Synthes 3
The Synthes LCP Proximal Tibial Plate 4.5/5.0 ispart of the LCP Periarticular Plating System, whichmerges locking screw technology with conven-tional plating techniques. The LCP PeriarticularPlating System is capable of addressing complexfractures of the – distal femur with the LCP Condylar Plates
4.5/5.0, – proximal femur with the LCP Proximal Femoral
Plates 4.5/5.0 and the LCP Proximal FemoralHook Plates 4.5/5.0,
– proximal tibia with the LCP Proximal Tibial Plate4.5/5.0 and LCP Medial Proximal Tibia Plates4.5/5.0.
The Locking Compression Plate (LCP) has combi-holes in the plate shaft that combine a dynamiccompression unit (DCU) hole with a locking screwhole. The combi-hole provides the flexibility of axial compression and locking capability through-out the length of the plate shaft.
Note: For information on fixation principles usingconventional and locked plating techniques, please refer to the Synthes Large Fragment Lock-ing Compression Plate (LCP) Technique Guide(036.000.019).

AO Principles
In 1958, the AO formulated four basic principles which havebecome the guidelines for internal fixation.1 Those principles,as applied to the LCP Proximal Tibial Plate 4.5/5.0 insertedwith Periarticular Aiming Arm Instruments, are:
Anatomic reductionFacilitates restoration of the articular surface using guidewires for reduction and insertion of cannulated screws. Pre-contoured plate assists reduction of metaphysis to diaphysis.
Stable fixationLocking screws create a fixed-angle construct, providing angular stability.
Preservation of blood supplyTapered end simplifies submuscular plate insertion, improvingtissue viability. Limited contact design reduces plate-to-bonecontact and vascular trauma.
Early, active mobilizationPlate features combined with AO technique create an envi-ronment for bone healing, expediting a return to optimalfunction.

4Reduce articular surface
Instrument
394.350 Large Distractor, complete
Reduce the fracture fragments and confirm reduction underimage intensifier control. Fragments may be reduced usingindependent Kirschner wires; however, Kirschner wire holesare also provided in the plate to help achieve provisional reduction, plate position, or fixation.
The locking screws do not provide interfragmentary or plate-to-bone compression; therefore, any desired compressionmust be achieved with traditional lag screws. The angularfragments must be reduced and compression must be ob-tained before applying the LCP Proximal Tibial Plate 4.5/5.0with locking screws. Alternatively cannulated conical screws 5.0 mm can be used through the head of the plate tocompress the plate to the bone.
Technique tips– Before reduction, application of an external fixator or
large distractor may facilitate reduction of the joint.– To verify that lag screws will not interfere with plate
placement, hold the plate laterally to the bone.

319.701 Measuring Device for Cannulated Locking Screws and Cannulated Conical Screws 5.0 and 7.3 mm
511.774 Torque Limiter, 4 Nm, for AO/ASIF Quick Coupling for Reamers
Important: Before proceeding, confirm plate head place-ment.
Use clinical examination and image intensifier control to con-firm that:– Screw trajectories in the proximal locking holes are parallel
to the joint in the transverse plane, and the plate is oriented properly on the plateau;
– Screw and plate placement are consistent with the pre -operative plan; and
– Alignment of the plate to the shaft of the tibia is correctin both the AP and lateral views. Placement of the plate at this point will determine the final flexion/extension reduction.
Use the measuring device for cannulated locking screws tomeasure for screw lengths of the anterior and posterior headscrews. Screw length is indicated by the etched line on theguide wire.
For proper screw length measurement, the measuring devicemust contact the end of the wire guide for screws 5.0.This will place the tip of the screw at the tip of the guidewire.
Note: If the plate shifts during screw insertion, the guidewires must be removed and reinserted for the screws to lockto the plate properly.
Technique tip: The self-drilling, self-tapping flutes of thecannulated conical screws 5.0 mm make predrilling andpretapping unnecessary in most cases. In dense bone, thelateral cortex can be predrilled using the cannulated drill bit 4.3 mm, for screws 5.0 mm.
Surgical Technique
16 Synthes Periarticular Aiming Arm Instruments for LCP Proximal Tibial Plate 4.5/5.0 Technique Guide
Remove the anterior and posterior wire guide assemblies andinsert the appropriate length screw over the guide wire andinto the bone, using the cannulated hexagonal screwdriver.Locking screws may be inserted using power equipment. However, final tightening should be done by hand.
Note: To compress the plate to the lateral tibial plateau, it isnecessary to use a conical screw before inserting any lockingscrews. Conical screws should be replaced with lockingscrews after reduction is complete.
Important– It is recommended to keep the central locking bolt con-
nected and not place a screw centrally at this time. One ofthe connecting points anterior, posterior or central in theplate head must remain in place to ensure that the aimingarm alignment is maintained.
– If the 4 Nm torque limiting attachment is unavailable, donot tighten the screws to the plate under power. Performfinal tightening by hand.
