President’s Message
TheNationalBlackNursesAssociationiscontinuingitslegacyinmakingadifferenceintheprofessionofnursing.While thenursingprofession is the largestsegmentof thenation’shealthcareworkforce, lessthan6percentoftheprofessionalregisterednursesarecom-
posedofnursesofcolor.And,yetpeopleofcolormakeupadisproportionatenumberofpatientswithmultiplechronicdiseases.NBNAiscommittedtoamissionofeliminatinghealthcaredispari-ties.NBNAstandsfirmbehindtheInstituteofMedicinereporttohelpournationpreparemorediverseprofessionallicensednursestotakeonleadershiprolesatthehospital,theboardroomandwithinthepoliticalarena.Aswecontinue tomove forward, our goalsmust continue to include support for thede-
velopment of a cadreof ethnic nurses reflecting the nation’sdiversity; advocacy for culturallycompetent,accessibleandaffordablehealthcare;promotionoftheprofessionalandeducationaladvancementofethnicnurses;educationofconsumers,healthcareprofessionalsandpolicymakersonhealth issuesofethnicminoritypopulations;developmentofethnicminoritynurseleaders inareasofhealthpolicy,practice, educationand research; andendorsementofbestpracticemodelsofnursingpractice,education,andresearchforminoritypopulations.NBNAhasfocusedontheIOMReportontheFutureofNursing.Membersareactivelypartici-
patinginleadershiprolesfortheirStateActionCoalitions.TheNBNAAdHocCommitteecontin-uesitsdevelopmentofastrategicapproachtorespondtotherecommendationstoIncreasetheproportionofnurseswithabaccalaureatedegreeto80percentby2020;and(2)Recommenda-tion7:Prepareandenablenursestoleadchangetoadvancehealth.NBNAhasalsoestablishedadiversityagendatoprepareitsmembershiptoassumeleadershippositionsacrossalllevels.Theofficers,boardmembers,andmembersarecontinuingtousetheirsynergisticpowerto

Lest We Forget: The Bachelor of Science in Nursing Conundrum Sandra Webb-Booker, PhD, RN
THe GOLDMARK RePORT, recommended the BachelorofScienceinNursing(BSN),astheentrylevelforprofes-sionalregisterednursesin1923.Almost70yearslater,we
continuetoeither readaboutorwitnessobjectiveaswellasheated opinionated discussions regarding the entry level for“professional nursing practice.” Most discussions cite dataaboutactualandprojectedfigures regarding thesupplyanddemand for nurses. Some additional factors influencing the“professionalnursingpractice”debatearetechnology,health-careoutcomesandcomplexities,expansionofclinicalnursingknowledge,andtheneedforincreasedautonomyinnursing.Shifts in health care delivery, from traditional hospital-
centered inpatientcaretomoreprimaryandpreventivecare,havealsoplayedasignificantroleinthesediscussions.Nursesworking in today’s healthcare system must practice acrossmultiple settings—thosewithin thehospital, aswell as thosewithinthecommunity.Nursesmustalsoexercisehigherlevelsofindependencewhenproblem-solvingandprovidingclinicalcare,i.e.beitcasemanagement,supervisionoflicensedandunlicensedpersonnel,and/orthecoordinationofcareandvitalhealthcareresources.Theeducationalpreparationandscopeofpracticeassoci-
atedwiththeBSNpreparedNursehasopeneddoorsthatcanlead to professional certifications in specialty areas such ascriticalandemergencycareandtheassumptionofexpandedrolessuchasaclinicalcareprovider,designer,manager,andcoordinator.TheBSNdegreeprovidesthefoundationneededforgraduatenursingeducation.WhereaslimitationsinthepresentscopeofADNeducation
hasledtothecreationofvulnerabilitiesforitsgraduates’futurescopeofprofessionalnursingpractice.TheAssociateDegreeNurse (ADN) is educationallyprepared to function in variousclinicalcapacities,buthasalimitedscopeofpracticeinnursingleadershipandmanagement,wellness,andcommunityhealth.ChangesintheeducationalpreparationoftheADNareneededto lessen thesevulnerabilitiesandenhanceADN’sviability inthehealthcareclimatetodayandtomorrow.TheFutureofNursingproposaladvocatesthat,atamini-
mum,theRegisteredProfessionalNurseberequiredtoobtainaBSNwithin10yearsafterinitiallicensure.ADNscannotbecontentwiththe“statusquo”.Todate, three summits havebeenheld,wherebynurses
throughout the state of Illinois have come together to meetanddiscusstheiropinionsandconcernsrelativetolegislativeproposals regarding the future“ProfessionalNurse”status inIllinois.The“summit”processiscreditedwithfacilitating“opendialogue amongst and between nurses of all communities(ruralandurban)andallethnicities,colorsandcreed.”Lessonslearned during the “1985 entry into practice struggle, morethan25yearsago,hassuccessfullybeenplacedintoaction,andmutuallyproductiveworkingrelationshipsexistsbetweenallparties.
However,formanyChicagoChapter NBNA members, re-defining theeducational needsofthe“ProfessionalNurse”,hasbeenreminiscentofthestrugglefacedbymanyCCNBNAmem-bersinthe1980s.Thenotionof“mandatingneweducationalrequirements”,forthosenurseswhohavesuccessfullyearnedthe “Professional Nurse” licensure status, based on currentand past legislationwithout the incorporation of a “grandfa-ther clause,” is an area of paramount concern forCCNBNAmembers.InreadingtheRobertWoodJohnsonFoundationInitiative
on theFutureofNursingat the InstituteofMedicinewefindthatRecommendation#15.says:Four-yearnursingprogramsshouldworkwithCommunityColleges thatoffer theADNtodeveloparticulationagreementsthatfacilitatethetransitionofADNgraduates intoBSNprograms.Thiswould incentexist-ingADNstoreturntoschooltocompletetheirbaccalaureatedegrees,whileenhancingtheireducation,preparationandlon-gevityinthenursingprofession.TheNationalLeagueofNurs-ingAccreditingCommissionandtheAmericanAssociationofCollegesinNursing(AACN)areencouragedtocollaborativelyworktogetherforthedevelopmentofaccreditationstandardswhichfacilitatearticulationagreementsbetweenADNandBSNnursingprograms.TheCCNBNAsupportstheInstituteofMedicine’srecom-
mendationtohaveprofessionalnurseswithAssociateDegreescontinuetheirnursingeducationandcompleteaBSNthrougharticulation initiatives with four-year universities as stated inrecommendation#15.We strongly encourage community colleges, vocational
programsandothereducational supportsystems toprovidethe basic academic support needed to ensureminority andunderservedpersonsdesiringtobecomeprofessionalnursesare prepared tomeet the admission criteria that will enablethemtobeadmitted toandsuccessfullymatriculate throughBSNprograms.Weencourage thecoordinationof educational resources
providedbyprofessionalorganizations,universitiesandotherstakeholders.Onecentralciteisneededtohouseinformationpertainingtoprograms,grantsandscholarshipsthatprovidefinancialassistancetoADNsmatriculatinginBSNprograms.The CCNBNA believes the establishment of the BSN
as theentry level for “professional nursingpractice”will alsoimpact the salaries of all practicing nurses. Institution of theBSNinitiativeshouldpromoteareductioninthenumberedofunlicensedpersonnelperformingnursingskillsinmanypatientcaresettings.

The Changing Healthcare Environment: Application of Technology in Nursing and Patient Education.

iNtRoDUctioNIntheworldwheretechnologyisnotonlybecomingmore
complex, but resourceful, nursing education has been chal-lenged toendorse theuseofnursing informaticconcepts inpracticeandeducation.Advocating theability to incorporatetechnologyintonursingeducation,theAmericanNursesAsso-ciation,theAssociationofCollegesofNursing,andtheInstituteofMedicine,stronglyencouragednursingschoolstointegratetheuseoftechnologyintheirnursingcurriculum(ezell,2011).evidence-basedresearch indicatesthatsuchtechnologyen-hances critical thinkingwhile reinforcing core knowledge forpractice.Technology has influenced all areas of human endeavor
includingthewaywelearnandsocialize.Intoday’shealthcareenvironment, increased acuity of the patients and the use oftechnologyatthebedside,promptedemployerstopreferhiringnurses who have the skills necessary to practice in clinicalandnon-clinicalsettings.Useoftechnology inclinicalsettingsprovide thestudent immediateaccess tomedical information.Thenurseeducatorwillhavetobeawareofcurrenttechnologyandhow itcanbeusedtoadvancenursingeducation.Thereareadvantagestoeachtypeoftechnology.Whileeachitemoftechnologymayenhancethelearningexperienceforstudents,itmayalsojeopardizetheconfidentialityofotherstudentsandclients.Facultymustbeawareoftheseissuesandemployeverypossibleprecautiontoprotectbothpatientsandotherstudentsfromviolationofprivacy.Responsetothetransformationofhealthcareandquality
initiativeshaveresultedinintegrationoftechnologyinnursingeducationcurricula.Sometechnologytoolsusedincludesimu-lation,interactivecasestudies,digitaltextbooks,onlineinstruc-tionalmode,PDAs,electronicmedicalrecordanddocumenta-tionsystems.Technologycanalsohelp the faculty todeliverthe course material effectively, improve student and facultyaccesstoinformationandresources,reducecostoflearningand educational resources and enhance learning. Attitudesandwillingnessofthefacultytoembracenewideasininstruc-tionaldesigncanhelppromotestudentlearningoutcomes.Integratingtechnologyintonursingcurriculaismucheasier
now than it was in the past. Amajority of college studentsownmobile technologysuchasan iTouchorsmartphones,reducingtheinitialhardwarecost.Softwareleadingprovidersareavailable foreveryplatform including tabletssuchas theiPad.Softwarereferencesaresignificantlylessexpensivethantextbooks and if the software is required for coursework, fi-nancialaidtypicallywillcovercosts.Confidentialityagreementsare overcoming barriers in facilities that have been reluctanttoallowmobiletechnologyduetopotentialprivacyconcerns.The vital role of mobile technology in patient safety can nolongerbeignored.Nursingfacultycanchoosefromhundreds
of references and decide what best fits the needs of theirspecificstudentpopulationandcourse.IncreasingtheuseoftechnologysuchasPDAintheclassroomandclinicalsettingsmovesnurseeducatorsclosertopreparingnursestomeettheInstituteofMedicine’scorecompetenciesandTheJointCom-mission’spatientsafetygoals.
coNclUsioNRapidchanges in technologyand intergenerationaldiffer-
ences in learningbringanew focuson theneedsof facultyto be able to engage students in the traditional face-to-faceclassroom.Faculty rolesarechangingascomputerassistedtechniques bring more autonomy to the students; studentsof diverse ages and background are expecting computerliteracy from their instructors. The increased role of technol-ogyinhealthcareanditstransformationalimpactonthatfield,allowingnurses tounderstandcurrentand future trendsandthus,integratingtechnologyintonursingeducationinordertoeffectivelyprepare students for anew, technologically-drivenhealthcareenvironment.Inthefuture,thehopeistoseenursesroutinely synchronizing their handheld computers with stra-tegically centralized database ports throughout a healthcareagency.

“there is just toomuch content to be covered.”Neither isit acceptable to only lecture from the podium using thedreadedPowerPoint©.Thisarticledescribessome innova-tiveways that nurse faculty areusing technology tomakelearningmoreinterestingandmorecongruenttotheoverallexperiencesoftheyoungergenerationofnursingstudents.Italso introducesseveral,notso frequentlyused, technol-ogytoolsthatarenowonthehorizon.
flippiNg the clAssRoom Berret (2012) describes what is popularly known as
the ‘flipped classroom.’ In this concept, students are heldaccountable for accessing course content outside of theclassroom through faculty prepared audio visual lectures.Students have the option to gain the information from avariety of sources conducive to the way they learn best.Thislearningcantakeplaceindramaticormodestformatse.g.,voiceovers,YouTube©oronlinesearches.Intheclass-room,studentsgettheopportunitytospendindividualtimeworkingwithprofessorsonwhat theyhave learnedso far.Duringthistimevarietyofteaching/learningstrategiessuchascasestudiesandnarrativepedagogyareemployed.Thisis an idealwayof learning forGenerationY studentswhoare known to respondwell to coaching and reassurancesaswellaspeerteaching.This‘flippedclassroom’givesstu-dentstheopportunitytoexpressthemselvesinalessformalway,oneinwhichtheyinteractcloselywithpeersaswellasteachers.Theliteraturereportsmanydefinitions,myths,andprosandconsofflippedteaching.ProfessorsTennesonandMcGlasson of evangel and Southwest Baptist universitiessummarizestheconceptasonethat“moveslecturemate-rial out of the classroom through online delivery... extendconversationoutsideofclassthroughthreadeddiscussion,move ‘homework’ into the classroomwhere the instructorcanserveas‘guide;’anduseopeneduptimefordiscussionandpractice.”(Baker,2000)Sohow is thisdone?Oneway is for educators touse

Homeworkmanagers, like CengageNOW©alsoprovidetoolstoprepareyourstudentsbeforeclass. Itisnodoubtthat‘Flippingtheclassroom’leavesmoreclasstime available for hands on, collaborative, active-learningexperiences.

whichtheteacherself-assesshisorherlearningstylesandmatchtheteachingwiththeappropriateandavailabletech-nology.Thereareseveraltechnologytoolsthatcanenhancetheclassroompresentationwhilecoveringvastamountsofcontentinaninterestingway.MostfacultyarecommittedtotheirPowerPoint© lecture,however thismodality isquicklybeingreplacedbyPrezi©auniqueprogramwhichenablestheeducator toengage the studentusingaconceptmapandstorytellingapproach.Other smart classroom tools such as Polleverywhere©
allow for immediate feedback in the classroomaswell asawaytostimulatestudentsintoparticipation.Clickers©arealso being widely used to obtain immediate feedback onlearning.ThewidelypopularYouTube©isoftenusedtoshowshort
videosofwhat isbeing taught. It is importanthowever forfacultytopreviewthevideototheendbeforeintroducingittostudents.Forfacultywholovetomovearoundtheclassroomwhile
teaching, the program Doceri© can be used to transformtheiPadintoamobilepresenter.Manypublishersresourcessuch as elsivierevolve© can be utilized via the iPad. WellknowntoolssuchasGoogledocs,GooglesitesandLive-Binder©arequiteusefulwhengroupprojectsareassignedastheyallowfortrackinggroupparticipationaswellasstor-ageofprojectcontent.Finally,toolssuchasLivescribe©Penallowforrecording
ofpresentationsforfuturediscussionandgrading.Skype©and Facebook© chats are also recommended for groupwork,collaboration,tutoringandexamreviews
sUmmARyThese are just a few of the tools you can use in your
Smart Classroom. There are many more available. Scott(2012)suggestswhen lookingat technology tools foryourcourses,avoidusingtechnologyjustbecauseitis“new”or“cool”.Insteaddefinethetool’spurpose,makesureitcom-plementstheactivity,andalignswithyourteachingstyle.

nursingprogramtoofferanonlineMaster of Science in Nursing(MSN). This provides an oppor-tunityfornurses,particularlyAfri-can-American,topreparetomeetprimaryhealthneeds.OuronlineMSNprogramallowsflexibilityfornurseswhowanttoadvancetheircareersthatalreadyworkinthefield.Althoughitisonline,theprogramemphasizesandrequiresthesamerigorofclinicalpracticums,withsupervisedpreceptorsinthestudent’shomelocation.AsAfrican-Americanandminoritynurses,itisimportantthat
wearecontinuallyawareofcontemporarytrendsinhealthcareandprepare tomeetmarketdemands.The futurehasneverbeenbetterforcareeradvancementasanursepractitioner.

practitioners,thevalueofaprogressivenursingeducationhasexponentially increased. The global policy think tank, RANDCorporation,estimatesthatthenumberofnursepractitionerswillnearlydoubleby2025.Thisdemandisfurthertriggeredbythecriticalshortageofprimarycarephysiciansofasmanyas63,000toofewdoctorsby2015,accordingtotheAssociationofAmericanMedicalColleges.With tens of millions of previously uninsured Americans
projectedtoenterthehealthcaredeliverysystemoverthenextfewyears,nursepractitionersareinacriticalpositiontomeetchanginghealthcaredemands.Astheanticipatedshortfallofprimary-care providers threatens the system’s capabilities,more nurses will be expected to pursue advanced nursingdegrees.Giventhehighpercentageofhealthdisparitiesthatoccur
inminoritycommunitiesandchronichealthconditionssuchashypertension and diabetes, having the cultural competenceand education can bemore effective in preventative health-care.Assuch,therewillbegreaterneedforAfrican-Americannursepractitionerswhohave training,expertise,andculturalsensitivitytoeffectivelytreatpatients.In response to this risingdemand fornursepractitioners,

TeLLINGONe’S PeRSONAL story is an excellent way ofgivingvoicetosilentissuesandproblemsthatcouldderaila student’s career aspirations. If the profession of nurs-
ing is committed to improving workforce diversity, then, anevidence-basedapproachmustbeimplementedthatincludesearlyscreening,detectionandinterventionsforstudentswhoneed assistance to succeed. The goal must be to ensureadmission, progression and graduation of underrepresentedminority(URM)students.Helpingstudentsbecomesuccess-fulinachievingtheirfirstdegreecanbuildtheirconfidencetocontinue theireducation to themaster’sanddoctorate level.Thisarticlesummarizestheacademicjourneysoftworecentgraduatenursingstudents,anddiscussesacademicbarrierssomestudentsfaceoncetheymatriculatefromthepre-nursingphaseintoanursingprogram.

encethatwassponsoredbytheBlackNursesAssociationofGreaterCincinnati.Theconference themewas “Pathways toHighereducation.”Asanursingstudent,Iwasexcitedtobeinthepresenceofprofessionalnurses.Thethreespeakersattheconferencewereoutstanding.Theytalkedaboutthefutureofnursing,theimportanceoflife-longlearningandtheneedforcontinuous professional development. each speaker sharedhis/herstrugglesandjoysofnursingschools,butmoreimpor-tantly,theydiscussedhowtobecomeasuccessfulandpro-ductiveprofessionalandwaystogivebacktotheprofessionandhelpothers.Theirstoriesofsuccessopenedmyeyesandignitedmyspirit;yes,Icouldandwouldachievemydreams.Iamsharingmystoryinhopethatotherswillreaditand
knownottogiveupwhenitseemsimpossibletoobtainyourgoals.OneofthespeakersattheMay19conferenceencour-agedme to givemy experience a voice to help others.MynameisCassieGardner.Iamanursingstudentinanassoci-ate degree in nursing (ADN) program. This nursingprogramis operated as a hospital-based school. I, similar to one ofthe speakers, have alwayswanted to be a nurse. I had theacademiccredentialstoboostmyopportunitiestoachievemygoalsofenteringandgraduatingfromnursingschool.Igradu-atedfromhighschoolswitharecordofhavingearnedA’sandB’sfromgradeschool.Withsuchanexemplaryschoolrecord,myfamilyandIassumedthatmylevelofperformancewouldcontinueandthatIwouldexcelincollege.In2008,Istartedcollegeandtomysurprise,IlearnedthatI
struggledwithtesttaking.NotonlywasIshockedthatIwasnotperformingwellinclass,butIwasatalossastohowtomoveforwardandwhatshouldbemynextsteps.Nooneinformed
meofavailableresources.Ididnotknowifthereweretutoringor academic assistanceprograms available on campus. Forthefirsttimeinmylife,Ifoundmyselfonapathofacademicfailure.Oneparticularprofessorencouragedmetoseekhelpfora“learningdisability.”HowcouldIgofromA’sandB’stobeingtoldthatImayhavealearningdisability;itwasunbeliev-able.Ofcourse,thisfeedbackonlyaddedtomyfear,confusionand frustration.Nothavinganacademicsupportsystem ledtomyworsefearoffailingnursingschool.AlthoughIpassedmyclinicalcourse,Ifailedthetheorycourse.Thefailureofthetheorycourseledtomydismissalfromthenursingprogram.Being dismissed from nursing school started an entirely
new set of emotions and experiences thatweremovingmefartherandfartheraway frommydream,andthestressandsenseoffailureweremakinglifelessthanwhatIhadplannedformyfuture.Then,inthesummerof2009,IwasdiagnosedwithCombinedAttentionDeficitHyperactivityDisorder(ADHD).Duetomypreviousoutstandingacademichistoryandnobe-havioral issuesduringmyprimaryeducationyears,myfamilyandIwereinastateofcompletedenial.Iwasconsideringthepossibilityofquitting,givinguponmydreamandnotlivinguptomyfullpotential.Myfamilyandpeershadalwaysheldhighexpectationofme.Therefore,Ihadtheaddedpressureoflet-ting themdown.At least, thediagnosis gaveme somethingtoworkwithandaplacetostartcorrectingthe issue. I trieddifferentmedicationsandtreatmentregiments.Finally,Ifoundamedicationthatworksforme. Iwasalsofortunatetohavenursingprofessorswhodidnotgiveuponme.Fortwoyears,myprofessorsworkedwithmeandhelped
mereturntoschooltopursuemydreamofbecominganurse.Sincereturningtoschool, Ihaveacceptedsomethings,andthesethingshavecontributedtomyprogressioninthenursingprogram. Ihaveacceptedthat Iamnotyournormalstudentand that Iamadifferent learner. Ihave learned toadaptmystudyroutinesandseekoutenvironmentsthatareconducivetomylearningstyle.Myprofessorshavenothingbutexcellentthingstosayaboutmeclinicallyandpersonally.Iowemysuc-cesstothemcaring,beingpatientandcompassionateabouttheirstudents.IknowthatIamoneofthefortunatestudentstohavehadfacultymemberswhocaredbeyondtheirclassroomresponsibilities. At theCincinnati Conference, not only did I receive en-
couragement,butIwasalsogivenanetworkandplatformtograduateandbecomesuccessfulincontinuingmyeducationtoachieveallthatIwantedtobecomeasaprofessionalnurse.IamgladtobewhereIamtoday.IgraduatedDecember2012withmyassociatedegreeinnursing.InJanuary2013,Istarted

Dr.MarthaDawsonspokeabouttheimportanceofcontinuingyoureducationwhenshegaveanoverviewoftheInstituteofMedi-cine(IOM),RobertWoodJohnson’sFutureofNursingReport.Iplantocontinuemyeducationtothedoctorateofnursinglevelwiththehopeofbecomingacertifiednurseanesthetist.Iwasso impressed with the educational conference provided bytheCincinnatichapterthatIattendedthe2012NationalBlackNursesAssociationAnnualeducationConferenceinOrlando,Florida.Itwasanawesomeexperience.
peRsoNAl ADjUstmeNts to AvoiD fAilURe August2009wasthebeginningofmynursingschooljour-
ney,andtheonsetofmanychallengesthatIhadtoface.MynameisAndrettaGriffin-Maxwell.IamarecentgraduatefromaBSNprogram.Duringmynursingeducation,IwasacceptedintoafederalgovernmentfundedprogramthatwassupportedbytheHealthResourcesandServicesAdministration(HRSA)WorkforceDiversity(WFD)grant.Theprogramwasenrichmentfor AcademicNursingSuccess (eANS). Iwas selectedwithnine junior level nursing students to participate in the eANSprogram. My group was the third cohort of students to beselectedforparticipationinthisprogram.Facultyandstaffofthisprogramprovideda three-day intensiveorientation; theyexplained the requirements ofwhat it took to be successfulinnursingschool.Apaneloffaculty,professionalnurses,andpreviousandcurrentnursingstudentssharedtheirstoriesandjourneywithus.Nevertheless,afterthefirsttwotothreeweeksintheprogram,thechallengesIfacedinnursingschoolweresooverpoweringthatIfeltlikeIneededtochangemajorsandchangeschool.Icried,whichseemedlikeeveryday,becausetheworkload
wasextremelydemandingandseemedunbearable.Iwantedtogiveup.Iwasdevastated,depressed,anddisappointedwithmyselffornotbeingabletohandlethechallengeandthetransi-tionfrompre-nursingtonursingschool.Astheweekspassed,thingswentfrombadtoworst.Ifellbehindinreadingassign-ments;Icouldnotcomprehendthematerials,andIwasfailingall tests, andmy clinical performance had deteriorated. TheeANSprogram facultywasmeetingwithstudentsweekly toaccesstheirperformanceandprovideassistanceasneeded.However, I continued to struggle with managing my time,courseworkload,andspiritualhealth.AlthoughIwasreceivingsupport and help, Iwas suffering, and I neededmore help.Iwantedmy lifeback; the joy,peace,happiness,motivation,andthedeterminationIhadinpre-nursing.IwantedthefrowntoreversebackintothesmileIoncehadpriortoenteringthenursingprogram.Therefore, I began to turn tomy faith and pray. If I was
goingtobesuccessful,Ineededmorehelpandguidance.TheeANSprojectdirector(principalinvestigator)andtheprogramfacultyfacilitatorbegantoprovidemewithmoreindividualizedtime.Theprojectdirectorwasalsomyadvisor.SheandIhadacrucialconversationwheresheexplainedupfrontthatsomeofthingsshewouldadvisemightsoundhardanddifficultformetohearandaccept.Shequicklyidentifiedthatextracurriculaactivitiesandtimemanagementweremajorfactorscontribut-ing tomyacademicproblems.Sheadvisedme, forfinancial
reasons,tomoveoutofmyapartmentandmovebackhome.Shediscussedalso the fact that I needed towithdraw fromtheROTC,andmetwiththeadvisorandme.TheROTCwasimportanttomebecauseIwantedtojoinsomebranchofthearmsservices,anditwasamajorsourceoffinancialsupportto fundmy education.Next, she explained that I needed toreducetheamountoftimeIwasspendingonmyhairandnails.Althoughthesedecisionsweretoughandtheadvicesoundedharsh,Itrustedheradvicebecauseshehadacaringattitude.Shestatedheradvicewasthesameshewouldgiveherowndaughter. Iknewinordertobesuccessful, Ihadto letgoofsomethingsandfocusonwhatIwantedmostinlife.Ifollowedallofheradvice.Sheadvisedmetomeetwiththefacultyfacili-tatorandfollowtheactionplanthattheywouldputinplaceforme.Shealsoarrangedformetohaveanindependentstudycoursewiththefacultyadvisor.Thefacultyfacilitatorwasverypassionateandwaswillingto
helpmewithmyacademicstruggles.Wedidweeklyone-on-onestudysessions.ThestudysessionshelpedmerealizewhatIwasdoingwrong,andwhyIcouldnotcomprehendthemate-rial.IdiscoveredthatIwasnotactivelyreadingtheinformation,notpayingattentiontokeywords,ignoringmedicaltermsthatIdidnotunderstand,andusingthewrongtechniquesforstudy-ing.Onceweidentifiedmyproblems,thingsstartedfallingintoplace. Ihadbeguntorealizethat thechallengeswerenot todestroyme,butIhadtounlearnsomestudyhabits,andlearnnewmethodsofunderstandingandapplythecoursecontentto clinical situations. One of the best lessons I learnedwasthatIwasnotstudyingtopassanexam,butmyperformanceonexamsdemonstratedthatIhadmasteredthecontentsoIcouldapplytheknowledgetoreal lifesituations.Astheproj-ectdirectorremindedmestudyingandtakingexamswasnotforearningagrade,butthegradewastheby-productofmylearning.Bymythirdsemester,mysmileandself-confidencereturned,and Ibegin tosharewhat Ihad learnedwithotherstudents. The support from the eANS program and facultywasthecornerstoneofmysuccess innursingschool.Sincegraduation,nursesintheBirminghamBlackNursesAssocia-tion (BBNA)havebecomemyprofessionalmentors,supportsystem,andthenetworkformyfirstnursingjob.

students start taking nursing courses andgoing to clinicals.Students might have difficulty with reading comprehension,testtakinganxiety,knowledgeapplication,timemanagementand critical thinking skills. For many students, memorizingmaterialhasbeen theirprimarystudymethod toprepare forexamsinhighschoolandinmanyoftheirpre-nursingcollegecourses.Memorizationdoesnotleadtolearningandsuccess-ful test taking in science-basedprofessionsbecausecourseinformationisthebuildingblockfortheapplicationoflearnedcontentinclinicalandlabsettings.Inaddition,priorsemesterlearningbecomesthefoundationforintroductionofnewideasandconceptsinthenextsemesterandfuturecourses.There-fore,informationmustberetainedandusedasknowledgebystudentsastheyprogressinthenursingcurriculumfromfun-damentaltoadvancedcourses.Nursingprogramsandother

professional schools do not allocate resources for remedia-tioninbasiccollegecoursesorprovidetutorialassistanceforstrugglingstudents.TheDivisionofNursing,BureauofHealthProfessions,HealthResourcesandServicesAdministrationisawardinggrantstoschoolsofnursingtoincreasethenumberof nurses from disadvantaged and minority population whosuccessfullymatriculateandgraduatewithaBSNdegree.Thegoal is thatby increasingdiversity innursingaccesstocare,qualityofcareandthedeliveryofculturallycompetentcarewillimprove.Facultymust appreciate cultural and learning stylediffer-
ences that may affect students’ performances. As was thecasewithCassieandAndretta,greatnursingprofessorsandinstructors will look for an underlying cause of a student’sproblem.Student-centeredfacultyalsowillprovideunpleasantfeedbackinawaythatissupportiveandnotdemeaning.InABlueprint forReform: TheReauthorization of theelementaryandSecondaryeducationAct(2010),itwasclearlystatedthatgreatteachers,notthecolorofstudentsskinortheirparentsincome,thatdeterminestudentsuccess.ThisreportcalledforreformoftheUSeducationsystemtoensurethateverystudentgraduatereadyforacollegeeducationandacareer.Smith,Williams-Jones,Lewis-Trabeaux,andMitchell(2012)
identifiedstudents’perceptionsoffacilitatorsandbarriersthataffectacademicsuccess.Thesesauthorsreportedthatsuchfactorsasfacultyencouragement,facultyavailability,usingdif-ferent learningmethods,obtainingassistanceattheproblemsolvingandanalysislevel,andself-directedlearningbehaviorswere important factors to help studentswith their academicperformance.Helpingstudentsfromunderrepresentedback-groundstoidentifyandcapitalizeontheirlearningstylescanbecriticaltosuccessinmatriculatingandgraduatingfromnursingprograms.Somestudentsmayhavetounlearncertainstudyhabitsandidentifynewlearningstylestohelpthemsucceed.Peer-to-peer mentoring has also improved nursing studentsperformance.TheeANSprogramwasdesignedsothatseniorcohortswouldmentorincomingjuniorcohorts,andtherewerepeermentoringwithinthecohorts.Toimprovetheirchanceofadmissiontonursingprograms
and to successfully navigate the nursing curriculum, pre-nursing students should work with their college advisors todeterminetheirareasofweaknesses,strengths,opportunities,and threats. If learningandacademicdeficienciesare identi-fiedearly,studentscanreceivehelppriortoenteringnursingprograms.Facultyandpre-nursingadvisorsmust rememberthatschoolsinAmericaarenotallcreatedequally,andmanystudentsarriveatcollegeunderpreparedfortherigorsofcol-legelevelstudyandthedemandsofmoreself-directivelearn-ingbehaviorsfromstudents.Therefore,thesestudentsshouldbeguidedtocampusresourcesearlyintheircollegelife.Pre-nursingadvisorsshouldhelpdevelopplanstoaddressidenti-fieddeficitsandconnectstudentstonursingadvisors.Nursingprogramsandtheliteraturehavedocumentedthat
pre-nursinggradesandperformance insciencecoursesarepositivepredictorsofhowwellstudentswillperforminnursingschool and the likelihood the studentswill graduate (Seago,Keane, Chen, Spetz and Grumbach, 2012). Adding to thestudents’struggletosucceedacademicallyareotherbarriers
thathavebeenwelldocumentedinnursingandotherliterature.Loftin,Newman,Dumas,Gilden,andBond(2012)providedathorough reviewof the literature thataddressedbarriers thatURMstudentsface innursingschools.TheirreviewincludedarticlesthatstudiedexperiencesofAfricanAmericans,NativeAmericans, Hispanics, Asian, and international nursing stu-dents.ThereviewidentifiedalistofbarriersthatURMstudentsface.Thesebarriers includedculture isolationandloneliness,family issues, emotional andmoral support, financial needs,discrimination and racism. In their study, Smith, Williams-Jones,Lewis-Trabeaux,andMitchell (2012) listedfive factorsthatcouldcontributetostudents’failuretoprogressinnursingschool. Factors identified in these author’s study that couldbecomebarrierstostudentsuccessincludedacademicwork-load, test-takinganxiety, financialworries, family responsibili-ties,andworking.Inconversationswiththestudentshighlightedinthisarticle,
eachstudentreportedexperiencingatleastfourofthesefivefactorsorallfive.StudentsfromURMgroups,includingmales,alsoreportedstereotypingbytheirfaculty,peersandpatients.Manynursingprogramsareproactively tryingtoaddresstheneedsofURMstudents.Socialization, acceptancebypeersandfaculty,andafeelingofinclusivenessandbeingrespectedasapartof the teamarealso importantcontributors tostu-dents’success,(Smith,Williams-Jones,Lewis-Trabeaux,andMitchell, 2012; Loftin, Newman, Dumas, Gilden, and Bond,2012).ABlueprint forReform:TheReauthorizationof theel-ementaryandSecondaryeducationAct(2010),callforustopreparestudents success, the rigorof learningandmeetingtheneedsofstudents.Professionalschoolmustbegintolearnwhysomestudents fromURMexcelandothersdonot.Ourfocusneedtoshiftfromwhatastudentcannotdotohelpingstudentsgetreadytosucceed.
sUmmARyToimprovestudentschanceofmatriculatingandgraduat-
ing fromnursingschools,a lotofworkneeds tooccurpriortostudents’admissiontonursingprogram.Therefore,nursingprogramadministratorsmustbegintodevelopinterprofessionalcurriculumdesignsthatincludeexposingstudentstoapplica-tionandteam-basedstylelearningintheirpre-nursingscienceandliberalartcourses.Pre-nursingstudentsmustunderstandthat theirpre-nursingscienceandmathcoursesprovide thefoundation for theirnursingcourses.Therefore, theyneed tounderstandtheconceptspresentedinthesecoursestohelpthem with knowledge application and learning in their corenursingclasses.NursingassociationscanplayamajorroleinhelpingURM
studentsbydevelopingtutoringandmentoringprograms.ManychaptersofNBNAhaveexperiencewithdesigning,implement-ing, and supportingmentorship programs that are aimed atincreasing theprogressionandgraduation ratesofstudents.These chapters have created partnerships with communitycolleges and universities. The NBNA and affiliated chaptershaveacritical role in increasingnursingdiversityandhelpingtheprofessiontoachievetheIOMmandateof80%BSNpre-parednursesby2020,anda50%increaseinthenumberofnurseswithaterminaldoctoratedegree.

Iproceededwithcautionbut Ibeganmy lectureonhowAfricanAmericanshavethehighestratesofhypertension,dys-lipidemia,diabetes,andobesity;allofwhichcanbepreventedand/or controlled with proper diet and exercise. My sistersuggestedweincorporatenewfruitsandvegetables intoourdietat leastonceamonth.This turned intoa recipesharingconversationamongthewomen.Thereyouhaveit! IwasabletoturnmySoulFoodloving
familyintoonethatisn’tafraidtotrynewfoods,understandstheimportanceoflifestylemodification,andwillhopefullyde-crease their chancesofbeingdiagnosedwith apreventabledisease.Thissimplegatheringhasevencausedarippleeffect.My family members are taking the new healthier recipes toworkwiththemwhichitcreatingabuzzamongsttheircowork-ers.Whoknewpeopleinthesouthwouldgetsoexcitedabouttheirhealth!everyonecanalltakesmallstepstomakeourcommunity

In order to continuegrowingashealthcareprofessionalsandimprovepatientcare;changemustbeginwithinourselvesbymeansofexploringourthoughtsandperceptionsandhowourunderlyingexperiencesandbeliefsimpactpatientcare.Theideaofwhatwethinkwebecomeisaninterestingconcepttoconsiderbothprofession-allyandpersonallyinthefieldofnursing.As a nurse we come across people of different races,
beliefs,religions,socioeconomicstatus,thoughtsandpercep-tionseverysingleday.Whilecaringforpatient’sweoftentimesbring inourownbeliefsofwhat is rightorwrongandgoodorbadwhen itcomes toprovidingcare forourpatients. It’shumannaturetohavesubconsciousthoughtsthatariseaboutthingsandpeoplewecomeacrosseveryday.Duringmyclinicalexperienceasanursingstudent,Icame
acrossmanyexperienceswhere IsawmedicalprofessionalsmakingjudgmentsaboutpatientsandattimesIwasguiltyofthisaswell.Ifoundmyselfthinkingabouthowourownpercep-tions,beliefs,valuesandjudgmentsmayimpactpatientcare.Arethewomenwholiveinpovertywithnoinsurancetreateddifferently thanthosewhoaresuccessful?Ourmindsareso
powerfulthatwecanmakeourselvesbelieveanythingwewantandthosebelievesinturnimpactthewayweinteractandtreatpeople. If youwant to believe that themotherwho has notseenherbaby in intensivecare for threeweeks isahorriblemother youcanconvinceyourself tobelieve thatandbasedupon thatbeliefour interactionsmaybedifferent than if shecametoseeherbabyeverysingleday.Nowthequestiontoconsideriswhythismotherhasnotbeenabletoseeherbabyis shedepressed?Has shehadapreviousmiscarriageandcannotbeartodealwiththepossibilityoflosinganotherchild?Perhapsshedoesnothave transportationorababysitterathome?Wedon’tknowherstory.It’s so important to consider the values that nursing is
baseduponwhenprovidingcare.Althoughwearealldifferentandour lifeexperiencesshape thepeoplewebecome.Oneaspectthatremainsconstantisthatweareallhumanbeingshavetherighttobetreatedwithdignity,respectandcompas-sion;regardlessofourlifestory.eventhoughitmaybehumannaturetomakevaluejudgments,itisimportanttobeawareofhow these judgments, our ownbeliefs and perceptions canimpactpatientcare.Throughawarenesswecanbegintomakeachangewithineachofourselvesandworktowardsachievingacommongoalofprovidingdignified,respectfulandcompas-sionatecareforallpatients.

conducting Health education Classes (HeC) for residents.Shreveport/BossierRescueMission(SBRM)wasfoundedin1955toprovideshelterandagospelservicetomentravelingthroughShreveport,accordingtotheSBRMwebsite.Astheneedformoreservicesgrew,SBRMexpandedandnow,forover25years,has functionedasanarkof refuge formen,women,andfamilieswithchildrenwhoare inneedof food,shelter, and clothing. Pursuing the passion of JesusChristtoliftupthehungry,homeless,abused,andaddictedistheorganization’s mission and this mission is truly exemplifiedthrougheveryareaofserviceprovidedatSBRMincludingtheHeCtaughtandfacilitatedbydevotedmembersofSBNA. SBNA has consistently provided this service for four

D eSPITe NUMeROUS initiatives, federal funding, philan-thropic support, and research findings that emphasizebestpracticestoincreasediversityinnursing,thenursing
professioncontinuestostrugglewithgraduatingapopulationthatmirrorsthepluralisticsocietytheyserve.Specifically,thenursingprofessionhassoughttoincreasethepresenceofun-derrepresentedminorities (URM);AfricanAmerican,HispanicandNativeAmerican.TheseURMconstitutealmost30%oftheUnitedStates’population (UnitedStatesCensusBureau,2011)yettheBureauofHealthProfession’s(2010)2008surveyofregisterednursesrevealsthattheycompriselessthat10%of thenursingworkforce.Highattrition rates, learningneedsnotbeingmetandashortageofminorityfacultyarejustafewoftheobstaclesthatcontinuetochallengeeffortstoincreaseminorityrepresentationinnursing.It iswidely accepted that increasingethnic and racial di-
versity innursingwillhelpeliminatehealthcaredisparities.Adiverse workforce is likely to improve the cultural climate ofinstitutions and promote the delivery of culturally competentcare.Thisdoesnotsuggestthatonlyminoritiescancareforminorities. In contrast, it underscores the need for all healthcare professionals to increase their cultural competence sotheyarebetterpositionedtomeetthehealthcareneedsofadiversepopulation. That being said, theNational League ofNursing (2003)
calledforaneducationalreformthatemphasizestheneedforachangeinparadigms.Reformeffortsmustincludepracticesthatconsidertheculturalcompetenceofeducators (Nationaleducation Association, 2008; Beard, 2009) and stress thesignificanceoftheteaching/pedagogycomponentofnursingeducation(NationalLeagueforNursing,2003;Benner,2009;American Association of Colleges of Nurses, 2011). Nursinginstitutions should adopt andevaluate reformmeasures thatseek topromoteaclimate that is inclusive,meet theeduca-tional needs of all students and prepare students to deliverculturallycompetentcare. Multicultural education (Me) is a reform effort that has
beenusedtorestructureeducationalinstitutionsandimprovetheculturalclimateofschools.Ithasbeenusedinteacheredu-cationand researchsuggests that it improves theacademicoutcomesofallstudents(Zirkel,2008).Itassumesthateduca-torsacceptthatdiversityenrichesournationandprovidesuswithopportunitiestoviewperspectivesfromtheexperiencesof others. It increases self-awareness and strengthens theeducator’sabilitytocreatelearningenvironmentsthatmeettheeducationalneedsofdiverse learners.More importantly,Mehelpsincreasethegraduationofamorediversepopulation.
WithMe, twomajorparadigmsareconsidered;deficiencyand difference orientation. Sleeter andGrant (2009) reportedthatwithdeficiencyorientation,someeducatorssubscribetothebeliefthatURMaredeficient intherightskillsandknowledge.Thisapproach to teachingsupports lowexpectationsanddi-rectsattentionawayfromtheevaluationandadoptionofteach-ingstrategiesthatpromoteequityintheclassroom.Conversely,differenceorientationrecognizestheimportanceofhavinghighexpectations forallstudentsandmodifying instructionalprac-ticesinawaythatbuildsuponthestrengthofallstudents.Banks(2008)conceptualizedMetohelpfacilitateitsadop-
tion.HeidentifiedfivedimensionsofMewhichinclude;ContentIntegration, Knowledge Construction, Prejudice Reduction,empoweringSchoolCultureandSocialStructureandequityPedagogy. Together these dimensions cultivate an environ-mentthatstrengthenstheculturalcompetenceofbothteacherandlearner.Thefirstdimension,ContentIntegration,examinesthe extent to which educators use experiences and beliefsof different cultures to illustrate key concepts. KnowledgeConstruction looks at how knowledge is created and helpsfacultyandstudentsexaminetheirpersonalattitudestowardsdiversity.Conceptsareviewedfromtheperspectivesofothersandstudentsareremindedthatwhatisreadisonlyonewayof viewing things. The third dimension,PrejudiceReduction,focuses on eliminating destructive stereotypes and promot-ing cultural competence. With empowering School Cultureand Social Structure, institutions are challenged to uncoverinequitable practices and hiddenmessages related to diver-sitythataresilentlyportrayedtostudents.Thelastdimension,equityPedagogy,emphasizesandfacilitatesthemodificationofteachingmethodsinawaythatfacilitatestheacademicsuc-cessofallstudents.Inclosing,strengtheningtheethnicandracialmixofnurs-
inghingesupontheprofession’sabilitytoovercomenumerousbarrierstograduatingURM.TheInstituteofMedicine’sReport(2011), The Future of Nursing, Leading Change, AdvancingHealth,assertsthatnursingeducationneedsatransformation.However, itmustbeacknowledgedthatfacultyplayapivotalrole in this transformationand theirwillingness to implementrequisite changes that facilitate the graduation of a diversepopulation iscrucial.WhileMe isnotapanacea, itprovidesa working plan to help advance the educational system. Itempowersnurseeducatorstoteachinawaythatisculturallyresponsive,andenablesthemtograduateadiversegroupofstudentswhoarebetterpreparedtodeliverculturallycompe-tentcare.Itisthisauthor’shopethatnursingwilladoptMeandstudyitseffectivenessinincreasingdiversityinnursing.

the BeAtiNg BReAst cANceR pRogRAmThe Beating Breast Cancer Program (BBCP) [PI-Carol
Ferrans, PHD, RN; (NIH-NIA P30 AG022849; NIH-NCI P50CA106743; IMHHDP60MD003424]was initiated in2010asa4-yearacademic-communitycollaborativeventure.Thepur-pose is todevelopanddisseminateacommunity-wide inter-ventionforAAW,designedtoincreaseearlydetectionofbreastcancer, with two key components: a short film to addressmisconceptionsaboutbreastcancerandfears;andcommu-nitynavigationtofindandobtainano-costorlow-costmam-mogram.TheBBCPissupportedbydiverselocalcommunityorganizations including the American Cancer Society-Illinois(ACS)andtheNationalBlackLeadershipinitiativeonCancer.A core component of the BBCP intervention is a DvD,
developed for this study, that addresses cultural beliefs andfears thathavebeen identifiedassignificantbarriers toearlydetectionpractices.TheDvDincludesfiveAAWcancersurvi-vorsfromdifferentsocioeconomicgroupswhoaddressbeliefs,fears,copingstrategiesandthesignificanceofbreastcancerearly detection. The DvD received a National Telly award in2011andisendorsedbytheAmericanCancerSociety.The intervention is implemented by registered nurses
whoaremembersofCCNBNA,ChietaPhiNursingSororityandLambdaPiAlphaNursingSorority (CommunityPartner)whohavereceivedresearchethicsandBBCPtraining.Thesegroupshave longstandinghistoriesofhealthpromotionandsupport of patients and community residents and, thus, areverysuitabletoimplementtheBBCPintervention.eachinter-ventionsessionincludesviewingtheDvDfollowedbyadiscus-sionandcompletionofsurveystoassesschangesinattitudesand beliefs following the education session. Participants are
giventotebagswith(1)informationonbreastcancerearlyde-tection,low-costandno-costmammogramsandmammogra-phyandbreasthealthand(2)acopyoftheDvDtosharewithfamily, friendsandothers.The intervention is implemented inindividual,smallandlargegroupsettingswhereAAWgather.WomenwhodesirenavigationservicesarefollowedbyaUni-versitystaff.On a day to day basis, theUniversity Partner is respon-
sible for theoverallconductandadministrationof theBBCPincluding: trainingofnurseeducatorsandqualitymonitoring;identifying and selecting event venues; establishing effectivecommunicationswiththenurseeducatorsandtheCommunityPartnercoordinators;overseeingtheimplementationofsched-uledeventsandinitiatingandmaintainingrelationshipswithkeyorganizational representatives.Allactivitiesand functionsaredesignedtosupportBBCP’sgoaltoensureaneffectivebreastcancerprogramthatincreasesmammographyscreeningandenhancesthewellbeingofAfrican-Americanwomen.TheCommunityPartnerservesasthecommunityadvocate
andisresponsibleforimplementingtheBBCPintervention,as-sessing theprocessandoutcomesof educational sessions,communicatingfindings toCommunityCoordinatorsand theUniversityPartner.Communitycoordinatorsserveas liaisonsbetween nurse educators and the University partner by: (1)keepingopen linesofcommunicationwith theUniversity; (2)ensuring protocol adherence; (2) matching nurse educatorswithselectedsites;and (3)ensuringnurseeducators imple-ment the intervention and assess outcomes of interventionsessions.
sUmmARy The University-Community partnership is an appropriate
approach for promoting mammography screening amongAAW.Overthepast1.5years,245BBCPsessionshavebeenconductedwith4,750womeninattendance.Ninetynineper-centoftheAAWindicatedtheDvDwasvaluable;89%statedtheylearnedsomethingnewinthesessions;85%reportedtheDvDhelpedthemdecidetogetamammogram;and760haverequestedassistanceinfindingaffordablemammograms.Pre-liminaryfindingsindicateeducationaloneisnotenoughtopro-motemammographyscreening;16%ofthewomenrequestednavigationassistanceinlocatingmammographyandfinancialassistance.TheDvDisacorecomponentofthisinterventionandisavail-

incomeoruninsuredpatientswasthatevenifwedidmanagetodiagnosethemintime,theywouldnotcomebackfortreat-ment,” said Bryant, Director of the Hollings Cancer CenterOutreachServicesProgramattheMedicalUniversityofSouthCarolinainCharleston.Theprogramaddressescancerdispari-tiesamongmedicallyunderservedpopulationsinSouthCaro-lina’sLowCountryregion.“They were afraid of the cost and they didn’t trust the
system,”shesaid.“Soweneededtocreateasystempatientswould trust so they could get both diagnosed and treated.”Bryantexpandedoutreacheffortsandchangedthecommu-nicationapproach.Shealsowantedpatientstobeawareofclinicaltrialsand
othertreatmentoptions.Sheusedamobilescreeningvantolinktheregion’scommunityhealthcenterstoHollingsandtoprovidecareinareaswheretherewereveryfewornoproviders.Forherdeterminationtopreventandtreatcanceramong
themedicallyunderserved,Bryanthasbeennamedoneof10recipientsoftheRobertWoodJohnsonFoundationCommunityHealthLeadersAwardfor2012.Theawardhonorsexceptionalmenandwomenwhohaveovercomesignificantobstaclestotackle someof themost challenging health and health careproblems facing their communities. Bryant will receive theawardduringaceremonyinSanAntonioonOctober17.“Whenwefindsomethingabnormal,weimmediatelycon-
nectthepatientwitha‘laynavigator,’”saidBryant.Navigatorsmakesure thepatientmakesdiagnostic follow-upand treat-mentappointments.“Andifnot,”saidBryant,“weliterallydrivetotheirhouseandknockonthedoor.”Bryant’seffortshavepaidoff.Thenumberofmobile-unit
screeningsincreasedfrom1,300in2006toover2,000inboth2010and2011.Morethanhalfofthepatientsscreenedwereuninsured or underinsured, and nearly two-thirds of thoseservedsaytheywouldnothavebeenscreened ifnot forthemobile van.Theprogramhasdecreased thenumberofpa-tientslosttofollow-upfrom11percentin2009tolessthan5percentin2010.Janice Ford Griffin, national program director for Com-

The Ethical Challenge of “Doing What Is Right” in the Nursing Setting

THeeTHICALCHALLeNGeof“doingwhatisright”inthenursing setting is one that is encountered many timesthroughouttheworkdaybytoday’snurses.Obviously,conventionaltheoriesofethicsareveryrelevant
inthenursingsetting.Forexample,categorizingbehavior,andin the instance of nurses, professional nursing behavior, asbeingethicallyrightifitinvolvesacting“forthegreatestgood,”to“effectrightness”orfor“whatisjust”andethicallywrongifitdoesnot,isnotonlynecessary,but,inmyopinion,essentialtogoodnursing.Whathappensotherwiseinthenursingsettingwhendirect
ethical issuesofrightorwrongbecomeparamount?Indeed,thismaybeboth the greatest ethical andprofessional chal-lengefacedbynurses.Frommyperspective,asasemi-retired,yetstillpracticing
registerednurseofmanyyears,perhapsthethreemostimpor-tantethicalissuesfornursestoday,are:1.ethicalissuesarisingfromimproperpatientrequestssuch
asbeinggivennon-physicianprescribedpharmaceuticals;2. ethical issues arising from co-worker createdwhistle-
bloweropportunitiessuchasaco-workercommittinganactofpatientabuseinthepresenceofothernursesandrequestingthemnottoreportit;and3.ethicalissuesarisingfromimproperdirectionsbysuper-
visorsoradministratorssuchasasupervisororadministratordirectingtheendorsementortheprovisionofanon-prescribedtreatmentonapatient.Accordingly, the ethical challenge of “doingwhat’s right”
becomesallencompassingandallowsforbothethicalcircum-spectionwhile additionally providing an ongoing opportunityforethicalenhancement.Intermsofethicalnursingbehavior,itprovidesguidelinesforchallengescenteredontheprincipleethical ideas of patient respect, professional responsibilityandethical resultsandoutcomes.Asapplicable to theworkofnursesinthehealthcaresetting,thesethreeideasareveryusefulasguidelines.The idea of “patient respect” is reciprocal and helps to
establish andmaintain an atmosphere of consideration andappreciationforthebasichumanityofeveryperson.Itreflectsthe “Golden Rule” and serves as the basis formy personalcreed and professional approach to interacting and dealingwithothers, namely: “Treat everyone theway I expect tobetreated.”Thistranscendsthesocialclassorcircumstances.Inmydeliveryofnursingcare,ahomelesspersondiagnosedwithHIv/Aidswouldbecaredforinnolessarespectfulfashionthanacelebritywithaminordisorder.Myrespectforthebasichu-manityanddignityofallpeopleextendstoeachofmypatients.
The idea of “professionalresponsibility”isobligatoryandentails an accepted obligationonmy part to provide nursingcaretopatientsinadutifulwayasrequiredbymyprofession.Itfurthersaystome,ethically,thatIammybrother’skeeperforwhomIamresponsible.Itmakesmefeelethicallyrighttobearandfulfillthisresponsibility.Thisbehaviorisastrategylearnedwhichenablesnursestoprovidepatientsatisfaction,timelinessand professional excellence. I always endeavor to carefullyevaluateordersprescribedforpatients inmycare—that theywillbeeffectiveandmeaningfulandnotoutofrange.Itisbothmyethicalandprofessional responsibility tobeawareof thespecifichealthneedsofmyindividualpatients.Lastly, the ideaof “ethical results” takes intoaccountnot
onlythe“rightness”ofendresultsofactions(i.e.,therightordesiredend),butalsothecourseandnatureofactionstakenalongtheway...withthe“meanstoachievingends(being)justasimportantastheendsthemselves.”Asappliedtonursing,this isaveryworkablemaximaswellasaneffectivewayofmakingethicaldecisionsbaseduponenvisionedoranticipatedresults.Anexampleofthiswouldbetheimplementationofanethnic diversity program designed to foster greater levels ofrespectforthecommonhumanityofbothpatientsandcare-givers.Inthisscenario,themeanstoachievingendsarejustasimportantastheendsthemselves.In conclusion, it is apparent that the ethical challenge of
“doingwhat is right” is both endemic to the nursingprofes-sionandthenursinghealthcaresettingand,additionally,onethatisencountered,andsuccessfullyaddressed,manytimesthroughouttheworkdaybytoday’snurses.

torstoahospitalinnorthernHaititotryandformarelationshipwiththehospitalinordertobetterservetheclinicinthecom-munityofPillatre.There isnoemergencyMedicalTreatmentandActiveLaborAct,(eMTALA)inHaiti,no“wehavetotreatregardlessoftheabilitytopay,”asintheU.S.InHaiti,it’smoneyupfrontornocare.Ourgoalwastomakearelationshipwiththehospital,findoutthecostsofmostcare,operations,ad-missionsandhospitalizationsandthensendthosepatientsweseeintheclinictothehospitalwhentheneedpresenteditself.Onceagain,Isignedupandbecamepartofateamof12goingtoCRUDUMHospitalinMilot,Haiti,inOctoberof2011.Unlikeworkingattheclinic,wehadnoideawhattoexpect.Weweregoingtoahospitalthatnooneontheteamhadeverbeento.Wehadnoideawhowasgoingtopickusupattheairport,noideawherewewouldbeworkingwhenwegot there,allweknewwasthatweweretheretoformarelationship.My friend and I were assigned to work in the intensive
careunit(ICU).Onourtourofthehospital,onourfirstday,welookedintotheICUtosee5or6patientsallappearingquiteill.Oneyoungladywhowas15yearsoldgrabbedmyheart.She
wascoveredfromheadtotoewithblistersinvariousstages,someopenandweeping,somescabbedover,herlipsswollengrotesquely,andhereyesalmostswollenshut.Wewerenotscheduled to beginworking untilMondaymorning (thiswaslateonSaturday)andIfeltsurethatthisyoungpatientwouldnotlivelongenoughformetocareforher.However,Mondaymorningcameandshewasstillthere.IlearnedthattheHaitianstaffwasunawareofwhat theyounggirlwassuffering fromandwastreatingherconditionwithintravenousantibioticsandlittleothercare.WethoughtthistobeStephenJohnsonSyn-drome.MyfriendandIwenttoworkbathingandattemptingtodebridetheyounggirl’swounds,Ihadneverseenapatientsocoveredinsores,andtherewasnotonespotonherbodythathadbeenspared.

Throughaninterpreteravailabletous,wetoldherthatwewere going to scrub herwounds in attempt to help her getbetter.Iattemptedtogethersomepainmedicationpriortousstartingthisprocess.IaskedandwastoldnobythenurseinchargeoftheICU.Therewasaverylimitedsupplyofmedica-tionanditwassavedforthosewhoreallyneededit,andfamilywhocouldaffordtopayforit.Ipersisted,pushingforpaincon-trolformypatient.IfinallygotadoctortoallowmetogivehertramadolIv—therewasonlyonevileinthehospital.Thefirstdaytheprocesstookabout45minutes.Thewhole
timethisyoungladygrimacedinpainbutnevercriedout,nevercomplained,andeventhankeduswhenwefinished.eachdaywecametoworkintheICUweweregreetedbyasmilefromtheyounglady.eachdayweproceededtoscrubherwounds,treatingthemmuchlikeyouwouldsevereburns,debridingthedeadtissueinhopesthathealingwouldtakeplace,allthetimeinteractingwiththeHaitiannurseshelpingtoteachthemwhatweweredoingandwhy.TheHaitiangirlcontinuedtoamazeuswithherrecoveryandheramazingattitude.Weaskedifwecouldpraywithher,shelovedthatandstartedtosing“Amaz-ingGrace”inCreole.Isangrightalongwithher,tearsrunningdownmy face, as I continued towork on herwounds. Fivedaysafterwehadfirststartedtocarefortheyounggirl,shewasalmostfullyrecovered!ShewastobedischargedonSat-urday,thedaywewereleaving.Shehuggedandthankedusforsavingherlife.Whataprivilegetocareforsuchandinspiringyounglady!OurlastnightatthehospitalinMilot,wehadaHaitianparty
atthemedicalresidencecompoundwherewehadslepteachnight—all themedicalstaff for thehospital liveson thecom-pound.WehadagreattimewiththeHaitianmedicalstaff,andwereabletositdownandtalkwiththedirectorofnursingfromthehospital.Shetoldusthatwewereoneofthebestteamstheyhadhadat thehospital.She loved thewayweworkedwiththeHaitianstaff,nevertakingover,butworkingalongsideoftheirnursingstaff,respectingthecultureandteachingthem
aswecaredforpatients.Sheencouragedustopleasereturnsoontocontinueteachingherstaff.Whatahugecomplementtoourteam!IwasabletojoinanotherteamthispastJuneof2012thatreturnedtoCRUDUMHospitaltocontinuethemean-ingfulrelationshipofcareandcompassion.IamsohumbledandhonoredtohelptheHaitianpeople
throughtheuseofmymedicalskills.Iamalsogratefulforthoseplaced inmypath thathavehelpedme to fulfill this life-longdreamandpassion.AsamemberofmylocalchapterIcon-tinuetoadvancemyeducation,nowpursuingmyMasterstobecomeaFamilyNursePractitioner.Mydesireistorunaclinicina ruralandunder-privilegedcommunityhere in theUnitedStatesatthelocallevelteachingdiseasepreventivehealthcarefocusing on diabetes and childhood obesity and promotinghealthylifestyleliving,andtoreturntoHaitiasanursepractitio-ner,teachingwithculturalsensitivityanddiversity,tailoringcaretotheirspecificneeds.

encouragewomen togetearlybreastcancerscreeningandtodecreasetheincidenceofthenumberofAfricanAmericanwomen dying from breast cancer deaths, the BBNABreastCancerInitiativecommitteewasformed.Themembersofthecommittee are Carthenia Jefferson, rN, esq., ProgramDirector;martha Dawson, DNP, rN, fACHeandtammy Davis, rN.TheBirminghamBNABreastCancerInitiative(BBNABCI)

WHeReAS LeADING health and nutrition experts agreethatnutritionstatusisadirectmeasureofpatienthealth.Under-nutrition and over-nutrition frequently contribute
to poor health outcomes, increased health disparities, risinghealthcarecostsandultimatelyundermine thepromotionofhealthandwellness.Whereas in theU.S., inadequateorunbalancednutrition-
alsoknownasmalnutrition—isoftennotviewedasa routinemedical concernand isoftennot recognizedor treated inaclinicalsetting.Under-nutrition isparticularlyprevalent incer-tainU.S.populations,includingminoritypopulations,hospital-izedpatients,andolderadults.Asmanyashalfofhospitalizedpatients and 35-85% of older long-term care residents areconsideredunder-nourished.Whereaspoornutritioncanresultinlossofleanbodymass,
leadingtocomplicationsthatimpactpatienthealthoutcomesincluding reduced recovery from surgery, illness, or chronicdisease, increased susceptibility to illness and infection, im-pairedwoundhealing,increasedriskforfallsandfracturesandincreasedmortality.Theseconsequences result in increaseduseofhealthcareresources.Whereas illnessand injurycansignificantlyspeedup the
lossofleanbodymass.Inaddition,theelderlyloseleanbodymassmorequicklyandtoagreaterextentthanyoungeradults.Traditionalweightassessmentsmayoverlookleanbodymass,asbodyweightandbodymassindexarenotaccurateindica-torsofleanbodymass.Whereas access to therapeutic nutrition—defined as the
use of specific nutrients, disease-specific nutrition products,and complete and balanced oral nutrition supplements tohelpmanage a health problem—is critical to helping restoreleanbodymass, resolvemalnutritionand thus improveclini-caloutcomes,reducehealthcarecosts,andkeeppeopleandcommunitieshealthy.Whereasdespitetherecognizedlinkbetweengoodnutri-
tionandgoodhealth,routinemedicaltreatmentandhealthcarecoverageintheU.S.havenotaddressedadequatetherapeuticnutrition in the hospital, faith-based, or community settings.Thismustchange if thegoalofhealthcare reform is tokeeppeoplehealthyandoutofinstitutionalizedcare.Therefore be it resolved thatmembers ofNational Black
Nurses’ Association support the need for direct, culturallycompetent nursing involvement in routine nutrition screeninganddeliveryofadequatetherapeuticnutritionacrossthespec-trumofhealthcare.
n TheNBNArecognizesthatroutinenutritionscreeningandtherapeuticnutritionmustbe incorporatedaspartof thestandard forevidence-basedcareacross thehealthcarecontinuum,sothatpatientswhoaremalnourishedaswellasthoseatriskformalnutritioncanbequicklyidentifiedandprovidedappropriate therapeuticnutrition in thehospital,thushelpingreducecomplicationsandtheneedfor inva-siveandexpensivemedicaltreatments.
n The NBNA recognizes the need for expandedMedicarecoveragetoincludetherapeuticnutritionthatmayberec-ommendedbyhealthcareprofessionals tohelpMedicarebeneficiaries improve their disease management andhealthoutcomes.
n TheNBNAisencouragedthatcertainstatesprovideMed-icaid coverage for therapeutic nutrition oral supplementstohelpMedicaidenrolleesincreasebloodglucosecontrol,withstanddialysis treatment, andmeetotherdefinednu-tritionalgoals for improvedchronicdiseasemanagementincludingdiabetes,renaldisease,cancer,andobesityandtheNBNAbelievesallstateMedicaidprogramsshouldpro-videsuchcoverage.
n TheNBNAisencouragedthatthere isanopportunity fornutritionscreeningandcounselingforobesityandchronicdiseaseaspartof thehealthcarereformessentialbenefitforpreventiveandwellnessservicesandchronicdisease.However,thereisanelementmissingfromthesepreventiveservices—prescribed nutrition therapy. Therapeutic nutri-tionmustbeincludedaspartoftheessentialhealthben-efit inoutpatientandcommunitycaresettings, ifnutritionscreeningandcounselingaretobeeffective in impactingpublichealth.
n TheNBNAalsosupportsincreasedemphasisonnutritionforhealthyagingthroughnewprovisionsintheOlderAmer-icansAct, includingpromoting routinenutritionscreeningforOAAmeal programparticipants and addressing theiruniquenutritionalneedswithnutritioninterventionsthatareculturallyandage-sensitiveandthatallowstatesmoreflex-ibilityinprovidingtherapeuticnutritionoralsupplementsinadditionto,notjustasareplacementfor,regularmeals.

2-days, workshop continues on Thursday, August 1, 8:00 am - 5:00 pm. Pre-registration is required and you must attend both sessions.
The ELNEC-Geriatric curriculum was developed to address the needs of geriatric patients facing life-threatening illnesses as well as meet the special needs of patients and families facing the end of life across various geriatric settings. The curriculum contains nine modules addressing critical aspects including: Principles of Palliative Care; Non-pain Symptoms at the End of Life; and, Goals of Care and Ethical Issues at the End of Life.
The ELNEC-Geriatric curriculum also includes several common threads integrated throughout, including:
• Thefamilyastheunitofcare.
• Criticalfinancialissuesthatinfluenceend-of-lifecare.
• Theimportanceofcultureasaninfluenceattheendoflife.
• Thecriticalneedforattentiontospecialpopulationssuchaschildren, the elderly and the uninsured.

The Mental Health First Aid program will provide nurses with knowledge to equip community residents to help persons with mental illness connect to care. This two day seminar will introduce nurse participants to risk factors and warning signs of mental health problems, provide an understanding of the impact of mental health problems on the community and provide an overview of common treatments.
