Greetings from the Philippine Medical Association! It is with great pleasure and an honor for me to convey warm greetings and felicitations to the offi-

cers and members of the Quezon City Medical Society on its 32nd Annual Convention and its observance, as well, of its 66th Foundation Day at the Crowne Plaza Galleria Manila, Ortigas Avenue, Quezon City with the theme : “Unity in Diversity for a Healthy Life Style”..

The theme chosen this year aptly describes the various approaches employed by all stakeholders in the pursuit of a common objective which is health. To emphasize the importance of “life style” as a signifi-

cant factor to achieve that goal, which is health, means that you believe in the importance of promotive and preventive aspects of delivering health care to our people which, to my mind, is laudable because it entails lesser cost. Having said that, your two-days convention promise to be enjoyable and memorable to all the delegates and participants.

It is with tremendous pleasure that I am congratulating the Organizing Committee headed by Dr. Norma Torregoza and the Executive Board for working hard in coming up with the theme “Unity in Diversity Towards a Healthy Lifestyle” in this 32nd Quezon City Medical Society Convention, June 23-24, 2016 at Crowne Plaza, Galleria Manila. Quezon City Medical Society

On the event of the 32nd Annual Convention of the Quezon City Medical Society it is truly a privilege for me to greet and extend my warmest greetings to all our colleagues and friends.

Our convention will serve as a venue wherein physicians, experts and allied professionals can mutually benefit from our planned activities. Our theme “Unity in Diversity Towards a Healthy Lifestyle” aims to

promote discipline, proper medications, optimal diet, exercise and healthy lifestyle to improve our over-all well being.

The fellowship night is also something to look forward to.

A highlight of the evening is the awarding ceremony and the induction of officers as they become our new executive officers, district presidents and members. It’s a great joy and privilege to be a part of the planning of this year’s convention. I’d like to take this

BRIEF HISTORY OF THE QUEZON CITY MEDICAL SOCIETY

(1950 – 2016)

The colourful history of QCMS began way back in 1950 when Dr. Petronilo Monsod, Dr. Demetrio Crispo Lacuna and Dr. Felipe Perez Arenas met at Max Restaurant Quezon City and organized what is known now as “The Quezon City Medical

Society”. There were 37 doctors from the different hospitals of Quezon City.

The inauguration of the Society and induction of officers was held on July 22, 1950 at the Socoro Hall of the Quezon Institute. Dr. Petronio G. Monsod was inducted as the first President by Dr. Rodolfo Gonzales, the President of the Philippine Medical Association.

The 1st Regional Convention of the PMA held in Davao City in 1951 was attended by members of the QCMS. The first PMA Convention to be held in this city (Quezon City) was on May 1958 at the Ateneo Grade School Loyola Heights in which the QCMS was host.

Since, there was no permanent office for the QCMS, Board Meetings were held in different places in Quezon City like Mt. Banawe Hospital, UDMC, V. Luna Hospital depending on who the elected president was.

Auxiliary to the QCMS was organized in 1965. And the first President was Mrs. Purita David.

The Society takes pride in having 8 PMA President coming from the QCMS namely: Dr. Pedro Mayuga 1960-61, Dr. Jesus Tamesis 1963-64, Dr. Jose Denoga 1965-66, Dr. Fe del Mundo 1969-70, Dr. Jose Pujalte 1985-86, Dr. Concordia Pascual 1994-95, Dr. Gil Fernandez 2001-2003 and Dr. Rey Melchor Santos 2008-2010.

On January 10, 1970 a Contract of Lease for an office space at the PMA Auditorium was signed allowing the QCMS to hold office there for 10 years for a fee of P10,000. This was negotiated by Dr. Elpidio C. Panganiban, QCMS President (1969-1970) and by Dra. Fe Del Mundo, PMA President (1969-1970).

On May 19, 1986, a Contract of Lease of the present office of the QCMS in the New PMA Building for a period of 99 years for an initial sum of P100,000 excluding the monthly rental was signed by Dr. Jose Pujalte and Dr. Jesus de Jesus of the QCMS Building Committee then. Activities included community development and civic action; continuing medical education program: socials, fellowship and sports development program and support to the Bukal ng Pagasa Day Care Center at West Riverside SFDM.

The QCMS used to be a component society under the Central Tagalog Region of the PMA which included Caloocan Medical Society, Pasay-Paranaque Medical Society and Valenzuela Medical Society. The first PMA Governor for the Central Tagalog Region was Dr. Jesus de Jesus. Previously the region was represented by a Councilor. With its desire to be able to involve as many members as possible in the activities of the society, Dr. Benjamin Tamesis QCMS President 1975-1976 subdivided the Quezon City Medical Society into four districts following the political districts I, II, III and IV with a district councilor for each.

The following were significant events during the 1980’s: 1. The 1st Annual Convention of the Quezon City Medical Society was organized under the Presidency of Dr. Jesus de Jesus

with Dr. Juliet Santos as Overall Chairman and was held at Garden Hotel, Makati Metro Manila on June 14, 1985. 2. On May 19, 1986, a Contract of Lease of the present office of the QCMS in the new PMA Building was signed by Dr. Jose

Pujalte, PMA President at that time and Dr. Jesus de Jesus Chairman of the QCMS Building Committee. 3. The first QCMS Duckpin Bowling Tournament was organized in 1986 by Dr. Jesus de Jesus, Dr. Antonio Cabigas and Dr.

Cesar Meris.

A great change came about in the 5th decade 1991-2000 since the founding of the QCMS. This was the ushering of the computer age in our area. The society acquired a computer to systematize the vast records of the membership.

The QCMS became a member of the Local Health Board of the Quezon City government. In 1995 under the Presidency of Dr. Rey Melchor Santos, the bid of the Quezon City Medical Society to become a separate PMA Region was successful and was ratified by the PMA House of Delegates. Dr. Santos became the first PMA Gov-ernor for the Quezon City Region.

In 1996 under the Presidency of Dr. Milagros Encarnacion the Benefit and Assistance Fund was created to give welfare bene-fits to our members in the form of Disability Benefit, Death Benefit and Legal Benefit. The first Chairman was Dr. Cesar E. Meris who also formulated the Benefit and Assistance Code which was approved.

Important events 1990 – 1999 1. The QCMS became a separate region of the PMA in 1995 2. Dr. Rey Melchor Santos became the first PMA Governor for the Quezon City Region 3. The QCMS Benefit and Assistance Trust Fund was established in 1996

In year 2000 coinciding with our Golden Anniversary, the Quezon City Medical Society with Dr. Cesar E. Meris as President and Dr. Juliet Santos as PMA Governor, won the 1st PMA award for “Most Outstanding Component Medical Society” Category I

First Prize also known as Icasiano Award.

From year 2000 thereon the Society further made progress by leaps and bounds: The QCMS office was expanded and renovated in 2003. A new construction was made adjoining the old office thereby doubling the office space. This was started during the presidency of Dr. Ricardo Costes and governorship of Dr. Ofelia Borje and finished during the term of Dr. Benjamin Lim, President and Dr. Cesar Meris as PMA Governor.

In 2003 the PMA Board of Governors decided that each PMA region should have a flag that would represent the region. Dr. Ce-sar Meris the PMA Governor for the QC Region designed the Regional flag which was presented to the Board and was approved. This flag also became the official flag of the Quezon City Medical Society.

The first Mission-Vision statements were made in a Seminar workshop January 25-26, 2003 at Sulo Hotel, Quezon City attended by officers of the Quezon City Medical Society.

Summary of events in the decade 2000 – 2009 1. In 2000 the QCMS won its first PMA Icasiano Award, First Prize. 2. The QCMS office was renovated in 2003 and expanded to double its office area. 3. The first flag of the Quezon City Medical Society was designed and approved by the Board in 2003. 4. The first Mission and Vision statement of the QCMS was made and approved in 2003. 5. In 2003 the QCMS Hymn was composed by Dr. Salvador Flores and adopted by the Association 6. The Fifth District of the QCMS was created in 2007

QCMS Past Presidents were appointed as members or Chairmen of various Commissions and Committees of PMA: Dr. Amelio Frias, Dr. Benjamin Li, Dr. Lilia Matulac were appointed to various PMA Committees. Dr. Antonio Cabigas was appointed Chairman of COMELEC (2010), Emergency and Disaster (2006 up to the present) and Dr. Diosdado Salcedo, Chairman Com-mittee on Disaster (1990). Dr. Salvador Flores, Chairman Committee on Disaster (2009) followed by Dr. Ricardo Costes who was also Chairman of PMA Indigency Clinic, Dr. Arnold Cachapero Yu became Editor in Chief of PMA Publications, Dr. Cesar Meris was appointed Chairman of PMA Audit Commission, Dr. Hector Santos was Chairman PMA Medical Missions Commit-tee, Dr. Fidelis Labrador was Chairman of Doctors Inn and also Overall Chairman of PMA Foundation Day and Medicine Week.

In year 2006-2007, a fifth district of QCMS was created by the Board under the Presidency of Dr. Marianne Dobles.

In the year 2006 Dr. Herminia Gozar then Chairman of the Ways and Means Committee, after her appointment as President of QCMS District V took the challenge and organized the District which became very successful that they won the Most Out-standing Award of the QCMS five times in succession.

In 2010 - 2011 under the Presidency of Dr. Herminia G. Gozar another renovation of the present office was undertaken, super-vised by Dr. Ferdinand Cercenia then the Chairman of the Building Committee.

During the destructive typhoon “Ondoy” in 2009, Dr. Cercenia, Dr. Dalida, Dr. Ahmed and many others worked tirelessly to as-

sist PMA and help the hapless typhoon victims.

Dr. Marianne Dobles was elected PMA National Treasurer 2011-2012. She was then appointed PMA Secretary General 2012-2013 while Dr. Albert Guevarra was elected PMA Treasurer 2012-2013.

Since year 2000 for the next 14 years the Quezon City Medical Society have won the most sought after award which is the “Most Outstanding Component Society (Icasiano Award)” of the PMA year after year.

2015 The Quezon City Medical Society, the largest component society of the PMA received several major PMA awards during the 109th PMA Annual Convention held last May 17-20, 2016 at the Waterfront Hotel at Lahug, Cebu City

The Society won 10 major PMA Awards after a year of activities consisting of various community outreach, scientific sessions, participation in PMA programs and many others. This was all carried out through the efforts of officers and members of the QCMS under the leadership of QCMS President Dr. Angela V. Cruz. The list of the PMA award 2016 won are:

CONVENTION GUIDELINES GUIDELINES FOR CONVENTION PARTICIPANTS 1.REGISTRATION A. Registration at the Convention site will start from 7:30 AM to 4:00PM. B. No official receipt of registration, no Convention Kit, whether life or regular QCMS member. C. Pre-registered members should present their official receipt and/ or ID for verification. D. As my term as Governor comes to a close, it is with gratitude and fulfillment that I look back at all that we have accomplished as a unified team. Accomplishments which just a year ago, were only goals which we envisioned. It is with great pride that I pre-sent a brief report of our achievements for the year 2015-2016.

First Quarter (June, July, August 2016) During our first meeting, we got to know each other and appointments were given, I was appointed as Governor-in-Charge of the Committee on Auditorium and Doctors Lounge and Committee on Indigency Clinic The integration of OWL@PMA to component societies. "One PMA, One Health, One Nation" and "PMA Empowering the Filipino physicians for Nation Building" were approved

as the official PMA theme and tagline respectively for the year 2015-2016. Empowering words which further reminded us to be one in brotherhood with our fellow physicians and one in service to our fellowmen to achieve our goal of one health for our nation.

Amendment of Physician's Act of 2015

2nd Quarter (September, October, November 2015) Reinstatement of PMA Certificate of Good Standing as a requirement for Philhealth accreditation. A move which further rein-forced the importance of our organization's presence in every health policy of our nation. Approval of the increase in honorarium and retainer's fees of the doctors of the Indigency Clinic.

3rd Quarter (December 2015, January, February 2016) A momentous meeting was held at one of the rarely visited places of our land - Marawi and Iligan City, where resolutions re-garding the recognition of additional subspecialties were particularly discussed.

4th Quarter ( March, April 2016) A busy period as final budget allocations were made for the renovation and improvement of the PMA building, grounds, ameni-ties, doctor's inn, indigency clinic and archives. Through our concentrated efforts and our deep commitment to our noble institu-tion, our beloved PMA home was given a fresh and modern look. A first after 25 years.

It is truly an honor to be alongside all of you, my fellow PMA batch 2015 - 2016. I would like to express my gratitude to our dynamic President Dra. Minerva P. Calimag; our supportive Vice President, Dra. Karen Y. Canol-Salomon; our meticulous Treasurer Dr. Oscar T. Cabahug ; our efficient Secretary General, Dra. Marianne L. Ordoñez-Dobles ; our dedicated Assistant Secretary, Dr. Albert C. Guevarra; my fellow dutiful co-governors, our ever diligent PMA staff and lastly my Quezon City Medical Society under the leadership of our President Dr. Angela V. As president of the most active, unified and biggest component society of the Philippine Medical Association, the year long activities from July 1, 2015 to June 30, 2016 was full of joy, excitement, fellowship and hardships. At this point I would like to express my thanks and gratitude to our past presidents, governors, Executive Board & Council and three secretaries, as without their full supports and cooperation, the following activities would not be accomplished.

Foremost the hardship was rewarded with 10 prestigious awards given during the 109th PMA Annual Convention, May 17-20, 2016 at Waterfront Hotel, Cebu City, namely: 1) the highest Icasiano Award Category I; 2) PMA Leadership Award; 3) Most Active Treasurer; 4) Most Active Secretary; 5) Child Advocacy; 6) Nutrition Project; 7) Adopt-a-Barangay Project; 8) Service Award; 9) Community Service; 10) Most Outstanding Physician.

Aware of the nationwide problem, about drug abuse, I coordinated with the Vice Mayor’s Office and conducted seminars about

the ill effects of drugs, early detection, when, where and how to refer cases for rehabilitation. We visited and gave gifts and snacks to Tahanan Drug Rehab Center in Payatas. Extended also seminar to Barangay Health workers in Cebu and Davao on the Integration of Drug abuse to Primary Health Care.

A year long medical and optical mission in different barangays in District I to V was done, so with prostatic screening and pap smear.

At the beginning of my term, I assigned different committees and met with them regularly to give solutions to the problem they encountered in their committees.

Not the least I met with the three secretaries as each of them got their job assignments.

It was a year long activities but fulfilling, thanks God as what was expected of us was accomplished and I enjoyed working with my cooperative Executive Board.

1. Call to order – The meeting was called to order at 12:40 p.m. presided by Dr. Wilfredo S. Tagle, President of Quezon City Medical Society.

2. Declaration of Quorum – The Secretary Dr. Teresita N. Agda reported that we have a total number of 249 at-tendees and all we need is only 200 attendees to have a quorum. So a quorum was declared. No objection was raised.

3. Approval of the previous minutes Dr. Tagle said that the minutes of the previous General Assembly Meeting held last June 26, 2014 was

printed in the Souvenir Program. Dr. Ma. Julieta Agnes E. Calleja moved for the approval of the minutes of the previous meeting last June 26,

2014 and was seconded.

4. Business arising from the minutes of the previous meeting. There is no business arise from the minutes of the previous meeting. 5. New Business 5.1. President’s Report: Dr. Wilfredo S. Tagle.

Dr. Tagle flashed his report in a slide show presentation. Term started June 27, 2014 from that time the real work begins. We have different Executive Council Meeting & Board Meeting. Total of 86 meetings Executive Council Meeting, this is the time were we plan all the activities of the organization, while he

Board Meeting is the time that the board approved all the activities of the organization. RTD’s: 23 round table discussion

For PMA activities, QCMS was very active this year for our support to the PMA Secretary General Dr. Dobles, PMA Governor for Q.C. Region Dr. Henson & other members of QCMS who are in PMA.

Minutes of the General Assembly Meeting QCMS also have a lot of Community Activities. Total of 65 Child Advocacy, we have 9 projects are ongoing for several years. Culture & Arts: only a few know how to draw and photograph. We only have 4 activities Other activities:

1.To encourage members to be active, we had a Doctober Feast 2.Leadership of Dr. Ferdinand Cercenia, we had an Annual Fun Run 3.Medical Mission: We had a Papal Visit

We would like to thanked the Past President for their active support for the whole year. We have a total of 209 activities. That means more than half of this year we are working for QCMS.

New members: Means we are declining # of new member. Problem was started when PHIC did not required a certificate of good standing from PMA & Comp. Society. The Executive Council should work double time to attract new members.

Regular members: Not really decreasing but there is a trend to decrease. Life Members: we have to let them know that they can avail of the life membership status in QCMS

Strategies to attract members and to get more active members we have different mode of communication to our office:

We would like to remind the members that we have different membership benefits: Disability: P10,000, every 5 years you can claim Legal Aid: P8,000 and Death: P12,000

Income statement: since we have a declining membership of course financially we decrease. For earning, we have almost 8Million goes up for 2012, 2013, 2014 & declining 2015. With regards to expenses it is increase from almost 5Million per annum goes down to 2012, 2013, increase in 2014 and down in 2015. Net income of course there is a down trend for the specific of the financial statement it will discuss later by our Treasurer.

6. Treasurer’s Report – Dr. Richie Roy Ryan Reyes Dr. Reyes greeted good afternoon. I am here to give you the summary of our financial statement of the society for fiscal year 2014-2015. FISCAL PERIOD (June 1, 2014-May 31, 2015) 1. Total Receipts – Php 6,816,299.54 2. Total Disbursements – Php 4,535,576.00 3. Excess receipts over disbursements: Php 2,280,723.54 4. Interest Income – Php 384,001.45 5. Beginning Cash Balance – Php 37,159,307.45 6. Ending Cash Balance – Php 39,824,032.54 7. Total Excess – Php 2,664,724.99

The QCMS funds divided into various banks as the Depository Banks: Citibank, which is 2013 global bank of the year Banco de Oro; Metrobank & Bank of Phil. Islands (BPI) are the top 3 banks of the Philippines China bank & Security bank are the top 10 banks in the Philippines.

7. Other matters: 1. QCMS Care to Share Foundation – Dr. Realiza G. Henson Dr. Henson greeted good afternoon. I am standing before you today are not as PMA Governor for QC Region but as President of the QCMS Care to Share Foundation, Inc. The QCMS Care to Share Foundation was conceptualized during the presidency of Dr. Marianne O. Dobles in 2006-2007 at that time she thought of a need for an implementing arm of the QCMS for its various projects & programs because of the money that the QCMS has that we can’t just spent it right then & there because it is for the members benefits. It is for projects of the society for its advo cacies & for its participation in the PMA aside from its operational expenses. So their noble idea was uploat by the Executive Board & was incorporated & registered in the Security & Exchange Com mission in 2009.

The objective of which: 1. To provide educational support for the under privilege people of QC 2. To provide educational support for the dependent and/ or beneficiaries of the members of the QCMS 3. To provide support to the charitable projects of the QCMS

Since it conception and since it is registration were been saving money for projects & programs and it was only in 2012 that we begun and some of the project that were thought of are: 1. Bato Bato Pick – a diagnostic procedure for kidney stones & prostate hypertrophy. From 2012 to pre-

sent, we have screened 980 patients in different barangay of QC. 2. Cleft Palate/Lip Operation: with the generous help of our reconstructive surgeon, Past President of

QCMS Dr. Hector M. Santos, Jr. He doesn’t do the operation, he also reconstruct the deformity of the patients. And he does it for “FREE”. Thank you Dr. Santos. To date 40 patients.

Minutes of the General Assembly Meeting

Another program is a Child Advocacy of having a scholar from the indigent children of QC. Was thought of & was given to the Care to Share Foundation by the Quezon City Medical Society. Presently we have 2 scholars from Busilak Learning Center our adopted learning center. We have interviewed another 1 and it has to be approved by the Board so hopefully this year we have 3 scholars. They are Elementary students and hopefully we can support them up to college. One dream of the QCMS Care to Share Foundation is to build an indigency center for minor surgical pro-cedure & dental, rehabilitation for optical services & for basic laboratories in affordable prices. We will not be able to realized this if not for your help. So this report is an appeal to your generous heart to please donate to our foundation. “To a might think that your donation will just be a drop in the bucket, but this drops is a bucket will make a bucket full” Thank you in advance for your generous support. QCMS Lot/Building Acquisition Dr. Cercenia asked, this was discussed in the previous General Assembly regarding the plan of the society to acquire a property. Before I was the Chairman of the Committee involve in the project. May I know what is the current status. Before we allotted 5 million & we added another half million because of the increasing price of the property, this is a land acquisition, the society still planning it to acquire a prop-erty? Dr. Tagle said that we have a standing committee on this a committee on Building Construction & Acqui-sition which is headed by Dr. Benjamin Lim. And since Dr. Lim is not around, Dr. Guevarra is one of the committee member.

Dr. Guevarra said that for the acquisition of the lot for QCMS was ratified during the presidency of Dr. Henson. As to inquire of Dr. Cercenia, as of now we do not have a lot because as of now the money that we allotted is not enough. We all know that we have to vote the lot area that in the center of QC that would be around Q. Avenue or North Avenue. As of now we are still in the process of having funds, this is a careful study. In the previous meeting of the committee there is a suggestion to get a lot is to loan it in the bank, however this suggestion was turn down because that will entail a big money that will hold loan interest. The money was belong to the members of the society so we don’t compromised it. Until now we

are still looking for a possible sites with a good price for us. Dr. Sison announced the QCMS Prestige ID Card aside from PMA Power card. This card will except you from Car coding. QCMS Merchandise “Doctor on Call ”Car plate amounted to P500 and QCMS Pin amounted to P100 Dr. Henson clarified as a PHIC Requirement: As a requirement for Phil Health accreditation a Good standing status is still required, although PHIC does not require you to submit a certificate of good stand-ing, The PMA submit every month the up to date list of good standing members to the PHIC & PRC. So wherever you are renewing your accreditation or license, they will just click your name & they will know if you are updated or not in the membership.
