WHAT IS THE PROPERTY TAX/RENTREBATE PROGRAM?A Pennsylvania program providing rebates onproperty tax or rent paid the previous year byincome-eligible seniors and people with disabilities.

HOW TO APPLY?Submit the paper application in this booklet to theDepartment of Revenue. Instructions are inside.

NEW FOR 2017Federal veterans’ disability payment and state veterans’payments are no longer required to be included in eligibilityincome. See the instructions for Line 6 on Page 7 foradditional information.

STATE SUPPLEMENTARY PAYMENT RECIPIENTSThe State Supplementary Payment (SSP) is not includedon your SSA-1099 form. The Department of HumanServices will issue annual statements to verify your SSPbenefit. SSP is still considered reportable income. As withother Social Security income, only half of the SSP incomeneeds to be included on Line 4 of the Property Tax/RentRebate claim form.

SOCIAL SECURITY RECIPIENTS WITH PA ADDRESSESIf you were a Pennsylvania resident for all of 2017, you donot have to submit proof of your Social Security incomeincluding Social Security retirement and SupplementalSecurity benefits. The Social Security Administrationprovides Social Security income information to the PADepartment of Revenue. The PA Department of HumanServices will provide State Supplementary Paymentinformation to the department.

However, you or the person who prepares your claim willneed these statements to correctly calculate the amount ofyour rebate. If none of these documents are available, youor your preparer will need to estimate the amount youreceived during the year. If the dollar amount you provideis not correct, the department will adjust the amount ofyour rebate based upon income amounts reported directlyto the department by the Social Security Administration orthe Department of Human Services.

SOCIAL SECURITY RECIPIENTS WITHOUTPA ADDRESSESIf your address in Social Security Administration recordswas not a Pennsylvania address for 2017, you must submita copy of one of the following documents as proof of your2017 Social Security income: Form SSA-1099 reportingyour 2017 Social Security benefits, a statement fromSocial Security that reports the monthly or yearly SocialSecurity/Supplemental Security Income benefits youreceived during 2017, or a bank statement showing theamount of Social Security/Supplemental Security Incomebenefits deposited into your account during 2017.

PHILADELPHIA RESIDENTSPlease read the special filing instructions on Page 11.

You are eligible for a Property Tax/Rent Rebate for claimyear 2017, if you meet the requirements in each of thethree categories below:

a. You were 65 or older as of Dec. 31, 2017; b. You were not 65, but your spouse who lived with you

was 65 or older as of Dec. 31, 2017; c. You were a widow or widower during all or part of

2017 and were 50 or older as of Dec. 31, 2017; or

d. You were permanently disabled and 18 or olderduring all or part of 2017, you were unable to workbecause of your medically determined physical ormental disability, and your disability is expected tocontinue indefinitely. If you received SupplementalSecurity Income (SSI) payments, you are eligible fora rebate if you meet all other requirements.

NOTE: If you applied for Social Security disability benefitsand the Social Security Administration did not rule in yourfavor, you are not eligible for a Property Tax/Rent Rebateas a disabled claimant.

When calculating your total eligible annual householdincome, exclude one-half of your Social Security benefitsas shown in Box 5 of your SSA-1099 statement, one-halfof your Supplemental Security Income benefits, one-half ofyour State Supplementary Payment benefits and one-halfof any Railroad Retirement Tier 1 benefits as shown onForm RRB-1099.

a. Property Owners - Your total eligible annual house-hold income, including the income that your spouseearned and received while residing with you, was$35,000 or less in 2017.

b. Renters - Your total eligible annual householdincome, including the income that your spouseearned and received while residing with you, was$15,000 or less in 2017.

You must report all items of income, except the nonre-portable types of income listed on Pages 8 and 9, whetheror not the income is taxable for federal or PA income taxpurposes.

NOTE: There may be differences between eligibilityincome and PA-taxable income. Please see specific lineinstructions for each category of income.

To file as a property owner, renter, or owner/renter, you mustmeet all requirements for one of the following categories:OWNER a. You owned and occupied your home, as evidenced

by a contract of sale, deed, trust, or life estate held bya grantee;

b. You occupied your home (rebates are for yourprimary residence only); and

c. You or someone on your behalf paid the 2017property taxes on your home.

RENTER a. You rented and occupied a home, apartment, nursing

home, boarding home, or similar residence inPennsylvania;

b. Your landlord paid property taxes or agreed to makea payment in lieu of property taxes on your rentalproperty for 2017 (see Page 12); and

c. You or someone on your behalf paid the rent on yourresidence for 2017.

OWNER/RENTER a. You owned, occupied, and paid property taxes for

part of the year and were a renter for part of the year; b. You owned and occupied your home and paid

property taxes and paid rent for the land upon whichyour home is situated; or

c. You paid rent for the home you occupied, and paidproperty taxes on the land upon which your home issituated.

CAUTION: As a renter, if you received cash public assis-tance during 2017, you are not eligible for any rebate forthose months you received cash public assistance. Pleasecomplete a PA-1000 Schedule D (enclosed in this booklet).

IMPORTANT: Please send photocopies, since the depart-ment cannot return original documents. Print your SocialSecurity Number (SSN) on each proof document that yousubmit with your claim form.• If you are age 65 or older, provide proof of your age.• If you are under age 65 and your spouse is age 65 or

older, provide proof of your spouse’s age.• If you are a widow or widower age 50 to 64, provide

proof of your age and a photocopy of your spouse’sdeath certificate.

• If you are permanently disabled, age 18 to 64, you mustprovide proof of your age and proof of your permanentdisability.

PROOF OF AGENOTE: If you receive Social Security or SSI benefits andhave proven your age with the Social SecurityAdministration, you do not need to submit proof of age.IMPORTANT: The department accepts photocopies of thefollowing documents as proof of your age. Do not sendyour original documents since the department cannotreturn original documents.• Birth certificate

• Blue Cross or Blue Shield 65 Special Card• Church baptismal record• Driver’s license or PA identification card• Hospital birth record• Naturalization/immigration paper, if age is shown• Military discharge paper, if age is shown• Medicare card• PACE/PACENET card• PassportThe department will not accept a Social Security card orhunting or fishing license as proof of age.If you have questions on other types of acceptabledocuments, please call the department at 1-888-222-9190.PROOF OF DISABILITY• For Social Security disability, SSI permanent and total

disability, Railroad Retirement permanent and totaldisability, or Black Lung disability, provide a copy of youraward letter.

• For Veterans Administration disability, provide a letterfrom the Veterans Administration stating that you are100 percent disabled.

• For Federal Civil Service disability, provide a letter fromCivil Service stating that you are 100 percent disabled.

• If you do not qualify under any of the disability programsmentioned above, did not apply for Social Securitybenefits, or do not have a letter from the VeteransAdministration or Civil Service Administration, you mustsubmit a Physician’s Statement of Permanent Disability(PA-1000 PS), enclosed in this booklet. The form mustdescribe your disability as permanent and yourphysician must sign the statement to certify that theinformation is true and accurate to the best of his/herknowledge and belief.

IMPORTANT: The Physician’s Statement of PermanentDisability cannot be used if you were denied SocialSecurity disability. The Department of Revenue has thelegal authority to require additional evidence that you arepermanently disabled and eligible for a rebate.HOUSEHOLDS WITH MORE THANONE QUALIFIED CLAIMANTOnly one member of your household may file a claim evenif more than one person qualifies for a rebate. If someoneother than your spouse appears on the deed or the lease,please complete a PA-1000 Schedule F (enclosed in thisbooklet). You may apply for only one rebate each year.DECEASED CLAIMANTTo be eligible for a rebate, the claimant must have lived atleast one day of a claim year, owned and occupied andpaid taxes or rented and occupied and paid rent for theclaim year during the time period the claimant was alive.The property tax paid for a deceased claimant will be

prorated based upon the number of days the claimant livedduring the claim year. See Schedule A for the calculation ofthe prorated property tax rebate.

To determine if a deceased claimant is eligible for a rebate,a deceased claimant’s claim form must also include anannualized income amount in the calculation of totalhousehold income. See Schedule G, specifically theinstructions for Line 11g, for information on the calculationof annualized income to be included in household income.A copy of the death certificate must also be included withthe claim form.

A surviving spouse, estate or personal representative mayfile a claim on behalf of a deceased claimant. A personalrepresentative can also have a previously filed rebateissued in his or her name, instead of the name of thedecedent, in certain circumstances. Please see sectionsentitled SURVIVING SPOUSE, AN ESTATE, andPERSONAL REPRESENTATIVE for details.

SURVIVING SPOUSEThe surviving spouse can file the completed claim andinclude a copy of the death certificate and a letter statingthat he/she was the spouse of the claimant at the time ofdeath. The surviving spouse may sign on the claimant’ssignature line.

ORIf the surviving spouse is eligible to file a claim, he/she canfile under his/her own name instead of submitting a claimusing the deceased individual’s claim form.

The surviving spouse should print his/her name, address,and Social Security Number (SSN) in Part A, and followthe filing instructions. The surviving spouse should furnishproof required for a first time filer. Do not use the labelthe department sent to the decedent in the booklet. Thesurviving spouse should enter the deceased spouse’s SSNand name in the spouse information area, and fill in theoval “if Spouse is Deceased”, located in the area next tothe Spouse’s SSN on the claim form.

AN ESTATEThe executor or the administrator of the claimant’s estatemay file the claim and submit a Short Certificate showingthe will was registered or probated. When there is nowill and there are assets (an estate), submit a copy ofthe Letters of Administration. A Short Certificate orLetters of Administration can be obtained from the countycourthouse where the death is recorded. The person filingthe claim form on behalf of the deceased person may signon the claimant’s signature line.

PERSONAL REPRESENTATIVEIf a person dies and there is no will, the will has not beenregistered or probated or there is no estate, then a personalrepresentative may file a claim on behalf of an eligibledecedent. A decedent’s personal representative mustsubmit a copy of the decedent’s death certificate and areceipted copy of the decedent’s funeral bill showing that

the personal representative personally paid the decedent’sfuneral expenses in an amount that is greater than or equalto the amount of the property tax/rent rebate to which thedecedent is entitled.If a person dies after filing a claim and there is no will, or ifthe will has not been registered or probated, or there is noestate, then a personal representative can also requestthat the department change the rebate to be issued intohis/her name. In cases where the rebate check has beenreceived but cannot be cashed, the check must bereturned with a request to have the rebate issued in thename of the personal representative. The decedent’s per-sonal representative must submit a copy of the decedent’sdeath certificate, and a receipted copy of the claimant’sfuneral bill showing that the personal representativepersonally paid the funeral expenses in an amount that isgreater than or equal to the amount of the property tax/rentrebate to the claimant is entitled.If you have any questions regarding the filing of a claim onbehalf of a deceased claimant, please call the departmentat 1-888-222-9190.PRIVACY NOTIFICATIONBy law, (42 U.S.C. § 405(c)(2)(C)(i); 61 Pa. Code §117.16)the Pennsylvania Department of Revenue has the authorityto use the Social Security Number (SSN) to administer theProperty Tax or Rent Rebate Program, the Pennsylvaniapersonal income tax and other Commonwealth of Pennsyl-vania tax laws. The department uses the SSN to identifyindividuals and verify their incomes. The department alsouses the SSN to administer a number of tax-offset andchild-support programs federal and Pennsylvania lawsrequire. The commonwealth may also use the SSN inexchange-of-tax-information agreements with governmentalauthorities.Pennsylvania law prohibits the commonwealth fromdisclosing information that individuals provide on incometax returns and rebate claims, including the SSN(s), exceptfor official purposes.

You must fill in your Social Security Number and enteryour county and school district codes even if usingthe preprinted label. If you are not using software toprepare your claim and your label is correct, place yourlabel in Part A. If you or your preparer are using software toprepare your claim, or if any information on the preprintedlabel is incorrect, discard the label. If not using a label,follow the instructions for printing letters and numbers andcompleting your name and address.If your spouse lived in a nursing home the entire year donot include his/her Social Security Number on the claim

form. He or she may qualify for a separate rebate on therent paid to the nursing home.IMPORTANT TIPS: There are two lines to enter youraddress. For the First Line of Address, enter the streetaddress. If the address has an apartment number (APT),suite (STE), floor (FL) or rural route number (RR), enterit after the street address. If the street address and theapartment number, suite, etc. do not fit on the First Line ofAddress, enter the street address on the Second Line ofAddress and the apartment number, suite, etc. on the FirstLine of Address. For the Second Line of Address, enter thepost office box, if applicable. If there is no post office box,leave the Second Line of Address blank.The U.S. Postal Service prefers that the actual deliveryaddress appears on the line immediately above the city,state and ZIP code. Do not include any punctuation suchas apostrophes, commas, periods and hyphens.• Use black ink. Another color such as red ink will delay

the processing of your rebate claim.• Do not use pencil or pens labeled as gel pens or any red

ink.• Print all information on your claim neatly inside the

boxes.• Use upper case (capital) letters. Use a blank box to

separate words.• Print one letter or number in each box when entering

your Social Security Number, name, address, dollaramounts, and other information. If your name, address,or city begins with Mc, Van, O’, etc., do not enter aspace or a punctuation mark.

• Completely fill in all the appropriate ovals on yourclaim form.

As a claimant, you must provide your birth date, telephonenumber, county code, school district code, and, if applicable,your spouse’s Social Security Number, birth date, and name.If your spouse is deceased, completely fill in the oval “IfSpouse is Deceased” in Part A of the form.IMPORTANT: County & School Codes - You must enterthe two-digit county code and five-digit school district codefor where you lived on Dec. 31, 2017, even if you movedafter Dec. 31, 2017. Using incorrect codes may affect yourproperty tax rebate. The lists of county and school districtnames and the respective codes are on Pages 15, 16 and17. If you do not know the name of the county or schooldistrict where you reside, you can either 1) check the

county and school property tax bills used to completethis claim if you are a property owner, or 2) obtain thisinformation from the Online Customer Service Center atwww.revenue.pa.gov.

Line 1 - Please fill in the oval that shows your correctfiling status. Fill in only one oval. Filling in more than oneoval may reduce the amount of your rebate.(P) Property Owners: Fill in this oval if you owned andoccupied your home for all or part of 2017 and did not rentfor any part of the year. If your deceased spouse’s name ison your deed or tax bills, include the decedent’s SocialSecurity Number and name.(R) Renters: Fill in this oval if you rented and occupiedyour residence for all or part of 2017.(B) Owner/Renter: Fill in this oval if you owned and occu-pied your residence for part of 2017, and also rented andoccupied another residence for the rest of 2017, or if youowned your residence and rented the land where yourresidence is located.EXAMPLE: John pays property taxes on a mobile homethat he owns and occupies. His mobile home is on landthat he leases. John may claim a property tax rebate onthe mobile home and a rent rebate on the land. Please read each description andselect the type of filer that applies best to your situationas of Dec. 31, 2017. A surviving spouse age 50 to 64 iseligible for a rebate as a widow or widower, while asurviving spouse who is 65 or older can file as a claimant.A surviving spouse under 50 may be able to file a claim fora deceased claimant if the deceased was 65 or older.Please complete the claim form using your Social SecurityNumber, name and address, and supply all appropriatedocumentation. a. You must report the total household income you earnedand/or received during 2017 for each category, whichincludes your spouse’s income earned and/or receivedwhile residing with you. All claimants must submit proof of annual income.IMPORTANT: The department reserves the right torequest additional information or make adjustments tofederal data if credits or deductions were taken to reduceincome.CAUTION: Spouses may not offset each other’s incomeand losses.The department has the legal authority to require evidenceof the income you report on your claim. The following liststhe kinds of income you must report and the documentsyou must submit as proof of the reported income. You mustinclude the income that your spouse received whileresiding with you. See Pages 8 and 9 for a list of the kindsof income that you do not need to report.NOTE: Print your Social Security Number on each ProofDocument that you submit with your claim form.Line 4 - Include one-half of your 2017 Social SecurityBenefits as shown in Box 5 of your benefit statementSSA-1099, one-half of your 2017 SSI, one-half of your 2017Social Security Disability Income, and one-half of your2017 State Supplementary Payment. No documentation isrequired, if using a PA address.Line 5 - Include one-half of your Railroad RetirementTier 1 Benefits. Submit a copy of form RRB-1099.CAUTION: The total income from old age benefitprograms from other countries, such as Service CanadaOld Age Security, must be converted into U.S. dollars andreported on Line 6.Line 6 - Include the gross amount (not the taxableamount) of pensions, annuities, Individual RetirementAccount distributions, Tier 2 Railroad Retirement Benefits,and Civil Service Disability Benefits. Do not include BlackLung Benefits federal veterans’ disability payments, orstate veterans’ benefits. State veterans’ benefits includeservice connected compensation or benefits of any kindprovided to a veteran or unmarried surviving spousepaid by a commonwealth agency or authorized underthe laws of the commonwealth. Submit photocopies ofpension/annuity benefits statements along with otherforms 1099 showing income for 2017.IMPORTANT: Do not include rollovers from IndividualRetirement Accounts and employer pensions. However,proof must be provided. Proof includes, but is not limitedto, a federal Form 1099-R showing a rollover or other

documentation indicating that the distribution was rolledinto a new account.If you have one or more distributions from annuity, lifeinsurance or endowment contracts reported on Form1099-R that are included as interest income on your PA-40Personal Income Tax Return, please write “Included asInterest Income on PA-40” across the top of any Form1099-R for such distributions and include copies of all yourForms 1099-R with your claim form.Line 7 - Report interest and dividends received or creditedduring the year, whether or not you actually receivedthe cash. If you received dividends and capital gainsdistributions from mutual funds, report the capital gainsdistributions portion of the income as dividends, notas gains from the sale or exchange of property. Includeinterest received from government entities. You must alsoinclude all tax-exempt interest income from direct obliga-tions of the U.S. government, any state government, or anypolitical subdivision thereof in the amount shown on Line 7.SUBMIT THE FOLLOWING:• A copy of your federal Form 1040 Schedule B or your

PA-40 Schedule A and/or B; or copies of any federalForms 1099 you received; OR

• A copy of the front page of your PA or federal income taxreturn verifying the income reported on Line 7.

IMPORTANT: If you received capital gains distributionsfrom a mutual fund, you must use PA Schedule B or thefront page of your PA tax return to verify your income. Ifyou have PA tax-exempt interest income, you must includefederal Form 1040 Schedule B along with a copy of thefront page of your federal tax return.Line 8 - Include gains or losses you realized from the saleof stocks, bonds, and other tangible or intangible propertyas well as any gains or losses realized as a partner in apartnership or shareholder in a PA S corporation. Do notinclude capital gains distributions from mutual fundsrequired to be reported on Line 7.NOTE: The nontaxable gain on the sale of your principalresidence must also be reported on this line. If you realizeda loss from the sale of your principal residence, this lossmay be used to offset any other gains you realized fromthe sale of tangible or intangible property. However, anynet loss reported on this line cannot be deducted from anyother income. rollovers (must provide a copy of federal Form 1099-Rindicating rollover or other supporting documentation).

CAUTION: If a claimant had significant income that is nottypically received in equal amounts throughout the claimyear, or if the number-of-days method does not accuratelycalculate the annualized income, the claim may be filedusing an alternative method for determining the annualizedincome amount to be included on Line 11g. A worksheetmust be included to show how the amount of annualizedincome was determined in those instances. The worksheetmust clearly show how the income was determined and anexplanation of the reason for deviating from the ScheduleG method. Examples of when the claim would be filedusing an alternative method would include instanceswhere there is income from a one-time event such as again on the sale of stock, lump sum payments from an IRAor annuity, an amount is reported as an inheritance or apayment is received as a beneficiary on a life insurancepolicy. Revenue from slots gaming is providing general propertytax relief to all Pennsylvania homeowners. Supplementalproperty tax rebates, equal to 50 percent of taxpayers’base rebates, are available to provide extra relief tohomeowners who need it the most.Homeowners in Pittsburgh, Scranton and Philadelphia witheligibility income of $30,000 or less will receive additionalpayments, as will homeowners in the rest of the state whomeet the same income-eligibility requirement and pay morethan 15 percent of their household income in property taxes.

IMPORTANT: If you are eligible for a supplementalpayment above the maximum rebate, the department willcalculate it for you. Please follow the instructions for Lines13 and 14 on Pages 9 and 11 of this booklet to completeyour rebate application; do not adjust the amounts onLine 14.

Since its very first game went on sale in 1972, the PennsylvaniaLottery has contributed nearly $28 billion to programs thatinclude property tax and rent rebates; transportation services;care services; prescription assistance; and a broad range oflocal services provided by Area Agencies on Aging.

20 PA-1000 Pennsylvania Property Tax or Rent Rebate Program

My Fellow Pennsylvanians:

The Property Tax/Rent Rebate program provides rebates up to $975 each year to hundreds ofthousands of older Pennsylvanians and residents with disabilities. This is one of the many programs funded by the Pennsylvania Lottery that benefit older Pennsylvanians.

The Pennsylvania Lottery has funded property tax relief for seniors since the early 1970s and isthe only lottery in the nation that devotes all proceeds to programs that benefit older residents.Since its inception, the Lottery has contributed nearly $28 billion to programs that have grown toinclude the Property Tax/Rent Rebate program, a free and reduced-fare public transit program,the low-cost prescription drug programs PACE and PACENET, long-term care services and the52 Area Agencies on Aging that serve all 67 counties, as well as hundreds of full- and part-timesenior community centers across the state.

All of these programs and services are part of Pennsylvania’s commitment to ensuring healthier,happier lives for its 2.9 million older residents. In fiscal year 2015-16, every day, the Lotteryhelped to provide older adults with nearly 20,700 prescriptions, more than 24,000 meals, over105,000 rides, and more than $834,000 in care services – all adding up to more than $1 billionin annual support.

The Property Tax/Rent Rebate program is available to qualified older Pennsylvanians and permanently disabled residents. Every qualified resident should use the program. If you think afriend, neighbor or family member may qualify for a rebate, tell them about the program. I donot want a single senior to miss out on the help they need.
