    CLEMENTS HIGH SCHOOLSTUDENT HEALTH SERVICES

    Dear Clements High School Parent/Guardian,

    Please contact me regarding your students health issues, extended illnesses andhospitalizations. I can better serve your student to promote or improve his/her healthand/or reduce lost time from the classroom with your input.

    Please be familiar with the procedures below:

    Clinic Procedures :

    When a student feels ill or is injured at school, it is important that the student go to theclinic rather than call or text from their mobile phone PLEASE NO TEXTING.

    When your student arrives in the clinic, first aid is administered if needed or an initialassessment is made. Your student is welcome to contact you - whether or not they wish

    to go home. All clinic visits are documented in our electronic records to protect yourstudents attendance record. The items bulleted below are reasons that a student must stay home or leave school , otherwise the decision to pick-up your student or allow themto drive home is up to you as the parent.

    Temperature of 100.4 degrees or greater within the past 24 hours (without feverreducing medication)

    Vomiting and/or diarrhea before school or at school An undetermined rash Feeling too ill to remain at school

    Your student must attempt to contact you about going home. If unable to reach you,your student should leave you a phone message and contact an adult previouslydesignated by you as an alternative contact. Clinic staff must personally speak withparents of student drivers before they can drive home (regardless of the age of your

    student).If your student signs out to go home through the CHS clinic, he/she is automaticallyexcused for the remainder of the day - even if you get the automated call stating thatyour student missed one or more periods. If your student stays home the next day orpart of the next day, a note from you to the attendance office is required within 2 days.

    MedicationsYour student is not allowed to carry any medication during the day with the exception ofEpi-pens, inhalers or insulin (with the proper paperwork completed each school year).Medications that your student may need during the school day must be furnished by youand stored in the clinic in the original container with the proper written authorization. Ifyour student has daily medications, he/she will be asked to come to the clinic betweenclasses.

    Temporary disabilitiesIf your student needs assistance getting to classes, please send a note describing thedisability and modifications needed. Five minutes passes may be issued with elevatorand book assistance as needed. The 5-minute pass will have an expiration date. Yourstudent should have this pass with him/her and available for inspection. PE exemptionsmore than a week require a doctors note.

    Immunizations

    You are responsible for providing and updating the required immunization records toenter school and remain in school.

    We are here to help your student make the most of his/her educational experience. Ifyou have any questions, please call or come by the clinic. You are always welcome.
