TB Screening As of January 1, 2015, the California Education
code regarding TB screening has changed. School employees may now
undergo a TB Risk Assessment administered by a health care
professional instead of having a TB skin test. SCOE will not be
offering the Risk Assessment to school districts. School districts
with employees or volunteers who want to take advantage of this
option under the law will need to establish their own policies and
make arrangements with a health care provider of their choosing.
3
Slide 5
TB Screening The risk assessment is still required every four
years. When the risk assessment is completed by the health care
provider, they will sign it and provide a copy to the employee or
volunteer to give to the district office for their file. Staff who
have tested positive in the past and had a documented negative
chest x-ray on file must now be screened every four years and
questionnaire kept on file with their district office. The
questionnaire for people with a past history of a positive TB test
is different from the Risk Assessment. 4
Slide 6
TB Screening Contact your school nurse when you have staff due
for this type of screening. She will complete the questionnaire
with the employee at no cost to the district. SCOE will continue to
provide TB skin testing clinics for school districts who choose
that option. Dates, locations, & cost in Back to School Packet

about chronic conditions, medications, ongoing health concerns,
attendance problems, SSTs First Aid/CPR course recommended Nurse
will do trainings for health concerns when required 7
Slide 9
Medications Always call the school nurse when a student needs
to take medication at school and when the parent brings a refill,
even if it is the same medication they have been taking. Never give
a medication that has not been checked by the school nurse.
Medications to be given to the student during the school day must
have an order from a California licensed doctor, nurse practitioner
or physician assistant and a parent signature (this includes over
the counter medications such as Tylenol, ibuprofen, Benadryl,
lotions, ointment, eye drops). 8
Slide 10
More about Medications Medications must be in the original
labeled container. Although medications can be given during school
hours, we encourage parents to try to give medications at home when
possible. Secretaries or health clerks should make a copy of
medication orders when they arrive and put the copy in the students
health insert. 9

Immunizations Checking for required immunizations Go to
www.shotsforschool.orgwww.shotsforschool.org Click on k-12 or
Seventh Grade (depending on what you are looking for). All of the
information you need for kindergarten school entry, seventh grade
Tdap and transferring students is on this website. 15
Slide 17
Immunizations There is also a section on the website about
exemptions! Exemptions can be medical (Physicians note, temporary
or permanent) or personal beliefs. Beginning January 1, 2014, there
was a new law that went into effect regarding immunization
exemptions! Parent must receive information about the benefits and
risks of immunizations and the risks of vaccine-preventable
diseasesa perfect time for them to do this is when they have a
physical exam by their health care provider. They have the new
forms! here is the link to the form (just in case):
http://eziz.org/assets/docs/CDPH-8262.pdfhttp://eziz.org/assets/docs/CDPH-8262.pdf
it is also available in several languages New form signed by parent
and health care provider Form can be completed no sooner than 6
months before requirement Parents must still show a copy of
immunization record for immunizations that have been completed
Exemptions are immunization specificmust be listed on affidavit
16
Slide 18
Immunizations There is an exemption for religious beliefs on
the new form. Families who choose this option do not need the
signature of a health care provider. Signing the back of the CSIR
is no longer an option for an immunization waiver! If you still
have the blue CSIR cards with a space for an exemption on the back,
they are out of date. School districts must keep a list of students
without immunizations in case of an outbreak requiring exclusions.
Continue to fill out CSIR if child does have some immunizations.
17
Slide 19
Immunizations Conditional Admissions This issue is also
addressed on the website Sometimes children are in the process of
finishing their immunizations and can be admitted on the condition
that they complete their immunizations when they are due. This is a
conditional admission. Can get complicated, ask your school nurse
Requires ongoing follow-up Notices of Required Immunizations
18
Slide 20
Immunizations Reviewing CSIR or immunization record of incoming
students Let school nurses know about new students School nurse can
access PHD Immunization Registry if parent cannot find or access
record (it may be there). She can also do verification of disease
for chickenpox. 19
Slide 21
Immunizations Do not allow a child to enroll and begin
attending school until you either have an immunization record of a
signed exemption form for the child. Parents should have a copy of
their childs immunizations. This applies to not only students
enrolling in kindergarten, but also students in other grades
transferring from other schools. Once the student begins attending
without immunizations, it may be difficult to get the family to
either follow up with immunizations or provide a record. 20
Slide 22
Immunizations Kindergarten When parent picks up the K
Registration Packet, remind her/him that their child will probably
need four year shots and it should be done before the child is
registered and the record needs to accompany the registration
material. Information is in packet. When parent brings in
registration packet, an immunization record should be with it Check
the record while the parent is there. If immunizations are missing,
give them a completed Required Immunizations letter refer them back
to their health care provider. 21
Slide 23
Immunizations Kindergarten Emphasize that the child is not
officially registered until immunizations are completed. If you
allow the child to attend school, most likely you will have a long,
time-consuming process of bringing the child into compliance. Work
with your administrator & school nurse 22
Slide 24
Immunizations Kindergarten You have an additional step of
entering the immunizations on to the CSIR. Enter M/D/Y Update
immunization schedule in K Packet in May (on PHDs website) Forms
will be on website CSIR is also available on the immunization
website: www.shotsforschool.org 23

Immunizations Kindergarten Report of Kindergarten Immunizations
due to state October 15 th, so not much time to deal with
noncompliant students (read parents). State no longer sends you a
hardcopy. Will send you a letter with instructions on doing report
online Make a copy of report before you forward it to the state or
you may lose it (cannot be saved). 24

7 th grade Tdap (whooping cough) For the 2012-2013 school year,
and all future school years, all students entering, advancing or
transferring into 7th grade will need proof of an adolescent
whooping cough booster immunization (called Tdap) for school in the
fall. This new school immunization law affects all students in
public and private schools.new school immunization law For
deadlines and instructions on filing the report for this
requirement, see this website: http://www.shotsforschool.org/
http://www.shotsforschool.org/ 25
Slide 27
7 th Grade Tdap (continued) As with all required immunizations,
it is very important to enforce this requirement. No student should
be allowed to attend 7 th grade unless this requirement is met.
There is no grace period and no conditional admission for this
requirement. Parents who want to sign a waiver must follow the same
procedure as for any other immunizations. Note: This is the
immunization requirement that all of the schools were audited for
in Spring of 2012 by the CDE. Apportionment may be adjusted if a
student was attending school without meeting the requirement.

First Grade Physical (CHDP) Information and physical form are
in K Packet (Also, will be on SCOE HSD website), but not needed
until 1 st grade. Physical can be within 18 months of starting
first grade. Required to have physical or waiver signed within 90
days of starting 1 st grade (before Thanksgiving break) Record CHDP
exam in students Health Insert. 27
Slide 29
First Grade Physical (CHDP) Suggested timeline for implementing
mandate: First notification in K Packet Track with immunizations
Fall for FIRST Graders: Letter #1 with physical form start of
school Letter #2 with physical form and waiver beginning of Oct.
Letter #1 with physical form in May for kindergartners Some schools
have chosen to make this a kindergarten requirement (which works
well since children usually see their health care provider for
immunizations before beginning school anyway) Visit this website
for more details on this mandate:

Other things to remember Schools that have AEDs: Someone at
your school site should be responsible for checking the batteries,
providing maintenance and logging the checks monthly or as per your
school policy. Schools with Epi-Pens: As of January 1, 2015, ALL
schools are required have an unassigned epinephrine auto-injector
program. Someone at your school site should be responsible for
checking the Epi-Pens and logging the checks monthly. 30
