Junior/Senior Prom Student Behavior Contract “City of Stars”, Mountain Gate Country Club Friday, April 28, 2017 - 7:00pm-11:00pm This contract must be completed and turned in to Mr. Kennedy in Room 36 by 3:15pm on Wednesday, April 26, 2017. No exceptions will be made. Faxed copies will not be accepted. The price for Prom is $85 presale until April 1 st then $95 from April 2 nd – April 26 th . Payment is non-refundable. Proof of payment must be attached to this contract either in the form of a receipt, stamp, or signature from the finance office. All school fees including the graduation fee, parent volunteer hours, etc. need to be paid by April 26 th in order to attend Prom. Please give yourself enough time to complete all necessary requirements stated above prior to submission of behavior contract. Students without a signed contract will not be allowed into the dance – NO EXCEPTIONS. DANCE RULES: 1. The dance hours are 7:00pm-11:00pm. The deadline for arrival by SMCHS students and their guests is 7:30 pm. All students will be allowed to leave at 11:00pm. Students must be picked up by 11:15 pm. 2. All students must show a valid 2016-2017 SMCHS photo ID and purchased admission before entering the dance. Any student without proper ID and contract will not be allowed to enter the dance. All guests of SMCHS students must show a valid student ID or contact info and signature of the dean or principal of the school which they attend. 3. In our continued efforts to ensure a safe environment for our students and their guests, no alcoholic beverages or illegal substances of any kind will be permitted. If a student’s or guest’s behavior gives reasonable suspicion that he/she may be under the influence of alcohol, they will be required to submit to a breathalyzer test. In the event of a failing test or refusal to cooperate with school administrators, parents will be notified and must pick up their son/daughter and/or their guest at the Prom venue. Being in possession of or under the influence of alcohol or drugs is grounds for disciplinary withdrawal from school and possible arrest. Students and guests may not smoke, regardless of age. All those attending the dance are subject to search upon arrival including, but not limited to, Interquest search dogs. Any vehicle that transports students to the dance, including a limo service, is subject to search by the Interquest search dogs and/or search by faculty and administration of SMCHS according to the policies found in pages 26- 28 of the SMCHS Parent-Student handbook. 4. Digital cameras, camera phones and non-digital cameras are permitted at SMCHS dances. However, neither SMCHS students nor their guests may take pictures without the expressed consent of those being photographed, nor may pictures be posted online or distributed without the consent of those in the photograph. Content of photographs must not contradict SMCHS policies or values. If a student or their guest is in violation of this policy, the SMCHS student will be given SaturdaySchool and may face serious disciplinary action. 5. Chaperones attend the dance for the protection and safety of the students and therefore MUST be shown proper respect. Students and guests are to follow the directions of chaperones and appropriate event staff. 6. This event is semi-formal. Students or guests wearing inappropriate, revealing or provocative attire will not be admitted. Attire must conform to these guidelines and the guidelines on Page 22 of the Parent-Student Handbook 2016-2017. 7. Chaperones are instructed to intervene when they witness inappropriate and/or provocative dancing or body contact. Students who do not comply with chaperons' reprimands will not be allowed to dance and may be subject to further disciplinary action. 8. Pre- and post-dance events are NOT school sponsored and will NOT be chaperoned or attended by chaperones. 9. Students on Strict Probation or students placed on Strict Probation and whose term extends through the date of the dance will not be able to attend. 10. Former students with poor disciplinary history or who have been asked to withdraw from SMCHS due to disciplinary reasons will not be approved as Prom Dance guests. 11. SMCHS students are responsible for their guests’ behavior at Prom and are to inform their guest of these regulations. Students are only allowed to bring 1 guest. Guest payment must be made when submitting contract. 12. Guests must be at least 13 years old and in high school and may not be older than 20 years old. All guests will be asked to provide picture identification (high school ID or valid state issued license) verifying age and identity.
