    OSH STANDARDSRule 1053.01

    All work accidents or occupational illnesses in

    places of employment resulting in disablingcondition or dangerous occurrence as definedin Rule 1053.2 shall be reported by the

    employer to the Regional Labor Office or dulyauthorized representative using theprescribed Form (DOLE/BWC/HSD-IP-6 ) .

    Assignment File is maintained by the

    Primary HSO.

    Primary HSO shall be responsible for

    ensuring the integrity and preservation ofevidence.

    All original written and signed statements by material.

    All original audio/visual tapes of interviews andtranscripts of the same.
