on behalf of appleton Medical Center Foundation’s Board of directors, i am privileged to share our 2010 Report to the Community with you. This report contains a summary of appleton Medical Center Foundation’s recent accomplishments, and provides the Foundation an opportunity to

say “Thank You” to the individuals and organizations who have supported our cause.

Your charitable gifts allow the appleton Medical Center leadership team to plan strategically for a strong future. a significant example of how your gifts make a difference can be seen daily in the Hospital’s corridors as Collaborative Care becomes an integral part of the patient experience.

over the past three years as president of the appleton Medical Center Foundation Board of directors, i have been inspired by the steadfast generosity of our community. With an uncertain economic future facing us, we have come to realize even more how much appleton Medical Center depends on the charitable care that the Foundation provides. as i leave the Board presidency in the capable hands of dr. Appleton Medical CenterEast Care Pavilion

ppleton Medical Center’s eight-story east Care Pavilion opened on July 19, 2010. The Pavilion’s 112 rooms replaced those in the Hospital’s original 1958 building.

The east Care Pavilion was designed to support ThedaCare’s revolutionary Collaborative Care model of inpatient care. Collaborative Care consists of a care team made up of a physician, nurse and pharmacist. The care team is supported by care managers, therapists and other clinicians who work with the patient and their family to create a single plan of care from admission through discharge. Since implementing Collaborative Care at appleton Medical Center, ThedaCare has documented shorter lengths of patient stays, lower costs per stay, and improved patient satisfaction.

The Hospital’s new nursing units and private rooms include design innovations that not only support Collaborative Care, but offer more effective, efficient, and customer-focused care in every unit. in addition to process innovations, the transformation utilizes bedside work stations, wireless computers, and nursing workspaces in corridors adjacent to patient rooms.

Many physician partners at appleton Medical Center participated in the Collaborative Care planning process and have been instrumental in its success. “as we continue to spread Collaborative Care to each nursing unit, and work with different specialty physicians, we gain more insight into providing better care for various medical conditions,” said Kathryn Correia, president of appleton Medical Center and Theda Clark Medical Center and ThedaCare senior vice president.

“The changes at appleton Medical Center allow patients to experience a higher quality of care because they no longer need to wait for the nurse, doctor, pharmacist, or other medical personnel to track each other down and figure out what to do next to progress the patient,” said Matthew Furlan, chief operating officer for appleton Medical Center and Theda Clark Medical Center. “The east Care Pavilion provides a quiet and private environment for our patients and space for the team of caregivers to develop one plan of care with the patient and their family. The result is higher quality at a lower cost, which will, in turn, pay for the new building,”he added.

Thanks to the generosity of donors who support the appleton Medical Center Foundation, each year the Foundation Board of directors awards grants to the Hospital for the purpose of purchasing needed medical equipment establishing new procedures. in 2010, the appleton Medical Center Foundation was pleased to award $273,076 in grants to support services and capital needs at the Hospital that would otherwise be unavailable to our patients. grants awarded in 2010 include:

I-sTaT point-of-care testing analyzer for the emergency services DepartmentCode STeMi is an important and innovative program the Foundation supports at appleton Medical Center for cardiac patients. i-STaT point-of-care testing analyzers allow physicians to ability to process blood samples at the patient’s bedside to assist in the rapid diagnosis

of life threatening conditions. a grant from the Foundation enabled the emergency Services department to purchase this critical tool for emergency physicians.

DasH 4000 Cardiac Monitor for the emergency services DepartmentWhen additional cardiac monitors were needed to respond to the high volume of chest pain patients who are seen in the emergency Services department, the Foundation provided funds to purchase a daSH 4000 cardiac monitor.

fetal Monitors for the birth Center Fetal monitors ensure safety for patients (and their babies) while in labor. With the purchase of three fetal monitors, there are now monitors in every labor room in the Birth Center. The information from these monitors enables the care team to make decisions quicker should an emergency arise.

Upgrade to the Interventional Cardiac Catherization labThe interventional Cardiac Catherization lab upgrade provides improved image quality for diagnostic accuracy and better patient treatment plans. it also gives providers quicker access to needed equipment in situations such as Code STeMi.

appleton Medical Center Main entrance and foundation Donor Wall RenovationsFunding was provided for a local artist to design a new Foundation donor Wall and two pieces of decorative art located on the first floor at appleton Medical Center. The cosmetic enhancements are part of the transformation of appleton Medical Center and the construction of the east Care Pavilion.

Foundation Grants

The sarah schmitt scholarshipin 2004, the emergency Physicians at appleton Medical Center (aMC) established a scholarship in memory of Sarah Schmitt, a clinical technician in the aMC emergency Services department who was studying to become a registered nursed when she died unexpectedly. The Scholarship is offered to aMC emergency Services department staff and their dependents for continuing education to receive a BSn or become a Rn or nurse practitioner. Special consideration is given to those with similar aspirations and goals as Sarah. Recipients of the 2010 Sarah Schmitt Scholarship were Victoria neuman and Kimberly Werfal, both of whom are attending Concordia University.

The sharon stanlik scholarship

in 2007, Sharon Stanlik established a scholarship in memory of her mother, virginia Plewa. The scholarship is available through the generosity of the Sharon Stanlik endowment Fund and is intended for the benefit of students pursuing a degree and/or certification in the nursing field or for professional development of nurses. The 2010 Stanlik Scholarship was presented to lindsay bahn of Berlin Wi, who is attending Marquette University.

The Jayme Reetz scholarshipJayme Reetz was employed at appleton Medical Center (aMC) for 12 years as a registered nurse in the Cardiac intensive Care and Post-anesthesia Care Units. in 1994, Jayme died as the result of complications following a double lung transplant. in her memory, a scholarship fund was established specifically designed for aMC employees who are continuing their education to receive a BSn or

studying to become a Rn, nurse practitioner or clinical nurse specialist. Since Jayme’s passion lay in cardiology, intensive care, aClS instruction, and surgery, these areas are highly considered when the selection is made. Recipients of the 2010 Scholarship were Kara brochtrup and Juliann stanis, both of appleton Wi, and Kimberly Werfal of neenah Wi.

June 2nd :Dr. James V. o’leary discussed the role of the da vinci® robot in gynecology. attendees had the opportunity to experience firsthand how the robot functions. after learning about the da vinci® surgery system, guests received a hands-on experience in the operating room.

July 27th : Dr. Mark Hermans and Dr. James McGovern spoke about the role of the traditional internist and the history of the hospitalist movement. They shared their insights and experiences of the transition of inpatient care from the primary care physician to the hospitalist. They also discussed how hospitalists are involved in improving the functions of the Hospital.

august 4th : Dr. Cyril Walsh of the appleton Medical Center emergency Physician group and Julie Thompson, a registered nurse and program coordinator, explained how Code STeMi saves lives of heart attack patients. Code STeMi allows the emergency Services department to diagnose and treat patients showing symptoms of a heart attack within the first hour after they arrive at the Hospital.

august 24th: Dr. Mark Moyad presented a one-of-a-kind Healthcare Forum when he shared insights from his book “dr. Moyad’s no BS diet Health advice:a Step-by-Step guide to What Works and What’s Worthless.” as director of Preventative and alternative Medicine at the University of Michigan Medical Center, dr. Moyad has maintained a consulting practice on complementary medicine for the past 16 years.

appleton Medical Center Foundation’s “laughter is the Best Medicine” raised over $19,000 at their event on February 20th at Trolley Square Waterfront Banquets in appleton. Funds raised were used to support the Foundation’s Healing Hearts and Pulmonary Rehabilitation Funds. The gathering offered guests a relaxed, fun-filled evening while being entertained by ComedySportz-Milwaukee.

on July 19th, 144 golfers joined forces to support both appleton Medical Center and Theda Clark Medical Center Foundations at north Shore golf Club. The event raised over $76,000! While golfers participated in various on-course contests, 40 non-golfers enjoyed a cooking demonstration presented by Chef eric Floriano from north Shore golf Club, and wine tasting with amber Mihna, Certified Wine Specialist with general Beverage Sales Company. Chef eric prepared three dishes that guests sampled while amber discussed three unique wines she paired with the food selections. a hors d’oeuvre reception and live auction followed in the main ballroom.

gifts to appleton Medical Center Foundation can have an immediate impact when needed equipment is purchased or current initiatives are underwritten. gifts can also provide lasting support through an endowment. no matter what your charitable objectives are, you can accomplish them by giving in a variety of ways, including:

Gifts of Cash – Cash gifts make an immediate impact on patient programs, community education, and research initiatives. gifts may be made in memory or honor of a friend or loved one.

endowed named funds – donors may establish endowed named funds to be administered to support a particular hospital program or initiative with a gift of $10,000 or more, payable over three-to-five years. endowed funds help ensure that important Hospital programs will be sustained in perpetuity.

Gifts of appreciated assets – gifts of non-cash assets such as common stock can offer valuable tax advantages when compared with cash gifts. in addition to avoiding capital gains tax on the appreciated value of the stock, donors receive a tax deduction for the full cash value of their gift of stock.

Gifts that Return Income – Significant tax advantages make life income agreements such as charitable trusts and gift annuities an attractive way to offer the opportunity to receive income during a donor’s lifetime while ultimately supporting the caring mission of appleton Medical Center Foundation.

Gifts by Will – Simply inserting the desired numbers into the sentence, “i bequeath $____ or ____% of my estate to appleton Medical Center Foundation” will cost nothing today, but will help the hospital in the future, and help to reduce the donor’s estate taxes.

Annual Partner Campaignin 2010, the appleton Medical Center Foundation announced the implementation of an annual Partner Campaign.
