Day of appointment: Please bring the following with you to your appointment: Your insurance card, your

Physician’s name and address, and the location of where you had your prior

mammogram films.

Cost: Insurance Coverage with: United HealthCare, BlueCross BlueShield, CIGNA, AETNA,

Health Net and other select carriers Non-Insured patients pay $167.00, includes the

Radiologist reading. We are pleased to offer Full Field Digital Mammography and

CAD Computerized reading.

We will verify your insurance benefits and inform you if there are any charges that you will be responsible for.

Patients under the age of 30 must provide a Doctor’s referral. Baseline mammograms recommended at age 35-39, (unless family

history or symptomatic) and most insurance plans cover women every year over 40 years old.

We are able to accommodate breast implant patients.

Sweet Potato Salad

Who says you can only use white potatoes for a potato salad? This dish will be a welcomed surprise at your next summer picnic.

Prep Time: 25 minutes

Nutritional Facts:

Calories: 100

Carbohydrates: 16g

Protein: 2g

Fat: 3g

Saturated Fat: 0.6g

Sugars: 5g

Dietary Fiber: 2g

Cholesterol: 5mg

Sodium: 95mg

Potassium: 250mg

Ingredients: 3 large sweet potatoes (about 13-ounces each)

3 stalks celery, diced

2 green onions, sliced

4 slices turkey bacon

2 tablespoons olive oil

1 tablespoon apple cider vinegar

1/8 teaspoon ground black pepper

1/2 teaspoon garlic powder

Directions:

1. Peel sweet potatoes and cube. Place sweet potatoes in a large pot. Cover with water and bring to a boil. Boil until tender about 5-7 minutes. Drain potatoes and rinse with cold water; let cool.

2. While potatoes are cooking, cook bacon and chop. 3. In a small bowl, whisk together, olive oil, apple cider vinegar, pepper and garlic powder. 4. Place sweet potatoes in serving bowl and add celery, green onions and turkey bacon. Pour

dressing over salad and toss until potatoes are coated. Serve cold.

The PUHSD Incentive Program has 3 tiers:

How do I show proof of my online tracker?

Take a snapshot and send it as an attachment with your forms. What proof do I need for my PCP wellness check, gym membership, weight loss program, etc.?

This is an honor based program. You only need to attach a tracker/log of your choice, a selfie, and a certificate to the EAP Webinar.

How do I submit my forms?

You can scan the form, take a snapshot, or send an email with your activities to [email protected]

Why do I keep receiving these emails? How many times do I have to complete the Tiers?

You only need to submit one entry to each of the tiers. We want to remind participants to get their information in by the deadline.

SPRING INTO FITNESS AT THIS 4-WEEK WORKOUT SERIES!

Dance, tone, strengthen and burn 2 the beats all while you have a fun time with the Creators of EDM

BODY FITNESS® - A dance party workout with bootcamp inspired combinations. Plus, dress to impress to

different themed workouts nights & win giveaways each week! We launch on April 5th with a Dance Flash

Mob!

April 26th - Unleash your Hero - Superhero Night

Certified instructors will teach a 60-minute workout on stage to bumpin’ DJ beats, while they motivate,

inspire and push your limits - modification will be instructed. Then make sure to stick around after the

workouts to ask questions and meet the instructors.

IGNITE NIGHTS at OdySea is a FREE community event that is family friendly — all ages and fitness

levels are welcome. Please bring your own mat. Spread the word and share the fun!

Do you want to be part of our Phoenix Union High School District Wellness

Ambassador team to help create a healthy campus culture?

Wellness ambassadors serve as PUHSD’s key communicators of the wellness program to

direct employees to wellness programs, activities, and resources. Ambassadors play a

vital role in supporting the promotion of health and wellness among faculty and staff. As

an ambassador, you will act as a liaison between your school and the wellness

coordinator. The role is voluntary with minimal time commitment.
