This letter will explain the pre-participation physical exam process and the collection of all pre-season paperwork required by the NCAA and the MSU Athletic Training/Sports Medicine Department. We request that each athlete complete all necessary pre-participation paperwork that is available on our Electronic Medical Records System - SportsWare. The instructions below will assist the student-athlete in completing all necessary pre-participation paperwork through SportsWare. This system will allow the AT/SM staff, as well as the student-athlete, to view all necessary documents, injuries, rehabilitations, and treatments throughout the academic year. This is part of your medical clearance process and must be completed prior to participating in Intercollegiate Athletics. You cannot participate until you complete all necessary documentation required by the AT/SM Staff. Medical Eligibility Checklist Each year the Athletic Training Staff must have the following forms on file for every athlete (we organize our files by years of participation at MSU, not eligibility or academic status). 1. Completed Sportsware Medical Records portal. 2. Completed Physical/Medical History forms or Yearly Update. 3. Sickle Cell Test 4. ADD/ADHD Medication documentation (IF APPLICABLE; updated yearly) Medical History/Physical Forms The NCAA requires that all student/athletes trying out for intercollegiate teams must have a pre-participation physical examination prior to beginning practice. We would like all initial exams to be conducted by the Montclair State University Team Physicians and Athletic Training/Sports Medicine staff here at MSU during the scheduled times, unless this is impossible due to other obligations. All the physicals will be done in The MSU Health Center, Blanton Hall. *The final decision on medical eligibility is the responsibility of the MSU Team Physicians.* ********All physicals must be within 6 months of the start of practice ********

Please check which forms you will need this year:

First Year Playing @ MSU The First Year Physical Packet is for all freshman or transfers- anyone trying out for an MSU team for the first time. A major portion of the initial pre-participation exam will require each student-athlete to complete an extensive medical history, including immunizations. Each candidate should report irregularities, surgeries, fractures, allergies, or other pertinent medical information. This would include disabilities, such as hearing, vision, or verbal. • Any student athlete with a pre-existing medical condition that has been documented should bring copies

of any pertinent information (i.e. x-rays, reports, etc.) with them to the pre-participation exam. Examples of what we are looking for include heart murmurs, asthma, diabetes, allergic reactions, surgeries, missing organs, metal plates, etc. This information may help avoid missing practice time if this type of information is deemed necessary by the medical staff.

• ADD/ADHD medication requires special documentation. If you have been diagnosed with one of these conditions and/or take medication for ADD/ADHD please see our website.

• Sickle Cell Trait Status: Following the recommendation of the NCAA we are requiring blood test documentation about the Sickle Cell Trait status of all first year athletes, both freshman and transfers. Documentation must be a blood test, either done at birth or a recent blood test.

• Concussion Baseline Test: All first year athletes at Montclair will be required to complete an online concussion baseline test prior to starting practicing. This test will be conducted on campus, either when an athlete is on campus for a physical or by calling (973) 655-5250 for an appointment. All athletes are required to take this baseline test except swimmers and track athletes. (Divers and pole vaulters must take a baseline test.)

• Failure to report any proper medical information or information withheld for the purpose of gaining clearance may disqualify an athlete from participation. Incomplete medical information also may lead to incorrect medical treatment.

Second & Fourth Year Playing @ MSU

The second year form simply requires updating us on your current medical conditions. This can be completed by a physician or the Athletic Training staff.

Third & Fifth Year Playing @ MSU

At the start of the year athletes are required to get a complete physical examination.

Insurance Explanation and Parent Information Forms - annually Please read the Insurance Explanation Form on the website. This will explain the Sports Athletic Accident Policy. All student athletes must complete and submit their insurance information and copy of your card before the athlete will be permitted to compete in preseason practices. This information helps us coordinate off campus care should the athlete sustain an injury. These should be completed by the day of your preseason physical exam or update. We also request a copy of your family insurance cards (front & back). Disclosure, Consent, Concussion Acknowledgment Forms - annually All student athletes must complete these forms before he/she will be permitted to compete in preseason practices.

Please follow these instructions to enter your information into the Sportsware system: Step 1: Sign up to SportsWare Online Access www.swol123.net and click “Join SportsWare” button, it takes you next page look like picture below.

Type School ID “MSUREDHAWKS” in the box and click “Next” button.

Enter your First name, Last name, Montclair email, and select “Montclair State U.” for your group in pull down box. Then click “Send” button.

When this message box comes up, you have signed up for SportsWare successfully!

Step 2: Log in to SportsWare

When your athletic trainer accepted your sign up request, you will get an email from “admin”, looks like the picture below.

Open e-mail and click a link under your name.

You will be redirected to a password set-up page.

Type your e-mail address which you use for sign up, and set your password. (We recommend using your

CWID# as your password)

After password is set up, back to www.swol123.net and log in.

You are now on your Dashboard within SportsWare.

Step 3: Entering Your Data Click “My Info” at the top of your dashboard

You will then be directed to a General Information page : Fill out all required information indicated by the “*”. You can upload your photo if you like. You do not need to put your SSN.

Then, click the Address tab on the top: Fill out your Primary (Home) Address. If you live on campus, or have off campus housing, please input your local address as the Secondary Address.

Click the Emergency tab on the top: Then fill our Primary Emergency Contact. You can fill out Secondary Emergency Contact if applicable.

Then click Insurance tab on the top: Fill out your insurance information and upload your insurance card (Front & Back). To upload your insurance card (Front & Back) take a clear picture with your phone and email it to yourself, or scan it to your computer. Upload as JPEG/PDF file. (If you have any issues uploading your insurance card, or insurance questions, please contact the Athletic Training staff)

Then click Medical tab on the top: Fill out all six alert tabs, drugs taken, and doctor information.

Then click “SAVE” and return to your dashboard. If your screen looks like the picture below, you missed some required items and will have to go back to fill them out.

Step 4: Medical History Once back on your dashboard please click the Medical History tab:

Complete the medical history questionnaire page. Fill out the date and your sport. Leave the Evaluator name blank. Answer each question ‘YES’ or ‘NO’. If you put ‘YES’, please give us a little more detail in the comment section.

Hit SAVE, then go back to your Dashboard. Step 5: Fill Out Forms From you Dashboard, click the “Forms” tab on the top. You will be taken to the Forms page (See Below) and complete the listed forms. To open a form you must select it and then click the ‘open’ tab on the left side.

In this section you will find your Concussion Education Form, Disclosure Form, Assumption of Risk, and YOUR REQUIRED YEAR PHYSICAL PACKET. Read the entire document thoroughly and fill out the required sections. Then, scroll up to the top of the page and click “SAVE & SUBMIT”. A pop-up window will appear to electronically sign your forms. Complete the electronic signature process for each form. Then return to your dashboard.

YOU MUST DOWNLOAD AND PRINT OUT THE ENTIRE PHYSICAL PACKET TO BRING TO YOUR PHYSICIAN FOR YOUR PHYSICAL.

If you are a first year athlete or transfer at Montclair, you must provide us with proof of your Sickle Cell Trait status. Remind your physician of this at the time of your physical. Documentation must be a blood

test either done at birth or a recent blood test. If you have any questions or difficulty, please contact the Athletic Training staff between the hours 10am-5pm.
