2. For more information, refer to the attached communication.

Participation of interested personnel to this activity shall be subject to the discretion of theSchools Division Superintendents, and to the provisions of DECS Order No. 28, s. 2001 entitled"Prohibiting the Commercialization of the DECS Organization through Endorsements andAccreditation of Goods and Services," DepEd Order No. 9, s. 2005 entitled "InstitutingMeasures to increase Engaged Time-on-Task" of "No-Disruption-of-Classes Policy," andall other existing DepEd policies and regulations, and ensuring compliance therewith.

CODE OF CONDUCT! hereby certify that my participation to the SYL INTERNATIONAL PROGRAM IN ASIA is voluntary. I understand that the following are prohibited: the

use of alcohol, tobacco or illegal drugs, discriminatory comments, violence and sexual misconduct. I understand that !, being a participant, amresponsible for my conduct at all times and I must comply with the safety regulations, environmental protection, policies and behavior set forth bySYL and the host country. 1 understand that any violation of the above will be ground for expulsion without refund of the registration fee ! paid. Ifurther understand that the leadership exchange activities are exciting, character building and fun, however, I also understand that it wili bestrenuous, physically and emotionally challenging. I will cooperate with other members and the staff without complaints and murmur, and I will domy best to maintain a positive attitude. I agree to abide with the above and I am enthusiastic in participating in the program.

In consideration of the right to attend and participate, the participant hereby:

1. That my participation to the program is final and voluntary.2. That I will pay Ten Thousand (P10,00.00) pesos upon signing up. This fee is intended for initial application and this fee is non-refundable.3. That I will pay the remaining amount a month before the scheduled travel.4. That in the event I will withdraw from the program for personal and other reasons, the ten thousand cannot be refunded.5. Authorize SYL or any of its agents to provide, obtain, or authorize any reasonable incidental and emergency medical treatment for the

participants in the event of illness, injury or incapacity and hereby accepts all responsibilities.6. Acknowledges that there is an element of risk involve in any activity like travel outside owns home and community; certifies that the

participant is physically, mentally and emotionally capable of attending and participating in the activities; assumes all risks of financialresponsibility for any loss or injury to the participant that may occur as a result of participant's negligence or misconduct and indemnifiesand holds SYL harmless from against any and all cost of the suit and actual attorney's fees in court and suffered by SYL as a result of orarising out of the participant's negligence and misconduct.

7. This consent and acknowledgement of risk shall not be amended, supplemented, or abrogate without the written consent of SYL and thatthe participant and his/her parents have read this consent form and understood its contents.

8. The participant (and if the participant is a minor), his or her parents or guardians has read this form and understands its contents.
