After receiving your two cleanings, simply complete the member satisfaction survey at https://www.surveymonkey.com/r/sodsurvey2020 within the specified time.

Dominion will confirm that the two cleanings were performed by a participating network dentist and will send the primary subscriber a check for all applicable family members.
