I’m Back!!It is my pleasure to take on the role

of CFPM president for another term. First of all, my thanks to Mike Turcotte for doing an outstanding job in this role last year. The CFPM’s loss is the College of Chiropodists of Ontario’s gain.

As I am writing this it is January and we have just endured a week of brutally cold weather. Where I am, minus 26, so says my car temperature gauge. Maybe the choice of the word brutal is wrong in this case, because the cold weather that we get here in most of Canada affords us the opportunity to take part in all varieties of wonderful winter sports, skiing, snowboarding, skating, snowmobiling, etc..

The point I am trying to make is that there are always two ways of looking at things. So are you a glass part full or a glass part empty individual. Myself I choose to be a glass part full type of guy, looking for the positive in any situation.

Podiatric medicine in Canada is at a turning point and I think that this coming year will be as important of a year that has ever existed for our profession. Looking forward, I see a lot of positives. Glass half full!!

The most important occurrence is the HPRAC review that is presently underway in Ontario. So why is a review of the scope of practice in Ontario so important to a Canadian organization. Well first the greatest number of practitioners in Canada come from and practice in Ontario, approximately 7 in 10. They represent the greatest number of members of the CFPM and we will do all that we can to support positive changes in any province.

Second, the repercussions of this scope review will be felt

across the country. One consistent title used throughout the country will aid with insurance companies and the general public understand what we do, making our professional lives simpler. A new school of podiatry will finally allow all Canadian trained practitioners to upgrade their skills here in Canada and have it mean something. This will also allow other provinces to use Ontario as a guide to improve their scope of practice in the future.

Finally, I hope that this review may finally bring peace to a divide that has been present in our common profession for many years.

However, even though I always like to look at the positive, one must always be mindful of the negative. The reality is that there are many regulated and non-regulated professionals out in the marketplace that want to take some or all of our scope as part of their own. As the best qualified and trained professions in podiatric medicine, we must always defend that we represent the top echelon of the medical foot care field. At the CFPM we take this role seriously and will continue to make sure that all levels of government, insurance companies and employers know what we can do and why we are the best at it.

To do this, however, we need your help. Make the commitment and become a member of the CFPM and if you are not already, your provincial society as well. Governments and insurance companies look at membership numbers as a way to see who they should be listening to when issues arise. I want to be able to say to them we represent the majority of practitioners.

Consider donating to our “Foothold on our Future” campaign. This fund will allow us to follow through with the HPRAC review and then the next step when the government examines their recommendation. The CFPM is committed to follow this long and involved process to the end where in my opinion we will see a bright new future for podiatry and podiatric medicine in our Country.

FootholD on the Future Ensuring Chiropodists Are Heard

The Health Professions Regulatory Advisory Council (HPRAC) review of regulation of foot care in

Ontario continues.As many of you are aware, the

Canadian Federation of Podiatric Medicine (CFPM) is actively involved to ensure a strong voice for its membership’s interests.

The review, which dates back to a 2007 referral by Ontario’s Minister of Health and Long-Term Care, is intended to look at “issues relating to the regulation of chiropody and podiatry and provide advice as to whether and how there should be changes to existing legislation regarding these related professions,” as well as “an analysis of the current model of foot care in Ontario, issues regarding restricted titles, and whether the existing limitations on the podiatrist class of members should continue.”

The following update is intended to keep you informed of recent developments and activities:

hPrAC ProcessHPRAC has wrapped up the first phase of its review of the current model of foot care in Ontario and has moved on to other aspects of the minister’s referral, including:

• Issues relating to the regulation of chiropody and podiatry;

• Whether and how there should be changes to existing legislation regarding chiropodists and podiatrist;

• Issues regarding restricted titles; and

• Whether the existing limitations on the podiatrist class of members should continue.

To this end, the College of Chiropodists of Ontario (COCOO) has submitted a formal application seeking changes to the regulation of chiropody and podiatry in Ontario.

The CFPM welcomes many of the points raised by the College of Chiropodists of Ontario and aligns itself with the College in its desire to ensure a foot care workforce for Ontario that offers the highest possible standards of care and a full scope of practice. Interested parties are able to review the Applicant’s proposal at: http://hprac.org/en/reports/resources/Chiropody_Podiatry_Applicant_ proposal.pdf

HPRAC most recently request-ed stakeholders’ views on this proposal.

CFPM’s responseThe CFPM submitted its response to the COCOO proposal.

Whilst supporting the broad aim expressed by COCOO that there should be a single inclusive profession drawing on the skills and expertise of both chiropodists and podiatrists, adopting the title ‘podiatry’, the CFPM nevertheless remains unconvinced that the model proposed in this instance is necessarily appropriate for the health needs of Ontario into the future. It is the firm view of the CFPM, based on available evidence, that a new model of podiatry education provision (incorporating both the current classes of chiropodists and podiatrists) should be adopted, embracing a skills ‘escalator’ approach that allows workforce flexibility, recognises the range of existing skills of both chiropodists

and podiatrists, and enables the development of a service that best meets the needs of the Ontarian population it seeks to serve.

A “one scope/one practice” podiatry model in which demonstrated competency, and not your origin of education, is the single criteria for admission will make Ontario a leader in podiatric care.

Your roleAs engaged foot care practitioners, with a vested interest in the future of chiropody and podiatry in Canada, you too have a role to play.

Look for information on the HPRAC website and in CFPM announcements as this process continues. We anticipate more opportunities for Ontario practitioners to provide comments.

The Canadian Federation of Podiatric Medicine continues to be a strong voice for foot care professionals and will ensure that its memberships’ interests are represented at every step of the HPRAC review process. A report containing HPRAC’s findings and recommendations is expected to be submitted to the Hon. Eric Hoskins in Summer 2015.

Enjoy an early summer getaway while updating on podiatric dermatology. This educational meeting aims to act as an update in the subject of

foot dermatology and will start by looking at skin assessment and then focussing on the common foot dermatoses seen in practice and how they may be managed. Skin tumours will also be covered and the day will include a discussion of some newer developments in the treatment of foot infections such as warts and fungal foot infections.

Dermoscopy is a relatively new technique which has been widely adopted across Europe in the field of Dermatology to assist in the assessment of pigmented and other types of skin lesion. This simple hand held device allows the user to visualise structure deeper in the skin, not visible to the naked eye improving diagnostic accuracy. This introductory session will cover the basic principles of Dermoscopy and allow delegates the opportunity to try out these devices. In addition, there will be a presentation on melanoma and the foot discussing a simple technique to assist in the recognition of suspicious lesions, ensuring early referral.

UK Podiatrist, Dr. Ivan Bristow is recognized internationally for his contribution to education and research in podiatric dermatology. Since the commencement of his career, he has held a strong interest in dermatology establishing the first foot dermatology clinic in the UK in 1992. He has continued his career, obtaining a Master’s Degree from the University of Oxford and promoting podiatric dermatology worldwide working with podiatrists, dermatologists and general practitioners on various education programmes.

CFPM Mentorship ProgramBy Stuart Berry

What do Mr Miyagi, Obi Wan Kenobi, Steve Jobs and Florence Nightingale

all have in common?I know what you’re thinking,

what has this got to do with podiatric medicine?

Well, all had mentees! Daniel LaRusso in the Karate Kid was mentored by Kesuke Miyagi, finally winning the All-Valley Karate Tournament. Luke Skywalker hears the voice of his mentor prophetically “the force be with you”, “use the force”. In an interview with American talk show host Charlie Rose, Zuckerberg talks about his inspiring mentor Steve Jobs and how Jobs gave him advice about how he could build a team that was as focused as Zuckerberg on building “high quality and good things”. In healthcare Linda Richards was mentored by Florence Nightingale during her time at the first ever secular nursing school in St Thomas’ Hospital London, England, only to go on and become the first professionally trained nurse in the US, founding nursing programs in both the US and Japan.

These relationships clearly have merit. Through them we can accomplish our full potential even better than we could on our own – win sporting tournaments, develop multibillion dollar companies, completely change the face of nursing globally. Oh, not forgetting the guy who helped defeat the Galactic Empire in the Galactic Civil War!

We are not merely talking about casual connections here. Where people may pass like ships in the night, or snatch one word of encouragement here and there. Mentorships are solid, firm, trusting relationships or arrangements. They are nurtured and may be even be hard work. Just think ‘wax on – wax off’ in The Karate Kid! They require commitment.

But they are WORTH IT!Did you know that the term ‘mentor’ comes from Greek

mythology? Odysseus left for the Trojan war and had to leave behind a trusted counsellor for his son. He left behind Mentor.

And so today our CFPM Mentors act also as ‘trusted counsellors’, offering support, encouragement and guidance for their paired mentee.

Product Review – priMED GlovesBy Stuart Berry

My mission begins. I collect together some CFPM products and decide to give them my own personal testing session in clinic. Completely subjective here – no double blind

trials or placebos, just me, a handful of patients and some products. Let’s see what happens…

I go armed into clinic with three different types of clinical glove available through the CFPM member’s glove program and three foot products.

The gloves I am trying out are the ‘PrimaTouch Vinyl’, ‘PrimaTouch Classic Nitrile’ and the ‘PrimaTouch Sense Nitrile’.

It’s the middle of winter, February 4 to be exact and its cold. My hands are beginning to suffer and are getting drier by the day. I am therefore pleased to see that all gloves are powder free – my hands might live to see another day, I think to myself. I have found that powdered gloves used to dry out my hands, and I also worry about the powder when treating wounds so, for me at least, powder-free was adopted long ago.

I go for the PrimaTouch Vinyl gloves first of all. Having experiment-ed with vinyl gloves in the past, I know they can be an economical choice for an examination glove. I was pleased to see documented that ‘PrimaTouch Vinyl’ meets and exceeds all Canadian Health Standards for medical examination gloves. Previous experience with PVC gloves shows they may not be as strong or dextrous as other types of examination gloves. However I was pleased to see that these stood up to the challenge. I found them useful for general clinical cleaning and foot examination, however I did find that I was wanting another style of glove when more tactile precision was needed – for example when debriding or enucleating.

I wondered whether I would miss that reassuring ‘thwak’ sound that only a latex glove seems to be able to give you, as you pluck it onto your hand. I like that sound. (I find myself pausing and plucking - these are the kind of things that you think about when suddenly the ‘thwak’ isn’t here anymore, as is the caes with nitrile gloves). But, all that said, the ‘synthetic latex’ of the Nitrile gloves did have a nice feel. Although not as ‘elastic’ as a regular latex glove, the PrimaTouch Classic Nitrile never felt as though it was about to rip or tear as it was put about its paces in clinic on my beefy hands. I soon got used to the ‘nitrile’ feel of the glove and found that I could debride, enucleate, resect and examine without feeling restricted. Knowing that they are latex-allergen free is also reassuring. Even the ‘medical blue’ colour of the gloves gives the clinic a look of ‘medical freshness’ about it. So after a few patients I soon forgot my crazy old habits of trying to making noises with my gloves! I liked these gloves.

But then I tried the ‘Prima touch Sense’. Advanced Nitrile technology provides exceptional tactile sensitivity, dexterity and strength, the literature tells me. Ok, we’ll see, I thought, after I was still thinking the ‘Classics’ left me feeling quite nimble fingered. I subjectively tested the temperature gradient of my patients feet

using the back of my hand and I could not believe how insensate other types of glove have felt in comparison to these. It was immediately easier to feel a temperature gradient, but also the most subtle of sensations. I found I could detect the most subtle of imperfections – a slight feathering of callus here, a slightly rougher nail there. I could see these really coming into there own for those who do very dextrous clinical work – wound care, perhaps. Plus, these gloves come in boxes of 200, rather than the usual 100’s so you even cut out on cardboard waste – good for the clinic and the environment I thought!

I was pleased to have the chance to try out some of the CFPM products. In the same inquisitive, subjective fashion, I am going to have a look at what other products the CFPM supply. Watch this space.

Is Your Online Marketing Up to 2015’s Standards?

And if you’re looking to keep your waiting room full, be sure that the Web Presence you build for this year includes these 3 vital strategies.

Mobile CompatibilityYour website obviously needs the basics, like the location of your practices, the services you offer, and a way to schedule appointments. But patients also expect performance – not just in speed, but in functionality. And since the mobile internet subscriptions outnumber fixed connections 3:1 in the Americas, having a site that performs well on smart devices like phones and tablets is crucial. You want to provide all your patients with a satisfactory experience, and that experience starts on your website. It’s especially important to do this because Google gives higher rankings to mobile sites when a search comes from a smart device.

There are two ways to do this – with standalone mobile sites and responsive mobile design. Standalone sites are the traditional method, and they still deliver results. But responsive design is the industry trend. This method of coding hard-wires mobile compatibility into your main website without the need for a separate mobile site, ensuring it will display perfectly on any device. If you’re starting to build a website now, this is the option you’ll want to consider most heavily.

reputation trackingOnline review sites exist for one purpose – telling consumers what they might experience before they buy. That means one of two things for your practice – free marketing, or a large obstacle. So if you don’t have a way to keep track of your online reviews, you need to find one. Though it’s possible to keep track of your reviews by periodically checking the most popular sites, your practice may want to consider reputation tracking software. Services like these provide automated progress reports of your website’s health, collecting reviews and compiling them all in one place. Some even include analytics that allow you to know what your patients appreciate and what needs work. From there, you can devise a plan to generate

more positive reviews, and to start getting that extra marketing push that comes from a sterling reviews page on Yelp or HealthGrades.

Search engine optimization (Seo)Search engines like Google are the modern equivalent of the yellow pages – if you don’t appear prominently, your practice won’t be seen. The goal, then, is clear – push your practice higher in the search engine rankings.

If you’re well-versed in HTML, there are some basic techniques you can put to use on your own, like keywording and meta-tagging. For everyone else, it’s often worthwhile to look into an SEO service. Convenience aside, do-it-yourself SEO is particularly difficult because of the amount of change Google makes to their algorithms on a regular basis. The average number is 500-600 per year, and in 2014 there were at least 13 “major” changes that caused major shake-ups in the SEO industry. If you’re willing to monitor those changes and make adjustments on a regular basis, DIY can be a viable but extremely labor-intensive option.

All in all, the bottom line is this: your website can only do so much on its own. In order to excel in the digital marketplace, it’s necessary to think beyond the traditional practice website. The additional marketing efforts discussed here are quickly becoming industry standards.
