    Personnel Lifting PlatformsWAC 296-882-100

    Scope

    This rule applies to the lifting, lowering, and transporting of personnelusing personnel platforms connected to cranes or derricks.

    Personnel platforms can be either suspended from the crane or derrick bywire rope or chain, or attached to the boom of the crane or derrick. Usingarticulating boom cranes with suspended platforms is not allowed

    Important!

    Do not use cranes or derricks for personnel lifting when there is aless hazardous way to accomplish the work.

    Cranes and derricks are designed and intended for materialhandling, and are not required to meet the same designstandards as manlifts, aerial lifts, bucket trucks, and otherconventional personnel-lifting equipment.

    The lifting or lowering of personnel using material handling cranes

    and derricks is prohibited unless all of the applicablerequirements of WACs 296-882-100 through 296-882-60030 aremet.

    ASME B23-0.1Within the general scope defined in Section I, ASME B30.23 may apply

    to hoisting equipment covered within certain volumes of the ASMEB30 standard, that is used to lift, lower, or transport personnel in aplatform, by wire rope or chain, from hoist equipment, or by aplatform that is mounted on a boom of the hoist equipment. Thelifting of personnel is not allowed using some ASME B30 Standard

    equipment. The ASME B30 volume addressing the hoistingequipment to be used shall be consulted for the applicability of the

    ASME B30.23 volume.

    Section 23-0.2: Intent of ASME B30.23

    The intent of this volume is to establish minimum design criteria,equipment features, and operational procedures for using materialhoisting equipment to lift personnel. Use of this volume will not raise thelevel of safety of the material hoisting equipment to that obtained byusing equipment specifically designed and manufactured for liftingpersonnel. The ASME B30 standard hoisting equipment is designedand intended for material handling and is not required to meet the samedesign standards required of conventional personnel lifting equipment(ANSI/SIA A 92 Aerial Platforms). Lifting and lowering of personnel

    using ASME B30 Standard hoisting equipment shall be undertaken onlyin circumstances when it is not possible to accomplish the task by lesshazardous means. Unless all of the applicable requirements of thisvolume are met, the lifting or lowering of personnel using ASME B30Standard equipment is prohibited.

    You must:

    for the period of personnel lifting- An anti-two-block device is installed on the hoisting systems- On cranes and derricks with variable angle booms there is a

    boom angle indicator that is clearly visible to the operator- Has a device that clearly shows the operator the booms

    extended length, unless the boom length does not affect the loadrating, on cranes with telescoping booms

    1.2.2 Hoisting Equipment(a) Features(1)Hoist equipment live boom or live load capabilities shall be removed for

    the period of personnel lifting.(2) Rope or chain hoisting systems, that are used for personnel lifting,shall have an anti-two-block device installed.(3) Hoisting equipment with a variable angle boom shall have a boomangle indicator readily visible to the operator(4) Hoisting equipment with telescoping booms shall have a boom lengthindicator, readable from the operators station, unless the load rating isindependent of the boom length.(5) Hooks used for attachment of a personnel lifting platform shall be ofa type that canbe positively locked closed and that will prevent theplatform lifting bridle from being dislodged.(6) Hoisting equipment shall have automatic brakes such that when theequipment operating controls are released the motions are brought torest.(7) A holding device (such as a load hold check valve) shall be providedin hydraulic or pneumatic systems to prevent uncontrolled movement of

    - Has hooks, when used to attach a platform, that

    prevent uncontrolled movement of the hoisting equipment if a

    WAC 296-882-40005Initial inspections on personnel platforms

    You must

    Have a qualified person inspect personnel platforms before initial

    use and at each new job site to make sure the requirements of WAC

    296-882-100 through WAC 296-882-60030 are met

    2.1.1 Personnel Lifting Platform (Inspection)(b) Frequency(1) Initial Inspection. Prior to initial use and at each new job site all

    platforms shall be inspected by a qualified person to verifycompliance with the provisions of this volume.

    Correct any hazardous condition before using the platform.

    Keep the most recent, dated inspection records available for the

    duration of the lift operation.

    2.1.1(b)(2) Regular Inspection(a) Frequent. The platform, suspension system, attachment points, andany platform motion controls shall be inspected at least once each day,before use, by a designated person. The inspection is to identifyconditions that have been specifically indicated by the platformmanufacturer, or a qualified person, as potentially creating a hazardousoperating condition. The inspection shall address at least those itemslisted in Appendix III. Any conditions found that constitute a hazard shallbe corrected prior to lifting personnel.2.1.1(b)(2)(c) Records(1) For frequent inspections, dated records for the hoisting equipmentand personnel platform shall be made and kept by the platform user forthe duration of the personnel lift operation

    WAC 296-882-40015Periodic inspections on personnel platforms

    You must:

    Make sure a qualified person performs a periodic inspection of

    personnel platforms at least once a year, according to manufacturersinstructions

    Correct any hazardous condition before using the platform

    Keep the most recent, dated inspection records with the platform

    and available for review

    Note: Platforms that have been out of service for more than a year dont

    2.1.1(b)(2)(b) Periodic. At least once every 12 months, or as required by theplatform manufacturer, a periodic inspection of the platform shall beperformed by a qualified person in accordance with the instructionsprovided by the manufacturer. Platforms which have been out of servicefor 12 or more consecutive months shall receive a periodic inspectionbefore use.2.3.1 (b)(1) Any hazardous condition disclosed by any inspection shall becorrected before use of the platform is resumed.     WAC 296-882-40020Adjustments and repairs on personnel platforms

    You must:

    Make sure any adjustments or repairs to the platform is done bya designated person

    Adjustments or repairs to the suspension system must be done

    by a qualified person.

    Make sure replacement parts and repairs are equal to or exceed

    the original equipment specifications.

    Make sure the manufacturer or a qualified person has approved

    any modifications, in writing, before they are made.

    Keep records of any repairs to the structural components of the

    platform

    Make sure all welding repairs are done by a certified welder.

    2.3.1(b)(5) Any adjustment or repair to the platform suspension systemshall be done by a qualified person2.3.1 (b)(1) Any hazardous condition disclosed by any inspection shall becorrected before use of the platform is resumed. Adjustments and

    repairs shall be accomplished by designated personnel.

    You must

    Test as follows:

    - Test to 125% of the platforms rated capacity- Hold the platform suspended for 5 minutes with the test

    load evenly distributed on the platformNote: This may be done at the same time as the trial lift

    2.2.1(b)Proof Testing(1) At each new job site, prior to hoisting people in the personnelplatform, the platform and rigging shall be proof tested to 125% of theplatforms rating. With the proof test load evenly distributed in theplatform, the platform shall be hoisted and held in a suspended positionfor not less than five minutes. After the proof testing, the platform shallbe inspected by a qualified person. Any damage revealed by theinspection shall be corrected and another proof test conducted.

    2.2.1(b)(3) After any structural repair or modification, a platform shall beproof tested to 150% of the platforms rating. Boom attached platformsshall use the test procedure described in para. 23.2.2.1(b)(1).Suspended platforms shall be tested by raising the loaded platform to aheight, then lowering it at a speed of not less than 100 ft/min. Once alowering speed of 100 ft/mion or more is reached, the platform descentshall be halted by applying the hoisting equipment brakes and then the

    platform allowed to hang for a period of not less than five minutes. Afterthis test and hanging period, the platform shall be inspected by aqualified person. Any damage revealed by the inspection shall becorrected and another proof test conducted.

    Test attached platforms in the following order:

    - Test to 150% of the platforms rated capacity.- Hold the platform suspended for 5 minutes with the test

    load evenly distributed on the platform.- Have an appointed person inspect the platform and

    rigging.- Correct any deficiencies.- Perform another test after any deficiencies are corrected.

    (3) After any structural repair or modification, a platform shall be prooftested to 150% of the platforms rating. Boom attached platforms shalluse the test procedure described in para. 23.2.2.1(b)(1).2.2.1(b)(1)

    At each new job site, prior to hoisting people in the personnel platform,the platform and rigging shall be proof tested to 125% of the platformsrating. With the proof test load evenly distributed in the platform, theplatform shall be hoisted and held in a suspended position for not lessthan five minutes. After the proof testing, the platform shall be inspectedby a qualified person. Any damage revealed by the inspection shall be

    corrected and another proof test conducted.

    Your responsibility:

    To make sure all duties are performed by the correctly appointed person.

    WAC 296-882-50005Job site management

    You must:

    Make sure job site management performs the duties listed in Table 2.

    Table 2Job site management duties

    Designate a Personnel Lift Supervisor for each personnel

    lift operation.

    Require that all personnel associated with the lift receive the

    briefings and safety indoctrinations specified in this volume.

    Prohibit the lifting of personnel whenever the operator:- Does not feel physically or mentally fit to perform the

    operation.- Has been working for more than 10 hours prior to

    the start of the lift or the lift will not be completedbefore the operator has been working for 12 hours.

    - Did not have at least eight hours off immediatelyprior to the work shift containing the personnel liftoperation.

    Designate the appropriate number of signalpersons, ground

    crew, and platform occupants to perform the personnel liftsafely.

    - In suspended and boom attached platforms withoutboom motion controls, one occupant shall beappointed as the platform signalperson. This

    Section 23-3.1: Personnel

    23-3.1.1 Management(a) Responsibilities(1) Management shall require that the provisions of this volume areunderstood and applied at the operational levels, and that theappropriate portions of this volume are included in the pre-lift briefing

    information.

    (2) Management shall designate qualified individuals at each job site toperform the duties of the Personnel Lift Authorizer. As a minimumthese individuals shall be responsible for:(a) verifying the need for a personnel lift;(b) verifying the equipment to be used for the personnel lift meets therequirements of Chapter 23-1;(c) authorizing the personnel lift operation(d) requiring the personnel lift be accomplished in accordance with theprovisions of this volume.

    (3) Job site management shall require that the provisions of this volumeare implemented during personnel lifting operations.

    (4) Job site management shall designate a Personnel Lift Supervisor

    vessels, not to lift personnel when the list or trim of the vesselexceeds five degrees. If mobile hoisting equipment is placed onfloating vessels, operators shall not lift personnel when the list ortrim of the vessel exceeds one degree.

    (5) Job site management shall require that all personnel associated withthe lift receive the briefings and safety indoctrinations specified in thisvolume.

    (6) Job site management shall not allow or require any operator to liftpersonnel under the following circumstances:(a) The operator does not feel physically or mentally fit to perform theoperation.(b) The operator has been working for more than 10 hours prior to thestart of the lift or the lift will not be completed before the operator hasbeen working for 12 hours.(c) the operator did not have at least eight hours off immediately prior tothe work shift containing the personnel lift operation.

    (7) Job site management shall designate the appropriate number ofsignalpersons, ground crew, and platform occupants to perform thepersonnel lift safely.(a) In suspended and boom attached platforms without boom motioncontrols, one occupant shall be designated as the platform signalperson.This person shall be responsible for communicating with the operatorand/or other designated signalpersons.

    (8) Job site management shall require that the hoisting equipment usedto lift personnel shall be in compliance with the requirements prescribedin this volume and the applicable hoisting equipment ASME B30 volume.

    Assign qualified individuals at each job site who will be responsible for

    the duties listed in Table 3.

    Verifying

    - The need for a personnel lift- That there are no less hazardous alternatives to

    performing the work, or providing access to thearea. The personnel lift shall not be authorizedwhen less hazardous means are feasible.

    Verifying the equipment to be used for the personnel lift

    meets the requirements of the chapter governing thatequipment

    Authorizing the personnel lift operation

    Reviewing and approving the personnel lift plan. Retain the

    plan as part of the job site records.

    Verifying that conditions provide clear visibility. When

    conditions such as darkness, fog, or snow prevent clearvisibility, do not authorize personnel lifting.

    Authorize personnel lifting over, under, or in the vicinity of

    power lines, in accordance with the requirements Chapter296-45, Safety standards for electrical work

    Section 23-3.2: Practices

    23-3.2.1 ManagementJob site management shall have a Personnel Lifting Management and

    Approval Program that requires the criteria of this volume to be met priorto lifting personnel. The program shall, as a minimum, require:(a) The Personnel Lift Supervisor to prepare a personnel lift plancontaining at least the information shown in Appendix II. This plan shallbe presented to the Personnel Lift Authorizer for review/approval. Theplan shall be retained as part of the job site records.(b) The Personnel Lift Authorizer to verify that there are no lesshazardous alternatives to performing the work, or providing access tothe area. The personnel lift shall not be authorized when less hazardousmeans are feasible.(c) The Personnel Lift Authorizer to verify that conditions provide clearvisibility. When conditions such as darkness, fog, or snow prevent clearvisibility, the Personnel Lift Authorizer shall not authorize personnellifting.(d) The Personnel Lift Authorizer to only authorize personnel lifting over,under, or in the vicinity of power lines, in accordance with therequirements of Appendix I;(e) personnel platforms not to be used in winds in excess of 20 mph(32.2 km/hr) at the hoisted platform height, electric storms, snow, ice,sleet, or other adverse weather conditions which could affect the safetyof personnel.(f) the Personnel Lift Supervisor to verify that each of the requirements inthis Section have been met;(g) the Personnel Lift Supervisor to hold a pre-lift briefing attended bythe hoisting equipment operator, platform occupants, and ground crew.If individuals are changed during a series of personnel lifts, each newperson shall be appropriately briefed by the Personnel Lift Supervisor.Contents of the briefing shall, as a minimum, cover:(1) the operations section of this volume(2) the assignment and responsibilities of each person involved in the liftoperation

    (3) the procedures to be followed(4) guidance on general and specific safety precautions(5) special signals for the operation(6) unique considerations of the lift(7) work to be accomplished during the lift(8) if applicable, the responsibilities and assignments contained in

    Appendix I when lifting personnel near electrical power lines

    (h) the Personnel Lift Supervisor to terminate personnel lifting operationsif hazardous conditions develop during the lift(i) personnel platforms to be only used for personnel, their tools, andsufficient material to do their work. They shall not be used for solelytransporting bulk materials.(j) a qualified person to evaluate the safety concerns of the operationalenvironment and verify the platform and hoisting equipment are suitablefor use. Additionally, special work circumstances may require furtherprecautions. Precautions such as, but not limited to the following shallbe taken:(1) When welding is to be accomplished from the personnel platform,suitable electrode holders shall be provided to protect them from contactwith any conducting components of the platform(2) Job site management shall instruct operators of hoisting equipment,installed on floating vessels, not to lift personnel when the list or trim ofthe vessel exceeds five degrees. If mobile hoisting equipment is placed

    on floating vessels, operators shall not lift personnel when the list or trimof the vessel exceeds one degree.(3) When personnel lifts are conducted over water, personnel flotationdevices shall be provided and required to be worn. Personnel fallprotection devices with quick release features shall be provided, andrequired to be worn.
