SIGN OF THE TIMESSigns went up all over Chicago area-based Franciscan St. James Health reading, “Does this add value for our patients?”

The signs were posted following a PHA executive training program in Lean methodologies. What did (or did not) add value became part of the conversation at executive and leadership meetings.

Meanwhile, Lean teams began such projects as reducing physician scheduling protocols, central-line infections, and inpatient readmissions, receiving patients upon inpatient admission, and decreasing RAC audit losses.

By focusing on value to the patient, Franciscan St. James Health is expected to save $2.5 million.

SITUATION Scott Memorial Hospital wanted to reduce transition-of-care time from its emergency department to its Med Surg nursing unit.

SOLUTION While earning Lean certifications through PHA, team participants within the Critical Access Hospital committed to methodologies that put the patient first. They found that reducing the anxiety experienced by the patient while waiting to be moved from the ED to a regular room trumped reducing interruptions for nurses during shift changes. A team decision was made to remove the “no fly zone,” or the time between 6:15 and 7 o’clock in the morning and evening during during which no patients could be transported to another unit.

IMPACT With PHA guidance, the improvement initiatives reduced ED to Med Surg transition time at Scott Memorial by two-thirds.

ELECTRONIC HEALTH RECORDS Meaningful Use (MU) of electronic health records is composed of dynamic stages that will continue to evolve and challenge groups to move outside their comfort zones.

SITUATION Columbus Regional Hospital needed to get more than 35 providers to Stage 1 Meaningful Use in 2012 using two disparate Electronic Health Records systems. While the hospital had a lot of expertise in one of the EHR products, it had limited experience with the other.

SOLUTION The hospital partnered with PHA to assist its staff as they learned a new EHR system and applied the federal Meaningful Use objectives to their everyday workflows. PHA provided a HIPAA Security Risk Assessment, a Readiness Assessment, and Attestation Support and Guidance.

IMPACT PHA worked with ONC to determine the true intent of the law and how the data needed to be captured to be compliant with MU. PHA was then able to facilitate conversations between the client and its vendors to ensure the changes were made allowing for timely Meaningful Use attestation.

Security breaches can leave providers and practice administrators personally liable, facing monetary fines and jail time if they knowingly fail to protect the privacy of electronic patient health information.

SITUATION Rush Memorial Hospital officials wanted to ensure that Electronic Patient Health Information (ePHI), either at rest or in motion, was secure under the best practices possible.

SOLUTION With the latest assessment and validation tools, PHA supplemented Rush Memorial’s IT security/privacy measures through a Network Security/Risk Analysis that analyzed the data at rest and in transit, assessed both physical and virtual spaces, reviewed existing policies and procedures, sought vulnerabilities in system configurations and networks, conducted on-site interviews with select staff members, and presented a threat vulnerability assessment and mitigation plan.

IMPACT With a PHA review, Rush Memorial is well positioned to protect its ePHI as it meets the standards for privacy and security under Meaningful Use and HIPAA.

PREDICTIVE MODELING AND COMPUTER SIMULATION

ASSET ALLOCATION: RIGHT PLACE, RIGHT TIMEFrom small practices to large health systems, organizations face the constant challenge of managing operations. Utilizing data from your organization, PHA uses predictive modeling and computer simulation techniques to determine the true probabilistic nature of such scenarios as staffing, facility design, readmissions, and clinical conditions.

AMBULATORY SURGERY CENTER STAFFING

Collaborating with Purdue’s School of Biomedical Engineering, PHA applied predictive modeling/computer simulation to IU Health Arnett Hospital & Physicians’ ambulatory surgery center to understand how layout and staffing levels affect patient access and to determine proportions of staff and rooms to match patient-demand predictions. A high-fidelity simulation confirmed what leadership suspected was needed to increase patient access and decrease average length of stay: one additional anesthesiologist.
