Internal Inspection of CS-1-9011A 5 8 and FW-1-349 5 353

As part of an in-service inspection audit by the NRC, inspections were performedon check valves CS-1-9011A, CS-1-9011B, FW-1-349 and FW-1-353.

All four valves were visually inspected to evaluate their overall physicalcondition, operability and possible defects. The results of the inspectionsare presented below.

Inspection of CS-1-9011A:

CS-1'-9011A disc was removed to facilitate inspection of the seating surfaces.The overall physical condition of the valve is excellent. Special attentionwas given to the condition of the seat, disc, disc stop, disc nut, disc nutlocking device, disc stud threads, swing arm support pins, cap, body internalsand visible welds. All of the mentioned items appeared to be in excellentcondition. The movement of the disc was smooth and unrestricted. No degrada-tion or evidence of excessive deformation was found at the disc stops'ointof impact. The locking device was intact with no signs of wear. The minorsurface corrosion visible should not interfere with valve operability.

Inspection of CS-1-9011B:

CS-1-9011B disc was also removed to facilitate inspection of the seating surfaces.The overall physical condition of the valve is excellent. Special attention was

given to the condition of the seat, disc, disc stop, disc nut, disc nut lockingdevice, disc stud threads, swing arm support pins, cap, body internals andvisible welds. All of the mentioned items appeared to be in excellent condition.Disc movement was smooth and unrestricted. No degradation or evidence ofexcessive deformation was found at the disc stops'oint of impact. The lockingdevice was intact with no signs of wear. The only notable surface corrosion isvisible in the photographs. This corrosion indication seems to be a result ofstanding water in the line and should not compromise the valve's operability.

Inspection of FW-1-349:

FW-1-349 was removed and disassembled for the inspection. The overall physicalcondition of the valve was good. Special attention was given to the duo check

pin, spring, seating surfaces and ease of movement. The spring was at theproper stiffness with no signs of abnormal wear. The seating surfaces wer'e

slightly pitted and were subsequently lapped to produce a better seal. Move-

ment of the duo check mechanism was excellent with no binding and positiveclosure.

Inspection of FW-1-353:

FW-1-353 was removed and disassembled for inspection. The overall. physicalcondition of the valve was good. Special attention was given to the duo checkpin, spring, seating surfaces and ease of movement. The spring was at theproper stiffness with no signs of abnormal wear. The seating surfaces weresli'ghtly pitted and were subsequently 1apped to produce a better seal. Move-ment of the duo check mechanism was excellent with no binding and positiveclosure. Minor line corrosion debris was stuck to internals. The amountof corrosion was minor and was only remarkable as part of an otherwiseoperable valve.

Internal Visual Inspection of Unit 1 Main Steam LineReverse Flow Check Valves: MS-1068, 2066, 3062 and 4062.

As a part of an In-service Inspection Audit conducted by the NRC, the reverse.flow check valves immediately downstream of the main steam isolation

valves'FCV-41,

42, 43 and 44) were visually inspected to evaluate their overallphysical condition, cleanliness and operability. The details of each valve'sinspection are presented herein. This work was conducted using MaintenanceShopwork Followers MM-1-83-067. MM-1-83-068, MM-1-83-069 and MM-1-83-070.These valves are Schutte 5 Koerting Company valves.

Inspection of MS-1068~ Reverse Flow Check Valve:

This valve was inspected first on the afternoon of April 13, 1983. During thecourse of the inspection, the disc was raised by hand which allowed water fromthe other side of the valve to .enter the valve body and the steam line. NuclearPlant Problem Report DC1-83-MM-P0054 was written to resolve the problem. Thewater between the isolation and check valves and in the check valve was drainedon April 14, 1983. The valve was reinspected and all water was removed.

The general physical condition of the inside of the valve was good. All sur-faces were covered with a loose coating of oxide which is typical for this typeof valve. The seat, however, was free of rust and in good condition. As

evidenced by the fact that the valve held back water merely by the weight ofthe disc against the seat, the seat can be presumed to be in good order. Move-

ment of the tail link on the rock shaft was smooth. Two set screws securingthe tail link to the rock shaft were found in place. The disc nut locking pinwas found in place, secure. No debris was found in the valve body.

Inspection of MS<066, Reverse Flow Check Valve:

This valve was insoected on April 15, 1983,.and found to be dry and in goodphysical condition. All surfaces, except the seats, were covered with a loosecoat of oxide which is typical. Movement of the tail link on the rock shaftwas smooth. The two set screws securing the tail link to the rock shaft werefound in place. The disc nut locking pin was found securely in place. No

debris was found in the valve body.

Inspection of MS-3362, Reverse Flow Check Valve:

This valve was inspected on April 12, 1983, and found to be dry and in good

physical condition. All surfaces, except the seats, were covered with a loosecoat of oxide which is typical. Movement of the tail link on the rock shaft

This valve was inspected on April ll, 1983, and found to be dry and in goodphysical condition. All surfaces, except the seats, were covered with a loosecoat of oxide which is typical. Movement of the tail link on the rock shaftwas smooth. The two set screws securing the tail link to the rock shaft werefound in place. The disc nut locking pin was found securely in place. Nodebris was found in the valve body.

Provision for In-service Inspection of Reverse Flow Check Valves:

In order to provide access for in-service visual inspection of these fourvalves, a design change request has been prepared to modify the drain lines onthe bottom of each valve to allow a Fiberscope to be inserted into the valve.This will allow the position and condition of the disc, tail link and lockingdevices to be inspected during refueling outages or other outages.

Resolution of Open Items

An IST Program working meeting was held at Diablo Canyon on February 7, 8and 9, 1983, between PGandE personnel, NRC Staff and EGRQ consultants.During this meeting a list of open items was generated. These items werelisted in a notegram from KQRG dated February 22, 1983 and titled, "TripReport for the Pump and Valve Inservice Testing Program Working Meeting forDiablo Canyon, Unit 1". The resolution of these open itens follows:

Open Item A.2

"Open iten for the utility to determine if allvalves with fail-safe actuators are being testedin accordance with IWV-3415."

PGandE Resolution

At DCPP all valves with fail-safe actuators are tested in accordance withthe intent of IWV-3415: "When practical, valves with fail-safe actuatorsshall be tested by observing the operation of the valves upon loss ofactuator power".

Open Item A.5

"Open iten for the NRC and utility to determineappropriate wording for not exercising the reactorvessel head vent valves (8078A, B, C, RD) quarterly."

PGandE Resolution

During the working meeting both the NRC and PGandE agreed that the ReactorVessel Head Vent Valves 8078A, B, C and D should not be exercised duringpower operation. The failure of one of these valves in the open positionis highly undesirable and nay jeopardize the reactor coolant pressureboundary integrity.

PGandE Resolution

The Auxiliary Feedwater Check Valves FW-348, 361, 369, 371, 373 and 375shall be partial-stroke exercised during paver operation and full-strokeexercised at a cold shutdown frequency using the auxiliary feedwater pumpdischarge. This is referenced in Cold Shutdown Statement K.

Open Item C.2

"Open item for the utility to determine a m thodand frequency for exercising or inspecting to verifyclosure of the main steam line check valves."

XGandE Resolution

The blain Steam Line Check Valves 5$-1068, 2066, 3062 and 4062 shall bedisassembled and/or internally inspected to verify OPERABILITY'. Thisinspection will be performed at a frequency of at least once per five years(naninal every third refueling outage). This is referenced in ReliefRequest P2. These valves were included in "the initial inspection resultson those valves that are to be disassanbled to demonstrate OPERABILITY".

Open Item F.13

"Open item for the utility to determine if thecentrifugal charging pumps discharge check valves,8478A and B, are full-stroke exercised quarterly."

XGandH Resolution

The Centrifugal Charging Pumps Discharge Check Valves 8478A and B shall bepartial;stroke exercised quarterly and full-stroke exercised at refuelingoutages. This is referenced in Relief Request P4.

XGandE Resolution

The Accumulator Discharge Check Valves 8956A, B, C and D shall be partial-stroke exercised during refueling outages since neither full-stroke exercisingnor disassembly are feasible. This is referenced in Relief Request ¹12.

The Containment Hydrogen Sampling Return Check Valves VAC-252 and 253 shallbe full-stroke exercised open at a cold shutdown frequency using the hydrogensampling system with flow indication. ,This is referenced in Cold ShutdownStatement ¹27.
