SENS DE DEROULEMENT Patient Package Leaflet in Accordance With the Pharmacists’ Regulations (Preparations) – 1986 The medicine is dispensed with a doctor's prescription only Plavix Plavix 75 mg 300 mg Film-coated Film-coated Tablets Tablets Active Ingredient Each Plavix 75 mg tablet contains: Clopidogrel (as Hydrogen Sulfate) 75 mg Each Plavix 300 mg tablet contains: Clopidogrel (as Hydrogen Sulfate) 300 mg Inactive ingredients: see section 6. Read this leaflet carefully in its entirety before using the medicine. Keep this leaflet; you may need to read it again. This leaflet contains concise information about the medicine. If you have further questions, refer to the doctor or pharmacist. This medicine has been prescribed for the treatment of your ailment. Do not pass it on to others. It may harm them, even if it seems to you that their ailment is similar. This medicine is not usually intended for children and adolescents. If a side effect worsens or if a side effect not mentioned in this leaflet appears, please refer to the doctor or pharmacist. 1. WHAT IS THIS MEDICINE INTENDED FOR? The medicine is intended for the prevention of atherothrombotic vascular events (heart attacks, cerebral stroke or vascular death) in patients with atherosclerosis who had a myocardial infarction, ischemic cerebrovascular event, or who suffer from peripheral arterial disease, and in patients suffering from unstable angina pectoris, including patients undergoing implant of a stent by catheterization and who are being treated with aspirin. The medicine is intended for prevention of atherothrombotic and thromboembolic events (heart attacks, cerebral stroke, emboli or vascular death) in combination with aspirin, in patients suffering from heart rhythm disorder such as atrial fibrillation type, who are not suitable for treatment with anti-coagulants of the vitamin K antagonist type such as warfarin (Coumadin), anti-thrombin or anti-factor Xa. Therapeutic Group: Platelet aggregation inhibitors, from the thienopyridine group. 2. BEFORE USING THE MEDICINE Do not use this medicine if: - you are sensitive (allergic) to clopidogrel or to any of the additional ingredients contained in the medicine (see section 6). - you suffer from a medical condition that presently causes bleeding, such as: peptic or duodenal ulcer, intracranial haemorrhage. - you suffer from acute liver impairment. Special warnings regarding use of this medicine: Before beginning treatment with the medicine, tell the doctor if: • you are at risk for bleeding, such as: a medical condition that puts you at risk for internal bleeding (e.g., peptic ulcer) problems with the blood system that put you at risk for internal bleeding (internal bleeding in the tissues, limbs or joints in your body) you recently underwent serious injury you recently underwent surgery (including dental surgery) you are due to undergo surgery (including dental surgery) within the next seven days • you suffered from a cerebral stroke in the last seven days • you suffer from kidney or liver dysfunction • you have suffered in the past from an allergic reaction (hypersensitivity) to any medicine given to you for treatment of the medical condition for which this medicine has been given. During treatment with this medicine: • You must inform the doctor if you are planning to undergo surgery (including dental surgery) • You must inform the doctor immediately if you develop a medical condition (also known as thrombotic thrombocytopenic purpura or TTP) which includes fever and subcutaneous bruises which can appear as red patches, with or without unexplained extreme fatigue, confusion, yellowing of skin or eyes (jaundice) (see section 4 “Side Effects”) • Inform the doctor immediately if you develop acquired type A hemophilia (see section 4 “Side Effects”), a condition which requires discontinuation of treatment with Plavix • If you are injured or wounded it is possible that bleeding will continue for longer than usual. This effect is related with the mechanism of action of the medicine which is meant to prevent the formation of blood clots. With regard to minor injuries or cuts, such as a shaving cut, it usually poses no problem, however, if you are bothered by the bleeding, you must refer to the doctor immediately (see section 4 “Side Effects”) • It is possible that the doctor will ask you to undergo blood tests Plavix is not intended for use in children or adolescents. If you are taking or have recently taken other medicines, including non-prescription medicines and food supplements, tell the doctor or pharmacist. In particular, inform the doctor or pharmacist if you are taking: - Medicines that may increase your risk of bleeding such as: • Oral anticoagulants, used to reduce blood coagulation • Non-steroidal anti-inflammatories (NSAIDs) usually given for the treatment of muscle or joint pain and/ or inflammation • Heparin or other injectable medicines used to reduce blood coagulation • Ticlopidine, another anti-platelets preparation • Medicines from the SSRI group (selective serotonin reuptake inhibitors) including but not restricted to fluoxetine or fluvoxamine or medicines from the SNRI group (serotonin-norepinephrine reuptake inhibitors) given to treat depression and anxiety - Omeprazole, esomeprazole or cimetidine, medicines given to treat abdominal pain - Fluconazole, voriconazole, ciprofloxacine, or chloramphenicol, medicines given for the treatment of bacterial or fungal infections - Efavirenz, a medicine to treat HIV infection - Carbamazepine or oxcarbazepine, medicines given for treatment of epilepsy - Moclobemide, a medicine given for the treatment of depression - Repaglinide, medicine to treat diabetes - Paclitaxel, medicine to treat cancer. If you have experienced severe chest pain (angina or heart attack), it is possible that the doctor will prescribe you Plavix together with acetylsalicylic acid, an ingredient that is found in many medicines that act as pain relievers and to reduce fever. Short term use with acetylsalicylic acid (no more than 1,000 mg in a 24 hour period) should not cause problems, however, with regard to prolonged use in other situations, you must consult with your doctor. Use of this medicine and food: This medicine may be taken with or without food. Pregnancy and breastfeeding: It is preferable not to take this medicine if you are pregnant. If you are pregnant or suspect that you are pregnant, you must inform the doctor. If you become pregnant while taking this medicine, consult with the doctor immediately. Do not breastfeed while taking this medicine. If you are breastfeeding or planning to breastfeed, consult with the doctor before taking this medicine. Driving and using machinery: It is not expected that the medicine will affect your ability to drive or operate machinery. Important information about some of the medicine’s ingredients: This medicine contains lactose. If you have a sensitivity/intolerance to sugars, consult with the doctor before taking the medicine. The medicine contains hydrogenated castor oil which may act as a laxative. 3. HOW SHOULD YOU USE THE MEDICINE? Always use according to the doctor’s instructions. Check with the doctor or pharmacist if you are uncertain. The dosage and manner of treatment will be determined by the doctor only. The usual dosage including in case of atrial fibrillation (an irregular heartbeat) is: one 75 mg tablet per day to be taken orally with or without food, and at the same time each day. If you have experienced severe chest pain (a heart attack or an unstable angina pectoris), the doctor may instruct you to take a 300 mg dosage (one 300 mg tablet or four 75 mg tablets) one time only, at the beginning of treatment. Afterwards - the recommended dosage is one Plavix 75 mg tablet a day, taken as described above. You must take the medicine for as long as the doctor continues prescribing it for you. If you accidentally take too high a dosage: Refer to the doctor or go to a hospital emergency room immediately, due to the increased risk of bleeding. If you forget to take the medicine: If you forget to take the medicine but remember within 12 hours from the fixed dosing time, take the tablet immediately and take the next tablet at the fixed time. If you forget to take the tablet and more than 12 hours have passed since the last fixed dosing time, skip that dose and wait for the next fixed time, take the tablet, and continue to take the medicine as usual. Do not take a double dose to compensate for the missed dose. Do not exceed the recommended dosage. If you took an overdose, or if a child has accidentally swallowed the medicine, refer immediately to a doctor or proceed to a hospital emergency room, and bring the package of the medicine with you. Do not chew! Swallow the medicine with water. The medicine may be taken with or without food. If you stop taking Plavix: Do not stop taking the medicine unless instructed to do so by the doctor. Consult the doctor before stopping treatment. Do not take medicines in the dark! Check the label and the dose each time you take medicine. Wear glasses if you need them. If you have further questions about using the medicine, consult with the doctor or pharmacist. 4. SIDE EFFECTS As with any medicine, use of Plavix may cause side effects in some users. Do not be alarmed by the list of side effects. You may not suffer from any of them. Consult with the doctor immediately if the following signs appear: • Fever, signs of infection or extreme fatigue. These may indicate a rare decrease in certain blood cells. • Signs of a liver dysfunction such as yellowing of the skin and/or eyes (jaundice) with or without subcutaneous bleeding appearing as red patches, and/or confusion (see section 2 “Before using this medicine”). • Swelling of the mouth or skin problems such as rash, itching and blisters. These can indicate an allergic reaction. The most common side effect is bleeding. The bleeding may occur as gastrointestinal bleeding, on the skin as bruising or hematoma (irregular bleeding or subcutaneous bruise), as nasal bleeding or blood in the urine. In a small number of cases there were reports of bleeding in the eye, intracranial bleeding, bleeding in the lungs or in the joints. If you suffer from prolonged bleeding during treatment with the medicine: If you were injured or wounded it is possible that bleeding will continue for longer than usual. This is related to the mechanism of action of the medicine which is meant to prevent the formation of blood clots. With regard to wounds or mild injuries, such as a shaving wound, it usually poses no problem, however, if you are bothered by the bleeding, you must refer to the doctor immediately (see section 2 “Before Using the Medicine”). Additional side effects: Frequent side effects (may affect up to 1 in 10 people): diarrhoea, abdominal pain, dyspepsia or heartburn. Infrequent side effects (may affect up to 1 in 100 people): headache, peptic ulcer, vomiting, nausea, constipation, gastrointestinal bloating, rash, itching, dizziness, tingling sensation or numbness. Rare side effects (may affect up to 1 in 1,000 people): dizzy sensation (vertigo), enlarged breasts in males (hypertrophy of the breast tissue). 1

Very rare side effects (may affect up to 1 in 10,000 people): jaundice, severe abdominal pain with or without backache, fever and breathing difficulties sometimes accompanied by coughing, respiratory tract bleeding, pneumonia, narrowing of the airways, general allergic reactions (for example, overall sensation of heat with discomfort until fainting), drug hypersensitivity syndrome such as dermatitis and systemichypersensitivitysyndrome(DRESS),swellingofthemouth, skin blisters, allergic skin reactions, inflammation in the mouth, drop in blood pressure, confusion, hallucinations, joint pain, muscle pain, changes in the sense of taste, acquired type A hemophilia.Frequency unknown (cannot be estimated from the existing data): hypersensitivity reactions accompanied by chest or abdominal pain, cross hypersensitivity reactions with other medicines from the thienopyridine group such as ticlopidine or prasugrel (see "Special warnings regarding use of this medicine"), acute generalized exanthematous pustulosis (AGEP).It is possible that your doctor will notice changes in your blood and urine tests.If a side effect worsens or if you notice side effects not mentioned in this leaflet, consult your doctor. (www.health.gov.il) that directs you to the online form for reporting side effects.5. hoW shoULd thIs medIcIne Be stoRed?Avoid poisoning! This medicine and any other medicine must be kept in a closed place out of the reach of children and/or infants to avoid poisoning. Do not induce vomiting unless explicitly instructed to do so by the doctor.Do not use the medicine after the expiry date (exp. date) appearing on the package. The expiry date refers to the last day of that month. Plavix 75 mg: Store at a temperature below 25°C. Plavix 300 mg: Store at a temperature below 30°C.6. fURtheR InfoRmAtIonIn addition to the active ingredient, Plavix 75 mg and Plavix 300 mg also contain:Mannitol, Macrogol 6000, Microcrystalline cellulose, Low substituted hydroxypropylcellulose, Hydrogenated castor oil.In the tablet coating:Hypromellose, Lactose monohydrate, Triacetin, Titanium dioxide,Redironoxide,Carnaubawax.How does the medicine look like and what are the contents of the package:The tablets are packaged in a blister pack.The75mgtabletsarecoated,roundandpink.Ononeside “75” is impressed and on the other side “1171” is

impressed.The300mgtabletsarecoated,oblongandpink.Ononeside “300” is impressed and on the other side “1332” is impressed.For the 75 mg dosage, there are packages of 10, 20, 30, 60 and 100 tablets.For the 300 mg dosage, there are packages of 4, 30 and 100 tablets.Notallofthepackagesizesaremarketed.This leaflet does not contain all the information about your medicine.
