Welcome to the 5th Medical Group, Minot AFB, ND. Our goal is to deliver timely and high quality health care to our beneficiaries. This handbook is intended to provide you with an outline of the services we have available to you, your family and the Minot AFB community. The 5th Medical Group has an enrolled TRICARE population of 12,500 people and is home to the largest Personnel Reliability Assurance Program (PRAP) in the Department of Defense.

The 5th MDG is fully accredited by The Joint Commission. This professional civilian accrediting body conducts periodic inspections of civilian and military health care facilities to ensure medical care and services rendered meet or exceed national standards. Accreditation assures our patients that the staff of this facility adheres to the highest standards of medical care and administration.

PATIENT RIGHTS

All persons obtaining care in this medical treatment facility are entitled to certain rights and are also subject to certain responsibilities. The observance of these rights and responsibilities by both patients and facility

personnel is vital to delivering patient care and services in an appropriate and efficient manner.

You have the right to: 1. Quality care and treatment consistent with available resources.

2. Considerate and respectful care, with recognition of personal dignity, psychosocial, spiritual,

and cultural values and belief systems. An environment of shared respect which is free from discrimination.

3. Privacy and security of protected health information. Communicate with your healthcare

providers in confidence as allowed by law and regulation.

4. Receive information about the individuals providing your care, treatment and services. A choice of healthcare provider, either within the UnitedHealthcare (UHC) network or the Military Treatment Facility (MTF).

5. An explanation of diagnosis, treatment and prognosis of illness in easily understood terms.

Ask questions and receive timely answers to those questions.

6. Any and all necessary information in non-clinical terms so you can make knowledgeable decisions on consent or refusal for treatment. Refusing care is allowed by law.

7. Make recommendations, ask questions, or file complaints to the clinic’s Patient Advocate. A fair and efficient process to appeal medical necessity decisions by your MTF or by United Healthcare Military and Veterans that includes both internal review and independent external review.

8. Know if the MTF proposes to engage in or perform research associated with your care or treatment.

9. Care and treatment in a safe environment.

10. Be informed of the facility’s rules and regulations that relate to patient or visitor conduct.

11. To be transferred to another medical care facility only after you receive complete

information and explanation concerning the needs for and alternatives to such a transfer. Access to emergency care without preauthorization where and when acute symptoms are so severe that a “sensible layperson” would want emergency care to prevent serious harm or death.

12. Understand the charges for care and your obligation for payment.

13. Make sure your wishes regarding your healthcare are known even if you are not able to

communicate or make decisions for yourself. Choose someone to make decisions on your behalf, if you cannot do so.

PATIENT RESPONSIBILITIES

You are responsible to: 1. Respect the rights of other patients and the MTF staff. This includes not destroying property, yelling, swearing, or physically assaulting anyone.

2. Become educated about United Healthcare Military & Veterans coverage, options, and rules, to include the required fees.

3. Tell your healthcare professional everything you know about your illness or condition, past healthcare, and any and all medications or supplements you take. 4. Inform the doctor or nurse about any changes in how you feel.

5. Participate in decisions related to your healthcare and ask questions if you do not understand any part of the proposed treatment. 6. Follow your plan of care developed in coordination with your provider. 7. Maximize healthy habits such as exercising, not using tobacco products, eating a healthy diet, and preventing the spread of disease. 8. Accept the consequences if you refuse to follow the plan of care. 9. Keep appointments or call to change or cancel them at least 24 hours in advance. 10. Report wrongdoing and fraud to appropriate authorities.

Patient Advocate The Patient Advocate program is designed to help each and every patient with clinic policies, procedures, complaints, and suggestions. For this reason, each clinic has a Patient Advocate representative. There are signs in each patient examination/treatment room identifying your patient advocate. Patient Safety The Patient Safety program allows patients and staff to communicate concerns about patient safety issues that occur before, during, and after care is received.

MEDICAL ADMINISTRATION Eligible Beneficiaries: Generally, active duty personnel and their family members, activated Air Reserve and Air National Guard members, retirees and their family members, American Red Cross employees, and certain categories of DoD, Non-Appropriated Funds, and Army and Air Force Exchange civilian employees and contractors (on a reimbursable basis, unless seen for a job-related injury or physical) are eligible for care.

Policy on Minors, children under 18 years of age: Either parent may confer authority to a third party to consent to medical, dental and/or surgical treatment for their minor dependents. This authorization is normally accomplished by execution of a Consent to Authorize Medical Care and Treatment of Dependents and maybe obtained from the base legal office. This document permits treatment only at the 5th Medical Group. A full power of attorney, sometimes referred to as In Loco Parentis, maybe be obtained from the base legal office and consents to care at any medical facility. For emergency treatment, in accordance with North Dakota Century Code 14-10-17.1, in the absence of parental authorization conferring authority to another adult, medical personnel may render medical emergency care and procedures deemed necessary to protect the life of a minor, if the parents cannot be contacted. In accordance with North Dakota Century Code 14-10-17, a provider may provide examination, care and treatment to a minor who is 14 years old or older for a sexually transmitted disease, alcoholism or drug abuse without parental consent. Also, a provider may provide a minor with a pregnancy test and its results without parental consent in accordance with North Dakota Century Code 14-10-19. Advance Medical Directives: North Dakota state law permits you to make your wishes regarding medical care in the event of a terminal or incapacitating illness known through the use of Living Wills and Durable Power of Attorney, also known as Advance Directives. The 5 MDG shall not discriminate against an individual based on whether or not the person executed an Advance Directive. The completion of an Advance Directive is an option, and lack of an Advance Directive will not hamper appropriate medical care. There are two limitations placed on your right to make these decisions: #1) Certain regulations limit the rights of military members to refuse care. In addition, treatment refusal may subject the member to Medical Evaluation Board (MEB) processing. #2) A doctor may refuse to recognize your Advance Directive as a matter of conscience or moral conviction. In such a case, every effort will be made to find a doctor who can honor your request. For more information, please contact the Base Legal Office at 701-723-3026.

ACCESS TO CARE

Emergency Services The 5 MDG does not operate an Emergency Department; however we do provide ambulance services. For on base emergencies, dial 911 from a landline or 701-727-1911 from a mobile phone. Appointment Cancellation/No-Shows Please cancel any medical appointment which you cannot attend to avoid being recorded as a “No-Show.” We request cancellations be made at least 24 hours in advance so the appointment can be offered to another patient. Referral Management A Primary Care Manager (PCM) may give a patient a referral to an off base provider for care not available in the clinic. Patients must stop by the Referral Management Center to activate their referral, receive traveling instructions and guidelines on reimbursable expenses. Referral Management services are located on the 1st floor.

PATIENT SUPPORT SERVICES

Beneficiary Counselor & Debt Collections (BCAC) The Beneficiary Counselor provides you with assistance dealing with your TRICARE claims and collection agency activities. Services also include one on one counseling as related to your current and post service TRICARE benefits. If you are an AD member considering elective surgery, you must stop by this office to obtain mandatory guidance as well as paperwork pertaining to your procedure prior to receiving the elective care.

Outpatient Records Military Treatment records are the property of the US Government and must be maintained at the 5 MDG. Outpatient Records is located on the 1st floor. Patients are not allowed to hand carry their medical records unless on PRP, PCSing to Korea, or other specific reasons that are annotated on your PCS orders. If you are empanelled to the facility and have a scheduled appointment on base, your record will be available, if requested by the provider. If you have an appointment off base, please stop by and pick up a copy of the required documentation needed for your appointment.

Release of Information The Release of Information office is located on the 1st floor in Outpatient Records. Services include transferring medical records, copying records, and the processing of all incoming and outgoing requests for medical information. All requests must be made in writing and must contain the appropriate legal authorization prior to release. We strive to provide you copies of your medical records within 30 calendar days of request.

Exceptional Family Member Program (EFMP) / Overseas Clearances This is an Air Force mandated program for all AD personnel with a family member who has a chronic disease, requires ongoing specialty care, requires specialized educational services or meets certain other criteria. Emphasis is on general medical services, medically related services, and special education services. The program screens all enrolled families for stateside assignments and all Air Force families with an overseas assignment by conducting a medical records review with the family member’s PCM to ensure all necessary services are available at the gaining base.

Third Party Collections By law, all military health care facilities must bill private insurance companies (third party insurers) for the care of military family members. It is mandatory that all non-active duty personnel complete a DD Form 2569, Record of Other Health Insurance, regardless of age, even if they do not have any

other health insurance. Sponsors are not allowed to sign the forms for their spouse or any child who is 18 years or older due to HIPAA regulations.

If you are enrolled in TRICARE Prime and you receive a bill for care, bring it to the Health Benefits Advisor located on the 3rd floor or call 701-723-5176.

TRICARE Online Website

TRICARE Online (TOL) is a secure web portal designed to increase access to care and information. Enrolled and registered beneficiaries have the capability to schedule and cancel appointments, and can request their prescription refills at any time, 24/7. The "Blue Button" tool allows authorized users the ability to view, print, and save their available health data. TRICARE Online Customer Service may be contacted at 1-800-600-9332.

MICARE SECURE MESSAGING MiCare Secure Messaging is an online service that will enable you to communicate securely and privately with your health care team. Registration is accomplished by filling out the MiCare Registration Form (pick up at Family Practice, PRP/Flight/Missile Medicine Clinics). After filling out the form, you will receive a confirmation email asking you to complete the registration process online. Once you have accomplished this you are ready to start messaging your care team through the RelayHealth portal: https://app.relayhealth.com.

PRIMARY CARE MEDICAL HOME

The 5th Medical Group’s Family Health Clinic operates as a Primary Care Medical Home (PCMH). The Family Health Clinic sees active-duty members who are not on PRP or FLY status, and all dependents. The PCMH is a model of care where patients have a direct relationship with a provider who coordinates a cooperative team of healthcare through ongoing preventive care. All enrolled beneficiaries are assigned a Primary Care Manager (PCM) which may be a physician, a physician assistant or a nurse practitioner. Changes to your assigned PCM can be discussed with your PCM team.

Your medical team will be invested in your care. The PCMH is designed to meet the majority of our patient’s physical and mental health care needs through a team-based approach to healthcare delivery that establishes and provides:

A respectful trusting relationship with all patients Enhanced access to meet patient needs Continuous and comprehensive care Medical management of all conditions Preventive and evidence-based care Proactive management of all health risks Health education and decision-making support

PATIENT CARE CLINICS

Ambulance Services Ambulance Services is located on the 1st floor and provides 24 hour emergency transport coverage seven days a week to the entire base population. It is staffed with both Active Duty Emergency Medical Technicians and civilian paramedics. Response is provided for 911 calls and flight line emergencies. Patients are transferred as needed to the civilian hospital in downtown Minot. For more information, call Ambulance Services at 701-723-5627. Dental Clinic The Dental Clinic is located on the 2nd floor and provides services to active-duty personnel only. Family members are encouraged to enroll in the TRICARE Dental Program (TDP) for services. Patient care is provided Monday-Friday, 7:30-4:30. For after-hours dental emergencies (e.g. severe pain, swelling, infection, or significant trauma), active-duty patients should call Ambulance Services at 701-723-5627. Family Health Clinic The Family Health Clinic is located on the 1st floor and provides acute, routine, and wellness care for the medical needs of TRICARE Prime beneficiaries, (who are not on PRP or FLY status) from birth to geriatrics. We offer the following services: evaluation and treatment of most acute and chronic medical conditions; the Behavioral Health Optimization Program, family planning (including vasectomies and contraception), diagnostic services, minor surgical procedures (such as ingrown toenail removal and skin biopsies), and a shaving waiver clinic.

Appointments: Appointments can be made by calling 701-723-5633, M-F, 0700-1600. The appointment line is closed on federal holidays and base down days. For after-hours non-emergency care, call the TRICARE Nurse Advice Line (NAL) at 1-800-874-2273 to be triaged by a nurse and/or to seek authorization for care off base. All clinic services are by appointment only, unless otherwise specified. You may also book an appointment with your assigned PCM at www.tricareonline.com. Walk-in for: Pregnancy testing (walk in to the lab)

Suspected urinary tract infection (women age 18 and older) Sore throat Suture removal Blood pressure test Depo-Provera injection (after being seen by your PCM) B-12 injection (after being seen by your PCM) Testosterone injection (after being seen by your PCM) Wart removal

Immunizations Clinic Immunizations Clinic is located on the 1st floor and is open Monday through Friday from 0900-1200 and 1300-1600 for walk-in services. Smallpox vaccinations are conducted every Thursday at 0730-0900. PPD/ TB Skin Tests have to be checked 48-72 hours after placement and will not be placed on Thursdays or 2-3 days prior to scheduled down days or federal holidays. Immunizations provides all necessary immunizations to active duty, their family members, and retirees, including well baby immunizations, deployment related shots, as well as the annual flu shot. Appointments are not required for Immunizations services. Allergy services are reserved Monday, Tuesday, Wednesday, and Friday from 0730-0900. Mental Health Clinic The Mental Health Clinic is located on the 1st floor and provides mental health services for Active Duty members and Active Guard personnel; family members and retirees are seen on a space available basis. The clinic is staffed by a psychiatrist, psychologists, social workers and mental health technicians. The clinic provides mental health counseling and outreach, Alcohol & Drug Abuse Prevention & Treatment (ADAPT), Family Advocacy Program (family maltreatment prevention and intervention), New Parent Support Program and numerous educational classes such as stress management, sleep hygiene, and anger management. Optometry Clinic The Optometry Clinic provides examinations, diagnosis, and treatment for ocular and visual disorders for AD personnel and family members age six and older.

Appointments for routine and acute eye conditions are available by calling the appointment line at 701-723-5633.

Pediatrics The Pediatrics Clinic is located on the 1st floor next to Immunizations and serves your child's health care needs from birth to up to 18 years old. The pediatric clinic performs well baby visits, sports physicals, routine check-ups and acute appointments. Walk-in services are provided for throat cultures from 0800-1100 and 1300-1500. Physical Therapy The Physical Therapy Clinic is located on the 2nd floor and treats patients based on self-referral or a referral from your PCM. The clinic performs exams, evaluations, and treatments for a full range of neurologic and musculoskeletal dysfunctions. For more information, call 701-723-5095.

Women's Health The Women's Health Clinic is co-located with the Family Health Clinic and provides a number of vital health care exams and counseling services for women. Although the 5 MDG does not provide obstetric care, this clinic does care for most other gynecological needs and issues affecting women, such as routine women's health exams and annual pap smears; evaluation for disease processes such as endometriosis; dysplasia clinic; and counseling on a variety of women's health concerns, such as birth control, infertility, hormone therapy, aging and menopause.

PRP/Flight & Missile Medicine Clinic The PRP/Flight & Missile Medicine Clinic is located on the 1st floor. This clinic serves the medical needs of active duty members who are on PRP and 2992 holders. We also serve the Air National Guard and civilians on PRP status. We offer the following services: evaluation and treatment of acute and chronic medical conditions, contraceptive needs, PHAs, occupational medicine exams, minor surgical procedures such as ingrown toenail removal and skin biopsies. If you have questions about your medical care, please place a telephone consult through the appointment line (701-723-5633) to our nurses to answer your questions.

Appointments: Appointments can be made by calling 701-723-5633, M-F, 0700-1600. The appointment line is closed on federal holidays and base down days. For after-hours non-emergency care, call 701-723-5627 to be triaged by the on call provider and/or to seek authorization for care off-base. PRP and 2992 holders should not use the Nurse Advice Line (NAL). All clinic services are by appointment only, unless otherwise specified. You may also book an appointment with your assigned PCM at www.tricareonline.com. Flight Medicine Return to Status clinic:

Members who need to return to status (2992 holders) can return to the clinic M-F 0730-0800 and 1300-1330. Members will be seen in order of arrival.

Fast track clinic:

Members who responded “yes” to the unit’s 10 questions and are sent by their CO/PRP monitor can use the fast track clinic for PRP dispositions. Members will be seen by a tech between the hours of 0730-1530 and will be dispositioned by a Competent Medical Authority in order of arrival.

Public Health The Public Health mission is to prevent disease, disability and premature death. Public Health has three elements: Force Health Management, Community Health, and Medical Standards.

Bioenvironmental Engineering The Bioenvironmental Engineering flight’s primary duties are promoting Occupational & Environmental Health (OEH) and Chemical, Biological, Radiological & Nuclear (CBRN) and HAZMAT response readiness. The OEH mission is accomplished with the application of Industrial Hygiene concepts to identify, quantify, control and track personnel’s exposures to chemical, biological, radiation, and physical hazards in 84 high-risk industrial work centers across two wings. The findings of the OEH surveys are instrumental in efficiently directing medical surveillance exams for workers. We also support Team Minot’s mission by providing rapid and reliable detection capabilities as well as Health Risk Analysis (HRA) for a wide range of CBRN/HAZMAT contingencies to protect personnel and assets throughout an 8,500 square mile area and conduct vulnerability assessments to our water supply and of potential threats from Toxic Industrial Chemicals/Materials present in our area.

Gas mask/respirator fit test hours are Monday 0800-1600, Wednesday 1300-1600, Friday 0800-1100.

ANCILLARY SERVICES

Pharmacy Pharmacy services are available on the 1st floor of the clinic. These services include the filling and refilling of prescriptions for medications stocked by the clinic as well as consultation on the proper use, precautions, and side effects of medications. PHARMACY HOURS: Monday - Friday 0730-1630 Saturday & Sunday CLOSED Holidays, & Training Days CLOSED To refill a prescription, call 701-723-5052. For more information please call 701-723-5293. Radiology Our radiology department is located on the 1st floor and performs services via an authorized referral from your PCM. Routine procedures are provided on a walk-in basis. For more information, call 701-723-5316. Laboratory The laboratory is located on the 1st floor, near Radiology. Required tests are typically entered by your PCM. Walk-in pregnancy tests are offered daily during normal duty hours. For more information, call 701-723-5338.

Care Beyond the Capability of the 5 MDG: TRICARE beneficiaries, including Active Duty members, who do not call for an authorization prior to receiving urgent care services, may be billed under TRICARE Prime's point-of-service option. Always call for an authorization at the 5th Medical Group prior to seeking treatment at any urgent care facility. Depending on the results from your referred network provider, your PCM may contact you for additional instructions or make an appointment.
