadets;

    (i) who have passed final examination of three years B.Sc. Nautical Sciences Courses

    at T.S. CHANAKYA, MUMBAI; and

    (ii) who have undergone three months Pre-Sea training at any of the Governmentapproved Training Institutes such as T.S.CHANAKYA, T.S. REHMAN, T.S. JAWAHAR,MTI (SCI) and NIPM, CHENNAI, after production of identity cards issued by theShipping Master, MUMBAI/KOLKATA/CHENNAI.

    (h) Persons holding Permanent Immigration Visa, such as the visas of UK, USA andAustralia.

    (i) Persons possessing two years diploma from any institute recognized by theNational Council for Vocational Training (NCVT) or State Council of Vocational

    Training (SCVT) or persons holding three years diploma/equivalent degree frominstitutions like Polytechnics recognized by Central/State Governments.

    (j) Nurses possessing qualifications recognized under the Indian Nursing Council Act.1947.

    (k) All persons above the age of 50 years.

    (l) All persons who have been staying abroad for more than three years (the periodof three years could be either in one stretch or broken) and their spouses.

    (m) All children up to the age of 18 years of age. (At the time of re-issue at the age

    of 18 years, ECR stamping shall be done, if applicable)..

    (II) No emigration clearance is required for visiting Bangladesh, Pakistan andall countries in Europe (excluding CIS states), North America, Australia,Japan, New Zealand, South Korea, South Africa and Singapore.

    The eligible category should be mentioned in the blank space providedunder this column and supportive documents should be attached with theapplication. It may be mentioned that with effect from Oct 1, 2007 Governmentof India has abolished Emigration Check Required Suspension (ECRS ). Witheffect from October 2007, ECR passport holders travelling abroad for purpose otherthan employment will be allowed to leave the country on production of validpassport, valid visa and return ticket at the immigration counters at internationalairport in India. For employment purpose, such passport holders require emigrationclearance from the office of Protector of Emigrants (POEs). Otherwise, they will bestopped from travelling at the port of exit. Applicants are, therefore, advised to applyfor ECNR, if they are eligible, to ensure hassle-free travel abroad. It may be notedthat the passport booklet will only have ECR category stamped and in casethe passport booklet does not have ECR stamp, the applicant would bedeemed to have been granted ECNR status .

    COLUMN 16: In this column, while applying for the first time for the minor childrenwho are less than 18 years of age, the details of valid passports held by BOTH OR EITHER parents should be furnished in the relevant column. In such cases, passportto their minor child will be issued without any police verification. Further, in thecases where the parents do not hold valid passports, applications for such minorscan be made on the basis of three documents of parents details of which are given in

    para C(B) of Section IV of the Passport Information Booklet along with Standardaffidavit at Annexure I . In all such cases, passport to their minor child will be issuedon post-police verification basis. A declaration on plain paper as given at annexureH is needed to be filled up in each case. In case the minor child who is between 15and 18 years of age wishes to -obtain a full validity passport for 10 years, the samecan be issued only on submission of Standard Affidavit as in Annexure I and anythree of the 14 document as mentioned in para C (B) of Section IV of PassportInformation Booklet by the parents and on payment of fee equivalent to the normalpassport fee i.e. Rs. 1000/- for a 36 pages passport, as applicable for an adult.Otherwise the validity of the passport is restricted to five years or attaining the ageof 18, whichever is earlier. In case of single parent and children born out of wedlockor in case of parents who are judicially separated, a sworn affidavit before aMagistrate, stating the facts of the case along with documentary proof to besubmitted as per Annexure C . In the NORMAL COURSE the signature/consent of both parents is required for issue of a passport to the minor (Annexure H) .However, if in ca se the applicant parent is not in a position to get the consent of theother parent, FOR WHATEVER REASON , the parent applying for the passport of theminor may sign the form and submit a sworn affidavit as per Annexure G statingthe facts and circumstances of the case along with the application. The affidavitshould also state that in case of a court case he/she would be responsible and notthe passport office. In such cases, where only one parent is applying, the physicalappearance of the child may be requested to ensure the applicant's parent has theactual custody of the child.

    COLUMN 17: The applicant should give correct information. Suppression of any fact

    may lead to fine up to Rs.5000 per offence and other penal provisions as applicableunder the provisions of the Passports Act, 1967 as amended from time to time.

    COLUMN 18: In this column, the name and address along with Mobile or Telephonenumber/e-mail of person to be intimated in the event of death or accident is to befurnished.

    COLUMN 19: This column is a self declaration made by the applicant about owingallegiance to the sovereignty, unity and integrity of India, not voluntarily acquiringthe citizenship or travel document from any other country etc. Also this columncontains declaration in furnishing true information in the application form and anapplicant is aware that it is an offence under the Passport Act 1967 for giving any

    wrong information or suppression of any material information in getting thepassport. The applicant also declares that he has no other passport or traveldocument. Under the space provided, the signature or Thumb Impression (left handthumb impression for male and right hand thumb impression for female) should befurnished along with date and place of application.

    COLUMN 20: Photocopies of all documents that are attached as enclosuresalong with the application form should be listed in the blank spaces providedand each document self attested by the applicant.

    Fees Structure

    The following all-inclusive fee is to be paid along with the application form, either by

    Bank Draft in favour of the Passport Officer concerned or in cash. In case of DD, full

    name of applicant and application number to be written on the reverse of the draft.

    The Bank Code of Issuing Bank, the Demand Draft Number and date of issue of the

    Demand Draft should be mentioned.Details of payment of fee may be mentioned in

    the relevant boxes in the application form.

    1. Fresh Passport (36 pages) of 10 years validity

    (including minors between 15 to 18 years of age, whowish to get a 10 years full validity passport)

    2. Fresh Passport (60 pages) of 10 years validity Rs. 1,500/-3. Fresh Passport for Minors (below 18 years of Age) of 5

    years validity or till the minor attains the age of 18which ever is earlier.

    PENALTY FOR OFFENCES UNDER PASSPORT ACT, 1967

    Imposition of penalties for suppression of information under Section 12(1) (B) of Passport Act, 1967 is asgiven below:

    (a) Proof of address (attach one of the following):

    Applicants ration card, certificate from Employer of reputed companieson letter head, water /telephone /electricity bill/statement of runningbank account/Income Tax Assessment Order /Election Commission IDcard, Gas connection Bill, Spouses passport copy, parents passportcopy in case of minors.     (b) Proof of Date of Birth (attach one of the following):

    Birth certificate issued by a Municipal Authority or district office of theRegistrar of Births & Deaths;

    N.B.: In the case of applicants born on or after 26.01.89, onlyBirth Certificate issued by the Municipal Authority or the Officeof the Registrar of Births & Deaths is acceptable.

    (c) Citizenship Document if applicant is a citizen of India by Registrationor Naturalization.

    (d) Government/Public Sector/Statutory body employees should submit Identity Certificate in original (ANNEXURE B) along with StandardAffidavit Annexure I .

    (e) If the applicant is eligible for ECNR attach attested copy of supporting document ( see COLUMN 15 of the INSTRUCTIONS

    AND GUIDELINES FOR FILLING UP THE APPLICATION FORM ) .

    (f) If the applicant was repatriated at Government cost, enclosedocuments to show that the expenditure, if any, incurred by theGovernment of India on his/her repatriation has been fully refunded

    to the Government of India, Ministry of External Affairs.

    (g) If the applicant was ever deported to India, give details of EmergencyCertificate/Passport.

    (2) When applying for reissue of a passport after 10 years, attach:

    (a) Old passport in original with self-attested photocopy of its first fourand last four pages, including ECR/ECNR page.

    (b) Document mentioned at (1) (d), if applicable.

    (c) Document mentioned at (1) (e), if the old passport did not have ECNRstamp or it was issued when the applicant was a minor.

    (d) If there is any change in address, document mentioned at 1 (a).

    (e) If the old passport does not contain spouse name, copy of marriagecertificate issued by the Registrar of Marriage or affidavitas perspecimen in Annexure D .

    (3) When applying for a minors passport attach:

    (a) A Declaration affirming the particulars furnished in the applicationabout the minor child as per Annexure-H (signed by both parents),Annexure C (Single parents who are separated but not formallydivorced/Single parent of the child born out of wedlock), AnnexureG (when passport is being applied for by single parent or legalguardian) . Annexure I (when a minor between 15-18 years of ageapplies for a full validity 10 year passport OR in case either parentswho do not hold valid Indian passport while applying passport for theirminor child), as the case may be.

    (b) Attested photocopy of passport, if any, of both parents,applicable.

    (c) Original passports of parents should be presented forverification of particulars.

    (d) If one parent is resident abroad, a Sworn affidavit by the parentresident abroad attested by the Indian Mission along with affidavitfrom parent residing in India as well be submitted.

    N.B. : Ordinarily the consent of both parents is required for issue of apassport to a minor (below 18 years of age). However if it isabsolutely not possible due to any reason, the parent applying for apassport for his/her minor child may submit an affidavit (Annexure G)and based on the same passport application will be processed. In casewhere the parent(s) is/are resident outside India, such consent fromthe parent(s), in the form of a sworn affidavit, duly attested by theIndian Mission abroad, is acceptable. In the cases where the minor

    children who are less than 18 years of age, the details of validpassports held by BOTH OR EITHER parents should be furnished. Insuch cases, passport to their minor child will be issued without anypolice verification basis. Further, in the cases where the parents donot hold valid passports, applications for such minors can be made onthe basis of three documents of parents details of which are given inpara C(B) of Section IV of the Passport Information Booklet alongwith Standard Affidavit Annexure I. In all such cases, passport totheir minor child will be issued on post-police verification basis.Children of all ages including new born must apply for separatepassports. However, those below 15 years will be given 5 yearsvalidity passport or passport till 18 years of age. In case the minorchild who is between 15 and 18 years of age wishes to obtain a fullvalidity passport for 10 years, the same can be issued only on post-police verification basis on submission of Standard Affidavit as inAnnexure I and three of the 14 document of parents details asmentioned in para C (B) of Section IV of Passport Information Bookletand on payment of fee equivalent to the normal passport fee i.e. Rs.1000/- for a 36 pages passport, as applicable for an adult. In the caseof single parents or of parents who are separated but not formallydivorced, an affidavit at Annexure C is to be submitted.

    For Adopted Children :

    In case of Adopted Children the following documents are to be furnished:

    i) Valid adoption deed registered as per Indian laws

    ii) In the case of Christians, Muslims and Parsis, a court order grantingguardianship and allowing the child to be taken out of the Country.

    iii) Copy of the guarantee executed before the Court concerned.

    (B) CHANGE OF NAME

    I . Following marriage, remarriage or divorce:

    (a) A woman applying for change of name/surname in existing passportdue to marriage must furnish:

    (i) Photocopy of the Husbands passport, if any, and

    (ii) An attested copy of marriage certificate issued by Registrar of Marriage OR an affidavit from the husband and wife alongwith a joint photograph, (Specimen at Annexure D ).

    (b) Divorcees applying for change of name OR for deletion of spousesname in existing passport must furnish

    (i) Certified copy of Divorce decree.

    (ii) Deed poll/sworn affidavit ( ANNEXURE E )

    (c) Re-married applicants applying for change of name/spouses namemust furnish:

    (i) Divorce deed/death certificate as the case may be in respect of first spouse, and

    (ii) Document as at (a) above relating to second marriage.

    II. In other circumstances for change of name, the applicant (bothmale and female) should furnish:

    (i) Deed poll/sworn affidavit (ANNEXURE E) :

    (ii) Paper cuttings of two leading daily newspapers in original (one dailynewspaper should be of the area of applicants permanent and presentaddress or nearby area).

    (C) OUT OF TURN ISSUE OF PASSPORT UNDER TATKAAL SCHEME

    (A) If an applicant desires to obtain his passport under the Tatkal Scheme, aVerification Certificate as per the specimen at ANNEXURE F and Standard Affidavitas Annexure I should be submitted along with the TATKAAL fee. The PassportIssuing Authority shall retain the right to verify in writing the authenticity of theVerification Certificate from the official who has issued it. All applicants seeking apassport out of turn under the TATKAAL Scheme are advised to submit theirapplication, documentation and fee as specified below. No proof of urgency isrequired for Out-of-Turn issue of passport . Post Police Verification shall be donein respect of all passports issued under the Tatkaal Scheme.

    (B) The applicant also has the option to obtain a passport under Tatkal Scheme onsubmission of three documents from the Fourteen documents as mentioned below,provided one of the three documents is a photo identity document and atleast one of the three is amongst the documents indicated at (a) to (i) and a Standard Affidavit( Annexure I ) on non-judicial stamp paper duly attested by a Notary:

    (a) Electors Photo Identity Card (EPIC);

    (b) Service Photo Identity Card issued by State/Central Government, Public SectorUndertakings, local bodies or Public Limited Companies;

    (c) SC/ST/ OBC Certificates;

    (d) Freedom Fighter Identity Cards;

    (e) Arms Licenses;

    (f) Property Documents such as Pattas, Registered Deeds etc.;

    (g) Rations Cards;

    (h) Pension Documents such as ex-servicemens Pension Book/Pension Paymentorder, ex-servicemens Widow/Dependent Certificates, Old Age Pension Order,Widow Pension Order;

    (i) Railway Identity Cards;

    (j) Income Tax Identity (PAN) Cards;

    (k) Bank/ Kisan/Post Office Passbooks;

    (l) Student Photo Identity Cards issued by Recognized Educational Institutions;

    (m) Driving Licenses; and

    (n) Birth Certificates issued under the RBD Act

    (o) Gas Connection Bill

    (All above documents to be produced in original along with self-attested copies)

    The TATKAAL fee is in addition to the applicable passport fee and payable either incash or DD in favor of Passport Officer concerned. The additional fee for out of turnTATKAAL passport, would be as follows:

    (D) For issue of passports to owner, partners and directors of Companieswhich are members of CII, FICCI & ASSOCHAM, the applicants have to submitVerification Certificate as at Annexure J along with Standard Affidavit atAnnexure I .

    (E) In case an applicant is in possession of a Verification Certificate (VC) and orthree (3) documents as mentioned in para C(B) (a) of Section IV , and Standardaffidavit as in Annexure-I , but does not wish to pay the additional fees as requiredunder Tatkaal, he/she will be issued passport on post- police verification basis in thenormal course.

    (F) CASES OF LOST/DAMAGED PASSPORTS: The applicant has to fill the passportapplication form and submit the same along with following deeds:

    Ans. It is up to you to change the address. It is always better if your

    passport indicates your current address. For this purpose, you needto fill up form-II and submit it along with valid proof of your

    residential address at Passport Office of the area of where you

    reside.

    Question 2. My Passport has been issued by one Passport Office. Can

    I apply for services in another Passport Office? Ans. Yes. You can

    apply for miscellaneous services at Passport Office where you are

    presently residing.

    Question 3. I dont have ECNR stamp in my Passport. Can I get it on

    the basis of my provisional matriculation or above certificate issued

    by the University/Board concerned? Ans. We are no longer

    affixing ECNR stamp in passports. In fact it is not even printed in the

    new passport booklets. Currently, only ECR stamp is being put in

    passports if applicant is not eligible for ECNR. However, you are

    entitled to ECNR status if you possess qualification of Matriculationor above. Even you can get ECNR status on the basis of provisional

    matriculation certificate if the mark sheets of all semesters of your

    course are available with you.

    Question 4. I have downloaded the application form from your

    website. Can I fill it up & submit? Ans. Yes.

    Question 5. I have downloaded the application form for passportfrom the website. However in the column where specimen signatures

    of the applicant are required it says put the signatures also on the

    stickers option, which is not available on the net. What should I do in

    this case. Ans. In this case, you need to put your signatures only

    below column 19 of the application form.

    Question 6. Is Identity Certificate mandatory for State/Central

    Government/PSU employees? Ans. Yes. Identity Certificate

    (Annexure-"B") issued by the competent authority in prescribed

    Performa is a must for these employees for getting their passports,

    without police verification.

    Question 7. I am working in ICICI Bank? Am I required to furnish the

    Identity Certificate? Ans. Only those persons working in companies,

    which are owned by the State/Central Governments, need to submit

    Identity Certificates. Employees of Private Banks are not required

    to submit Identity Certificates for getting passports.

    Question 8. I got my passport 2 years ago. I got married recently.Should I include my spouses name in my passport? If so, what is the

    procedure? Ans. It is better to include your spouses name in your

    passport. For this, please file up Form No.II(EAP-II) form and submit

    it along with marriage certificate, photocopy of spouses passport (if

    she has one) and proof of stay (if the present address is different

    from the one in the passport). Note : The procedure is the same if

    one wants to include her husbands name in the passport.

    Question 9. I have a one-month-old baby? Does the baby require a

    separate passport Or his/her name can be endorsed in one of the

    parents passport? Ans. Endorsement of childs name in parents

    passports is not allowed anymore. As per the amended law, minors

    should have separate passports.

    Question 10. In case of a minor child passport, whose father is

    abroad, whether consent is to be obtained to apply for a passport? If

    so, what is the procedure? Ans. Yes. Consent of the spouse, who is

    abroad, is required for submitting the application. It should be in the

    form of a Sworn Affidavit duly attested by the Indian

    Embassy/Consulate abroad.

    Question 11. What is the difference between Re-issue and

    Renewal of a passport? Ans. Passports are Re-issued after the expiry

    of their 10-year validity period.If you had been issued a Short

    Validity Passport (i.e. a passport of validity less than 10 years) apply

    for renewal to extend the normal validity of the existing passport to

    10 years. In the case of Re-issue of passport a normal fee of

    Rs.1000/- will be charged whereas for extending the validity

    (renewal) no fee is charged.

    Question 12. My passport expired 4 years ago. Can I renew it? What

    is the procedure for it? Ans. In such a case, you have to fill up

    Application Form No. I(EAP-I) form and submit it along with

    original/photocopy of your expired passport and proof of your

    residence/address. However, since the passport expired 4 years ago,

    fresh police verification will be required in your case.

    Question 13. I am a student staying away from my parents.

    Where do I apply for passport and what proof of residence shall I

    have to furnish? Ans. For instance, if you are a student staying in

    Rohtak (which falls under the jurisdiction of RPO, Chandigarh) and

    your parents are staying in Hissar which also falls under the

    jurisdiction of RPO Chandigarh , You can apply at RPO, Chandigarh.

    For this purpose, along with your form you have to provide four

    Personal Particulars forms. Please also indicate your hostel

    address/the place where you are staying in Rohtak as present

    address and your parents address as Permanent address. In case

    you are studying and staying in Bangalore , you can either apply to

    the Passport Office Bangalore or in the Passport Office in whose jurisdiction your parents address (permanent address) is located.e.g

    Delhi. In the later case , fill up an additional set of Personal

    particulars form giving the address in Bangalore. For proof of

    residence at your hostel/place of stay, either you should provide a

    letter from the Principal/Warden or proof of residence of the

    guardian with whom you are staying along with the guardianship

    affidavit. You should also provide proof of your parents address (i.e.

    permanent address).

    Question 14. What is the difference in procedure betweenapplying in person and by Post? Ans. While you apply in person,

    two copies of all the documents duly self attested have to be handed

    over at the counter along with the originals, which will be returned

    after verification at the counter immediately. While applying by

    post, you have to submit two copies of all the documents duly

    attested by a Gazetted Officer bearing his name seal and address of

    his office.

    Question 15. I am a salaried person and Income tax is deducted from

    salary. What are the documents I need to submit for ECNR status?

    Ans. Form 16(a) for the last 3 years along with copies of

    acknowledgement of your returns.

    Question 16. I shifted to Delhi 7 months back from Bangalore. I

    have to apply for a passport. As per instructions, one should have

    stayed for atleast one year at a given address. Can I apply for apassport at Passport Office, Delhi? If so, please let me know the

    procedure. Ans. Yes. You can apply at Passport Office, Delhi. For

    this purpose, you have to submit proof of seven months stay in Delhi

    and also mention the previous address in the application form for

    which you are also required to submit 2 additional Personal

    Particulars forms along with the application.

    Question 17. I have to travel to the UK next week on a company

    assignment. I dont have passport. Can I get a new passport under

    the Tatkal Scheme? Ans. Yes. You can apply under Tatkal scheme

    and pay the Tatkal fee of Rs. 1,500/1000 as the case may be, and a

    Verification Certificate (Annexure-F) or three documents as

    mentioned under Tatkal Category of Passport Information Booklet

    along with Standard Affidavit (Annexure-I).

    Question 18. What documents are required when there is a

    change in name on account of Divorce.

    Ans. (i) Divorce deed duly authenticated by Court and (ii) Deed Poll

    or Sworn Affidavit as per specimen in Annexure D given in the

    Passport Information booklet attached to the application.

    Question. 19. Is it mandatory to submit Birth Certificate in cases

    of those who are born on or after 26.01.1989. Ans. Yes, Birth

    Certificate issued by Municipal authorities only is acceptable forthose born on or after 26.01.1989.

    Question 20. In a case of Muslim passport holder, whether entry of

    second wife name is allowed on passport. Ans. Yes.

    But the entry of first wife name will remain intact if

    divorce is not obtained.
