This directory provides a list of UW Health Senior Preferred’s network providers. This directory is for UW Health Senior Preferred’s service area in Dane County. This directory is current as of June 30, 2016. Some network providers may have been added or removed from our network after this directory was printed. We do not guarantee that each provider is still accepting new members. To get the most up-to-date information about UW Health Senior Preferred’s network providers in your area, you can visit www.seniorpreferred.org or call our Customer Service at (608) 775-8077 or (800) 394-5566. For people who are deaf, hard of hearing or speech impaired please call TTY/TDD 711 or (800) 877-8973, which is free of charge, or you may call through a video relay service company of your choice. Interpreter services are provided free of charge to you. A customer service representative is available to assist you Monday through Friday from 8:00 a.m. to 8:00 p.m. From October 1 through February 14, we are also available to assist you on Saturdays and Sundays from 8:00 a.m. to 8:00 p.m. Other Pharmacies/Physicians/Providers are Available in our Network.

Section 1 – Introduction This directory provides a list of UW Health Senior Preferred’s network providers. To get detailed information about your health care coverage, please see your Evidence of Coverage. The fact that a provider, clinic or hospital is listed in this directory does not mean that the services provided by them are covered under UW Health Senior Preferred. Network Providers The “network providers” listed in this directory have agreed to provide you with your health care services. You may go to any of our network providers listed in this directory; however some services may require a referral. If you have been going to one network provider, you are not required to continue going to that same provider. In some cases, you may get covered services from non-network providers. While you are a member of our plan, it is important to know that you must use network providers for your medical care and services, with a few exceptions. The exceptions are urgent and emergent services when you are out of the network (generally, when traveling out of the area), renal/kidney dialysis, and cases in which UW Health Senior Preferred has authorized use of out-of-network providers. See Chapter 3 of your Evidence of Coverage for more specific information about emergency, out-of-network, and out-of-area coverage. Non-emergency services by non-network providers will require a referral authorization from your network provider. Referral authorization must be approved before services can be rendered. If you should receive a bill for the full cost of your covered services from a non-network provider, submit it to UW Health Senior Preferred for processing and determination of your liability, if any. Please note that although services are provided through the UW Health Senior Preferred network delivery system, the continued participation of a doctor, hospital or facility cannot be guaranteed. We recommend that you confirm the participation of the provider before scheduling an appointment. What is covered if you have a medical emergency? You may receive covered emergency medical care whenever you need it worldwide. Our plan covers ambulance services in situations where getting to the emergency room in any other way could endanger your health.

If you are seen in the Emergency Room and are admitted, we will communicate with that provider to manage and follow up on your care.

If your emergency care is provided by a network provider, you are entitled to follow-up care to be sure your condition continues to be stable. If your emergency care is provided by an out-of network provider, you are entitled to one follow-up visit. We will then try to arrange for network providers to take over your care as soon as your medical condition and circumstances allow. For more information, see the Medical Benefits Chart in Chapter 4 of your Evidence of Coverage. What is the service area for UW Health Senior Preferred? UW Health Senior Preferred service area consists of Dane County. Please refer to the map on the following page. How do you find UW Health Senior Preferred providers in your area? Network providers are listed in this directory by State, County, City, and Provider Type. The providers are then listed in alphabetical order by facility. The UW Health Senior Preferred network may change at any time. You will receive notice when necessary. An updated directory is located on our website at www.seniorpreferred.org. You may also call Customer Service for updated provider information. Nurse Advisor Program UW Health Senior Preferred provides a Nurse Advisor Program, which you may call to receive medical advice from a registered nurse, 24 hours a day, 365 days a year. Introduction

This booklet provides a list of UW Health Senior Preferred’s network pharmacies. To get a complete description of your prescription coverage, including how to fill your prescriptions, please review the Evidence of Coverage and UW Health Senior Preferred’s formulary.

We call the pharmacies on this list our “network pharmacies” because we have made arrangements with them to provide prescription drugs to Plan members. In most cases, your prescriptions are covered under Senior Preferred only if they are filled at a network pharmacy. Once you go to one pharmacy, you are not required to continue going to the same pharmacy to fill your prescription but can switch to any other of our network pharmacies. We will fill prescriptions at non-network pharmacies under certain circumstances as described in your Evidence of Coverage.

You can get prescription drugs shipped to your home through our network mail order delivery program. For more information, please contact us or see the mail order section of this pharmacy directory.

All network pharmacies may not be listed in this directory. Pharmacies may have been added or removed from the list after this directory was printed. This means the pharmacies listed here may no longer be in our network, or there may be newer pharmacies in our network that are not listed. This directory is current as of June 30, 2016, for distribution on January 1, 2017. For the most current list, please contact us. Our contact information appears on the front and back cover pages.

How do I fill a prescription at a network pharmacy? To fill your prescription at a network pharmacy, you must show your Member ID card. If you do not have your ID card with you when you fill your prescription, you may have to pay the full cost of the prescription (rather than paying just your co-payment). If this happens, you can ask us to reimburse you for our share of the cost by submitting a claim to us. To find out how to submit a claim, call our Customer Service. Can I receive a 90-day supply of “maintenance” drugs? The plan offers a way to get a long-term supply of “maintenance” drugs on our plan’s Drug List. (Maintenance drugs are drugs that you take on a regular basis, for a chronic or long-term medical condition.) Some retail pharmacies in our network allow you to get a long-term supply of maintenance drugs. Some of these retail pharmacies may agree to accept a lower cost-sharing amount for a long-term supply of maintenance drugs. Other retail pharmacies may not agree to

accept the lower cost-sharing amounts for a long-term supply of maintenance drugs. In this case you will be responsible for the difference in price. Your Provider Directory tells you which pharmacies in our network can give you a long-term supply of maintenance drugs. Pharmacies offering a 90-day supply of maintenance drugs are identified by a 90 symbol following the pharmacy’s name. You can also call Customer Service for more information (phone numbers are printed on the back cover of this booklet). Filling prescriptions at a non-network pharmacy Generally, we cover drugs filled at an out-of-network pharmacy only when you are not able to use a network pharmacy. The following are a few exceptions when we will pay for a prescription filled at a pharmacy outside of our network.

Getting coverage when you travel or are away from the plan’s service area If you take a prescription drug on a regular basis and you are going on a trip, be sure to check your supply of the drug before you leave. When possible, take along all the medication you will need. To help accommodate this, UW Health Senior Preferred does allow long term supply of maintenance drugs in which you are allowed up to a 90-day supply. Pharmacies offering 90 day contracted rates are identified by a 90 symbol following the pharmacy’s name. If you are traveling within the United States and territories and run out of your prescription drugs, you can call Customer Service to help locate a network pharmacy in the area. We cannot pay for any prescriptions that are filled by pharmacies outside of the United States and territories, even for a medical emergency or urgent care. What if I need a prescription because of a medical emergency or because I need urgent care? We will cover prescriptions that are filled at an out-of-network pharmacy if the prescriptions are related to care for a medical emergency or urgent care. In this situation, you will have to pay the full cost (rather than paying just your co-payment) when you fill your prescription. You can ask us to reimburse you for our share of the cost, (See your Evidence of Coverage, Chapter 7 for details in how to ask the plan to pay you back.) Remember, we cannot pay for any prescriptions that are filled by pharmacies outside of the United States and territories, even for a medical emergency or urgent care. Other times you can get your prescription covered if you go to an out-of-network pharmacy We will cover your prescription at an out-of-network pharmacy if you are getting a vaccine which is not covered by Medicare Part B and is administered in your doctor’s office. In this situation, you will have to pay the full cost (rather than paying just your co-payment) when you fill your prescription. You can ask us to reimburse you for our share of the cost less your normal copayment for the vaccine (including administration) less any difference between the amounts the doctor charges and what we normally pay. (See your Evidence of Coverage, Chapter 7 for details in how to ask the plan to pay you back.) Before you fill your prescription in any of these situations, call Customer Service to see if there is a network pharmacy in your area where you can fill your prescription. If you do go to an out of network pharmacy for the reasons listed above, you will have to pay the full cost (rather than paying just your co-payment) when you fill your prescription. You can ask us to

reimburse you for our share of the cost (See your Evidence of Coverage, Chapter 7 for details in how to ask the plan to pay you back.) How do I request reimbursement? When you go to a network pharmacy your claim is automatically submitted to us by the pharmacy. However, if you go to an out-of-network pharmacy because of the reasons listed above, the pharmacy may not be able to submit the claim directly to us and you will have to pay the full cost of your prescription. When you return home, simply submit the pharmacy print out including the name of the drug, dosage and quantity along with your receipt to the following address: Senior Preferred, Attn: Pharmacy Department, 1900 South Avenue, Mail Stop: NCA2-01, La Crosse, WI 54601. (See your Evidence of Coverage, Chapter 7 for details on how to ask the plan to pay you back.) Upon receipt, we will make an initial coverage determination on the claim.
