The Physician Factor: How MACRA Will Influence Post Acute UtilizationPhysician executives will explain what it means when a doctor’s reimbursement is directly tied to performance under MACRA. They will describe the adjustments their practices are making under the new Quality Payment Program, and answer critical questions about the effect MACRA will have on referral patterns to post acute care: Will MIPS-based quality measures drive more or less volume to skilled nursing? And what effect will bonus incentives from Advanced APMs have on a physician’s willingness to increase use of home health services and skip the SNF altogether?
The New BPCI Landscape & The Role of Post Acute CareA veteran convener and three early adopters of bundling will explain the coming arrival of BPCI Advanced and what it means for post acute providers. They’ll describe how SNFs and home health are positioned to add value to the new 90-day Advanced bundled payment program by way of care management and patient navigation. In additon, they’ll address the discontinuation of BPCI Model 3, and review options for being episode initiators, including moving upstream and forging risk-based partnerships with health systems and physician groups.

Quantifying the Value of Palliative Care Rehospitalizations for patients with chronic co-morbidities are responsible for a huge spike in costs when approaching end-of-life care. This session will make the argument that highly coordinated palliative care (administered before and during the hospice stage) not only improves outcomes but significantly drives down costs. We will quantify the effect palliative care has on reducing ED visits and acute admissions, and demonstrate its value (despite not qualifying for reimbursement) by producing a net savings to payers and risk-based providers.

Medicare Care Choices Model: Valuable Lessons from an Early-Adopter The CMS Medicare Care Choices Model was originally billed as a paradigm shift that allows patients to receive palliative care, yet still be eligible for curative care. One Florida hospice provider will share a firsthand assessment of the program at the halfway point in its four-year trial period. We’ll discuss what it’s like to operate in a Per-Beneficiary-Per-Month reimbursement model; the challenges of meeting curative and palliative care provider goals simultaneously; and recommendations for structuring a permanent dual-care model when the pilot expires in 2020.
