EMERGENCY BACKPACK WITH EQUIPMENT No.PQ/10/2016-17
The Punjab Emergency Service (Rescue 1122) intends to pre-qualify reputed Manufacturers / authorized Distributors / Importers / Sole Agents of Foreign principals/ Firms / concerns duly authorized by the principal registered with Sales Tax and Income Tax departments, having sufficient managerial, technical, financial capabilities for the procurement of the following:

1) Prequalification documents are available and can be obtained on a written request and on payment of prescribed fee (non-refundable and non-adjustable) from Procurement Wing of the Service at Rescue 1122 Headquarters on any working day during office hours immediately after publication of this notice up to 07-03-2017. 2) Applications along with required information must reach the Procurement Wing by 08-03-2017 at 11:30 am which shall be opened on the same date at 12:30 pm in the presence of applicant firms or their authorized representatives, who choose to be present, in the Conference Room of Rescue 1122 Headquarters located at Emergency Services Academy, 0.5 Km Motorway Service Lane, Thokar Niaz Baig, Lahore 3) No application shall be accepted after due date and time. In case of holiday, pre-qualification procedure shall be done on next working day at same place and time. 4) Applicant who does not fulfill the minimum requirements as stipulated in the prequalification documents shall not be pre-qualified whereas only the pre-qualified firms shall be entitled to participate in the subsequent procurement proceedings. 5) Procurement shall be carried out as per Punjab Procurement Rules. 6) The pre-qualification notice is also available on PPRA and Punjab Emergency Service websites, www.ppra.punjab.gov.pk. and www.rescue.gov.pk. 7) The prequalification process initiated through Prequalification Notice No.PQ/05/2016-17 appeared in the daily Nawa-i-Waqt dated 17-01-2017, Daily Times dated 18-01-2017 is hereby cancelled. Therefore, applicant firms are required to apply afresh.

1. Introduction
The Punjab Emergency Service (Rescue 1122) (here in after referred
to “Service”) has been established under the Punjab Emergency Service Act,
2006 for professional management of emergencies such as road traffic
accidents, buildings collapse, hazardous material incidents, fires, disasters,
etc. The procurements in the Service are governed under the Punjab
Emergency Service Financial Rules 2007 and Punjab Procurement Rules,
2014 duly notified by Government of the Punjab.
2. The Rescue 1122 is providing emergency service in all Punjab and has
managed a large number of emergencies and has rescued millions of
emergency victims and saved losses worth billion of rupees. The Service
intends to procure 800 Emergency Backpack (including Trauma/ airways kits,
Pulse Oximeter, Blood Pressure Monitor, Stethoscope, Glucometer, Splints,
cervical Collar, Portable Oxygen Cylinder etc.) for Motorbike Ambulances.
The Motorbike Ambulance Service is to be started in 9 Divisional
Headquarters of Punjab, to ensure effective service delivery and improved
emergency response in congested areas and during peak hours.
3. In this connection, applications are invited from Manufacturers /
authorized Distributors / Importers / Sole Agents of Foreign principals/ Firms /
concerns duly authorized by the principal registered with Sales Tax and
Income Tax departments, having sufficient managerial, technical, financial
capabilities as well as appropriate experience, for prequalification for supply of
Backpack for Motorbike Ambulance Service.

2. Instructions 1. The evaluation criteria given in this document shall be adopted for the
purpose of pre-qualification.
2. The applicant is required to go through all instructions, forms, terms
and conditions in the documents. Failure to furnish all requisite information /
documents not substantially responsive to the pre-qualification documents
shall be at the applicant’s risk and may result in the rejection of the
application.
3. All documents / statements submitted by the applicant for pre-
qualification shall be under Oath. In case any document or statement of the
applicant proves to be fake / false, non verifiable, fabricated or materially
incorrect information shall lead to disqualification of the applicant’s firm.
4. The pre-qualification shall be valid for said project. However if any
new project containing similar articles is approved before initiating of bids
invitation, the same may be considered and only prequalified applicants shall
be eligible to participate in subsequent bidding process.
5. This pre-qualification process shall be governed under Punjab
Procurement Rules, 2014, as amended from time to time and instructions of
the Government of the Punjab received during the completion of the process.
6. The application for pre-qualification shall not be accepted if:-
a) it is received after the time and date fixed for its receipt.
b) the prequalification documents are un-signed.
c) the prequalification documents are ambiguous;
d) the application is from the firms, black listed or suspended.
e) any erasing / cutting / crossing, etc. appearing in the
prequalification documents is not properly signed and stamped by
the authorized person.
f) the cash receipts (photocopy) for the purchase of prequalification
documents is not attached.

3. Documents Required
The request for pre-qualification must comprise the following documents in
booklet shape (2 sets) duly numbered with proper annexure.
1. Company profile.
2. Valid Certificate of Company’s / Firm’s Registration / Incorporation under the
relevant laws of Pakistan.
3. Valid Income Tax Registration (Status Active with FBR).
4. Valid General Sales Tax Registration (Status Active with FBR).
5. Submission of undertaking on legal valid and attested stamp paper of
Rs.100/- that the firm is not blacklisted and not involved in litigation with any
of Provincial or Federal Government Department, Agency, Organization or
autonomous body anywhere in Pakistan. In case involved in any litigation
process, proof of dispute resolution is required.
6. Project of similar nature and complexity completed in last 05 years.
7. After sale service repair facility (Workshop) for medical related equipment (Mandatory
in Lahore and preferable in all divisional headquarters of Punjab.)
8. Income Tax Returns of the applicant.
9. Proof of Average Annual Sales of last 3 years.
10. Audited bank statement, profit & loss statements and balance sheets of last
03 years along with comparison of each statement for 3 years.
11. Photocopy of deposits slip of pre-qualification fee.
12. Proof of Quality Control Standards followed by the firm for manufacturing and
supply of medical related equipment, if any.
13. Duly filled pre-qualification form.
14. Integrity Pact duly signed and stamped by authorized representative.
15. Undertaking (All terms & conditions and qualifications listed anywhere in the
pre-qualification document have been carefully read and agreed to and abide
by these terms & conditions.
16. Any other document which the applicant deems appropriate to support his
application for pre-qualification.
17. Covering letter duly signed and stamped by authorized representative for pre-
qualification.
Note: Verifiable documentary proof of the mandatory and other requirements
mentioned in this document must be submitted with the pre-qualification application.
Non-compliance shall lead to rejection of the applicant for pre-qualification.

5. Evaluation Criteria
EVALUATION CRITERIA FOR EMERGENCY BACKPACK WITH EQUIPMENT
Pass Marks: Based on conditions listed in this document, application not meeting the 70% pass marks limit will be rejected. All applicants scoring greater than or equal to 70% of the marks will be accepted in pre-qualification. PROVISO: Provided that if none or only one (single) applicant exceeds the 70% pass marks, then the Service shall decrease the pass marks limit to 65%. In other words, if two or more applicants exceed, then the pass marks will not be decreased to 65%.

Terms and Conditions General
1) The Emergency Backpack and equipment shall be suitable for use in
tropical climate of Pakistan where blown dust and grit prevail frequently
round the year.
2) As the Emergency Backpack and equipment shall be used for the
rescue purpose throughout the Punjab, the manufacturer shall take
every precaution about the machines against ingress of the harmful
ingredient.
3) The Emergency Backpack and equipment shall bear the name of
Rescue1122 written at suitable place on lateral and back side as
approved by Service.
4) The offered Emergency Backpack and equipment should be latest
model.
5) The supplier shall provide Maintenance Manual with each unit.
6) Inspection: Emergency Backpack and equipment shall be inspected
by the authorized Inspection Committee at Lahore upon delivery.
7) The applicant shall provide last five years performance certificate from
the different clients of the government / semi government /
organization.
8) The conditional / vague bid shall not be accepted.
9) The Service may constitute a “Committee” which may visit the
applicant’s manufacturing facility / repair facility / business point, if
necessary.
10) The pre-qualification shall be valid for said project. However if any
new project containing similar articles is approved before initiating of
bids invitation, the same may be considered and only prequalified
applicants shall be eligible to participate in subsequent bidding process

ANNEXURE-A Format for Covering Letter

Having examined the Pre-qualification document and Appendixes we, the undersigned, in conformity with the said document, offer to provide the said items on terms of reference to be signed upon the award of Purchase Order/Contract. 2. We undertake, if our proposal is accepted, to provide the items/services comprise in the contract within time frame specified, starting from the date of receipt of notification of award from the Purchaser. 3. We agree to abide by this proposal for the period of ____ days (as per requirement of the project) from the date of opening and it shall remain binding upon us and may be accepted at any time before the expiration of that period. 4. We agree to execute a contract in the form to be communicated by the (insert name of the Purchaser)_, incorporating all agreements with such alterations or additions thereto as may be necessary to adapt such agreement to the circumstances of the standard. 5. Unless and until a formal agreement is prepared and executed this proposal together with your written acceptance thereof shall constitute a binding contract agreement. 6. ANNEXURE-B INTEGRITY PACT
