At Duffy Health Center, our mission is to improve the quality of life for persons who are homeless or at risk of homelessness on Cape

Cod through community collaborations and the provision of integrated medical, mental health, addiction treatment, case management and housing support services.

Our team cares for Cape Cod residents who have complex medical, mental health, and substance use disorders. Our patients often lack

the social supports many of us take for granted.

2017 was a year of fiscal and program growth for Duffy Health Center! We expanded services, increased collaboration with our community partners, and our team provided integrated, quality care to 3,200 vulnerable Cape Codders.

As we reflect on Duffy Health Center’s 20th anniversary year, we honor our grassroots origins. Without the individuals who believed in our mission and were willing to invest their time, treasure, and talent to make Duffy Health Center a reality, we would not be where we are today. We honor their legacy by recommitting to our vision of Cape Cod as a community where all persons have access to quality health care, safe and affordable housing, and lives filled with hope and purpose.

Today, we look to the future. Collectively, we face many challenges: federal funds and Medicaid dollars are at risk; health care system changes require innovation in the face of ambiguity; the opioid epidemic continues its devastation; and Cape Codders struggle to find safe and affordable year-round housing.

This fall we are conducting our strategic planning work on how to meet these challenges. Now, more than ever, we need the support of our generous Duffy community. As our first supporters invested so much in our mission, so we now ask for your support to continue this work. We need not only your generous gifts, but for you to be a champion for Duffy. Let people know why you support the Duffy Health Center and how they, too, can make a deep impact on the lives of so many Cape Cod residents.

20 Years of Restoring Health and Rebuilding Lives

Nurses from the VNA set up a clinic at the NOAH Shelter to provide medical care to homeless patients.

The O’Neill Clinic is incorporated as a private non-profit at the NOAH Shelter.

The Health Resources and Services Administration awards a federal grant for the clinic to become a community health center serving persons experiencing, or at risk of homelessness.

The clinic is renamed Duffy Health Center in honor of Elizabeth and Mary E. Duffy. Duffy family funds enable the clinic to move to 105 Park St., which then expands to two additional sites.

Federal funding enables Duffy Health Center to begin offering mental health and case management services. Duffy becomes the first community health center on Cape Cod to begin evidence-based practice using Suboxone to help people with opioid use disorders.

Duffy’s “Housing First” model is implemented. Supported by state funding, this program includes permanent housing and supportive case management services. Today, it provides housing for 43 persons, reducing costs to the community while improving health outcomes.

Continuing program expansion, Duffy moved to the current location at 94 Main St. with support from the Health Resources and Services Administration and private donors.

Duffy is recognized as a Patient-Centered Medical Home. We expand hours, increase service options, and implement a team-based approach for patients.

The state’s Prevention Wellness Trust Fund is awarded to the Barnstable County coalition, including Duffy. Focusing on disease prevention and wellness, it links people to community partners such as the YMCA.

Duffy clinical staff continue to expand its evidence-based practice of Medication Assisted Treatment for persons with opioid use disorders. Over 400 persons are treated in 2016. In response to community needs, Duffy expands adult substance use disorder services and to start a treatment program for young adults ages 18-25.

Last year, your support helped Duffy Health Center to...

Complete treatment for 35 patients with Hepatitis C, with an 89% success rate. Together, we provided access to leading edge medications that actually cure this debilitating disease.

House 43 chronically homeless individuals; 85% of our Housing First clients remain housed after one year.

Provide follow-up care for 203 patients being discharged from Cape Cod Hospital’s inpatient psychiatric facility, with 93% connecting to a Duffy service.

Expand our Medication Assisted Treatment program from 240 to over 400 patients with opioid use disorders.

Place 56 people in temporary housing through the use of “In From the Streets” funds raised at the Gala; 33 of those individuals were subsequently placed into more stable housing.
