1 Introduction

These Electrical Safety Rules are intended for the use of such employees of PDO and its contractors who may be concerned with the operation of, or work on, PDO HIGH VOLTAGE and associated LOW VOLTAGE electrical systems. The Rules are additional to the generally applicable safety requirements of the Company, specifically the Permit-to-Work System (PR-1172) as operated by the Company, together with the PDO HSE Manual.

The application of these Rules is mandatory.

The authority for revision of these Rules rests with the Corporate Functional Discipline Head Electrical.

All copies of these Rules shall be issued in accordance with PR-1948, ESOP 1.

The Rules shall not apply to three & single phase LOW VOLTAGE APPARATUS inside workshops, laboratories, commercial, domestic and similar buildings or to portable electric tools, all of which are not considered to be part of PDO's electrical systems which come within the scope of these Rules.

The Rules shall not be applied to batteries for all purposes, instrument or telecommunication circuits operating at up to 110 volts DC and AC, nor to radio frequency, or low energy high voltages as found in cathode ray tubes, radar, air-conditioning systems/plants and cathodic protection systems.

However the principles of the safe working methods contained in subsection 5.4 of these Rules shall be followed, where practicable, when working on any LOW VOLTAGE circuits within the PDO concessions. ELECTRICAL SAFETY OPERATIONAL PROCEDURES (ESOP) PR–1948 (previously titled as SP-1108) are issued by the Corporate Functional Discipline Head Electrical to supplement these Rules and to define policies that assist in their implementation.

All copies of these PROCEDURES shall be issued in accordance with ESOP 1.

The ESRs forbid unauthorised staff to execute any electrical operational activity

It is however acceptable that:-

1. Unauthorised contractor staff carry out job-related-tasks, for which no electrical

authorisation is required, whilst under the direct supervision of authorised staff.

2. Contractor's equipment is used to execute the job.

Such equipment shall comply with the appropriate PDO HSE regulations.

2 Glossary

Electrical Definitions

For the purpose of these Electrical Safety Rules, the following definitions shall apply and where used shall be printed in capital letters. The definitions have been grouped into categories dealing with specific aspects of electrical safety

2.1.1 General Terminology

APPROVED

Of a type or method sanctioned for use within PDO.

(See Appendix A for method, device, reference document or stock reference)

DANGER

A risk of bodily injury or loss of life or health from shock, burn, asphyxiation or other causes.

DEAD

At or about zero potential and disconnected from any LIVE system.

EARTH, EARTHED or EARTHING

Connecting to or connected to the general mass of earth in such a manner as will ensure at all times that the body being earthed discharges immediately any electrical energy stored in it and furthermore is maintained at a safe potential with respect to the general mass of earth

When applied to conductors: all phases short circuited and effectively connected to earth either through switchgear with adequately rated EARTHING capacity or by APPROVED portable EARTHING leads or by any other APPROVED devices.

EXTRA LOW VOLTAGE (ELV) APPARATUS

Any apparatus, equipment and conductors which are normally operated at a voltage not exceeding 50V AC.

HIGH VOLTAGE (HV) APPARATUS

Any apparatus, equipment and conductors which are normally operated at a voltage exceeding 1000V AC.

ISOLATED OR ISOLATION

Disconnected from LIVE APPARATUS and conductors by an open disconnector or by an adequate physical gap.

LIVE

Electrically charged.

LIVE LINE WASHING

Washing of LIVE HIGH VOLTAGE insulators on overhead lines and substations with exposed conductors using APPROVED equipment and methods.

LIVE LINE WORK

Work on LIVE HIGH VOLTAGE overhead exposed conductors and other APPARATUS using APPROVED insulated tools and methods.

LOW VOLTAGE (LV) APPARATUS

Any apparatus, equipment and conductors which are not EXTRA LOW VOLTAGE APPARATUS and which normally operate at a voltage not exceeding 1000V AC.

SAFETY DISTANCE

The distance from the nearest LIVE HIGH VOLTAGE exposed conductor or from an insulator supporting a HIGH VOLTAGE conductor which shall be maintained to avoid DANGER.

SWITCHING

The operation of circuit breakers, isolators, disconnectors, fuses or other methods of making or breaking an electrical circuit and/or the application and removal of CIRCUIT MAIN EARTHS and LV EARTHS.

WORK IN A CONFINED SPACE

Work in an enclosed area where there is a possibility of concentrations of toxic, flammable or hazardous gases or where the oxygen content of the atmosphere could be reduced below 20% by volume.

WORKING PARTY

The persons under the direct supervision of a COMPETENT ELECTRICAL PERSON or an AUTHORISED ELECTRICAL PERSON.

2.1.2 Documentation, Permits, Notices & Procedures

ELECTRICAL SAFETY OPERATIONAL PROCEDURES

Instructions that are issued by the CFDH-E to supplement these Rules and to define policies that assist in their implementation.

i) ELECTRICAL SAFETY DOCUMENTS,

ii) ELECTRICAL PERMIT-TO-WORK, SANCTIONS-FOR-TEST, LIMITATIONS-OF-

ACCESS and

iii) ELECTRICAL ISOLATION/DE-ISOLATION CERTIFICATES

are all ELECTRICAL SAFETY DOCUMENTS.

CAUTION NOTICE

A notice in an APPROVED form attached to electrical APPARATUS or its controls conveying a warning against interference with such APPARATUS. These notices shall be attached to all points of ISOLATION from which APPARATUS can be made LIVE.

CERTIFICATE OF APPOINTMENT

A form of identification issued by the CFDH-E to appoint PDO (and non-PDO) staff to a level of electrical authorisation. (see Appendix G)

DANGER NOTICE

A notice in an APPROVED form reading 'DANGER' with any additional wording calling attention to the DANGER of approaching or interfering with such LIVE APPARATUS.

EARTHED NOTICE

A notice in an APPROVED form reading 'EARTHED' that shall be used to identify APPARATUS that has been used to apply a CIRCUIT MAIN EARTH to a circuit

ELECTRICAL IMMOBILISATION / MOBILISATION CERTIFICATE

A form of instruction signed and issued by a responsible member of a Custodian Department to a suitably authorised APPOINTED PERSON to ensure the safe immobilisation of selected LV electrical motor circuits prior to non-electrical work by a third party. (See Appendix E-3).

ELECTRICAL ISOLATION/DE-ISOLATION CERTIFICATE

A form of instruction signed and issued by a responsible member of a Custodian Department to a suitably authorised ELECTRICAL PERSON to ensure the safe ISOLATION of electrical circuits prior to non-electrical work by a third party. (See Appendix E-1).

ELECTRICAL ISOLATION/DE-ISOLATION CERTIFICATE

(allowing restoration of motive power for on-load checks to be made)

A form of instruction signed and issued by a responsible member of a Custodian Department to a suitably authorised ELECTRICAL PERSON to ensure the safe ISOLATION of electrical circuits prior to non-electrical work and on-load checks by a third party.

This CERTIFICATE shall allow the restoration of motive power, under the direct supervision of an ELECTRICAL PERSON, for on-load checks to be made by the third party. (See Appendix E-2).

ELECTRICAL PERMIT-TO-WORK (EPTW)

A form of declaration signed and issued by an AUTHORISED ELECTRICAL PERSON to a COMPETENT ELECTRICAL PERSON in charge of work to be carried out on any APPARATUS, for the purpose of making known to such person exactly what HIGH VOLTAGE APPARATUS is DEAD, ISOLATED from all LIVE conductors, has been discharged, connected to EARTH and on which it is safe to work. (see Appendix B).

An ELECTRICAL PERMIT-TO-WORK shall also be issued to a person in charge of work on LOW VOLTAGE APPARATUS when either it is necessary to:-

a) apply a CIRCUIT MAIN EARTH to the HIGH VOLTAGE side of a transformer feeding the LOW VOLTAGE APPARATUS

or

b) apply an LV EARTH to the busbars of a LOW VOLTAGE switchboard.

EXCAVATION CERTIFICATE

A form of declaration endorsed by all relevant service departments and issued under the Permit-to-Work system to ensure that all underground services and power cables are identified and safe methods of excavation are followed.

LIMITATION-OF-ACCESS (LOA)

A form issued and canceled by an AUTHORISED ELECTRICAL PERSON to a COMPETENT ELECTRICAL PERSON defining the limits and nature of work which will be carried out in the vicinity of LIVE APPARATUS. A LIMITATION-OF-ACCESS shall be issued when an ELECTRICAL PERMIT-TO-WORK or SANCTION-FOR-TEST is not applicable and when verbal instructions are not considered sufficient. (see Appendix D).

A LIMITATION-OF-ACCESS shall also be issued to a person in charge of work on LIVE HIGH VOLTAGE conductors or insulators supporting LIVE HIGH VOLTAGE conductors when LIVE LINE WORKING or LIVE LINE WASHING procedures are to be used.

PROCEDURES FOR APPOINTED PERSONS

An APPROVED procedure describing activities which can be carried out by an APPOINTED PERSON.

SANCTION-FOR-TEST (SFT)

A form of declaration signed and issued by a SENIOR AUTHORISED ELECTRICAL PERSON to an AUTHORISED ELECTRICAL PERSON in charge of testing HIGH VOLTAGE APPARATUS for the purpose of making known to such person exactly what APPARATUS is to be tested and the condition under which testing is to be performed. (see Appendix C).

SWITCHGEAR OPERATING PROCEDURES (SOP)

An APPROVED procedure detailing how to operate switchgear and how to make it safe to work on. The procedure checked and tested by consruction engineer and approved by document custodian and document author before up loading to live link.

SWITCHING PROGRAMME

An APPROVED schedule which lists in sequence the SWITCHING operations necessary to achieve a defined purpose on the system.

This schedule may include ISOLATION and EARTHING of HIGH VOLTAGE APPARATUS to make it safe prior to the issue of an ELECTRICAL PERMIT-TO-WORK or SANCTION-FOR-TEST.

WORK PERMIT

(The class A or B Permit-to-Work)

A form of declaration signed and issued by the custodian of equipment or plant to the person in charge (Permit Holder) of a working party, giving clearance to work on the specified equipment

or plant, detailing restrictions and precautions required to carry out the specified work safely. The permit shall state which other certificates or permits are required prior to starting work (eg. , ELECTRICAL PERMIT-TO-WORK).

2.1.3 Safety Equipment

ADDITIONAL EARTH

An earth of APPROVED type which is applied after the issue of an ELECTRICAL PERMIT-TO-WORK or SANCTION-FOR-TEST (for example an EARTH applied at a point of work).

These shall be attached or removed by (as a minimum) a COMPETENT ELECTRICAL PERSON holding the ELECTRICAL PERMIT-TO-WORK. When portable EARTHING leads are used for this purpose, an APPROVED label shall be attached with the legend 'AE'.

CIRCUIT MAIN EARTH

An earth applied by an AUTHORISED ELECTRICAL PERSON prior to issue of an ELECTRICAL PERMIT-TO-WORK or SANCTION-FOR-TEST. When portable EARTHING leads are used for this purpose, an APPROVED label shall be attached with the legend 'CME'.

KEY SAFE

An APPROVED device for the secure retention of all SAFETY PADLOCK keys used to lock means of ISOLATION, EARTHING or other safety devices in accordance with these Rules.

LV EARTH

An earth of an APPROVED design that may be used on LOW VOLTAGE APPARATUS when it is worked on under an ELECTRICAL PERMIT-TO-WORK.

SAFETY PADLOCKS

Locks of an APPROVED type with a unique key, that shall be used to lock switches, etc, at points of ISOLATION where the APPARATUS being worked on could be made LIVE in error. Only one key shall be supplied for each lock.

STANDARD ELECTRICAL PADLOCKS

Locks of an APPROVED standard type fitted to prevent access by unauthorised persons to power stations, substations or electrical

APPARATUS not subject to an ELECTRICAL SAFETY DOCUMENT. This type of lock shall fit the ‘electrical’ master key system.

2.1.4 Persons

Refer to ESOP-3 (PR-1948) for procedures on selection, training, assessment and appointment of Electrical Persons and Appointed Persons.

2.1.4.1 Electrical Person

A person (PDO or non-PDO) who has been appointed by the CFDH-E to any of the electrical authorisation levels defined below in Rules 2.1.4.2/3/4/5/6. ELECTRICAL PERSONS shall be subject to the licensing requirements of the Permit-to-Work system as operated by PDO.

2.1.4.2 Competent Electrical Person (PDO Staff) - CEP

A person who has sufficient technical knowledge or experience to avoid DANGER and has been appointed by the CFDH-E to receive and complete ELECTRICAL ISOLATION/DE-ISOLATION CERTIFICATES and receive ELECTRICAL PERMITS-TO-WORK and LIMITATIONS-OF-ACCESS.

This person may carry out LV SWITCHING, but only on single circuits with no possibilities of backfeeds.

The CERTIFICATE OF APPOINTMENT shall state the section of the electrical system to which it applies

2.1.4.3 Competent Electrical Switching Person (PDO Staff) - CESP

A COMPETENT ELECTRICAL PERSON who has been appointed in writing by the CFDH-E to carry out LV SWITCHING, including LV switchboards with possible backfeeds from main incomers or bus couplers, but excluding LV switchboards with incomers from the HV system.

For Electrical Technician should have complete at least 6 month period of CEP

The CERTIFICATE OF APPOINTMENT shall state the section of the electrical system to which it applies

2.1.4.4 Authorised Electrical Person (PDO Staff)- AEP

A COMPETENT ELECTRICAL SWITCHING PERSON who has been appointed in writing by the CFDH-E to carry out HV and LV SWITCHING, issue and cancellation of ELECTRICAL PERMITS-TO-WORK, LIMITATIONS-OF-ACCESS and receive SANCTIONS-FOR-TEST.

Electrical Technision should have completed one year period of CESP experiance and minimum of 10 switching program.

Electrical Engineer who have one year period of CEP experiance and minimum of10 switching program

Electrical Sumesible Pump Engineer approved and certified by ESP company can be appointed derect to AEP for defined electrical boundaries.

The CERTIFICATE OF APPOINTMENT shall state the section of the electrical system to which it applies

2.1.4.5 Senior Authorised Electrical Person (PDO Staff)-SAEP

An AUTHORISED ELECTRICAL PERSON who has been appointed in writing by the CFDH-E to carry out HV and LV SWITCHING, the issue and cancellation of ELECTRICAL PERMITS-TO-WORK, SANCTIONS-FOR-TEST and LIMITATIONS-OF-ACCESS.

The CERTIFICATE OF APPOINTMENT shall state the section of the electrical system to which it applies.

It may also include carrying out the duties of a CONTROL PERSON for a specified section of the electrical system

2.1.4.6 Control Person (PDO Staff)-CP

A SENIOR AUTHORISED ELECTRICAL PERSON who has been appointed in writing by the CFDH-E to manage a section of the electrical system within defined electrical boundaries.

The position of CONTROL PERSON can be either of the following:

a) The person operating from a control room specially equipped with system control facilities (particularly applicable to the operation of powerstations)

b) The most senior ELECTRICAL PERSON in a PDO Area (particularly applicable to Area Operations Electrical Supervisor positions)

or

c) Heads of Utility

The CERTIFICATE OF APPOINTMENT shall state the section of the electrical system for which the appointment applies.

2.1.4.7 Non-PDO Staff Electrical Persons

It shall be possible for contract staff to be authorised for any of the authorisation levels as defined in Rules 2.1.4.2/3/4/5/6.

The contractor shall nominate such staff in writing to PDO as having sufficient technical knowledge or experience, commensurate with the level of authorisation requested, to carry out the duties as defined in Rules 2.1.4.2/3/4/5/6 (whichever is applicable) in a safe and responsible manner.

The CERTIFICATE OF APPOINTMENT shall state that the authorisation shall only be valid for the purposes of commissioning and/or maintenance of APPARATUS and associated SWITCHING on PDO electrical systems as defined in the contract scope of work or within the EPC contract area

2.1.4.8 Appointed Person (PDO and Non-PDO Staff)

A person nominated by a PDO production unit or facility and appointed in writing by the CFDH-E to carry out specified activities, which may include limited SWITCHING in his unit.

The CERTIFICATE OF APPOINTMENT shall state the activities and any limited items of SWITCHING this person may carry out

3. General Requirements

3.1 DUTY

It is the duty of all persons, including contractors and sub-contractors employed by PDO, who work on the electrical systems within PDO areas, to make themselves thoroughly conversant with and to conduct their work in accordance with all Rules governing the operation or work they have to undertake on such systems.

Furthermore, it is the duty of these persons to use any means or appliance provided by PDO for securing safety and also not willfully to do anything likely to endanger themselves or others.

Contravention of these Rules shall constitute misconduct. Any person (whether PDO or non-

PDO staff) who contravenes these Rules shall be liable to dismissal or be otherwise dealt with in accordance with Personnel Disciplinary procedures.

3.2 OBJECTIONS

Any person objecting on operational or safety grounds to any instructions given to him regarding the operation of HV or LV APPARATUS or work on electrical equipment shall make these objections clearly known to the person issuing them.

This person shall then have the matter investigated and if necessary referred to a higher authority for a decision before proceeding.

3.3 ISSUE OF ELECTRICAL SAFETY RULES

A copy of these Rules shall be issued to PDO employees and to employees of PDO contractors and sub-contractors, involved in the operation of or work on PDO electrical systems. Each employee shall sign a receipt for their copy which shall be kept in good condition. The method of issue shall be in accordance with ESOP-1.

3.4 DANGEROUS OCCURRENCES AND ACCIDENTS

All dangerous occurrences and electrical accidents shall be reported immediately to a CP who shall report in accordance with an APPROVED procedure.

3.5 FAILURE OF SUPPLY

A failure of supply, from whatever cause, to or from any part of the HV systems shall be reported immediately to the CP for the section affected.

The CP shall report any serious interruption involving a large part of the facilities or an important single consumer in accordance with procedure PR – 1353 ( Follow- up procedure for Electrical Faults and Equipment Failure – PR -1353 ).

During failure of supply, all APPARATUS, conductors and equipment shall be considered LIVE, until ISOLATED and proved DEAD.

3.6 ADMITTANCE TO STATIONS

No person shall, without authority, enter any power station or substation premises belonging to or under the control of PDO. All such stations shall be kept secured with STANDARD ELECTRICAL LOCKS when unattended.

4. Authorisation of Electrical Persons and Appointed Persons

4.1 GENERAL

The authorisation of staff to SAEP, AEP, CESP, CEP, CP and AP operating and working under the scope of these Rules shall follow the definitions given in Rule 2.1.4.

4.2 APPOINTMENT

The method of training, assessing and subsequent appointing of such PERSONS is detailed in ESOP 3.

4.3 CERTIFICATE OF APPOINTMENT

4.3.1 A CofA shall be given to all ELECTRICAL PERSONS and to all APPOINTED PERSONS.

The CofA shall state the main activities these persons may carry out on PDO electrical systems, and the PDO Area to which it applies.

4.3.2 Another CofA shall be given to SAEP's who are to undertake the role of CP for a section of the electrical system within defined electrical boundaries in a specific PDO Area.

5. Procedures for Initiation of Work

5.1 ACCESS

5.1.1 Access To High Voltage Enclosures And Apparatus

Barriers (including plates on access ladders) restricting access to enclosures or cubicles containing LIVE HV conductors shall normally be kept locked. No person except an AEP or a person acting under his direct supervion shall have access to any such enclosure or cubicle in which a LIVE conductor is exposed.

All spout or busbar shutters shall be locked shut. In case of possible backfeed also the feeder shutters shall be locked shut. If the design of switch gear does not have this locking facility, other precautions to prevent DANGER shall be taken (e.g. padlocking of cubicle doors).

NOTE: Possible backfeed exists in case of ring circuit and multiple radial feeders!

5.1.2 Access to Underground Chambers or Cellars

In accordance with PDO policy a WORK PERMIT is always required for WORK IN A CONFINED SPACE. No person shall therefore enter any underground chamber or cellar until the requirements of the Permit-to-Work System for entry into a CONFINED SPACE have been implemented.

5.1.3 Climbing of Towers and Gantries

Gates and devices to prevent climbing of towers and gantries supporting HV conductors shall always be kept locked except when opened by an AEP under an ELECTRICAL PERMIT-TO-WORK, SANCTION-FOR-TEST or LIMITATION-OF-ACCESS.

5.1.4 Climbing on Wood Poles

Before any pole is climbed, it shall be sounded. No pole badly impaired by decay or damage shall be climbed until the person in charge is satisfied that it has been made safe.

No person shall work upon any pole or tower without making proper use of his APPROVED safety harness, which he shall examine to see that it is in good order, on each occasion before use.

No person shall climb any pole supporting LIVE HV conductors except in accordance with Rule 7.1.4.

5.2 SWITCHING PROCEDURES

5.2.1 General

5.2.1.1 No HV SWITCHING shall be carried out without the agreement of the CP responsible for that section of the electrical system.

5.2.1.2 In normal circumstances all HV SWITCHING shall be carried out only after a written APPROVED SWITCHING PROGRAMME has been agreed.

SWITCHING PROGRAMMES need not normally be prepared for SWITCHING on HV motor circuits unless required by the CP.

5.2.1.3 When the CP gives agreement for HV SWITCHING to be carried out he shall, if practicable, communicate directly with the person who is to carry out the SWITCHING.

SWITCHING to the CP's instructions, or with his consent, shall be carried out without undue delay.

All HV SWITCHING shall be reported to the CP as soon as possible after each operation or at an agreed stage of a SWITCHING PROGRAMME.

5.2.1.4 The remote opening and closing of HV circuit breakers may be carried out by unaccompanied suitably authorised operators.

All other HV SWITCHING shall be carried out by an AEP who shall be accompanied during the SWITCHING operations. (See also Rule 5.5.2.2 when applying CIRCUIT MAIN EARTHS).

5.2.1.5 When operating pole mounted switchgear by mechanical means from ground level, the operator shall wear rubber gloves of an APPROVED type and an APPROVED safety helmet.

5.2.1.6 Making LIVE or DEAD by signal or prearranged understanding after an agreed time interval is forbidden.

5.2.1.7 When switchgear or any other HV APPARATUS shows any sign of distress, its condition shall be reported immediately to the CP and it shall be examined before a decision is made about further operation.

5.2.2 Switching Across Electrical Boundaries

Before SWITCHING is carried out on any system which may affect other electrical systems, the CP agreeing the SWITCHING, shall communicate with the CP of the other system(s) concerned and the SWITCHING shall be recorded by all parties.

When telephone or radio messages are being used between CP's to co-ordinate SWITCHING PROGRAMMES between different specified sections of electrical systems, each message shall be repeated in full and written down before SWITCHING is performed and each actual operation shall be reported to the CP at the other end before the next operation can be executed.

5.2.3 Switching Under Emergency Conditions

In all cases when emergency SWITCHING has been carried out to avoid DANGER to persons or APPARATUS, the CP for the electrical system concerned shall be informed as soon as possible of any action taken. The CP shall report in accordance with an APPROVED procedure.

5.3 PRECAUTIONS TO BE TAKEN BEFORE WORKING ON OR TESTING HIGH

VOLTAGE APPARATUS

No person shall undertake any repairs, maintenance, alterations, cleaning or testing on HV APPARATUS until such parts of that APPARATUS are:-

(a) Switched off from all points of supply, made DEAD and proven DEAD by checking each phase of the circuit in an approved manner (refer ESOP 5).

(b) ISOLATED and all steps taken to secure with SAFETY PADLOCKS from all points of supply, including where practicable voltage and auxiliary transformers, common neutral equipment and any other source from which the APPARATUS and conductors may become LIVE and CAUTION NOTICES attached at all points of ISOLATION. The keys for the SAFETY PADLOCKS shall be placed in a KEY SAFE or another APPROVED security method shall be used.

(c) Connected to EARTH at all points of disconnection from HV supplies to such APPARATUS or between such points and the points of work or testing.

(d) Screened where necessary to prevent DANGER and DANGER NOTICES attached.

(e) Released for work by the issue of an ELECTRICAL PERMIT-TO-WORK to a CEP who shall be accompanied by another CEP during the progress of the work.

or

(f) Released for testing by the issue of a SANCTION-FOR-TEST to an AEP who shall be accompanied by another AEP or CEP during the progress of the testing.

If an HV testing supply is to be used then Rule 5.7.2 shall also apply.

and

(g) The person receiving the ELECTRICAL PERMIT-TO-WORK or SANCTION-FOR-TEST is made fully aware of the nature and extent of the work or testing to be done.

It is the duty of the person issuing the ELECTRICAL PERMIT-TO-WORK or SANCTION-FOR-TEST to ensure that these provisions are complied with.

Any work on LIVE HV conductors or insulators shall be carried out under the LIVE LINE WORKING Rules in Section 8.0 or LIVE LINE WASHING Rules in Section 9.0.

5.4 PRECAUTIONS TO BE TAKEN BEFORE WORKING ON OR TESTING LOW

VOLTAGE APPARATUS

5.4.1 No person shall carry out any repairs, maintenance, alterations or cleaning on LV APPARATUS until such parts of that APPARATUS are:-

(a) Switched off from all possible points of supply and made DEAD.

(b) ISOLATED from all possible points of supply. The ISOLATION shall normally be carried out by opening circuit breakers/switches and by removing fuses or links.

The operating handles of these circuit breakers/switches shall have been, where practicable, locked in the 'open' position with SAFETY PADLOCKS or similar type locking devices and CAUTION NOTICES attached at all points of ISOLATION. (see NOTE).

(c) Screened where necessary to prevent DANGER, and DANGER NOTICES attached to draw attention to LIVE APPARATUS near to the point of work.

(d) Proved DEAD with an APPROVED voltage testing device suitable for the rated voltage of the circuit. The voltage testing device shall be checked before and after use to check it is working correctly.

(e) EARTHED, where necessary, by an AEP, with LV EARTHS that have been applied in accordance with Rule 5.5.3 to ISOLATED LV conductors.

(f) Released for work to the CEP when the above precautions are complete.

and

(g) The CEP who is to be in charge of the work, is made fully aware of the nature and extent of the work to be done, and understands the precautions that he and his WORKING PARTY shall take to avoid DANGER from LIVE APPARATUS.

NOTE: If the APPARATUS at the point of ISOLATION is not provided with suitable means of locking or if SAFETY PADLOCKS cannot be applied, and the circuit cannot be ISOLATED by the withdrawal of fuses or links, then other suitable precautions shall be taken to avoid DANGER to person whilst working on the circuit.

5.4.2 An ELECTRICAL PERMIT-TO-WORK shall be issued to the CEP in charge of the work on LV APPARATUS and conductors when it has been necessary to apply a CIRCUIT MAIN EARTH to the ISOLATED HV windings of the transformer feeding the LV windings and connections (including incoming switchboard units) or when an LV EARTH is applied to the LV switchboard busbars.

When such an ELECTRICAL PERMIT-TO-WORK is issued to a CEP, he shall be accompanied by another CEP during the progress of the work.

5.4.3 Notwithstanding the requirements of Rule 5.4.1, essential testing or adjustments may be made with LV APPARATUS LIVE, provided that safe methods are used and APPROVED equipment or instruments required for testing are properly insulated for the rated voltage of the circuit.

If it is necessary for safety reasons, the CEP carrying out testing or adjustments shall wear APPROVED electrical rubber gloves.

If the testing or adjustments of LV APPARATUS requires covers to be removed so that LIVE terminals, windings or connections are exposed, then suitable precautions shall be taken to prevent persons coming into accidental contact or interfering with that APPARATUS.

The CEP in charge of the testing or adjustments shall be accompanied during the progress of the testing or adjustments.

5.5 EARTHING

5.5.1 General

No HV EARTHING switch shall be operated or CIRCUIT MAIN EARTH connected or removed except with the agreement of the CP.

5.5.2 High Voltage Earthing

5.5.2.1 All phases of the HV APPARATUS shall be tested with an APPROVED voltage testing device to ensure that they are DEAD.

For HV APPARATUS, EARTHING switches shall be used where provided.

Where EARTHING switches are not provided, then other APPROVED means of EARTHING shall be adopted including:-

(a) A circuit breaker be used to make the EARTH connection. The circuit breaker shall normally have its trip feature made inoperative before closing to EARTH, unless this is not practicable, when it shall be done afterwards. After closing, the circuit breaker shall be locked in the EARTH position. An EARTHED NOTICE shall be attached on the circuit breaker when it is in the EARTH position.

or

(b) Applying APPROVED portable EARTHING leads by an insulating pole or other APPROVED equipment to all phase conductors.

If this method of using portable EARTHING leads is to be used, the following sequence of procedures shall be carried.out:-

(i) Check that the circuit to be EARTHED is ISOLATED.

(ii) Ensure that each phase of the circuit is DEAD (only an HV testing device suitable for the rated voltage of the circuit shall be used).

The APPROVED voltage testing device shall be checked before and after use to check it is functioning correctly.

(iii) Examine the portable EARTHING leads prior to use to ensure that they are in good condition.

(iv) Connect the portable EARTHING leads to the EARTH system before securing them to the phase conductors. Care must be taken to ensure that good contact is made.

(v) EARTH and short circuit all phases even if work is to be carried out on one phase only.

and

(vi) When removing portable EARTHING leads disconnect them from the phase conductors first and the EARTH system last.

NOTE: 1 When portable EARTHING leads are used as CIRCUIT MAIN

EARTHS, an APPROVED label shall be attached with the letters 'C.M.E'.

2 When portable EARTHING leads are used as ADDITIONAL EARTHS, an APPROVED label shall be attached with the letters 'A.E'.

5.5.2.2 ACCOMPANIED SWITCHING

If a CIRCUIT MAIN EARTH is to be applied using portable EARTHING leads in accordance with 5.5.2.1(b), then the AEP carrying out this SWITCHING shall be accompanied by a CEP.

5.5.3 Low Voltage Earthing

5.5.3.1 Any LV EARTH applied on DEAD LV switchboards shall be in accordance with Rule 5.5.3.2 and their application shall be recorded in the 'Other Precautions Section' of the relevant ELECTRICAL PERMIT-TO-WORK.

5.5.3.2 For LV APPARATUS only APPROVED portable EARTHING devices shall be used as LV EARTHS. If these EARTHING devices are used then all phases and neutral of the LV APPARATUS shall first be tested with an APPROVED voltage testing device. (Only an LV testing device shall be used). The voltage testing device shall be checked before and after use.

5.5.3.3 The portable EARTHING devices shall then be applied in a similar manner to the sequence of procedures specified in Rule 5.5.2.1b(i), (iii) (iv), (v) and (vi).

5.6 APPROACH TO EXPOSED LIVE HIGH VOLTAGE CONDUCTORS OR

INSULATORS SUPPORTING THEM

5.6.1 Safety Distances

(a) The SAFETY DISTANCE (designated 'X' in Table 1 and Diagram 1 of page 26&27) shall be maintained at the respective system

voltages between any part of the person or object and the nearest LIVE HV conductor.

(b) A distance of 300 mm shall also be maintained, at all system voltages, from the portion of insulators supporting LIVE HV conductors which is outside the appropriate SAFETY DISTANCE from the conductors.

5.6.2 Approach of Persons

(a) No person shall, except, for the provision of Rule 5.6.2(b), allow any part of his body to approach exposed HV CONDUCTORS or insulators supporting such conductors within the SAFETY DISTANCES specified in Rule 5.6.1, unless the conductors have been made DEAD, ISOLATED and EARTHED and an ELECTRICAL PERMIT-TO-WORK or SANCTION-FOR-TEST has been issued.

(b) When a person is applying an APPROVED voltage testing device or phasing device to HV conductors contained within metal enclosed switchgear, it is allowable for those parts of the body to approach within the SAFETY DISTANCES specified in Rule 5.6.1 subject to APPROVED procedures.

5.6.3 Testing of Conductors

(a) When exposed HV conductors are not ISOLATED, the only objects to approach them or insulators supporting them, within the SAFETY DISTANCE specified in Rule 5.6.1 shall be APPROVED voltage testing devices.

(b) When exposed HV conductors are ISOLATED but not yet proved DEAD the only objects which shall approach them or the insulators supporting them, within the SAFETY DISTANCES specified in Rule 5.6.1 shall be APPROVED voltage testing devices.

5.6.4 Work Near Live Conductors

(a) When work is to take place in the vicinity of LIVE HV conductors the AEP shall, taking into account the nature and location of the work and the tools or objects to be used, determine how the SAFETY DISTANCES specified in Rule 5.6.1 have to be maintained by the persons working.

(b) If no ELECTRICAL PERMIT-TO-WORK or SANCTION-FOR-TEST is issued for adjacent APPARATUS, then the AEP shall issue a LIMITATION-OF-ACCESS to the CEP in charge of the work, specifying how the SAFETY DISTANCES from LIVE conductors shall be maintained during the course of the work.

NOTE: APPENDIX F gives guidance on the application of Rule 5.6 to various operational situations.

5.7 TESTING OF HIGH VOLTAGE APPARATUS BEFORE IT IS CONNECTED TO

AN ELECTRICAL SYSTEM

5.7.1 When HV APPARATUS is to have an HV test voltage applied before the connections are attached that would enable it to be made LIVE from an electrical system, the AEP responsible for applying the HV test voltage does not require a SANCTION-FOR-TEST if the HV APPARATUS does not yet come within the scope of the Electrical Safety Rules.

5.7.2 The AEP shall ensure that when a HV test voltage is being applied to APPARATUS, adequate precautions are taken to guard the APPARATUS to prevent DANGER.

These precautions shall include:-

(a) DANGER NOTICES being attached to the APPARATUS during the period the HV testing is taking place.

(b) All APPARATUS, particularly cables, are discharged to EARTH before and after the application of the HV test voltage.

(c) Any temporary conductors used for applying the HV test voltage shall be of adequate size and be easily visible.

(d) Test connections shall not be applied in a cell or compartment in which there are exposed LIVE HV conductors.

When such testing using an HV testing voltage supply is carried out, the AEP shall be accompanied by a CEP.

6. Issue And Control Of Electrical Safety Documents

6.1 INITIATION OF DOCUMENTS

6.1.1 A WORK PERMIT shall always be issued prior to issuing an ELECTRICAL SAFETY DOCUMENT in accordance with the Permit to Work Manual. The issued ELECTRICAL SAFETY DOCUMENT(S) and the WORK PERMIT shall cross-refer.

6.1.2 An ELECTRICAL PERMIT-TO-WORK shall be agreed by the CP and issued by an ELECTRICAL PERSON, under whose approval the work is initiated, before any work is carried out on any HV APPARATUS after it has once been connected to the HV system.

An ELECTRICAL PERMIT-TO-WORK shall be agreed by the CP and issued at site by an ELECTRICAL PERSON before any work is carried out on any LV APPARATUS that requires a CIRCUIT MAIN EARTH to be applied to HV APPARATUS.

The maximum number of ELECTRICAL PERMIT-TO-WORK that is allowed to be issued by one SAEP/ AEP at any one time shall be no more than 10. If more ELECTRICAL PERMIT-TO-WORK are to be issued, then additional SAEP/ AEP has to be made available who shall also comply with the above rule

A SANCTION-FOR-TEST shall be agreed by the CP and issued by a SAEP under whose approval the testing is initiated, before any testing is carried out on any HV APPARATUS after it has once been connected to and remains part of an HV system.

If an ELECTRICAL PERMIT-TO-WORK has been issued on HV or LV APPARATUS, then until that ELECTRICAL PERMIT-TO-WORK is canceled, a SANCTION-FOR-TEST shall NOT be issued on that same APPARATUS.

A LIMITATION-OF-ACCESS shall be issued by an AEP when verbal instructions are not considered sufficient, particularly when work is to be undertaken in the close proximity of LIVE uninsulated HV or LV conductors.

A LIMITATION-OF-ACCESS shall be issued by an AEP to allow LIVE LINE WORKING or LIVE LINE WASHING procedures to take place. The CP shall be informed when a LIMITATION-OF-ACCESS for these specified activities is issued.

6.1.3 An ELECTRICAL ISOLATION/DE-ISOLATION CERTIFICATE shall be issued by a responsible member of a Custodian Department to a third party who is to carry out non-electrical work on APPARATUS that needs to be ISOLATED before work commences.

An ELECTRICAL ISOLATION/DE-ISOLATION CERTIFICATE (allowing restoration of motive power) shall be issued by a responsible member of a Custodian Department to a third party who is to carry out non-electrical work and subsequent on-load checks on APPARATUS that needs to be ISOLATED before work commences.

An ELECTRICAL IMMOBILISATION/MOBILISATION CERTIFICATE shall be issued by a responsible member of a Custodian Department to a third party who is to carry out non-electrical work on LV motor circuits that needs to be immobilised before work commences.

The ELECTRICAL IMMOBILISATION/MOBILISATION CERTIFICATE shall only be used by in case the procedure is carried out by a suitably authorised APPOINTED PERSON, in accordance with the procedure as listed in ESOP 16.

6.2 PROCEDURE FOR ISSUE AND RECEIPT

6.2.1 An ELECTRICAL PERMIT-TO-WORK, SANCTION-FOR-TEST or LIMITATION-OF-ACCESS shall be issued to the person in charge of the work, who after reading its contents to the person

issuing the ELECTRICAL PERMIT-TO-WORK, SANCTION-FOR-TEST or LIMITATION-OF-ACCESS shall sign its receipt and its duplicate.

The person to whom an ELECTRICAL PERMIT-TO-WORK or LIMITATION-OF-ACCESS is issued shall be a CEP, who shall retain the ELECTRICAL PERMIT-TO-WORK or LIMITATION-OF-ACCESS in his possession at all times, and remain on site, whilst work is being carried out.

The person in charge to whom a SANCTION-FOR-TEST is issued shall be an AEP, who shall retain the SANCTION-FOR-TEST in his possession at all times, and remain on site, whilst tests are being made.

Where more than one WORKING PARTY is concerned, an ELECTRICAL PERMIT-TO-WORK or LIMITATION-OF-ACCESS shall be issued to the CEP in charge of each WORKING PARTY.

The issue of ELECTRICAL PERMITS-TO-WORK, SANCTIONS-FOR-TEST, ELECTRICAL ISOLATION/DE-ISOLATION CERTIFICATES, IMMOBILISATION/MOBILISATION CERTIFICATES and LIMITATIONS-OF-ACCESS shall be logged in a book provided for that purpose in a substation, unless the number of the relevant SWITCHING PROGRAMME is entered.

6.2.2 An ELECTRICAL IMMOBILISATION/MOBILISATION CERTIFICATE shall be taken by the third party to a suitably authorised APPOINTED PERSON requesting the immobilisation of the LV motor circuit prior to the non-electrical work commencing on such circuit.

An ELECTRICAL ISOLATION/DE-ISOLATION CERTIFICATE shall be taken by the third party to a suitably authorised ELECTRICAL PERSON requesting the ISOLATION of the APPARATUS prior to the non-electrical work commencing on such APPARATUS.

An ELECTRICAL ISOLATION/DE-ISOLATION CERTIFICATE (allowing restoration of motive power) shall be taken by a third party to a suitably authorised ELECTRICAL PERSON requesting the ISOLATION of the APPARATUS prior to the non-electrical work and subsequent on-load checks commencing on such APPARATUS. The ELECTRICAL PERSON shall not accept this type of CERTIFICATE (allowing restoration of motive power) unless the responsible member of the Custodian Department has agreed, at the time of issue, that no other WORK PERMIT shall be issued on the APPARATUS until the CERTIFICATE is canceled and returned to the Custodian Department.

Permission to commence and carry out the ISOLATION or immobilisation of the APPARATUS, under either type of ELECTRICAL ISOLATION/DE-ISOLATION OR ELECTRICAL IMMOBILISATION / MOBILISATION CERTIFICATE, shall be given to the ELECTRICAL PERSON or APPOINTED PERSON by the Custodian Department's representative at the site of the APPARATUS.

6.3 PRECAUTIONS DURING PROGRESS OF WORK

6.3.1 HV APPARATUS ISOLATED and EARTHED for work under the terms of an ELECTRICAL PERMIT-TO-WORK shall remain so until the ELECTRICAL PERMIT-TO-WORK has been cleared, returned to an AEP and cancelled.

The APPARATUS ISOLATED and EARTHED for testing under the terms of a SANCTION-FOR-TEST shall not be connected to the system without the agreement of the CP. The recipient of the SANCTION-FOR-TEST shall be responsible for coordinating all testing operations on the ISOLATED APPARATUS and for ensuring safety during the tests. He may, without reference to the CP, remove and replace EARTHS as necessary and carry out tests including making LIVE the APPARATUS concerned from a testing supply. This does not mean that the recipient of the SANCTION-FOR-TEST has to carry out the tests personally, but that they shall be carried out under his direct supervision.

6.3.2 The APPARATUS ISOLATED for work by a third party under an ELECTRICAL ISOLATION/DE-ISOLATION CERTIFICATE or IMMOBILISATION/ MOBILISATION CERTIFICATE shall remain so until the work is complete.

The APPARATUS ISOLATED for work and on-load checking by a third party under an ELECTRICAL ISOLATION/DE-ISOLATION CERTIFICATE (allowing restoration of motive power) may be DE-ISOLATED by the ELECTRICAL PERSON to allow on-load checks on the APPARATUS to be made by the third party, but only under the direct supervision of the ELECTRICAL PERSON.

6.4 PROCEDURE FOR CLEARANCE AND CANCELLATION

6.4.1 When work on APPARATUS for which an ELECTRICAL PERMIT-TO-WORK has been issued is completed, the recipient shall sign the clearance and return the ELECTRICAL PERMIT-TO-WORK to an AEP who shall cancel it and inform the CP.

Where more than one ELECTRICAL PERMIT–TO-WORK has been issued for work on HV APPARATUS associated with the same CIRCUIT MAIN EARTHS, the CP shall ensure that all such ELECTRICAL PERMITS-TO-WORK have been cancelled before the CIRCUIT MAIN EARTHS are removed.

When testing on APPARATUS for which a SANCTION-FOR-TEST has been issued is completed, the recipient shall sign the clearance and return the SANCTION-FOR-TEST to a SAEP for cancellation.

A circuit under SANCTION-FOR-TEST shall have all CIRCUIT MAIN EARTHS restored prior to cancellation unless agreement was reached with the CP at the time of issue to accept the cancellation of the SANCTION-FOR-TEST without these EARTHS in position.

The SAEP, before canceling the SANCTION-FOR-TEST shall enter under "Exceptions" the changes of state of EARTHING of the circuit and then inform the CP of the cancellation and of the operational state of the circuit.

When work for which a LIMITATION-OF-ACCESS has been issued is completed, the recipient shall sign the clearance and return the LIMITATION-OF-ACCESS for cancellation to an AEP.

When a LIMITATION-OF-ACCESS which allowed LIVE LINE WORKING or LIVE LINE WASHING procedures is canceled, then the CP shall be informed.

6.4.2 When work by a party under an ELECTRICAL ISOLATION/DE-ISOLATION CERTIFICATE or IMMOBILISATION/MOBILISATION CERTIFICATE is complete the Custodian Department's representative on the site of the APPARATUS shall return the CERTIFICATE to a suitably authorised ELECTRICAL PERSON or APPOINTED PERSON as a request to DE-ISOLATE the circuit and make the APPARATUS ready for operational use.

When the work and the on-load checks by a third party under an ELECTRICAL ISOLATION/DE-

ISOLATION CERTIFICATE (allowing restoration of motive power) are complete, the Custodian

Department's representative on the site of the APPARATUS shall return the CERTIFICATE to a

suitably authorised ELECTRICAL PERSON as a request to DE-ISOLATE the circuit and make

the APPARATUS ready for operational use.

7. Precautions to be Taken on Specific Items of Apparatus

7.1 WORK ON OVERHEAD LINES

7.1.1 Identification Of Circuits

7.1.1.1 Single Circuit High Voltage Overhead Lines

Where poles or structures supporting LIVE conductors are part of an

overhead line circuit whose route runs near, or in the immediate vicinity

of other overhead line circuits, then those poles or structures shall carry

an APPROVED label or symbol to enable easy identification of that circuit

from ground level.

7.1.1.2 Double Circuit High Voltage Overhead Lines

Where single towers or structures are used to support the LIVE conductors of two circuits, then each tower or structure shall carry APPROVED labels or symbols, so that a positive identification of each circuit, and the position of its conductors on the towers and structures, can be made both by persons from ground level, and by persons working up a tower or structure under an ELECTRICAL SAFETY DOCUMENT.

7.1.2 General

The procedures called for in Rule 5.1.3 and Rule 5.1.4 shall apply for all work on overhead lines.

7.1.3 Approach of Lightning

On the near approach of a lightning storm, all work on overhead lines shall cease immediately and the CEP in charge shall inform the AEP that his WORKING PARTY are not working on the overhead line.

7.1.4 Work on Poles Supporting The High Voltage Conductors of One Circuit

No CEP shall climb any such pole supporting LIVE conductors except under the direct supervision of an AEP, who shall ensure that the person does not ascend to a position in which it is possible for him to bring any part of his body or working tool within the SAFETY DISTANCES specified in Rule 5.6.

Additionally when work is to be carried out within one meter distance outside the SAFETY DISTANCES specified in Rule 5.6, the AEP shall issue a LIMITATION-OF-ACCESS to the CEP.

A DANGER NOTICE shall also be attached to the pole at a distance from LIVE conductors not less than those specified in Rule 5.6.

7.1.5 Work on Towers or Structures Supporting the High Voltage Conductors of Two Circuits

No person shall climb above the anti-climbing guards of such towers or structures supporting the LIVE conductors of two circuits, unless he is first issued with a LIMITATION-OF-ACCESS, stating the precautions he and his WORKING PARTY shall take to avoid DANGER.

An AEP shall personally ensure that DANGER NOTICES or APPROVED red flags are attached to all sides of the tower or structure above which, persons climbing shall be instructed not to pass. These DANGER NOTICES or APPROVED red flags shall be attached at a distance from the lowest LIVE conductor so that persons working on the steelwork beneath them cannot bring any part of their bodies or working tools within the SAFETY DISTANCES specified in Rule 5.6.

The AEP shall from the ground directly supervise all such persons climbing or working on the towers or structures.

7.1.6 Work on Single or Double Circuit High Voltage Overhead Lines with All Conductors Dead

7.1.6.1 For work on HV conductors the AEP in addition to the procedures called for in Rule 5.3 shall:-

(a) Identify the line to be worked on in an APPROVED manner at the point of work.

(b) Where practicable ensure that the line is DEAD by checking with an APPROVED voltage testing device.

(c) If from the point of work the conductors can be seen to be EARTHED, dispense the check with an APPROVED voltage testing device.

(d) Issue the CEP holding the ELECTRICAL PERMIT-TO-WORK with portable EARTHING leads that shall be used as ADDITIONAL EARTHS.

7.1.6.2 For work on HV conductors the CEP holding the ELECTRICAL PERMIT-TO-WORK shall:-

(a) Before the commencement of work apply ADDITIONAL EARTHS at or as near as practicable to the point of work. These portable EARTHING leads shall be used to bond together all conductors and be connected to EARTH.

An APPROVED label reading 'A.E.' shall be attached to the portable EARTHING leads.

(b) Remove and re-apply these ADDITIONAL EARTHS at different points of work on the line, but only if they are kept between the CIRCUIT MAIN EARTH positions.

(c) Dispense with the use of ADDITIONAL EARTHS if CIRCUIT MAIN EARTHS have been applied in the immediate vicinity at each side of the point of work.

(d) If the WORKING PARTY leaves the line at any time, check when they come back, that they have returned to the same line and that all EARTHS adjacent to the point of work, are still in position before allowing work to start again.

(e) Check that all members of the WORKING PARTY are off the line before removing all the ADDITIONAL EARTHS.

(f) Sign the 'Clearance Section' of the ELECTRICAL PERMIT-TO-WORK.

7.1.7 Work on Double Circuit Lines with one Circuit Live

7.1.7.1 Where the design of towers or structures enables the SAFETY DISTANCES as specified in Rule 5.6 to be maintained when persons are working on the DEAD conductors of one circuit with the other one LIVE, then in addition to the requirements of Rules 5.3, 7.1.6.1 and 7.1.6.2 the following precautions shall be taken before work commences.

7.1.7.2 The AEP shall:-

(a) At the point of work personally identify the circuit to be worked on, and instruct the WORKING PARTY how to obtain safe access, up the tower or structure, to the circuit's DEAD conductors.

(b) Attach an APPROVED green flag at ground level on the DEAD circuit side of the face of the tower or structure showing the safe access approach.

(c) Attach a DANGER NOTICE at ground level on the LIVE circuit side of the face of the tower or structure.

(d) Enter on the ELECTRICAL PERMIT-TO-WORK the wording of the circuit identification or symbol, that is attached to the tower or structure, of the circuit to be worked on.

(e) Instruct the CEP holding the ELECTRICAL PERMIT-TO-WORK, to climb ahead of the rest of the WORKING PARTY, and to attach APPROVED red flags to the cross arms of the tower or structure, between the two circuits, drawing attention to the DANGERS of approaching the LIVE circuit.

(f) Ensure that these APPROVED red flags are attached so that all members of the WORKING PARTY are aware that no part of their bodies or working tools shall go beyond them and come within the SAFETY DISTANCES specified in Rule 5.6.

(g) Remain on the ground, adjacent to the tower or structure, watching whilst members of the WORKING PARTY are working above the anti-climbing guards. Be prepared to warn them if they approach the LIVE circuit, and are in DANGER of bringing any part of their bodies or working tools, within the SAFETY DISTANCES specified in Rule 5.6.

7.1.7.3 The CEP holding the ELECTRICAL PERMIT-TO-WORK shall:-

Ensure that until the work is finished and the rest of the WORKING PARTY have descended to the ground, the conductors remain EARTHED at the point of work and the APPROVED red and green flags and DANGER NOTICES are kept in position.

7.1.8 Work on the Common Earth Conductor of Double Circuit Overhead Lines

7.1.8.1 Before work commences on the overhead common EARTH conductor of a double circuit overhead line, the AEP shall ensure that both circuits are made DEAD, and that the procedures called for in Rule 5.3 and Rule 7.1.6 are applied for both circuits.

7.2 WORK ON UNDERGROUND CABLES

7.2.1 Work on High Voltage Cables

For work on HV cables, the AEP in addition to the procedures called for in Rule 5.3 shall:-

(a) Ensure that no person touches or cuts into the insulation which covers the conductor, unless the conductor is DEAD, ISOLATED and EARTHED.

(b) Before issuing the ELECTRICAL PERMIT-TO-WORK to the CEP, identify the cable to be worked on at the point of work, and prove it dead by APPROVED spiking equipment or other APPROVED means.

7.2.2 Work on Low Voltage Cables

For work on LV cables, the CESP in addition to the procedures called for in Rule 5.4 shall:-

(a) Take all reasonably practicable steps to prevent the cable being made LIVE in error during the course of work.

(b) Keep any SAFETY PADLOCK keys, withdrawn fuses or links in a safe place during the course of work.

(c) Before the start of the work, clearly identify the cable at the point of work to the person in charge of the work.

7.2.3 Work In The Vicinity of Buried High Voltage Cables

7.2.3.1 Accidental striking of power or auxiliary cables buried in the ground can result in fatal or severe accidents in addition to possible loss of electrical supplies to important installations. To prevent such incidents it is necessary for a responsible member of the Custodian Department to issue an EXCAVATION PERMIT to the person in charge of the excavation.

7.2.3.2 If the excavations are in the vicinity of underground electrical cables then an AEP shall specify on the EXCAVATION PERMIT any additional precautions that shall be taken to safeguard the cables.

7.2.3.3 If in the view of the AEP the cables should be made DEAD, then an ELECTRICAL ISOLATION/DE-ISOLATION CERTIFICATE shall be issued to cover this SWITCHING.

7.3 WORK ON HIGH VOLTAGE TRANSFORMERS

7.3.1 For work on HV transformers, the AEP in addition to the procedures called for in Rule 5.3 shall:-

(a) Ensure that, where auxiliary or voltage transformers cannot be ISOLATED from the HV conductors, the LV fuses or links are withdrawn and kept in a safe place during the progress of the work.

(b) Where work is to be carried out on an HV/LV transformer, ensure that the LV windings are ISOLATED either by the opening of a switch or withdrawal of fuses or links, so that any possible backfeed is prevented during the progress of the work. Attach CAUTION NOTICES at the points of ISOLATION.

(c) Before issuing the ELECTRICAL PERMIT-TO-WORK to the CEP, identify at the point of work, the transformer to be worked on.

7.4 WORK ON HIGH VOLTAGE SWITCHGEAR

7.4.1 Withdrawable Apparatus

When work is to be carried out on withdrawable HV APPARATUS (e.g. a circuit breaker), then the AEP in addition to the procedures called for in Rule 5.1.1. shall:-

(a) Disconnect the withdrawable HV APPARATUS from all supplies and remove from its normal service housing.

(b) Discharge its conductors to EARTH, but they need not remain connected to EARTH.

7.4.2. Work on Busbar Spouts

When work is to be carried out on the busbars or busbars spouts of multi-panel switchboards then the AEP in addition to the procedures called for in Rule 5.3 shall:-

(a) Ensure that the section of the busbars on which work is to be carried out are made DEAD and ISOLATED from all points of supply from which it can be made LIVE.

(b) Ensure that the ISOLATING arrangements are secured with SAFETY PADLOCKS, and where practicable, the shutters of LIVE spouts locked shut.

(c) Attach CAUTION NOTICES at all points where the busbars can be made LIVE.

(d) Attach DANGER NOTICES, where applicable, on or adjacent to APPARATUS containing LIVE conductors at the limits of the busbar on which work is to be carried out.

(e) Test with an APPROVED voltage testing device that the busbars are dead, checking the voltage testing device before and after use.

(f) EARTH the busbar at the point of testing and where practicable lock the CIRCUIT MAIN EARTH in position.

Attach an EARTHED NOTICE to the circuit breaker or EARTHING device.

(g) Test with an APPROVED voltage testing device every spout where work is to be carried out, checking the voltage testing device before and after use.

(h) Issue an ELECTRICAL PERMIT-TO-WORK to a CEP.

7.5 WORK IN SUBSTATIONS WITH EXPOSED HIGH VOLTAGE CONDUCTORS

7.5.1 Safety Distances to Live Conductors

When work is to be carried out in substations where there are exposed LIVE HV conductors, then unless the whole APPARATUS and conductors are to be made DEAD, the AEP in addition to the procedures called for in Rule 5.3 shall:-

(a) Identify the area where work may be carried out safely and ensure that SAFETY DISTANCES called for in Rule 5.6 are maintained by the WORKING PARTY and the tools and equipment they are to use. If no ELECTRICAL PERMIT-TO-WORK or SANCTION-FOR-TEST is issued for adjacent APPARATUS in the substation, then he shall issue a LIMITATION-OF-ACCESS to the CEP in charge of the work, specifying how the SAFETY DISTANCES from LIVE conductors shall be maintained during the course of the work.

(b) If APPROVED barriers or ropes are necessary to define the safe area of work, ensure that these are clearly visible and are not tied or supported by any structure carrying LIVE APPARATUS or conductors. The safe area shall be identified by the attaching of green flags inside the barriers or ropes and DANGER NOTICES attached to adjoining LIVE APPARATUS.

(c) Ensure that ladders required for the work shall be of an APPROVED type and be kept secure with a STANDARD ELECTRICAL PADLOCK when not in use.

When ladders are used to gain access to DEAD APPARATUS, subject to an ELECTRICAL PERMIT-TO-WORK, these shall be locked in position with a STANDARD ELECTRICAL PADLOCK or by any other APPROVED means such that they cannot be removed unauthorised.

(d) Ensure that if ladders, scaffolding, cranes etc, are to be used, that the position that they shall be placed and the route through the substation to that position shall first be agreed. Scaffolding and cranes, etc., shall be connected to the substation EARTH system as soon as they are in position for work to take place.

7.6 WORK ON REMOTELY OR AUTOMATICALLY CONTROLLED HV

APPARATUS

7.6.1 Physical Danger

Serious bodily harm can be suffered by persons working on HV APPARATUS such as tap-change gear, circuit breakers, etc, due to the automatic or remote control feature being operated in error.

7.6.2 Precautions

For work on remotely or automatically controlled HV APPARATUS such as tap-change gear, circuit breakers, disconnectors, etc., the AEP before work commences shall:-

(a) Ensure that all remote control and automatic features are made so that they cannot be operated and, where practicable, secure with SAFETY PADLOCKS and CAUTION NOTICES.

(b) Take such precautions to ensure that no work will be in progress on the tap-changer, circuit breaker etc., when work is taking place on the controlling equipment associated wiring or relays, except when it is undertaken by an AEP or CEP acting under his direct supervision.

7.7 WORK AND TESTING ON HV MOTOR CIRCUITS (includes HV circuits with

an LV motor, eg. beam pump circuits)

7.7.1 As no electricity can be backfed from an ISOLATED HV motor circuit, the following procedure may be adopted if the CP and the AEP both agree to its use, for routine maintenance work only, without preparing a SWITCHING PROGRAMME for a particular circuit.

7.7.2 The AEP shall then, in sequence:-

(a) Open the HV motor circuit making the motor DEAD.

(b) ISOLATE the HV motor circuit and apply SAFETY PADLOCKS and CAUTION NOTICE at the point of ISOLATION.

(c) EARTH the HV motor circuit at the point of ISOLATION through the circuit breaker or by other APPROVED means, (ie. apply a CIRCUIT MAIN EARTH).

Attach an EARTHED NOTICE to the circuit breaker, if applicable.

(d) Issue an ELECTRICAL PERMIT-TO-WORK to the CEP at the point of work.

(e) Receive back the ELECTRICAL PERMIT-TO-WORK with the 'Clearance Section' signed off by the CEP when the work is completed.

(f) Notwithstanding the requirements of Rule 5.5.1 and Rule 6.3, remove the CIRCUIT MAIN EARTH and apply a 'megger' test to the HV motor circuit, to satisfy routine maintenance requirements.

(g) Cancel the ELECTRICAL PERMIT-TO-WORK, remove SAFETY PADLOCKS, CAUTION NOTICES, any DANGER and EARTHED NOTICES and restore the circuit breaker to the service position.

7.8 WORK ON LOW VOLTAGE SWITCHBOARDS

7.8.1 Due to the danger of electrical backfeeds from both HV transformers and LV incoming switchgear units, busbars, bus section units, all work on these LV items of APPARATUS require strict precautions and the issue of a LIMITATION-OF-ACCESS or an ELECTRICAL PERMIT-TO-WORK to the person in charge of the work.

7.8.2 If the design of the LV system or switchboard connections are such that complete ISOLATION of the incoming switchgear unit to be worked on requires the ISOLATION of the HV winding of an HV/LV transformer then, in addition to the opening of LV switchgear units, the following procedure shall be carried out by the AEP:-

(a) All such LV switchgear units shall be opened and, where practicable, a SAFETY PADLOCK applied.

CAUTION NOTICES shall be attached to such switchgear units.

(b) DANGER NOTICES shall be attached to LIVE APPARATUS adjacent to the point of work.

(c) The HV/LV transformer shall be switched out, ISOLATED and a CAUTION NOTICE attached at the point of ISOLATION.

(d) A CIRCUIT MAIN EARTH shall be applied to the HV winding of the transformer.

(e) An ELECTRICAL PERMIT-TO-WORK shall be issued at the point of work to the person in charge of the work.

(f) Any LV EARTH applied on the DEAD switchboard shall be in accordance with Rule 5.5.3 and its application shall be recorded in the 'Other Precautions Section' of the ELECTRICAL PERMIT-TO-WORK.

7.8.3 If work on LV switchboards does not require ISOLATION and EARTHING of HV APPARATUS then if LV EARTHS are applied on the LV APPARATUS their application shall be recorded in the Other Precautions Section' of the ELECTRICAL PERMIT-TO-WORK issued for the work.

7.9 WORK ON HIGH VOLTAGE STATIC CAPACITORS

7.9.1 For work on HV static capacitors the AEP in addition to the procedures called for in Rule 5.3 shall:-

(a) Apply APPROVED capacitor EARTHING devices to the capacitor frames.

(b) Apply APPROVED capacitor EARTHING devices to the common connection of each group of capacitors.

(c) Issue ADDITIONAL EARTHS to the CEP holding the ELECTRICAL PERMIT-TO-WORK and instruct him to apply these to the capacitor units at the point of work.

7.9.2 Capacitor units shall be short-circuited and remain short-circuited:-

(a) When removed from the circuit

or

(b) In cases where the AEP finds it is not practicable to apply the EARTHING requirements of Rule 7.9.1.

7.10 SWITCHING, WORKING OR TESTING IN HAZARDOUS AREAS.

7.10.1 General

All electrical work and/or electrical testing in PDO designated hazardous areas shall be undertaken in accordance with the requirements of the Permit-to-Work System supplemented, as may be applicable, by these Rules.

All the precautions called for in other sections of these Rules that apply to SWITCHING, work or testing on either HV or LV APPARATUS, shall apply to all the APPARATUS located in hazardous areas.

7.10.2 Additional Precautions

The person in charge of work or testing in hazardous areas shall ensure that:-

(a) Before removing any covers of certified APPARATUS the incoming connections, including the neutral, are made DEAD and have been ISOLATED at a unit outside the hazardous area.

(b) No SWITCHING, testing or other operations are carried out on certified APPARATUS, whilst its covers are removed and the electrical connections are exposed to the atmosphere.

(c) Suitable gas testing equipment is first used to check that no flammable gases, that could create a hazard, are present at the point of work or testing.

8. Live Line Working

8.1 GENERAL

8.1.1 To date no PDO personnel have been trained, authorised or equipped to carry out LIVE LINE WORKING procedures on HV conductors on the PDO electrical systems.

8.1.2 Contractors selected to carry out such LIVE LINE WORKING procedures in PDO shall have proven LIVE LINE WORKING procedures, adequately trained and experienced staff and the necessary well maintained LIVE LINE tools and equipment to carry out such work. (See ESOP 22 for guidance).

8.1.3 The AEP before initiating LIVE LINE WORK shall ensure that the Contractor's Supervisor has the written procedures, the necessary evidence and certificates on site to satisfy him that the requirements of Rule 8.1.2 are met. (See ESOP 22 for guidance).

8.1.4 The LIVE LINE WORKING and the safety of the team shall be the responsibility of the Contractor's Supervisor, who shall ensure that the procedure is carried out correctly and is present throughout the procedure to ensure that it is in accordance with the Rules in this Section 8.0.

8.2 LIVE LINE TOOLS AND EQUIPMENT

8.2.1 LIVE LINE tools and equipment shall be kept in a clean and dry condition and immediately before use be inspected by the Contractor's Supervisor to ensure they are clean, dry and in sound condition.

8.2.2 A clear mark shall be maintained on every LIVE LINE tool and equipment where appropriate, Indicating the limit of the safe handling distance which shall not be less than as indicated in Table 2 below.

8.3 SAFETY PRECAUTIONS

8.3.1 The AEP shall inform the CP of the nature and location of the LIVE LINE WORK to be carried out by the Contractor.

8.3.2 If the circuit to be worked on is controlled by an automatic circuit reclosing device, work shall not commence until the automatic reclosing features have been made inoperative.

8.3.3 The AEP shall issue a LIMITATION-OF-ACCESS to the Contractor's Supervisor who is in charge of the LIVE LINE WORK.

The LIMITATION-OF-ACCESS shall state the name of the circuit, its identification label or symbol, the precautions taken by the AEP and a brief description of the work to be carried out using the LIVE LINE WORKING procedure.

8.3.4 The AEP shall agree with the CP that if the circuit becomes dead due to the operation of protective equipment, the circuit shall not be re-energised without reference being made to him on site and his consent obtained.

The AEP shall normally remain on site throughout any LIVE LINE WORKING taking place on wood pole overhead lines, or in his absence, appoint a CEP to represent him with full authority to stop the work if he considers the Contractors are not working safely. (See ESOP 22 for guidance).

The AEP shall remain on site throughout any LIVE LINE WORKING that takes place in substations or on double circuit overhead lines. (See ESOP 22 for guidance).

8.3.5 No LIVE LINE WORKING shall commence in unfavourable conditions, such as rain, fog or high winds.

8.3.6 If weather or other unforeseen conditions make it necessary to suspend LIVE LINE WORKING, then the circuit conductors and the equipment shall be left in a safe condition and the CP informed.

8.3.7 Before work commences, all conductors and associated pole top, line and tower fittings, steelwork and insulators shall be thoroughly examined at the point of work, through binoculars if necessary, so as to minimise the possibility of failure of these parts during LIVE LINE WORK. This examination shall extend to include adjacent spans and supports on both sides of the point of work.

Before any pole is climbed it shall be sounded. No pole badly impaired by decay or damage shall be climbed until the Contractor's Supervisor is satisfied that he has made it safe for climbing, alternatively access to the top of the pole shall be by suitable means independent of the pole.

8.3.8 Before displacing conductors adequate precautions shall be taken to avoid DANGER and to ensure the safety of third persons, Safe clearances shall also be maintained between the line and other APPARATUS and objects.

8.3.9 No vehicle or person other than members of the LIVE LINE WORKING team shall be allowed in the near vicinity of the point of work while the work is in progress without the consent of the Contractor's Supervisor in charge of the LIVE LINE WORK. Temporary markers may be erected to ensure this requirement is met.

8.3.10 No person shall climb, move or work in such a position to bring any part of his body, clothing or any working tool, other than LIVE LINE WORKING tools or equipment, within the distances as indicated in Table 3 below. This distance shall be marked on the structure.

8.3.11 The Contractor's Supervisor shall be responsible for ensuring all personnel of the LIVE LINE WORKING team wear the protective clothing and equipment necessary for this work.

9. Live Line Washing

9.1 GENERAL

9.1.1. To date no PDO personnel have been trained, authorised or equipped to carry out LIVE LINE WASHING on HV insulators on PDO electrical systems.

9.1.2 Contractors selected to carry out such LIVE LINE WASHING shall have proven LIVE LINE WASHING procedures, adequately trained and experienced staff and all the necessary well maintained equipment to carry out such work. (See ESOP 22 for guidance).

9.1.3 The AEP before initiating LIVE LINE WASHING shall ensure that the Contractor's Supervisor has the necessary evidence and certificates on site to satisfy him that the requirements of Rule 9.1.2 are met. (See ESOP 22 for guidance).

9.1.4 The LIVE LINE WASHING and the safety of the team shall be the responsibility of the Contractor's Supervisor, who shall ensure that the work is carried out correctly and is present all the time the washing is taking place in accordance with the Rules in this Section 9.0.

9.2 LIVE LINE WASHING EQUIPMENT

9.2.1 Monitoring devices to check the conductivity of the water to be used shall be available on site.

9.2.2 The equipment shall have a nozzle which is such that the water ejected by it breaks into a spray before reaching the insulator being washed.

9.2.3 The nozzle and metal parts of the equipment shall be bonded together to EARTH by a suitable EARTHING lead which is not less than 70 mm2 copper equivalent in cross-section.

9.3 SAFETY PRECAUTIONS

9.3.1 The AEP shall inform the CP of the location of the LIVE LINE WASHING to be carried out by the Contractor.

9.3.2 The AEP shall issue a LIMITATION-OF-ACCESS to the Contractor's Supervisor who is in charge of the LIVE LINE WASHING.

The LIMITATION-OF-ACCESS shall state the name of the circuit, its identification or symbol and any precautions taken by the AEP.

The AEP shall normally remain on site throughout the LIVE LINE WASHING taking place on wood pole overhead lines, or in his absence, appoint a CEP to represent him with full authority to stop the washing if he considers the Contractors are not working safely. (See ESOP 22 for guidance).

9.3.3 The water being used for LIVE LINE WASHING shall have a conductivity of less than 400 micro siemens/cm. This shall be constantly monitored as a rise in the water temperature increases its conductivity. The water quality monitoring shall be done using an automatic monitoring device fitted with an audible alarm signal.

When no connection to a fixed EARTH plate is available on site for the bonding called for in Rule 9.2.3, the Contractor's Supervisor shall have a temporary EARTH electrode installed and check that its resistance is adequate for the operator's safety before commencing any LIVE LINE WASHING. (See ESOP 22 for guidance).

9.3.4 The safety clearance necessary between the hose operator and LIVE conductors will depend on the system voltage, the method of washing and the efficiency of the breakup of the jet of water.

The safety clearance necessary shall be agreed between the Contractor's Supervisor and the AEP depending on the Contractor's methods, equipment and experience.

9.3.5 The LIVE LINE WASHING shall not commence in unfavourable conditions such as high winds as this would affect the direction of the water jets.

9.3.6 Only one phase insulator shall be washed by the same jet.

9.3.7 No other LIVE conductor at a lesser distance than the one being washed shall be sprayed in error.

9.3.8 LIVE LINE WASHING shall stop in the event of a local lightning risk.

9.3.9 The Contractor's supervisor shall be responsible for ensuring that the LIVE LINE WASHING operator wears any protective clothing necessary for this work.

The AEP shall inform the CP when the LIVE LINE WASHING is complete.

10. USE OF APPOINTED PERSONS

10.1 GENERAL

10.1.1 For use in certain situations, or at specific locations, there is sometimes the need of a person who can carry out tasks that would normally be done by ELECTRICAL PERSONS as a small part of their operational duties.

Entering a substation, which has no exposed HV conductors, to read meters or inspect equipment, does not necessarily require the electrical experience of a person who has CEP status.

10.1.2 The role of an AP is generally defined in Rule 2.1.4.8.

10.2 SWITCHING DUTIES

10.2.1 An AP may be nominated by his Custodian Department to carry out specific limited HV SWITCHING from a position remote from the circuit breaker, or limited LV SWITCHING including immobilisation / mobilisation of selected LV motors using an APPROVED procedure. This SWITCHING shall be limited to the closing and opening of a circuit breaker or contactor controlling a generator, motor etc.

10.2.2 If the CP agrees to the AP carrying out this limited SWITCHING, then an APPROVED PROCEDURE FOR APPOINTED PERSONS shall be prepared for this purpose and submitted to the relevant Head of Utility, who after checking and verification will forward the procedure to the CFDH-E for approval before it is implemented.

Each APPROVED PROCEDURE FOR APPOINTED PERSONS shall have a unique reference number. (see ESOP 16 for guidance).

NOTE: All PROCEDURES FOR APPOINTED PERSONS shall be kept in one file together with the SWITCHGEAR OPERATING PROCEDURES.

10.3 APPOINTMENT

Applications for personnel to become AP shall be dealt with in accordance with ESOP 3.

APPENDIX F – WORKING AND ACCESS CLEARANCE

1.0 The Working and Access Clearances given in the following Tables of this Appendix are derived from the SAFETY DISTANCES 'X' (Table 1) specified in Rule 5.6.1 with the addition of 'Application Factors' suitable for the particular activity to be undertaken in the vicinity of exposed LIVE HIGH VOLTAGE conductors.

2.0 Appendix F1 specifies the Working and Access Clearances for work on overhead line poles when work is to be undertaken with the conductors LIVE.

(See Rule 2.3.6.4(a) and Rule 2.5.1.4).

3.0 Appendix F2 specifies the Working and Access Clearances for work in substations containing exposed LIVE HIGH VOLTAGE conductors.

(See Rule 2.3.6.4(a) and Rule 2.5.5.1).

4.0 Appendix F3 specifies the Working and Access Clearances that Contractors should be using when carrying out HIGH VOLTAGE LIVE LINE WORK on PDO wood pole overhead lines. (See Rule 2.6.3[x]).

IMPORTANT NOTE

This Appendix has been produced and added to these Rules to give AUTHORISED ELECTRICAL PERSONS operational guidance, when they are issuing ELECTRICAL SAFETY DOCUMENTS, in the types of situations covered in appendices F1, F2 and F3.

The Working and Access Clearance figures given should be used as a minimum requirement, and should be increased if practicable in all such situations.

The importance of the AUTHORISED ELECTRICAL PERSONS understanding the intention of Rule 2.3.6, and in particular Rule 2.3.6.4, is essential when using the Clearances given in this Appendix F.
