TO ASSEMBLE: Print and tape together the “Sequence Mat”
= Total of 3 sheets of paper taped together to make a poster.
Hang “Sequence Mat” at eye level for the student to see.
Print, Laminate and tape “Shapes Jumping Spots” in an array around the student on the floor a few feet from the poster.
TO PLAY: 1. Student will visually scan the “Sequence Mat” poster left to right and top to
bottom. 2. As student scans on each apple, he/she will jump on that corresponding
"Shapes Jumping Spot” that is on the floor.
3. Student is to keep their body facing forward at all times with eyes facing the “Sequence Mat” poster.
4. Student will be jumping forward, sideways, and backward onto the “Shapes Jumping Spots”.
5. PT can assist the student by using a pointer along the “Sequence Mat” to guide the student.
6. Next, encourage the student to visually scan independently. 7. Work toward “chunking” the shape jumps. For example, student will scan 2-
3 shapes, recall that information and jump without long pauses.
**Suggestion- Use a metronome (app) to time the jumps working to increase accuracy with increasing speed.

Description of Game: Child uses a score card to record pointsfor each golf ball he/she can putt into the holes. Each shape has an assigned score value for getting the golf ball into that shaped hole.
Materials Required: Empty cardboard box that measures approximately 17 inches
long X 8.5 inches high X11 inches wide. A standard box that holds 10 reams of copy paper works well.
Golf ball(s) and Golf Club Targets (included) Score Card (included)
Assembly Instructions: • Print and cut out targets. Apply the targets to the side of the box as
follows:• Turn the box upside down and attach the targets next to one
another on one side of the box. • Next, use scissors to cut out the three shaped areas on the box.
This will allow the putted ball to enter the box. • Print and cut score card for each child’s use.
How to Play:1. Child will stand on spot as designated by the PT and place the golf
ball at their feet.2. Child is to use the club to gently tap/putt the golf ball toward one of
the three shaped targets. Each target has a different point value.3. PT can allow the child to use the club to hit the ball more than once
on each turn as he/she attempts to score. Or the PT may allow only one putt per turn. The score card allows for 8 turns per child.
4. Child uses score card to record points that are earned for each turn.
Variations: - Do not keep score with the score card- Vary the distance between child and target- Do not use a golf club. Child may squat or be in prone while using his/her hands to roll a small ball forward into the target
