Provision of Social Case Study Report (SCSR)

BRIEF DESCRIPTION A comprehensive study report given to individual/family of the patient who wishes to avail services from government institutions and other helping agencies for medical/financial support. A SCSR contains confidential data like his/her personal information, socio-economic background, etc.

Issuance of Referral and Endorsement Letter

BRIEF DESCRIPTION For Referral Agencies like the Department of Health and National Mental Hospital and other government facilities are looking for a referral from the government unit so as to determine if the patient/client is indigent and has no means to support his/her medical needs. For Endorsement DSWD Region IV-A and Securities and Exchange Commission require the endorsement letter from the LGU-CSWD to check if the organization or group really exist and if its purpose and objective is attainable or not.

Provision of Assistance to Children in Conflict with the Law

BRIEF DESCRIPTION Provision of temporary shelter to male CICL as a venue for his security, rehabilitation and preparation in his reintegration to his family and community. Provision of temporary residential care to male Children in Conflict with the Law while the case being filed against him is going. Provision of protection, care, training and rehabilitation of CICL in a home-like environment for a maximum of six months or more based on the court order with an end view of reintegrating them to their families and communities. Facilitate the immediate assessment of their physical, psychological, medical, economic and para-legal needs and determine their best intervention program in preparation for their aftercare program.

Issuance of Solo Parent ID

BRIEF DESCRIPTION The Solo Parent ID is issued to any parent individual left solo or alone with the responsibility of the children due to death, criminal conviction, disability, legal separation, separation de facto at least one year, annulment, abandonment and unmarried parent who chose to keep their children with them and to any individual or family member who solely provides parental care and supports the child as a result of death, abandonment, disappearance and prolonged absence of the parent.
