IntroductionHair fall is a major problem that affects both men and women at some point in their lives. There are many causes of hair fall. The most common factor is androgenetic alopecia. The cause of androgenetic alopecia is hereditary and there is usually a genetic history of it, up to three generations, on either the mother’s or the father’s side. Balding in androgenetic alopecia is caused by the action of DHT (dehyrotestosterone), which is the active form of the hormone testosterone on the receptors present in the hair follicles. We classify balding in Hamilton–Norwood range of 1–7 for men and Ludwig range of 1–3 for women, to assess the severity of balding as depicted in the picture below.

The treatment options that have a definite role in the treatment of androgenetic alopecia are broadly divided into two. The first option is medical treatment and the second option is surgical hair transplant. Medically, there are only two medicines, namely Minoxidil lotion and oral finasteride tablets, that have a testified role in treating hair fall. Medicines work only in the early stages of balding, namely stages 1–4. They may stop the hair fall and retain the hair that are present. But these medical treatments have some disadvantages also, such as they need to be taken regularly, they have to be used lifelong, and they cannot help a bald person grow a head full of hair.

When the balding has progressed to stage 4 and beyond the “gold standard” for hair restoration the solution is hair transplant procedure. The greatest benefit of this procedure is that it offers a permanent solution. The transplanted hair are permanent, i.e. they stay for life; never fall; and need subsequent haircuts. The principle of hair transplant is that all of us have a zone of hair on the back and the sides of our scalp that are permanent or, in other words, are unresponsive to the DHT hormone, therefore, they never fall. Thus, we aim at redistributing these permanent hair follicles to the area of balding where they can add significant value.

Hair transplant to permanently correct baldness is done at our clinic by follicular unit transplantation (FUT) using the patented “follicle by follicle technique” .The hair that are transplanted to the bald area are permanent, appear absolutely natural, and do not look different from the existing or natural hair in appearance or direction of growth. You can comb them to your desired style, oil or shampoo them, and even shave them.

The procedure is done under local anesthesia on an outpatient basis. You would stay awake throughout. You can watch TV, listen to music, and take your lunch during the break. You go home immediately after the procedure. Depending on the area and number of follicle units, it usually takes approximately 2–6 hours for one session of hair transplant.

FUT vs FUEHair transplant is done by two techniques world over. One method is the follicular unit transplantation (FUT) method or the strip method and the other is the follicular unit extraction (FUE) or punch method or automated/robotic method. We usually do hair transplant through FUT, using the strip method, which is the best method and used by 90 per cent surgeons all over the world. We prefer doing FUE only in special circumstances, as FUT or strip method offers many advantages over FUE. There is a lot of marketing over the internet by a small group of surgeons who use direct hair implantation, or FUE technique; but this procedure has limitations and is not the ideal way for hair transplant in most cases. This FUE procedure is a blind technique. There is damage to the hair roots, which results in permanent loss of 10–30 per cent permanent hair roots even in the best of hands. The stock of permanent donor hair is limited in any individual, and thus this loss is significant. Adjacent follicles cannot be harvested during FUE as if two follicles are extracted side-by-side, it will leave a visible bald patch. At least 1–2 follicles are kept between harvested follicles, and thus it is difficult to get more than 1500–2000 follicles in one sitting, which is inadequate in most cases .Also, if 1000 follicle units are harvested from the back, there will be 1000 dot scars at the back of the head, which is much more scarring than 1 linear scar after taking a strip in the case of FUT. The scars in either case are visible only after shaving the head, they cannot be seen even when the hair are small.

Procedure Details After giving local anesthesia we take a strip of skin containing the hair and hair follicles from the back and sides of the scalp. The length of the strip depends on the amount of follicles required and the width of the strip is between 1–1.5 cm. After taking the strip, the gap is closed with a stitch, which is removed on the 10th day after the procedure. We do a “trichophytic closure” of the linear scar where hair grow through the scar and make the scar hardly visible. About 3–6 months after the treatment, the scar can hardly be made out. After harvesting the strip, we make small gaps at the front of the scalp to receive the tissues from the grafted strip. The strip is cut to give individual follicles, which are then transplanted into the slits made in the recipient area. It takes about 2–3 hours to place the grafts in their position. Overall the procedure takes about 6–8 hours and a full medical team, consisting of 8–10 people, work at the same time. The patient goes back home the same day and no stay with us is required.

ResultsIn the initial 3–4 weeks after the procedure, all the transplanted hair fall off. After a rest of another month, the hair start to grow out of the scalp again. The hair then grow at a particular speed – on an average at a rate of 1 cm per month. Therefore, a waiting period of 6 months after the surgery is required to appreciate the results of the hair transplant procedure.. Once the 6 months are complete, the hair that would have grown back would be your permanent hair. They would never fall, they’ll grow normally and would need haircuts. IntroductionLiposuction means removal of fat by surgery. In liposuction, the surgeon loosens and removes the fat from a specified area by using special instruments. Liposuction was first practiced in France in the late 1970s and since 1981, has been in practice in the United State and other parts of the world. It is the most commonly performed cosmetic surgery procedure in the world. Localised or generalised fat collection on hips, thighs, buttocks, abdomen, waist, back, cheek, jowls and neck, arms, upper torso, and other body areas can be reduced by this procedure.

Ideal Candidate Liposuction is not a cure for obesity, neither it is a substitute for dieting. People with reasonably normal weight and healthy elastic skin, who want to get rid of extra localised fat in certain areas, reap the best results from this procedure. Healthy elastic skin has the capacity to shrink after the surgery. Though patients of any age may undergo liposuction, it is not usually done if the skin too loose in a particular area. One could have loose skin due to various reasons, such as losing lot of weight, loose skin in the abdomen after pregnancy, or loss of elasticity of skin due to old age. In such cases, in addition to Liposuction, some of the skin may also have to be removed and muscles need to be tightened to get a satisfactory body figure.

Surgical Procedure:A small incision of about 3–4 mm is made (as in laparoscopy) a few inches away from the areas to be suctioned and suction cannulae (tube) are inserted. The incisions are usually made in the skin folds and their scars are hardly noticeable. On an average the body area consists of 60–85 per cent fat and the remainder content would be plasma, blood and serum; but this can vary depending upon the areas to be treated.

ResultsResults depend upon the expectations of the patients and the amount of fat that is suctioned from the given area. Usually, patients with good elastic skin who go for recontouring of particular areas are very happy. Also in case of some patients, all unwanted fat cannot be removed in the first sitting. In such cases, liposuction may have to be repeated. A second liposuction cannot be done on the same area until all the swelling has gone down from the area, i.e. about 4–6 months from the first surgery. However, liposuction of a different area can be done even as early as one month later.

On rare occasions, the surgery can cause rippled or uneven skin or areas of loose skin, which might require further surgeries to correct. Infection, healing problems, discolouration, unsatisfactory scar, etc. are possible but rare side effects. If used, general anesthesia carries its own, though few, hazards.

Abdominoplasty or “tummy tuck” is a cosmetic surgery procedure that makes the abdomen firmer. It tightens the muscle and fascia of the abdominal wall by removing excess skin and fat from the middle and lower abdomen. IntroductionThe term “double chin” is misleading.It is actually a neck problem and not a chin issue. It results from excess fat in the neck area that is immediately below the chin; this creates the appearance of an extra chin or a double chin.

Double chin can be a result of hereditary or lifestyle. Hereditary collections of fat that appears under the chin are difficult to lose by weight loss and exercise alone. The most common cause of a double chin is weight gain with loss of soft tissue elasticity.

Indications: Localised accumulation of fat.

ProcedureIn chin liposuction a small incision is made under the chin where unwanted fat is present. Through this gap, the fatty tissue is infused with a special solution, after which fatty tissues are removed via hollow tubes called cannulas,using the process of vacuum aspiration, i.e. creating negative air pressure. The incision is sutured with a fine thread and almost no scar is visible. Patients are advised to wear a chin strap for 3–4 weeks.

Final Result Some change can be seen immediately but the final results are apparent only after 2–3 months of the surgery, when the swelling and edema are resolved.

ProcedureAll patients are medical screened for fitness before surgery. The patient is asked to discontinue smoking and to avoid anticoagulants, such as aspirin, clopidogrel, herbal products and Vitamin E, for at least 1 week before the surgery, with doctor’s advice.

The procedure is done under general anesthesia. Incisions are made in less noticeable areas to minimise visible scarring. The most popular incision is under the breast, but in selected patients, the incision can be made around the nipple or through the armpit. You and your plastic surgeon will discuss which incision options are appropriate for your desired outcome. Incisions vary as per anatomy and patient–surgeon preference.

Breast size and shape are important, so be honest and open about your expectations when talking with your surgeon. Implant type and size would depend not only on your desired increase in size, but more importantly on your breast anatomy, skin elasticity, and body type.

RecoveryApproximate recovery time is 2–3 weeks. The patient is adviced to wear a breast support during the postoperative period.

There might be some wound healing and implant (if used) problems, but the overall risks of the procedure are minimal in healthy individuals.

Options for Breast Implants Saline breast implants are filled with sterile salt water. They can be filled with different amounts of saline that can affect the shape, firmness, and feel of the breast should the implant shell leak or be naturally expelled by the body.

Silicone breast implants are filled with an elastic gel. The gel feels and moves much like the natural breast tissue. Silicone gel implants feel and appear more natural. They are becoming increasingly more popular than saline implants.

Currently, saline breast implants are U.S. FDA-approved for augmentation in women of 18 years of age and older; and silicone breast implants are U.S. FDA-approved for augmentation in women of 22 years of age and older.

Saline or silicone implants can be advised at younger age for reconstruction purposes.

The type of implant, degree of enlargement desired, body type, and surgeon’s recommendations affects the method of insertion and the position of implants.

Layered sutures are done on the incisions made in the breast tissue; and sutures, skin adhesive, or surgical tape are used to close the skin. Over time, the incision line will fade and post-surgical swelling will resolve. Results of breast augmentation can be noticed immediately after the surgery.

Scars and ContracturesA scar is a result of body’s natural wound healing process and can be caused by acne, burns, surgery, or any kind of accidental injury. Scars are actually tissues that replace the damaged skin or tissues after an injury.

Although complete scar removal is not possible, but with the help of different types of surgery, scars can be healed considerably so that they are less visible.

A common method is the surgical removal of the scar tissues. After removing the scar tissues, the area is closed with small stitches, some of which are absorbable. Nonabsorbable sutures are removed 7–10 days after the surgery, and the best result can be seen in a few weeks’ time. It is a simple procedure, done under local anesthesia without any major complications or side effects.

Skin grafting and flapping are also commonly used techniques. In these procedures, the scar tissues are removed and then a skin graft or flap is placed on the area. This technique is generally used for contractures or if the scar covers a large area.

Keloid and hypertrophic scars are caused by excessive production of collagen. Keloid scars may increase in size over a period of time. On the other hand hypertrophic scars do not spread from the area of the initial wound and look like thick elevated lumps.

If keloids are present over the ear lobe or are small in size, they can be treated with surgical cutting.

IntroductionReconstructive surgery can correct functional impairments that are caused by traumatic injuries, such as facial bone fractures and breaks; congenital abnormalities, such as cleft palates or cleft lips; developmental abnormalities; infections and diseases; burns; and cancer or tumors. The surgery is usually done to improve function, but it may also be used to recreate a “normal” appearance.

Tumor removal, laceration repair, scar repair, hand surgery, and breast reduction surgery are the most common reconstructive procedures. Other examples are breast reconstruction after a mastectomy, cleft lip and palate surgery, contracture surgery for burn survivors, and creating a new outer ear when one is congenitally absent.

FACELIFT

IntroductionThe surgical procedure of facelift improves visible signs of aging on face and neck.

Ideal CandidatesCandidates that have developed some sagging in the mid face; have deep creases below the lower eyelids and from along the nose to the corners of the mouth; have problem of fallen or displaced fat; have developed jowls due to loss of muscle tone; or have loose skin or fat deposit under chin and jaw (double chin) would benefit from undergoing this procedure.

ProcedureThe surgery is usually performed under local anesthesia with intravenous sedation or general anesthesia.

Types: Traditional facelift, limited incision facelift, or a neck lift.

Traditional Facelift

In a traditional facelift, the surgeon makes an incision from the hairline at the temples, contining around the ear and ending at the lower scalp. Then the surgeon may redistribute the fat from the face, jowls, and neck; reposition the underlying tissue. Usually the deeper layers of the face and the muscles are also lifted.

THREADLIFT

IntroductionThread lift is a safer, convenient alternative to traditional facelift designed for minor facial sagging. It is less invasive and costs less.

ProcedureLocal anesthesia is given.

The surgeon inserts a hollow needle in the skin at a predetermined area, and then a barbed thread is slipped into the skin through the needle. The needle is then removed, and the doctor would lift the droopy areas with smooth and barbed sutures and secure them down. BLEPHAROPLASTY (EYE LID SURGERY )

IntroductionThe plastic surgery for eyelids, blepharoplasty is done to correct defects, deformities, and disfigurations of the eyelids; and to aesthetically modify the eye region of the face. With the removal or the repositioning (or both) of excess tissues, and the reinforcement of the corresponding muscle and tendon, the blepharoplasty procedure resolves functional and cosmetic problems of the periorbita, i.e. the area from the eyebrow to the upper portion of the cheek.

Older people normally undergo changes with age such as excess eye lid skin with and without protruding fat. Some also suffer from restricted vision because of droopy lids.

Although more common in women, now men are also increasingly opting for this type of surgery.

ProcedureIncisions are made along the natural skin lines or creases of the upper and the lower eyelids; the natural creases later hide the surgical scars, especially when affected in the skin creases below the eyelashes of the lower eyelid.

Depending upon the technique applied by the plastic surgeon, for instance in the case of a transconjunctival blepharoplasty, the incisions can be made from the conjunctiva, the interior surface of the lower eyelid.
