haul efforts at both the recipient site and the four potential donor sites at Port Canaveral. Restoration of vegetative cover in all disturbed areas at both the recipient site and the four (4) potential donor site(s) is a requirement.

2. CIF-002: Since the contract duration is 2 years and it is a probability that the second phase of the material transport won't be contracted right after the completion of phase 1, are we responsible for the maintenance and monitoring of the BMP's until both contracts are complete? RESPONSE: Contractor is responsible for complying with all conditions of the NPDES Permit for the full length of the contract.

3. CIF-003: If any vegetation is present on the existing stockpile at the cape do we need to separate the vegetation or can it all be loaded at once? RESPONSE: Organics will need to be separated and disposed over by the contractor at the SR-524 ILC recipient site and at the four (4) Port Canaveral donor site(s).

4. CIF-004: What is the engineers value of this project? RESPONSE: No engineer’s estimate has been performed.

5. CIF-005: Is a bid bond/P&P bond required? RESPONSE: Yes, see attached standard general conditions

6. CIF-006: Will liquidated damages be assessed if phase 1 is not complete within the 5 month time frame? If so what are the LD's per day? RESPONSE: The contract does not include a liquidated damages clause. However, the Bidder’s schedule required at the time of proposal submission should be realistic and based on the required number of trucks and turn around trips to complete as stated. The Bidder’s Schedule, Methodology and Equipment represents 30/100 points in the evaluation.

7. CIF-007: Are there any qualification requirements for the 3rd party company/personnel? RESPONSE:

8. CIF-008: Do we need to supply a site specific health & safety plan? If so, one at each location? RESPONSE: Yes

9. CIF-009: The SWPPP that was supplied with the bid specifications was not signed, will you be supplying a signed version for permitting? RESPONSE: The SWPPP plan is provided for use by the site contractor. The successful bidder will be required to apply for and obtain the NPDES permit.

10. CIF-010: Do we need to utilize Davis Bacon Wages? RESPONSE: No.

11. CIF-011: What do you want to do with the spoils from the swale that gets excavated? On the stockpile or dispose of off-site? RESPONSE: Muck and other unsuitable material (including organics) will need to be hauled offsite.

12. CIF-012: What line item do you want the cost for the dewatering and temporary impoundment basin to go, if needed? RESPONSE: Line Item 5: SWPPP Erosion Control Setup, Maintenance and Monitoring

13. CIF-013: What line item do you want the clearing & grubbing cost?

RESPONSE: Line Item 5: SWPPP Erosion Control Setup, Maintenance and Monitoring

14. CIF-014: Do we need to include clearing & grubbing at the Port location? RESPONSE: Yes at all four (4) potential sites.

15. CIF-015: On drawing C201 it states to protect/avoid foraging areas. Do you want silt fence around those (2) areas? RESPONSE: Silt fence is required around the stockpile. Lath with colored flagging placed on maximum 50’ spacing shall be used to mark the foraging areas.

16. CIF-016: Do we need to include a concrete apron & aggregate at the Port location? RESPONSE: A sediment tracking prevention area will be required at the four (4) donor sites at Port Canaveral. The Contractor will be required to maintain clean roadways in both locations and maintain good housekeeping practices throughout the contract.

17. CIF-017: Will the access road and concrete apron be left in-place upon completion of the project ? RESPONSE: Improvement in the FDOT SR-524 ROW will need to be restored to existing condition and sodded. The onsite access road can be left in place.

18. CIF-018: Will we be able to adjust our fuel cost if the fuel prices go up based on the FDOT fuel index? RESPONSE: Unit price adjustments will not be allowed during the duration of the contract.

19. CIF-019: Do you have specific work hours and work days that we can start loading & dumping? RESPONSE: No specific work hours or days are advertised with this project. Contractor should note that the haul route has been revised. Truck Routes will require use of I-95 and SR-528 to and from Port Canaveral to the ILC. The use of SR-524 from Industry Road to Cox road is prohibited for this project.

20. CIF-020: Is there a certain number of weather related delay days that you want everyone to incorporated into the bids? RESPONSE: The NOAA National Center for Environmental Information website: http://www.ncdc.noaa.gov/ will be used to establish historical (30-year) average rain days for each month of work. The contractor should anticipate the historical average monthly rain days in their bid schedule.

21. CIF-021: Can you confirm that a State Licensed General Contractor is required to bid this project? The Request for Proposal page mentions this but on page 002116-5 under D. Insurance/License Requirements it states that “Bidders must be properly licensed as a contractor to conduct business in the State of Florida”. RESPONSE: The language in the Request for Proposal document has been revised as follows: “The Canaveral Port Authority is seeking proposals from qualified Licensed Contractors for SR-524 Inland Logistics Center…” Bidders must be properly licensed as a contractor to conduct business in the State of Florida; however, do not necessarily need to be a Licensed General Contractor.

GENERAL NOTES

GENERAL STATEMENT

THIS SET OF PLANS REPRESENTS A DESIGN PACKAGE FOR GRADING, STOCKPILING AND FILL IMPORT ON THE CPA OWNED PROPERTYAREA NEAR THE INTERSECTION OF SR524 ANDI-95 IN COCOA, FLORIDA. THE LAND CLEARING WILL BE REQUIRED TO DEPOSIT FILL.

STORWATER MANAGEMENT WILL BE CONDUCTED BY BEST MANAGEMENT PRACTICES TO CONTROL EROSION, SEDIMENTATION, ANDRUNOFF.

NO UTILITIES ARE PROPOSED. NO BUILDINGS ARE PROPOSED.

THE PROJECT SITE IS LOCATED IN TOWNSHIP 24, RANGE 35, SECTION 23, WITHIN BREVARD COUNTY, COCOA, FLORIDA. ACCORDING TOTHE BREVARD COUNTY PROPERTY APPRAISER THE ADDRESS IS 4555 & 4565 SR 524, COCOA, FL 32926.

GENERAL NOTES AND DEMOLITION

1. THE ARRANGEMENT OF DRAWINGS AND/OR THE ORGANIZATION OF THE SPECIFICATIONS INTO DIVISIONS, SECTIONS ANDARTICLES ARE FOR CLARITY ONLY AND SHALL NOT CONTROL THE CONTRACTOR IN DIVIDING THE WORK AMONGSUBCONTRACTORS OR IN ESTABLISHING THE EXTENT OF WORK TO BE PERFORMED BY ANY TRADE. THE CONTRACTOR MAYSUBCONTRACT THE WORK IN SUCH DIVISIONS AS HE SEES FIT AND HE IS ULTIMATELY RESPONSIBLE FOR FURNISHING ALL WORKSHOWN ON THE DRAWINGS AND/OR IN THE SPECIFICATIONS.

IN THE EVENT OF INCONSISTENCIES BETWEEN PARTS OF THE CONTRACT DOCUMENTS OR BETWEEN THE CONTRACT DOCUMENTSAND APPLICABLE STANDARDS, CODES AND ORDINANCES, THE CONTRACTOR SHALL: (1) PROVIDE THE BETTER QUALITY AND/ORGREATER QUANTITY OF WORK; OR (2) COMPLY WITH THE MORE STRINGENT REQUIREMENT: EITHER OR BOTH IN ACCORDANCEWITH THE ENGINEER'S INTERPRETATION. TECHNICAL SPECIFICATIONS TAKE PRIORITY OVER GENERAL SPECIFICATIONS ANDDETAIL DRAWINGS TAKE PRECEDENCE OVER GENERAL DRAWINGS. ANY WORK SHOWN ON ONE DRAWING SHALL BE CONSTRUEDTO BE SHOWN IN ALL DRAWINGS AND THE CONTRACTOR WILL COORDINATE THE WORK AND THE DRAWINGS. IF ANY PORTION OFTHE CONTRACT DOCUMENT SHALL BE IN CONFLICT WITH ANY OTHER PORTION, THE VARIOUS DOCUMENTS COMPRISING THECONTRACT DOCUMENTS SHALL GOVERN IN THE FOLLOWING ORDER OF PRECEDENCE: THE OWNER CONTRACTOR AGREEMENT;MODIFICATIONS; ADDENDA; AND SUPPLEMENTARY CONDITIONS; THE GENERAL CONDITIONS; THE SPECIFICATIONS; THEDRAWINGS; AS BETWEEN SCHEDULES AND INFORMATION GIVEN ON DRAWINGS, THE SCHEDULES SHALL GOVERN; AS BETWEENFIGURES GIVEN ON DRAWINGS AND THE SCALED MEASUREMENTS, THE FIGURES SHALL GOVERN; AS BETWEEN LARGE-SCALEDRAWINGS AND SMALL SCALE DRAWINGS, THE LARGER SCALE SHALL GOVERN. ANY SUCH CONFLICT OR INCONSISTENCYBETWEEN OR IN THE DRAWINGS SHALL BE SUBMITTED TO THE ENGINEER WHOSE DECISION THEREON SHALL BE FINAL ANDCONCLUSIVE.

3. WORK SHALL BE IN ACCORDANCE WITH CANAVERAL PORT AUTHORITY (CPA) AND CITY OF COCOA RULES AND REGULATIONS. CPASTAFF SHALL BE THE REGULATORY AGENCIES POINT OF CONTACT TO VERIFY NECESSARY PERMIT CONDITIONS WITH, BUT NOTLIMITED TO, FDEP WATER/STORMWATER/SEWER, CITY OF COCOA, BREVARD COUNTY, SJWMD, AND EPA STANDARDS.CONTRACTOR SHALL KEEP A COPY ONSITE OF ALL PROJECT PERMITS AND SIGNED AND SEALED DRAWINGS.

4. BACKGROUND AERIAL AND TOPOGRAPHIC CONTOUR ELEVATIONS HAVE BEEN OBTAINED BY FDOT 2012 AERIAL IMAGERY ANDSJRWMD 2012 SHAPE FILES (LIDAR TRANSLATION), RESPECTIVELY.

5. EXCEPT WHERE THE PLANS AND SPECIFICATIONS PROVIDE THAT SUCH WORK SHALL BE PERFORMED UNDER THE CONTRACT FORTHIS PROJECT, ALL UTILITIES INTERFERING WITH CONSTRUCTION SHALL BE REMOVED OR ADJUSTED BY THEIR OWNERS AT THEIREXPENSE. THE CONTRACTOR SHALL ARRANGE HIS SCHEDULE TO ALLOW UTILITY OWNERS TIME FOR THE NECESSARYRELOCATION AND ADJUSTMENT OF UTILITIES AND RELATED STRUCTURES.

6. MATERIALS INTERFERING WITH CONSTRUCTION AND ABANDONED UTILITY LINES, PIPES, STRUCTURES, AND OTHERSUBTERRANEAN OBJECTS SHALL BE REMOVED. ALL MATERIALS NOT CLAIMED BY THE OWNER SHALL BECOME THE PROPERTY OFTHE CONTRACTOR AND SHALL BE DISPOSED OF IN A LEGAL MANNER.

7. ALL EXISTING AND PROPOSED UTILITY AND STORM WATER STRUCTURES WHOSE TOPS WILL BE EXPOSED WITHIN UNPAVED AREASSUBJECT TO VEHICULAR, BICYCLE, OR PEDESTRIAN TRAFFIC SHALL BE ADJUSTED SO THAT THE TOP SURFACE OF COVERS ORFRAMES SHALL BE NO MORE THAN AN 0.5 INCH ABOVE FINISHED GRADE, UNLESS OTHERWISE SPECIFIED ON THESE PLANS.WHERE SOD IS APPLIED, FINISHED GRADE SHALL BE NOMINAL HEIGHT OF GRASS AFTER SOD IS FIRMLY PLACED.

8. THE INDICATED LOCATION OF UNDERGROUND UTILITIES, STRUCTURES AND FACILITIES IS APPROXIMATE AND REFLECTS THE BESTINFORMATION AVAILABLE FROM SURVEYS AND RECORDS. IT IS THE CONTRACTOR'S RESPONSIBILITY TO VERIFY THE ACTUALLOCATION AND EXTENT OF UNDERGROUND FACILITIES AND NOTIFY OWNER & ENGINEER WITH ANY DISCREPRANCIES PRIOR TOPROCEEDING WITH ANY CONSTRUCTION ACTIVITY WHICH MAY AFFECT SUCH FACILITIES. ADDITIONAL BURIED UTILITIES MAY BEENCOUNTERED.

9. THE CONTRACTOR IS RESPONSIBLE FOR VERIFYING THE LOCATION OF ALL EXISTING UTILITIES AND COORDINATING WITH THEAPPROPRIATE UTILITY AUTHORITIES BEFORE CONSTRUCTION COMMENCES. CONTRACTOR MUST CALL SUNSHINE STATE ONECALL (OR PRIVATE LOCATER) FOR UTILITY LOCATES 48 HOURS PRIOR TO CONSTRUCTION AT 1-800-432-4770. ANY UTILITYCONFLICTS SHALL BE REPORTED TO THE ENGINEER AND RESOLVED PRIOR TO CONSTRUCTION.

10. 1. SAWCUT EXISTING PAVEMENT AT THE CONNECTIONS TO THE EXISTING ROADWAYS TO FORM A SMOOTH TRANSITION.

2. HORIZONTAL GEOMETRY REFERS TO ROADWAY ANNOTATIONS / LABELS, AS POSITIONED IN THE PLANS. DIMENSIONS ONROADWAY OR PARKING LOTS ARE TO EDGE OF CURB.

3. ALL CURBS (EXISTING OR NEW) SHALL BE PROTECTED FROM CONSTRUCTION DAMAGE. ALL CHIPPED OR CRACKED PORTIONS OFCURB SHALL BE REPLACED AT THE CONTRACTORS EXPENSE. IN ADDITION, ANY MORTAR, CONCRETE, SOIL AND OTHER DEPOSITSOR STAINS SHALL BE CLEANED TO RETURN THE CURBS TO THEIR ORIGINAL CONDITION.

PAVING, GRADING AND DRAINAGE

1. EXISTING ON-SITE AND OFF-SITE DRAINAGE PATTERNS SHALL BE MAINTAINED DURING CONSTRUCTION UNLESS OTHERWISEINDICATED ON THE PLANS.

2. ANY MUCK, CLAYED SOIL, HARDPAN AND/OR UNSUITABLE MATERIALS ENCOUNTERED, AS DEFINED IN FDOT INDEX #500, SHALL BEREMOVED AND REPLACED WITH SUITABLE MATERIAL, IN ACCORDANCE WITH SAID INDEX. THE CONTRACTOR SHALL USE AGEOTECHNICAL FIELD TECHNICIAN TO FIELD VERIFY THE REMOVAL OF UNSUITABLE SOILS. COPIES OF REPORTS SHALL BEPROVIDED TO THE ENGINEER.

3. ALL DRAINAGE STRUCTURES WITHIN PROJECT BOUNDARY SHALL BE CLEANED OF SAND, SILT, CONCRETE DEBRIS, ETC. ASREQUIRED DURING AND AT THE END OF CONSTRUCTION TO PROVIDE POSITIVE DRAINAGE FLOWS.

2. ANY PUBLIC LAND CORNER WITHIN THE LIMITS OF CONSTRUCTION IS TO BE PROTECTED. IF A CORNERMONUMENT IS IN DANGER OF BEING DESTROYED AND HAS NOT BEEN PROPERLY REFERENCED, THE CONTRACTORSHALL NOTIFY THE OWNER, WITHOUT DELAY, BY TELEPHONE.

3. ALL ITEMS DAMAGED SHALL BE RESTORED TO PRIOR CONDITION OR IMPROVED CONDITION.

CONSTRUCTION SEQUENCING ADVISEMENT:

CONTRACTOR IS ADVISED A SUBSTANTIAL MAINTENANCE OF TRAFFIC EFFORT ISREQUIRED BOTH ONSITE AND OFFSITE.

UTILITY NOTES (AS NEEDED):

1. ALTHOUGH WATER & SEWER UTILITY WORK IS NOT PROPOSED, RELOCATION AND COORDINATION EFFORTS MAYBECOME NECESSARY. B. ADJUSTING EXISTING UTILITY LIDS AND VALVES TO GRADE (ELECTRICAL, TELECOM, WATER, ETC).

C. PROVIDING PIPE DEFLECTIONS (HORIZONTAL OR VERTICAL).

D. PROVIDING JOINT RESTRAINTS (THRUST BLOCKS, MECHANICAL JOINTING, ETC). PROVIDING SPLICES, STRAIGHTDUCTING, AND/OR COUPLERS FOR ELECTRICAL WORK (SPECIFIED BY OTHERS).

E. PROVIDING TESTING OF WATER UTILITIES (BACTERIOLOGICAL, PRESSURE, ETC) .

OBSERVATIONS & QUALITY ASSURANCE:

1. THE CONTRACTOR SHALL NOTIFY THE ENGINEER OF RECORD PRIOR TO BEGINNING CONSTRUCTION AND AT/PRIORTO THE INSTALLATION OF THE FOLLOWING ITEMS, WHERE APPLICABLE:

1.1. CLEARING/ STRIPPING1.2. STRIPING/UNDERGROUND INSTALLATIONS1.3. FILL/PAVING/SODDING1.4. ELECTRICAL/LANDSCAPE ITEMS

2. STRIPPING - STRIP ALL SURFACES OF VEGETATION, STUMPS, DEBRIS, ORGANIC TOP SOIL (ORGANIC CONTENTGREATER THAN 5%), OR OTHER DELETERIOUS MATERIALS AS ENCOUNTERED. IF ORGANIC MATERIAL IS PRESENT INBOTTOM OF EXCAVATION THE CONTRACTOR SHOULD CONTACT THE ENGINEER OF RECORD PRIOR TO BACKFILLING.ORGANICS AND TOP SOIL SHALL BE DISPOSED OF BY THE CONTRACTOR AT NO ADDITIONAL COST TO THE OWNER.

3. FILL MATERIAL & COMPACTION - STRUCTURAL FILL SHOULD CONSIST OF INORGANIC, NON-PLASTIC, GRANULARSOILS CONTAINING LESS THAN 10% (BY WEIGHT) OF THE MATERIAL PASSING THE #200 SIEVE (RELATIVELY CLEANSAND WITH UNIFIED SOIL CLASSIFICATION OF SP, SP-SM, SW, OR SW-SM). REGRADED EXISTING FINE SANDS ARECONSIDERED GENERALLY ACCEPTABLE FOR FILL PRACTICALITY, EXCLUDING NEAR TOP SOILS OR ORGANICS.BACKFILL MATERIAL PLACED SHALL NOT EXCEED TWELVE (12) INCHES IN THICKNESS, AND COMPACTED TO AMINIMUM DENSITY OF THE 98% OF THE MODIFIED PROCTOR (AASHTO T-180).
