FACT: 24 percent of all potential years of life lost in Canada in 1996 were attributable directly to poverty compared to 31 percent for cancer and 18 percent for cardiovascular disease.

FACT: Prevalence of depression is 58% higher below the poverty line than the Canadian average.

“We often see patients with unusual health issues that stem from poverty, but it’s not always obvious what the underlying cause is. With the SW, we can get into the real issues more quickly”

FACT: People with low socio-economic status are more likely to be hospitalized for conditions where hospitalization could be avoided with early disease management.

FACT: 1 in every 10 patients you see are affected by low income.

FACT: Lowest income 20% has more than double the rate of diabetes compared with the highest income. (10% vs. 5% in men, 8% vs. 3% in women).

determinants of health (SDH). Learn how to:

• Promote SDH practice with your colleagues;

• Effectively engage with your local community, or take action at

the province or national levels.

Come gather with your fellow leaders, explore

what collective action on SDH might look like and

take action to best support your patients’ health.

FACT: Only 65% of children are developing well at the age of 5 in Canada’s poorest neighbourhoods vs. 80% in the richest.

Define “stretch goal” success for a SDH-focused gathering of Physician Leaders on May 11, 2016.
