Connecting with Community, Inc. (CWC) has been providing services through the Supported Living Services waiver (SLS) since 1995. The philosophy of CWC is to assist individuals in achieving greater independence. We believe that this is best accomplished by allowing the individual to have as much control over their life’s decisions as possible. We do this by engaging the individual in defining the challenges they face. We design activities that promote their capabilities. And we focus on connecting individuals, not fixing them. The services that we offer under this waiver include, Personal Care (PC), Homemaker Enhanced (HE), Mentorship, Supported Community Connections (SCC), and hourly respite. All services, except for the SCC services, are provided 1:1. SCC is completed in small groups with one provider that goes out in the community. For larger groups (more than 3) we use “The Buddy Bus” for our community based activities. These activities include volunteering, bowling/pool, YMCA, hiking/walking/field trips and dances or concerts on Friday nights. CWC also offers an innovative day program, “Club of Arts Learning Center”. The Resource Exchange is a Community Centered Board, responsible to facilitate eligibility, provide comprehensive case management and provide or purchase services and supports for children and adults with developmental disabilities. TRE’s three-county service area includes El Paso, Park and Teller Counties. The support programs we help administer are of no cost to the consumer and the type of insurance a person has does not have a bearing on whether or not a person receives HCBS, Early Intervention or Family Support services. The programs may pick up extra hours of supports where typical insurance caps out or may pay for supports that insurance will not cover. 0-17 years: C-HCBS Children’s Home and Community Based Services, CES Children’s Extensive Support, CWA Children with Autism 18 and over: SLS Supported Living Services, DD Developmental Disability Comprehensive FSSP Typically 0-17 years but may serve beyond that if necessary: Family Support Services 0-3 years: Early Intervention ServicesCategories of Service: Coordination of Services, Respite, Advocacy & Support
