Statement of Need: Cardiovascular disease remains the leading cause of death worldwide and recognizing patients at risk is the key to early interventions and decreased morbidity and mortality. This symposium is designed to provide healthcare profes-sionals with evidence-based diagnostic and management skills to improve healthcare outcomes and quality of life for patients with cardiovascular and metabolic risk factors. Target Audience: cardiologists, internists, family practitioners, endocrinologists, cardiac surgeons, electrophysiologists, emer-gency physicians, nurses and other healthcare professionals Educational Objectives: Upon completion of this program participants should be able to: · Identify patients at risk for cardiovascular disease · Be aware of cultural disparities associated with cardiovascular disease risk, including ethnicity, age, gender & smoking · Select appropriate diagnostic tools and therapeutic options · Improve patient care outcomes and quality of life · Describe ways to prevent cardiovascular disease

QUESTION: HOW SHOULD I DISPOSE OF ITEMS CONTAINING PROTECTED HEALTH INFORMATION (PHI)? AANNSSWWEERR:: All items containing PHI MUST be placed in the document destruction bins (shredding bins). Items include, but not limited to, are:

Patient Labels (IV Labels) Forms Reports / Monitor Strips Any item (paper, post-its, etc) that

contains Patient Information (PHI)

Clinical notes Physician orders Computer screen prints Patient wrist bands Yellow sticky notes with PHI Photographs with PHI

Disposal of IV Bags containing labels with protected health information is as follows: For IV Bags containing medications:

IV Bags must be placed in the blue pharmaceutical bins (which are located in controlled areas on the units).

IV labels do not have to be removed or blacked out prior to disposal. For IV Bags containing NO medications:

IV labels must be removed or blacked out with a permanent marker IV bags must be drained prior to placing in the trash
