Reflexiones/Conclusiones
DM con enfermedad multivaso: CABG de elección si es factibleMejor pronóstico que ICP “a igualdad de condiciones”
Ensayos clínicos: pacientes muy seleccionados, técnicas evolucionan…Individualizar la decisión de revascularización (Heart Team). Factores a considerar:
Riesgo quirúrgico (comorbilidades), campos distales para CABG
Factibilidad y complejidad ICP (nº stents, longitud, diámetro…), posibilidad antiagregación
Buscar revascularización lo más completa posible
Disfunción ventricular
TC: afectación distal (bifurcación) y/o otros territorios
Valorar ICP (si es factible y no compleja) cuando:Riesgo quirúrgico prohibitivo o elevado
Malos campos distales con menor probabilidad revascularización completa con CABG (posibilidad de procedimiento híbrido)
Multivaso con Syntax score bajo (≤22)
TC sin afectación bifurcación (aislado o + 1 vaso) con Syntax score no alto (≤32)
