This publication sets orth detailedrecommended procedures orusing Stryker Trauma devices andinstruments.
It oers guidance that you shouldheed, but, as with any such technicalguide, each surgeon must consider theparticular needs o each patient andmake appropriate adjustments when
and as required. A workshop trainingis required prior to irst surgery.
See package insert (V15013 andV15034) or a complete list o potentialadverse eects, contraindictions, warnings and precautions. Thesurgeon must discuss all relevantrisks, including the inite lietime o the device, with the patient, whenneccessary.
Warning:All bone screws reerenced inthis document here are notapproved or screw attachment orfxation to the posterior elements(pedicles) o the cervical, thoracicor lumbar spine.

Reduction and temporary ixation o racture ragments are technically andmechanically one o the more diicultparts in Osteosynthesis procedures.
Reduction is the act o restoring thecorrect position o the ragments. Itis extremely important in intra- andperi-articular ractures.Perect anatomical reduction reversesthe mechanism o injury whichcreated the racture displacementduring the injury. Logically, it callsor orces and moments in directionsopposite to those which produced theracture.
In large bone reduction, traction andsome basic angular and rotationalcorrections may be applied using aracture table with the disadvantagethat traction must be perormedacross at least one joint.
When external reduction is impossibleor insuicient, then the reduction may need to be perormed by ORIF withexposure o the racture ragments. These ragments are manipulated
using various orceps to achieveperect restoration o the anatomy. The newly developed Basic ReductionSet is designed and equipped withinstruments or the most commonreduction techniques in traumatology. One instrument case contains avariety o common orceps and otherinstruments required or surgicalreduction and temporary ixation. The Basic Reduction Set is part o theStryker plate and screw amily.
The combination o simplicity andversatility can solve the surgeon’s daily challenges or quick and accuratesurgical reduction.

Instruments
Repositioning ForcepsPointed orceps are typically instruments used or direct racturereduction.
Length 200mm
Using one Forceps
In oblique diaphyseal ractures, onebranch o the orceps is positioned oneach main ragment. With some pressure and simultaneousrotation o the handles the bone islengthened and the ragments arereduced.
Using two Forceps
Each ragment is held with a pointedreduction orceps.Lengthening is achieved by manual dis-traction while proper axial alignmentcan be controlled with the two orceps.
Using one Forceps or fat bone
suraces
The orceps can also be applied directly to at bone suraces or be used withshallow drill holes

Instruments
These small pointed reduction orcepsmay be used or direct or indirectracture reduction as they are moregentle to the periosteal sleeve.
Small, undisplaced ragments
Provisional fxation o small, undisplaced ragments with directapplication o the orceps to the bone.
Fracture reduction with plates
Indirect racture reduction with a plate. One branch o the orceps has a specialtip to ft the K-Wire holes in smallragment plates.

The serrated jaws o this reductionorceps oer a sae grip on the bone ordirect reduction o diaphyseal ractures. The orceps can also be used orindirect reduction o a ragment usinga plate in an antiglide unction.

The ball spike fts into the oval holes inbroad and narrow plates o the AxSOSand SPS Basic Fragment Set. Thelength o the spike has been designedto impinge on the bone surace ater

The pivoting swivel head allows aprecise positioning o the orceps inprovisional plate fxation. The widtho the head has been designed to ftthe narrow basic ragment plates usingorceps size 1 and the broad plates with
size 3 respectively.

For “closed” indirect reduction o proximal tibial or distal emoralractures, one or two points o the Mantis orceps can be appliedpercutaneously through stab incisions. The spiked disk can be used orincreased surace contact.

The versatile Farabeu orceps can beused to grasp and manipulate the iliacwing or as a reduction orceps withprovisional screws o 3.5mm or 4.5mmdiameter respectively.
It can also provide direct grip on boneor the manipulation and reduction o segmented diaphyseal ractures.

The insertion o Schanz Screws(Reduction Pins) allows manipulationo bone ragments with or without adirect view. The technique is mainly used in articular ractures or hardly reducible large bone ragments.

The Hohmann Retractors are part o the basic sot tissue instruments andmainly used to retract muscle or othersot tissues rom the surronding o the
bone without injuring or tearing any sot tissues.
In cortical bone, a small-tippedHohmann retractor can be used as alever to achieve reduction. The tip o the retractor is placed between the twocortices o a diaphyseal racture. It isthen turned through 180° to engagethe cortex o the opposite ragment. With a bending orce applied by theretractor, the two cortices can be both
opposed and realigned, allowing gentlereduction o the racture.
