With your help, last year, Hale Makua Health Services made strides in: Increasing the capacity of our home health and rehab programs Developing our local workforce Providing Hale Makua nursing home residents with physicians that can provide better continuity of care Planning for the future needs of Maui residents In 2018, Home Health by Hale Makua, which provides skilled nursing and therapy services to home-bound individuals, cared for 595 individuals in the comfort of their own homes. An increase of 27% over the number of individuals served by Home Health in 2017. Rehab by Hale Makua, which provides in-patient and out-patient occupational, physical and speech therapies at Hale Makua Kahului and Wailuku, helped 509 individuals recover from surgery, injury or illness in 2018. With the goal of helping individuals safely return back home, Rehab helped 48% more individuals than in the prior year. Further developing Maui’s local nursing workforce, aiming to eliminate the need for traveler nurses, we built upon the Nurse Aide Training Program that was started in 2017. We created a Nurse Aide Apprenticeship program to provide on the job training for newly hired Nurse Aides, and partnered with the University of Hawaii Maui College to provide education and clinical training for those individuals who may not want or be able to complete the schooling needed to become a Registered Nurse (RN) by developing a Licensed Practical Nurse (LPN) Pathway. With your help, last year, Hale Makua Health Services made strides in:

• Increasing the capacity of our home health and rehab programs

• Developing our local workforce

• Providing Hale Makua nursing home residents with physicians that can provide better continuity of care

• Planning for the future needs of Maui residents

In 2018, Home Health by Hale Makua, which provides skilled nursing and therapy services to home-bound individuals, cared for 595 individuals in the comfort of their own homes. An increase of 27% over the number of individuals served by Home Health in 2017.

Rehab by Hale Makua, which provides in-patient and out-patient occupational, physical and speech therapies at Hale Makua Kahului and Wailuku, helped 509 individuals recover from surgery, injury or illness in 2018. With the goal of helping individuals safely return back home, Rehab helped 48% more individuals than in the prior year.

Further developing Maui’s local nursing workforce, aiming to eliminate the need for traveler nurses, we built upon the Nurse Aide Training Program that was started in 2017. We created a Nurse Aide Apprenticeship program to provide on the

job training for newly hired Nurse Aides, and partnered with the University of Hawaii Maui College to provide education and clinical training for those individuals who may not want or be able to complete the schooling needed to become a Registered Nurse (RN) by developing a Licensed Practical Nurse (LPN) Pathway.

Recently graduated LPNs who are employed by Hale Makua will automatically be enrolled in our LPN Residency program to help in the transition of putting learned skills into practice. Most recently, we added a Nurse Residency program to help prepare new graduates and less experienced RNs to become independent practicing nurses.

A partnership was also established with Maui Medical Group to create a physician group that provides an admitting and attending physician for Hale Makua nursing home residents who do not have one, with the goal of eliminating the need to contract every six months with a new locum physician from the mainland. These local physicians provide better continuity of care for residents.

Grants totaling$215,000 from the County of Maui, plus unrestricted gifts from individual donors, helped provide doctors for Hale Makua nursing home residents who don’t have one.

Over the course of a year, 302 individuals were admitted from the community or from Maui Memorial Medical Center to Hale Makua by the doctors, and 874 individuals were attended to at Hale Makua by the physician. A total of 1,481 physician visits were provided.

Home Health by Hale Makua celebrated

50 years of providing skilled nursing and therapy care in the comfort of homes of Maui residents.

Worked with County of Maui Mayor, County Council and officials on a Resolution to lease 4.57 acres of the Kahului Community Center, adjacent to Hale Makua Kahului, to create a new state-of-the-art rehabilitation and senior living facilities, health care services, and senior oriented recreational opportunities.

Providing Physicians for Residents Who Don’t Have One

Shelley Cisneros, LPN, is a recent LPN Pathway graduate who was hired by Hale Makua. She’s pictured here taking the vitals of Hale Makua resident Phillip Gragas.

Over $30,000 in grants and donations from the HMSA Foundation, the State of Hawaii, The Hilton and Maria Acacio Unemori Fund, and the Walter & Cathy Scott Memorial Fund helped develop our nursing workforce in order to employ locally grown talent. In 2018, Hale Makua needed to contract up to 27 nurses who came from mainland agencies to cover our Licensed Practical Nurse (LPN)and Registered Nurse (RN) needs.

By graduating 10 LPN Pathway students in December 2018, of which 9 were offered employment and will participate in our LPN Residency program, and launching our own RN Residency program, the number of travel nurses contracted by the organization has been reduced to nine as of March 31, 2019.

Growing Our Local Nursing Workforce

In 2018, volunteers transported residents to their rooms for exercise approximately 4,335 times, served over 13,800 beverages during social hour, and led 240 games of bingo.

Last year, 90 volunteers contributed 4,611 hours assisting with fundraising events, clerical work, and spending time with residents talking story, playing games and going on outings.

Mahalo Donors from January 1 - December 31, 2018

The Roy H. & Lorraine M. Okumura Foundation donated $12,000 for food services capital or operational needs. The funds were used to purchase six hydration stations, one for each neighborhood on the Kahului campus, with the goal of ensuring that residents stay hydrated, thereby reducing the occurrence of urinary tract infections.

Each hydration station is equipped with a five gallon insulated cooler and 50 quart insulated ice chest, large enough so it does not require refilling throughout the day. These hydration stations are essential in providing drinking water to all residents in Kahului campus.

The May Templeton Hopper Fund of the Hawaii Community Foundation and the Harry and Jeanette Weinberg Foundation donated a combined $39,800 to provide financial assistance to 15 Adult Day Health by Hale Makua clients.

These scholarships provided an additional 500 client days. 100% of tuition recipients reviewed remained stable or improved in nutrition and social engagement, and 86% of scholarship recipients reviewed remained stable or improved in fall prevention and functional mobility (one client fell at home and was not seriously injured).

A $400,000 grant from the State of Hawaii for provided care and services for 76 individuals whose insurance did not cover the full cost of care or who have no insurance.

A total of 765 days of skilled services were provided. If these residents stayed the same amount of time in the hospital, the cost avoidance for the hospital would be about $1.1475 million
