NHS Forth Valley seeks to appoint an enthusiastic and innovative Specialty Doctor colleague
based at Forth Valley Royal Hospital. The successful applicant will join a team of 12 Emergency
Medicine Consultants, 4 Emergency Medical Practitioners and 5 Specialty Doctors.
NHS Forth Valley is undergoing a period of dynamic change in relation to the organisation of
Emergency & Urgent Care as part of an ongoing programme of modernisation and improvement in
relation to unscheduled care.
NHS Forth Valley provides acute services for a population of approximately 310,000 people in
Central Scotland. Acute services, emergency and complex care, and planned services are
provided for at Forth Valley Royal Hospital (FVRH)

Introduction to NHS Forth Valley NHS Forth Valley is one of 14 regional health boards in Scotland and serves a population of
nearly 310,000 in a diverse geographical area which covers the heart of Scotland and covers three
council areas, Clackmannanshire, Stirling and Falkirk.
NHS Forth Valley has an annual budget of approximately £570 million and is the largest employer
in the area, with approximately 8000 staff from a wide range of professional and support
occupations. Acute Services are based in our modern, purpose built 860 bedded, Forth Valley
Royal Hospital, which opened in 2011 and provides inpatient and outpatient services. It is
supported by a network of four community hospitals, 56 health centres, day centres providing care
and support for patients with mental illness and learning disabilities and a wide range of
community based services.
We are home to the Scottish Centre for Clinical Simulation and Human Factors, one of the most
advanced training facilities of its kind where medical staff are able to hone their skills using
computerised mannequins. In addition, NHS Forth Valley has been named in a survey of trainee
doctors as one of the top places to be in the UK for medical education and training.
There are two Health and Social Care Partnerships (HSCP) which cover the Forth Valley area –
one for Clackmannanshire and Stirling and one for Falkirk.

The Forth Valley Area
Forth Valley, in the heart of Scotland, is an exciting place to live and work.
Transport links are second to none with excellent motorway and rail links
and two international airports nearby, Edinburgh and Glasgow. House
hunters in the Forth Valley area are spoiled for choice. On offer are a large
array of established and new properties in a variety of attractive settings.
Education in the area is first class, with excellent provision of pre-school, primary and secondary
education. State schools in the area have strong reputations and there is also easy access to the
area’s private schools in Dollar and Crieff. On the further education front, Stirling boasts an
excellent University and access to the University cities of Glasgow and Edinburgh is easy. There
are also further education Colleges in Falkirk, Stirling and Alloa.
Stirling AreaA wee city with a big history, Stirling punches well above its weight for historic attractions and
spectacular scenery, not to mention shopping, places to eat and exciting events. Visit Stirling and

Falkirk AreaThe Falkirk area is steeped in history with a wealth of attractions, including two of the world’s most
unique, including, The Falkirk Wheel and The Kelpies in The Helix Park. Plus the Falkirk area has

Clackmannanshire AreaClackmannanshire lies between the majestic Ochil Hills and the River Forth and provides the
perfect year-round base to explore the stunning countryside, rich history, medieval castles and

Emergency Department
Forth Valley has a varied urban and rural catchment area which stretches to Tyndrum to the North, Loch Lomond to the West and encompasses all of Clackmannanshire, Falkirk and Grangemouth, lying right in the centre/heart of Scotland.
The Emergency Department at FVRH is a major unit open 24 hours a day. There are laboratory services on site and 24 access to X-ray and CT scanning.
In the year to 1st April 2016, a total of 63,296 patients attended the Emergency Department and 15, 845 attended the minor injuries unit located in Stirling Community Hospital. The Forth Valley Royal Hospital Emergency Department is very well equipped with 4 adult and 2 paediatric resus bays, a 14 bedded trolleys ‘major’ area and 8 cubicle treatments ‘minors’ area with additional triage, see-and-treat and mental health assessment areas.
Forth Valley Royal Hospital has been designated as a Trauma Unit in the current NHS Scotland reconfiguration of major trauma services, expected to go live in 2020. NHS Forth Valley will feed-in to the major trauma networks of both the West (Greater Glasgow & Clyde) and South East (Edinburgh & the Lothians) of Scotland.
Medical Staff
12 Consultants are in post. There is a Consultant shop floor cover 0800 – 2200 7 days per week and on call cover out with this time. 2 consultants have dual-accreditation with Paediatric Emergency Medicine.
4 Emergency Medicine Practitioners 5 Specialty Doctors A training grade rota comprising FY2 – ST6 provides cover to the department 24 hours per
day, seven days per week. We employ a number of Emergency Nurse Practitioners who provide a see-and-treat
service 14 hours a day, and are developing an ANP (Advanced Nurse Practitioner) service.
Administrative SupportThe post holder will have appropriate secretarial support and a computer with access to the NHS network and resources.
Training and EducationNHS Forth Valley places a high priority on training and development across all staff groups and strongly supports the continuing medical education programme for Consultants. There is a purpose built education centre with a library, lecture theatre and meeting rooms and video-conferencing facilities.

Forth Valley Royal Hospital has also developed a comprehensive clinical skills teaching suite for the teaching of resuscitation techniques and other procedural skills. The Scottish Centre for Simulation and Clinical Human Factors is based within the hospital. There are weekly postgraduate meetings involving all specialties. There are regular evening postgraduate medical meetings in the hospital for both hospital and primary care medical staff. Glasgow and Edinburgh with their Royal Colleges, University Medical Schools and postgraduate facilities are within easy reach.
Teaching
The Emergency Medicine environment promotes team working, excellent communication, prioritisation and management of undifferentiated presentations. The appointee will be expected to contribute to shop-floor teaching and training of FY2 doctors, Specialist Trainees and Specialist Registrars on the Training Programme. The shop-floor teaching is complemented by formal training via case presentations, topic discussions and educational meetings, which are Consultant led. There is an active in situ simulation programme within the ED
Middle grade staff for most major specialties; including paediatrics, medicine, surgery and anaesthetics are resident within the hospital 24/7 and they and the duty consultants are available to assist in patient management within Emergency Department, when required.
Postgraduate and Developmental Facilities
There is a purpose-built Conference and Education Centre at Forth Valley Royal Hospital. It houses the Postgraduate Library and audio-visual facilities and offers opportunities for a wide range of development activities for all staff groups. This is a modern, high quality facility which is widely used by groups throughout the country.
A wide range of full-text and e-journals are freely available. Internet access is available in the centres, and also in the emergency department.
There are regular postgraduate meetings in both hospitals with telemedicine links to Glasgow and Edinburgh with their Royal Colleges, university medical schools and postgraduate facilities are within easy reach. Forth Valley Royal Hospital is the site of the Scottish Clinical Simulation centre; where courses relevant to the Emergency Department are available and with which the department has close links.

Specialty Doctor Responsibilities
A Specialty Doctor in Emergency Medicine has responsibility for providing care for the acutely ill and injured patient, and working at a middle-grade level. To this effect they must:
Work as part of a multi-disciplinary team in order to support and provide an Emergency Medicine service within the Emergency Department.
Provide clinical leadership as the most senior doctor on shift in the Emergency Department on site overnight
Provide resuscitation of a high order of skill when indicated and initial treatment of the emergency patient in association and agreement with their specialist colleagues.
Provide advice and clinical review where appropriate to patients in the emergency department seen by more junior colleagues, as part of the emergency medical team.
After initial diagnosis and emergency treatment, arrange as necessary either the admission of the patient or transfer to the care of the appropriate specialty. Where indicated, this may involve calling the appropriate specialist to the Emergency Department.
Treat appropriate cases until discharge or transfer responsibility back to the General Practitioner. Out-patient care of a continuing nature will normally be referral of the patient to the appropriate specialty.
Assist the ambulance and other emergency services in all matters that improve the effect of transfer of patients from the site of the emergency to the hospital.
Ensure appropriate liaison with all emergency services, local authorities and other interested services, including Social Services, Health Visitors and others.
Provide inpatient medical care for head injured patients within the acute assessment unit.
It is expected that the successful candidate will be competent in procedural sedation and non-invasive ventilation, but training and supervision can be provided in these and other areas of critical care.
Have responsibility for ensuring active participate in continuing education.
Undertake audit in all aspects of Emergency Medicine, as part of annual appraisal and a programme of continuous quality improvement.
Assist and provide teaching for undergraduate and postgraduate students.
Actively participate in both departmental and organisational matters concerning clinical governance.
Support the delivery of national and local performance and quality standards including the 4 hour and HEAT 10 targets.
The requirements of this post will change through time in line with clinical advances and developments in services provided by the NHS in Forth Valley.

Conditions of Service
Doctors who have completed a minimum of two years’ service in the specialty doctor grade and/or in equivalent grades or who had an entitlement to six weeks annual leave a year or more in their immediately previous appointments shall be entitled to annual leave at the rate of six weeks per year.
This appointment is governed by the Specialty Doctor Terms and Conditions of Service as set out in the Circular PCS (DD) 2008/7
The salary scale for the post is currently £38,685 - £72,140 per annum (pro rata for part-time appointments).
The post-holder will hold full registration and a Licence to Practise with the General Medical Council.
This is a substantive appointment and is immediately available.
You will work on full shift rota, including night shifts, which is New Deal Compliant and Working Time Compliant. While at work you will be responsible for the initial management of self-referred patients presenting to the Emergency Department and arranging appropriate referral or discharge by close liaison with both the in-patient teams and local general practitioners, supporting the less experienced staff.
The rota for permanent middle-grade doctors in the ED has been designed separate to that of the training middle-grade doctors to ensure sustainability in terms of frequency of night shifts and weekend working. Full details are available on request.
For full time Specialty Doctors, this will comprise 9 Programmed Activities for direct clinical care, 1 programmed activity for professional development.
Hours of work for successful part time Specialty Doctors will be agreed on an individual basis.
Specialty Doctors in Emergency Medicine are offered a named EM Consultant as a mentor. We actively encourage such doctors to use study leave for CPD and career progression, and there is scope for development time to attain skills and competencies in complimentary specialities, such as anaesthesia / critical care. This post would suit candidates intending to pursue CESR via article 14.
We would welcome applicants looking to work less than full-time.
In accordance with advice contained in NHS Circular 32/1989 NHS Forth Valley will take direct financial responsibility for claims against hospital doctors arising from their negligence through acts and omissions of medical and dental staff employed by them. You are therefore not required to subscribe to a medical defence organisation. NHS Forth Valley’s indemnity will, however, cover only NHS Forth Valley duties. All medical and dental staff are advised to maintain their defence body membership in order to ensure they are covered for any work which does not fall within the scope of the indemnity scheme e.g. private work, ‘good Samaritan’ actions etc.

Contractual Details
Contracted activities for this appointment will be defined and agreed with the post holder and the Acute Site Director, Medical Directorate.
The post holder will be professionally responsible to the Clinical Director in Emergency Medicine and the Associate Medical Director, Medical Directorate and managerially responsible to the Site Director, Medical Directorate
NHS Forth Valley will require each Specialty Doctor to agree with the Site Director and Clinical Director, a job plan of fixed and other commitments, which will be reviewed on an annual basis and revised as necessary to meet the requirements of the local and national service issues.
Specialty Doctors will be required to participate in an annual appraisal and will be assigned a Secondary Care Appraiser in NHS Forth Valley.
If NHS Forth Valley Health Board considers that the there is a requirement for the post holder to relocate his or her home in order to take up the responsibilities of the post, relocation expenses will be available in accordance with the Board’s Relocation Policy. Support to the level of 15% of starting salary up to a maximum of £10,000 may be available, subject to the provisions of the policy.
Formal appointment to this post will be conditional upon satisfactory completion of all pre-employment checks, including satisfactory medical screening.
NHS Forth Valley does not negotiate salary placements. On commencement of employment, salary will be determined in accordance with Section 5 of the terms and conditions of service.
This appointment will be superannuable under the NHS Superannuation Scheme, which is contracted out of the State Earnings Related Pension Scheme (SERPS). New employees will automatically be enrolled in the scheme unless they opt out. The employment is subject to three calendar months notice on either side.

Pre-Employment Screening
Criminal Convictions - Protecting Vulnerable Groups (PVG) SchemeThe Protecting Vulnerable Groups (PVG) scheme has replaced the previous disclosure
arrangements for people who work with vulnerable groups. The duties of this post require the post
holder to undertake regulated work with children and young people (individuals aged under 18
years) and / or protected adults (individuals aged 16 or over who is provided with (and thus
receives) a type of care, support or welfare service).
The post holder will be required to be a member of the PVG Scheme and to consent to NHS Forth

Selection Criteria
Essential
MBChB or equivalent qualification
Full General Medical Council Registration and Licence to Practise
Have completed at least four years’ full-time postgraduate training (or its equivalent gained on a part-time or flexible basis) at least two of which will be in a specialty training programme in a relevant specialty (e.g. emergency and/or acute medicine) or as a fixed term specialty trainee in a relevant specialty; or shall have equivalent experience and competencies (suitable for ST3 and above applications).
Appropriate practical skills for emergency medicine work
Excellent communication skills, both written and oral
Ability to work well within a team, both leadership & followership skills
Demonstrable organisational and time management skills
A genuine interest in the specialty
Previous Emergency Medicine experience to allow the successful applicant to work at a middle-grade level.
